1.Acute extensive anterior myocardial infarction after medullary infarction:a case report

The medulla oblongata,situated at the caudal portion of the brainstem,serves as a critical regulatory center responsible for maintaining fundamental vital functions including respiratory and cardiovascular homeostasis.As a pivotal hub within the autonomic nervous system,it orchestrates the coordinated control of afferent and efferent neural pathways.Dysfunction of this region may precipitate life-threatening cardiorespiratory arrest,associated with substantial mortality rates.This case report presents a patient who developed acute extensive anterior myocardial infarction during treatment with dual antiplatelet therapy and moderate-intensity statins following acute medullary infarction.It is hypothesized that the pathogenesis may involve the acceleration of plaque erosion by the stroke-heart syndrome.This clinical case provides valuable insights into the complex neurocardiac interplay,particularly highlighting the imperative for enhanced recognition of brain-heart axis interactions in cerebrovascular pathology.
2.Incidence trends and age-period-cohort analysis of lip and oral cancer in China, 1990-2021

Objective:To analyze the incidence trend of lip and oral cancer in China from 1990 to 2021.Methods:The incidence data of lip and oral cancer in China from 1990 to 2021 were obtained through the Global Health Data Exchange (GHDx) database, including incidence rates by gender and age-standardized incidence rates. Joinpoint regression was used to analyze the trend of lip and oral cancer incidence rate. Age-period-cohort (APC) model was adopted to estimate the effects of Age, Period and Cohort on the incidence of lip and oral cancer in China.Results:The crude and age-standardized incidence rates of lip and oral cancer in China from 1990 to 2021 showed an increasing trend, and the crude incidence rates of the whole population, male and female in the year 2021 increased by 216.8%, 266.7% and 130.4%, respectively, compared with 1990. The age effect, period effect and birth cohort effect of the incidence risk of lip and oral cancer in China were statistically significant (all P<0.05). People of older the age and the more recent the birth cohort, the higher the incidence risk. Conclusions:The incidence of lip and oral cancer is on the rise in China from 1990 to 2021. Age, period and birth cohort are all the factors affecting the incidence of lip and oral cancer.

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
4.Current status and influencing factors of kinesiophobia in pregnant women with pregnancy-related low back pain

Objective:To explore the current status of kinesiophobia in pregnant women with pregnancy-related low back pain (PLBP) and its influencing factors, so as to inform the clinical development of intervention programs.Methods:Convenience sampling was used to select 270 pregnant women with PLBP who underwent routine obstetric examination from August to November 2023 at the Obstetrics Clinic of Affiliated Obstetrics and Gynecology Hospital of Jiangnan University. Pregnant women were surveyed using the General Information Questionnaire, Tampa Scale for Kinesio-Phobia, Falls Efficacy Scale-International, Pregnancy Exercise Self-efficacy Scale, and Low Back Pain Knowledge Questionnaire. Multiple linear regression was used to explore the factors influencing kinesiophobia in pregnant women with PLBP.Results:In 270 pregnant women with PLBP, the kinesiophobia score was (47.78±8.80) and the prevalence of kinesiophobia was 89.3% (241/270). Multiple linear regression analysis showed that lower extremity edema, pain, fear of falling, exercise self-efficacy, and knowledge of low back pain were the factors influencing kinesiophobia in pregnant women with PLBP ( P<0.05) . Conclusions:Pregnant women have a high level of kinesiophobia, and there are many influencing factors. Healthcare professionals should consider a variety of factors to develop an intervention program to reduce kinesiophobia and increase activity levels in pregnant women with PLBP.

Objective To investigate the infection status and changing trend of hepatitis C virus(HCV)infection in hospitalized patients in medical institutions,and provide reference for formulating HCV infection prevention and control strategies.Methods HCV infection surveillance results from cross-sectional survey data reported to China Healthcare-associated Infection(HAI)Surveillance System in 2020 were summarized and analyzed,HCV positive was serum anti-HCV positive or HCV RNA positive,survey result was compared with the survey results from 2003.Results In 2020,1 071 368 inpatients in 1 573 hospitals were surveyed,738 535 of whom underwent HCV test,4 014 patients were infected with HCV,with a detection rate of 68.93%and a HCV positive rate of 0.54%.The positive rate of HCV in male and female patients were 0.60%and 0.48%,respectively,with a statistically sig-nificant difference(x2=47.18,P<0.001).The HCV positive rate in the 50-<60 age group was the highest(0.76%),followed by the 40-<50 age group(0.71%).Difference among all age groups was statistically signifi-cant(x2=696.74,P<0.001).In 2003,91 113 inpatients were surveyed.35 145 of whom underwent HCV test,resulting in a detection rate of 38.57%;775 patients were infected with HCV,with a positive rate of 2.21%.In 2020,HCV positive rates in hospitals of different scales were 0.46%-0.63%,with the highest in hospital with bed numbers ranging 600-899.Patients'HCV positive rates in hospitals of different scales was statistically signifi-cant(X2=35.34,P<0.001).In 2020,12 provinces/municipalities had over 10 000 patients underwent HCV-rela-ted test,and HCV positive rates ranged 0.19%-0.81%,with the highest rate from Hainan Province.HCV posi-tive rates in different departments were 0.06%-0.82%,with the lowest positive rate in the department of pedia-trics and the highest in the department of internal medicine.In 2003 and 2020,HCV positive rates in the depart-ment of infectious diseases were the highest,being 7.95%and 3.48%,respectively.Followed by departments of orthopedics(7.72%),gastroenterology(3.77%),nephrology(3.57%)and general intensive care unit(ICU,3.10%)in 2003,as well as departments of gastroenterology(1.35%),nephrology(1.18%),endocrinology(0.91%),and general intensive care unit(ICU,0.79%)in 2020.Conclusion Compared with 2003,HCV positive rate decreased significantly in 2020.HCV infected patients were mainly from the department of infectious diseases,followed by departments of gastroenterology,nephrology and general ICU.HCV infection positive rate varies with gender,age,and region.
