1.Observation on the therapeutic effect of microsurgical vascular decompression under complete neuroendoscopy combined with nerve combing surgery in the treatment of trigeminal neuralgia and its impact on prognosis

Objective To explore the therapeutic effect of microsurgical vascular decompression(MVD)under complete neuroendoscopy combined with nerve combing surgery in the treatment of primary trigeminal neuralgia(TN),and its impacts on prognosis.Methods 108 primary TN patients from November 2021 to November 2023 were retrospectively selected and grouped into combined group and control group based on the surgical plan,with 54 cases in each group.The control group received MVD treatment under the microscope,while the combined group received complete neuroendoscopic MVD combined with nerve combing surgery.The total effective rate,pain degree,stress indicators,quality of life,and complications were compared between the two groups.Results The total effective rate of the combined group(96.30%)was greatly higher than that of the control group(83.33%),the difference was statistically significant(P<0.05).The visual analogue scale(VAS)scores of both groups after surgery were significantly lower than those before surgery,and the combined group was lower than the control group,the differences were statistically significant(P<0.05).After surgery,the levels of norepinephrine(NE),superoxide dismutase(SOD),and cortisol(Cor)increased in both groups compared with those before surgery,but the combined group was greatly lower than that of the control group,the differences were statistically significant(P<0.05).The generic quality of life inventory-74(GQOL-74)scores in both groups after surgery were significantly higher than those before surgery,and the combined group was higher than the control group,the differences were statistically significant(P<0.05).There was no difference of the total complication rate between the two groups(P>0.05).Conclusion MVD under complete neuroendoscopy combined with nerve combing surgery in the treatment of primary TN,can relieve pain,reduce stress response and improve the quality of life of patients.It is worthy of clinical promotion and application.
2.Association between Polymorphism of BPIFA1 Gene rs750064,rs1078761 Loci and Susceptibility to Asthma in Pediatric Patients with Allergic Rhinitis

Objective To explore the association between rs750064,rs1078761 loci polymorphisms of BPI fold containing family A member 1(BPIFA1)gene and susceptibility to asthma in pediatric patients with allergic rhinitis(AR).Methods A total of 136 cases of AR admitted to Heping Hospital Affiliated to Changzhi Medical College from March 2021 to May 2024,aged 2~10 years old,were selected as the observation group,including 70 cases of asthma with AR,the control group was 66 cases with AR alone.The laboratory indicators of the children were collected,the rs750064 and rs1078761 loci of BPIFA1 gene were genotyped by MassArray System.The differences in genotype distribution and allele frequency between the two groups were compared.The correlation between rs750064 and rs1078761 loci of BPIFA1 gene and susceptibility to asthma in children with AR was analyzed by unconditional Logistic regression.Results Compared with the control group,interleukin-6(IL-6),fractional exhaled nitric oxide(FeNO)and immunoglobulin E(IgE)levels in the observation group were significantly increased,while forced vital capacity(FVC)levels were significantly decreased,with statistical significance(t=-22.648~4.879,all P<0.05).The genotypes of rs750064 and rs1078761 of BPIFA1 gene in control group and observation group were in line with Hardy-Weinberg equilibrium law(χ2=1.492~5.549,all P>0.05),indicating population representation.The distribution frequencies of allele T and genotype TT at rs750064 of BPIFA1 gene and allele A and genotype AA at rs1078761 locus in the observation group were higher than those in the control group,and the differences were statistically significant(χ2=8.251~10.273,all P<0.05).The results of unconditional Logistic regression showed that in the co-dominant model(CC vs CT)of rs750064,the risk of CC genotype carriers was lower than that of TT genotype carriers(OR=0.537,95%CI:0.276~1.804).In the dominant model(CT+CC vs TT)and the recessive model(CT+TT vs CC),rs750064 polymorphism was associated with the risk of AR with asthma(all P<0.05).In the co-dominant model(GG vs AG),the risk of rs1078761 in GG genotype carriers was lower than that in AA genotype carriers(OR=0.498,95%CI:0.176~1.205).Under the dominant model(AG+GG vs AA)and recessive model(AG+AA vs GG),rs1078761 polymorphism was associated with the risk of AR combined asthma with statistical significance(all P<0.05).After adjusting various factors,rs750064 and rs1078761 were associated with the risk of AR combined with asthma under the three genetic models,and the differences were statistically significant(all P<0.05).Conclusion The polymorphism of BPIFA1 gene is significantly related to the susceptibility of asthma in children with AR.TT carriers of rs750064 locus and AA carriers of rs1078761 locus are more likely to develop asthma in children with AR.

5.Analysis of 23 Cases of Intrauterine Device Removal After Radiotherapy for Cervical Cancer

Objective To investigate the experience of intrauterine device(IUD)removal in patients with cervical cancer after radiotherapy.Methods A total of 23 patients with cervical cancer after radiotherapy underwent abdominal ultrasound guided hysteroscopic removal of IUD in our department from January 2020 to December 2022.For vaginal and/or cervical adhesions,blunt separation of adhesions was performed by using hysteroscope head or curved forceps under abdominal ultrasound guidance.If it was difficult to separate the adhesions for hysteroscope head entering the uterine cavity,a probe was inserted into the uterine cavity under ultrasound guidance,and the cervical canal was gradually expanded to 6-caliber dilation rod.Then the hysteroscope was inserted again into the uterine cavity.For obvious cervical atrophy that was tough in which cervical forceps could not be used to clamp the cervix,a 1-0 absorbable suture line was used to suture the anterior and/or posterior lips of the atrophied cervix at the top of the vagina,with an assistant firmly pulling and fixing the cervix.Results There were 4 cases of vaginal partial adhesions and cervical contracture,10 cases of cervical contracture,and remaining 9 cases having no vaginal adhesions and cervical contracture.Under hysteroscopy,there were 3 cases of cervical adhesions,2 cases of endometrial polyps,1 case of submucosal uterine fibroids,2 cases of uterine abscess,2 cases of incarcerated IUD,and remaining 13 cases having normal uterine cavity morphology.All the 23 cases of IUD were successfully removed by using abdominal ultrasound guided hysteroscopy(circular shaped in 12 cases,uterine shaped in 6 cases,V-shaped in 2 cases,Y-shaped in 1 case,T-shaped in 1 case,and umbrella shaped in 1 case).The surgical time was(19.2±10.9)min,and there were no complications such as false passage formation,uterine perforation,organ damage,massive vaginal bleeding,transurethral resection of the prostate syndrome,infection,embolism,or shock.The 23 cases were followed up for 2-24 months postoperatively,with a median of 12 months.One case continued concurrent radiotherapy and chemotherapy,3 cases continued post-loading radiotherapy,1 case continued chemotherapy,and 2 cases received targeted treatment(distant metastasis).The remaining 16 cases recovered well in regular reviews without complications such as abdominal pain,fever,or vaginal bleeding.Conclusions If the size and location of the cancer lesion do not affect the removal of IUD after radiotherapy for cervical cancer,it should be removed as soon as possible.The application of abdominal ultrasound guided hysteroscopy in IUD removal in patients with cervical cancer after radiotherapy is safe and feasible to a certain extent.
6.Study on the Extraction Technology of Fructus Gleditisae Abnormalis and GC-MS Analysis of Extracted Com- ponents

OBJECTIVE:To establish and optimize a extraction method of Fructus Gleditsiae Abnormallis ,and to analyze and identify chemical components of the extract simultaneously. METHODS :Fructus Gleditsiae Abnormallis was extracted with CO 2 supercritical fluid extraction (SFE)method. Based on single factor tests ,using extraction yield as index ,extraction temperature , extraction pressure and extraction time as investigation factors ,SFE technology was optimized with orthogonal test ,and validation test was performed. Chemical components in the extract were identified by GC-MS. Relative percentage of each component was calculated with area normalization method. RESULTS :The optimal SFE extraction technology of Fructus Gleditsiae Abnormallis was extraction temperature of 60 ℃,extraction pressure of 300 MPa and pression time of 15 min. Average extraction of 3 times of validation tests was 1.73%(RSD=1.78%,n=3). The 48 components in the extracts of Fructus Gleditsiae Abnormallis were identified,which accounted for 98.31% of the total amount of the extracts. The extracts of Fructus Gleditisae Abnormalis mainly included organic acids ,accounting for 36.99%,followed by alkaloids ,accounting for 12.59% in total. Main components were palmitic acid (16.62%),oleic acid (14.12%),N-aminotetrahydropyrrole(9.79%),2,6-dimethyloctane-1,7-dien-3-ol(5.95%), tetrahydropyran(3.83%),vanillin(3.39%),etc. CONCLUSIONS :SFE method of Fructus Gleditisae Abnormalis is established successfully,and the extract is mainly organic acids.

Benign infantile convulsions associated with mild gastroenteritis (BICE),known as convulsions with mild gastroenteritis,is a kind of non-febrile convulsions disease associated with acute infantile gastroenteritis.Patients with BICE often have mild gastroenteritis and tonic-clonic seizure manifestation,but no apparent disturbance of water,electrolyte and acid base was observed.BICE,the main etiological agent of which is Rotavirus,is a kind of self-limited disease with favorable prognosis,and no requiring of continuous anticonvulsant treatment.
