1.Analysis of preferences and demands of learners in nursing massive open online courses based on text mining

Objective:To deeply explore the thematic needs and characteristics of learners regarding course elements based on the review texts of nursing massive open online courses (MOOC), providing a reference for achieving effective alignment between digital nursing education content and learner needs.Methods:Data were collected from the review texts of 112 nursing courses on the Chinese University MOOC platform using a web crawler script written with Python′s Requests library. The collection period spanned from the course launch dates to December 31, 2023. Sentiment analysis and high-frequency words analysis were conducted using Chinese text Nature language processing library, and core themes of learners′ positive and negative reviews were extracted using the latent dirichlet allocation.Results:A corpus of 18 184 nursing MOOC review texts was constructed, with positive sentiment reviews accounting for 89.30% (16 238/18 184) and negative sentiment reviews making up 10.70% (1 946/18 184). Word frequency analysis revealed that most nursing MOOC serve as carriers for blended online and offline teaching models, with students being the primary target audience, though social participants were also involved. The reviews effectively mirrored real-world clinical nursing scenarios. The need of learners was categorized into three major themes: content design and assessment, course resources and teaching strategies, and software applications and platform functionality.Conclusions:This study, leveraging text mining technology, thoroughly investigated the three thematic characteristics of nursing MOOC needs of online learners and proposed targeted optimization recommendations. Future research could incorporate other online teaching platforms and comprehensively construct a sentiment lexicon for nursing online course reviews using big data modeling and machine learning algorithms. These would enable a holistic analysis of the digital nursing education landscape, allowing for precise improvements to address existing shortcomings.
2.Study on the brain functional network and structural-functional coupling in children with drug-resistant epilepsy

Objective:To investigate the changes in brain functional network and structural-functional network coupling in children with drug-resistant epilepsy (DRE), and to analyze their correlation with cognitive function, disease duration, and age of onset.Methods:This study was a cross-sectional study. Clinical and imaging data of 19 children with DRE who received consultation and treatment at the Affiliated Hospital of Zunyi Medical University from August 2021 to August 2023 (DRE group) were prospectively included. Another 27 age-and sex-matched healthy children were collected as the healthy control group. All subjects had 3D-T 1WI, T 2 fluid-attenuated inversion recovery, diffusion tensor imaging (DTI), resting-state functional magnetic resonance imaging (rs-fMRI) scans and Wechsler Intelligence Scale assessments. Independent sample t-test and Mann-Whitney U test were used to analyze the global and local topological attributes, as well as the structural-functional coupling (SFC) values at the whole brain and modular levels in two groups. Correlations between abnormal resting state brain functional network indicators and the Wechsler Intelligence Scale score [verbal comprehension index (VCI), perceptual reasoning index (PRI), working memory index (WMI), processing speed index (PSI), full scale intelligence quotient (FSIQ)], disease duration and age of onset was evaluated using a Spearman or Pearson correlation analysis. Results:Compared to the healthy control group, DRE group exhibited decreased VCI, PRI, WMI, PSI, FSIQ and the differences were all statistically significant (all P<0.05). Both brain functional networks had small world attributes. There was a statistically significant difference in the area under the curve of sparsity of degree centrality (DC) in the left pallidum between the DRE group and healthy control group (2.998±0.942, 4.992±1.945, t=-4.07, FDR corrected P<0.05). Compared with the control group, the DRE group had decreased SFC within the limbic network (LN) ( P<0.05), increased SFC within the sensorimotor (SMN) ( P<0.05), decreased SFC between the default mode network-LN ( P<0.05), and increased SFC between the SMN-attentional network (AN) ( P<0.05). There was no statistically significant difference in SFC at the whole brain level between the two groups. Correlation analysis indicated that DC in left pallidum in DRE group negatively correlated with the PSI ( r=-0.537, P=0.018), and SFC between the SMN and AN demonstrated a negative correlation with age of onset ( r=-0.537, P=0.018). Conclusion:The altered DC in left pallidum may be related to cognitive impairment in children with DRE, providing biomarker information for the study of neural mechanisms in children with DRE.

5.Effects of Zhoufei Pingchuan Capsules on Th17/Treg Balance and Related Inflammatory Factors in Peripheral Blood of Patients with Stable Chronic Obstructive Pulmonary Disease with Lung-kidney Qi Deficiency Syndrome

Objective To investigate the effects of Zhoufei Pingchuan Capsules on the balance of peripheral blood helper T lymphocyte 17 cell/regulatory T lymphocyte cell(Th17/Treg)and related inflammatory factors in peripheral blood of patients with stable chronic obstructive pulmonary disease(COPD)with lung-kidney qi deficiency syndrome.Methods Totally 40 COPD patients were randomly divided into the study group and the control group,with 20 cases in each group.Another 20 cases were in the healthy group.The control group was given tiotropium bromide powder inhalation,18 μg/time,1 time/d,inhalation;on the basis of the control group,the study group was given Zhoufei Pingchuan Capsules,3 pills/time,3 times/d,orally.All patients were treated for 8 weeks.The healthy group was not given any intervention.Forced expiratory volume in one second(FEV1),FEV1/forced vital capacity(FEV1/FVC),maximum mid-expiratory flow(MMEF),carbon monoxide diffusing capacity/alveolar ventilation(DLCO/VA),arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),COPD assessment test(CAT)score,Th17/Treg ratio,cytokines interleukin(IL)-17,IL-22,IL-10,and transforming growth factor-β1(TGF-β1)were compared before and after treatment.Results Compared with before treatment,the lung function indexes(FEV1,FEV1/FVC,MMEF,DLCO/VA),blood gas indexes(PaO2,PaCO2)and CAT score in the study group after treatment were significantly improved(P<0.05).After treatment,the mean values of MMEF,DLCO/VA,PaCO2 and CAT score in the study group were better than those in the control group(P<0.05).Compared with before treatment,the levels of Th17,IL-17 and IL-22 in the study group were significantly lower,and the levels of Treg,IL-10 and TGF-β1 were significantly higher(P<0.05).After treatment,there were significant differences in Th17,Treg,IL-17,IL-22,IL-10 and TGF-β1 among the three groups(P<0.01).Further pairwise comparison showed that Th17 ranked in the order of high and low was control group>study group>healthy group,Treg in the order of high and low was healthy group>study group>control group,the levels of IL-17 and IL-22 in the order of high and low were control group>study group>healthy group,and the levels of IL-10 and TGF-β1 in the order of high and low were healthy group>study group>control group,with statistical significance(P<0.05).Conclusion Zhoufei Pingchuan Capsules can improve the lung function,arterial blood gas and symptom score of patients with lung-kidney qi deficiency syndrome in stable stage of COPD.Its mechanism may be related to regulating the balance of Th17/Treg,down-regulating the levels of Th17,IL-17 and IL-22,and up-regulating the levels of Treg,IL-10 and TGF-β1,in order to reduce airway inflammation and regulate immune homeostasis.
6.Common incidence of catheter-related venous thrombosis between midline catheter and peripherally inserted central catheter:a Meta-analysis

Objective To compare the incidence of catheter-related venous thrombosis between midline catheters and peripherally inserted central catheters.Methods Randomized controlled trials and cohort studies on the incidence of venous thrombosis associated with midline catheters and PICC catheters were searched from CNKI,Wanfang database,VIP database,Web of Science,PubMed,Embase and Cochrane Library from inception to December 31,2022.Review Manager 5.4 software and Stata 14.0 software were used to analyse and describe the outcome indicators.Results A total of 16 studies were included,including 12 cohort studies and 4 randomized controlled studies,with 21853 subjects.The results of the Meta-analysis showed that the incidence of catheter-related venous thrombosis per thousand catheter days of midline catheters was statistically significant compared with PICC[RR=2.74,95%CI(1.21,6.21),P=0.016].There was no significant difference in the incidence of catheter-related venous thrombosis compared with PICC[RR=0.85,95%CI=(0.70,1.03),P=0.101].In the subgroups,the incidence of superficial vein thrombosis in the midline catheter was significantly different from that in the PICC[RR=2.36,95%C/=(1.56,3.58),P<0.001].Conclusion The current evidence shows that the incidence rate of catheter-related venous thrombosis per thousand catheter days and superficial vein thrombosis was higher for midline catheters than PICCs.Therefore,in clinical practice,vascular access devices should be selected reasonably,and the occurrence and development of catheter-related superficial venous thrombosis should be paid attention to,and clinical screening should be effectively carried out on the basis of a full evaluation.
7.Establishment of characteristic chromatogram and determination method of four components in Shangkeling spray

OBJECTIVE To establish a method for qualitative and quantitative analysis of Shangkeling spray ,which can be a certain foundation for the overall quality evaluation of Shangkeling spray . METHODS Eleven batches of Shangkeling spray were determined by high performance liquid chromatography (HPLC).The separation was performed on Ultimate ® XB-C18 column with mobile phase consisted of acetonitrile -0.1% phosphoric acid (gradient elution )at the flow rate of 1.0 mL/min. The detection wavelength was set at 210 nm,and column temperature was 35 ℃.Similarity Evaluation Software of Chromatographic Fingerprint of Traditional Chinese Medicine (2012 edition)was used for the establishment of HPLC characteristic chromatogram and similarity analysis;the chromatographic peaks were identified by comparing with the chromatogram of the reference substance . The contents of matrine ,oxymatrine,scopoletin and isoazinopyridine were determined by HPLC .RESULTS Totally 18 common characteristic peaks were demarcated for 11 batches of samples ,4 of them were identified ,i.e. peak 2(matrine),peak 3(oxymatrine),peak 6 (scopoletin),peak 7(isoazinopyridine). The similarity between the characteristic chromatogram of 11 batches of samples and the control characteristic chromatogram R was ≥0.990. The results of content determination methodology conformed to the relevant requirements. The contents of matrine ,oxymatrine,scopoletin and isoazinopyridine in 11 batches of Shangkeling spray were 14.48-44.86,32.53-69.76,11.28-20.96 and 10.36-22.49 μg/mL,respectively. CONCLUSIONS HPLC characteristic chroma -togram and quantitative analysis method of 4 indicator components are successfully established in this study ,which can be used to evaluate the quality of this preparation .
8.Frequency-Resolved Connectome Hubs and Their Test-Retest Reliability in the Resting Human Brain.

Functional hubs with disproportionately extensive connectivities play a crucial role in global information integration in human brain networks. However, most resting-state functional magnetic resonance imaging (R-fMRI) studies have identified functional hubs by examining spontaneous fluctuations of the blood oxygen level-dependent signal within a typical low-frequency band (e.g., 0.01-0.08 Hz or 0.01-0.1 Hz). Little is known about how the spatial distributions of functional hubs depend on frequency bands of interest. Here, we used repeatedly measured R-fMRI data from 53 healthy young adults and a degree centrality analysis to identify voxelwise frequency-resolved functional hubs and further examined their test-retest reliability across two sessions. We showed that a wide-range frequency band (0.01-0.24 Hz) accessible with a typical sampling rate (fsample = 0.5 Hz) could be classified into three frequency bands with distinct patterns, namely, low-frequency (LF, 0.01-0.06 Hz), middle-frequency (MF, 0.06-0.16 Hz), and high-frequency (HF, 0.16-0.24 Hz) bands. The functional hubs were mainly located in the medial and lateral frontal and parietal cortices in the LF band, and in the medial prefrontal cortex, superior temporal gyrus, parahippocampal gyrus, amygdala, and several cerebellar regions in the MF and HF bands. These hub regions exhibited fair to good test-retest reliability, regardless of the frequency band. The presence of the three frequency bands was well replicated using an independent R-fMRI dataset from 45 healthy young adults. Our findings demonstrate reliable frequency-resolved functional connectivity hubs in three categories, thus providing insights into the frequency-specific connectome organization in healthy and disordered brains.

9.Circulation miR-193 cluster serves as a promising biomarker for diagnosis of breast cancer

Objective:
To evaluate circulating miR-193 cluster as a biomarker for diagnosis of breast cancer (BC).
Methods:
A total of 130 BC patients and 45 healthy controls (HCs) were enrolled. The level of miR-193 cluster was analyzed by using GEO database. RT-qPCR was used to evaluate the level of miR-193 cluster in serum of participants. Receivers operating characteristic (ROC) curve analysis was manipulated to investigate their diagnostic value for BC.
Results:
In the screening stage, the results of GEO database indicated the expression of miR-193a-5p and miR-193b were decreased in the serum of BC patients with prior diagnostic value, and the significant decreased expressions of miR-193 cluster members, miR-193a-3p, -5p and miR-193b-3p, were observed in serum of BC patients compared with those in healthy controls by the determination of RT-qPCR(all P <0.01). In the validation stage, the AUCROC of miR-193a-5p and miR-193b-5p were 0.888 (95% CI :0.808-0.969) and 0.954 (95% CI :0.902-1.000), respectively. In addition, the combined AUCROC of these two miRNAs was 0.982 (95% CI :0.966-0.999).
Conclusion
miR-193a-5p and miR-193b-5p in serum could be served as diagnostic biomarker for screening of breast cancer, and the combined detections of the two miRNAs may exhibit more diagnostic efficiency and good clinical application potential.
10. Effect of diabetic management modes on diabetic nephropathy: a prospective study

Objective:
To explore the effect of "diabetes specialists-community general practitioners-community nurse co-management mode" and "diabetes specialist management mode" on diabetic nephropathy (DN) in primary medical institutions.
Methods:
Patients with type 2 diabetes admitted to Quanzijie Health Clinic of Jimusar County of Xinjiang Uygur Autonomous Region from October 2017 to March 2018 were enrolled. The Patients were divided into co-management group or specialist management group according to their administrative villages. The treatment plans of the two groups were formulated with reference to the current guidelines. The subjects of the co-management group were jointly managed by a fixed team composed of diabetes specialists from Jimusar Traditional Chinese Medicine Hospital, community general practitioners and community nurses from Quanzijie Health Clinic, and required to attend diabetes education courses every month. The diabetes specialist of Jimusar Traditional Chinese Medicine Hospital was responsible for the formulation and management of the treatment plan of the research object. Follow-up was fulfilled once every 4 weeks for 24 weeks in two groups. Before and after intervention, blood glucose, blood pressure, urinary albumin/creatinine ratio (UACR), estimated glomerular filtration rate (eGFR) as well as the utilization rate of angiotensin converting enzyme inhibitors/angiotensin Ⅱ receptor blocker (ACEI/ARB) were collected.
Results:
A total of 115 patients accomplished this study with 54 patients in co-management group and 61 patients in specialist management group. After 24 weeks of intervention, fasting glucose level, postprandial glucose level 2 hours after breakfast, glycosylated hemoglobin (HbA1c), Log UACR in co-management group and specialists management group were significantly decreased compared with baseline [fasting glucose level (mmol/L): 8.06±1.92 vs. 9.16±2.83, 8.21±2.10 vs. 9.06±1.89; postprandial glucose level 2 hours after breakfast (mmol/L): 12.26±3.78 vs. 14.11±5.28, 12.47±3.63 vs. 14.00±3.88; HbA1c: 0.074±0.014 vs. 0.082±0.023, 0.076±0.014 vs. 0.081±0.016; Log UACR (mg/g): 1.63±1.56 vs. 2.25±1.44, 1.84±1.65 vs. 2.43±1.56, all
