1.Progress in the application of minimal residual lesion detection in the precision diagnosis and treatment of esophageal cancer

The detection of minimal residual lesions represented by circulating tumor DNA can detect residual cancer lesions that cannot be displayed by traditional imaging in advance. The positive status and dynamic changes are closely related to the prognosis evaluation of various solid tumors, the effectiveness of neoadjuvant therapy, the prediction of postoperative recurrence risk, and the precise implementation of adjuvant therapy. This article summarizes the advantages and challenges faced by the application of circulating tumor DNA testing in individualized multidisciplinary diagnosis and treatment of esophageal cancer, in order to better carry out precise diagnosis and treatment of esophageal cancer under the guidance of minimal residual lesion detection.
2.Application of magnetic recanalization technology in treatment of complex refractory benign biliary stricture

Benign biliary strictures are frequently encountered after liver transplantation or as a result of iatrogenic injury, among others. Complex and refractory benign biliary obstructions refer to biliary strictures that persist or treatment fails despite multiple endoscopic therapies. Traditional endoscopic approaches demonstrate suboptimal therapeutic effects in treating such patients, with a high recurrence rate. Moreover, surgical operations are highly invasive, and the outcomes are also unsatisfactory. The magnetic dilation technology establishes an external drainage sinus tract via percutaneous transhepatic biliary drainage and positions a magnet above the obstructed biliary tract; through endoscopic retrograde cholangiopancreatography, another magnet is placed beneath the obstructed biliary tract. The two magnets mutually attract each other, exerting pressure on the scar tissue in the middle of the magnets, causing it to gradually become ischemic, necrotic, and detached, and subsequently discharged along with the magnets. The biliary epithelial cells surrounding the magnets gradually repair and heal, ultimately forming a new patent anastomosis. The magnetic dilation technology, through slow compression, realizes the concurrent process of “removing” scars and healing, avoiding severe local inflammatory responses and excessive tissue hyperplasia. Consequently, the magnetic dilation technology possesses advantages such as minimally invasiveness and reliable effects, gradually emerging as an effective modality for treating complex and refractory benign biliary stricture cases.
3.A neural circuit from paraventricular hypothalamic nucleus oxytocin neurons to trigeminal nucleus caudalis GABAergic neurons modulates pain sensitization in a mouse model of chronic migraine.

OBJECTIVES:
To investigate the role of a neural pathway from oxytocin (OXT) neurons in the paraventricular hypothalamic nucleus (PVN) to γ-aminobutyric acid (GABA) neurons (GABAergic neurons) in the trigeminal nucleus caudalis (TNC) in regulating pain sensitization in a mouse model of chronic migraine and to explore the underlying mechanisms.
METHODS:
A chronic migraine mouse model was established by intraperitoneal injection of nitroglycerin (NTG, 1 mg/mL, 10 mg/kg) on days 1, 3, 5, 7, and 9. The study consisted of four parts: PartⅠ: 24 male wild-type C57BL/6J mice were divided into four groups (n=6 in each), receiving single or repeated injection of NTG or saline, respectively. Immunofluorescence was used to detect c-Fos and OXT expression in the PVN. Part Ⅱ: 6 male OXT-Cre transgenic C57BL/6J mice were used for anterograde monosynaptic tracing combined with RNAscope and immunofluorescence to identify neural projections from PVN OXT neurons to TNC GABAergic neurons. Part Ⅲ: 30 male OXT-Cre transgenic C57BL/6J mice were bilaterally injected Cre-dependent chemogenetic activation virus into the PVN. These mice were randomly divided into five groups, with six mice in each group. Mice in the clozapine N-oxide (CNO) group and the control group were intra-peritoneally injected with 0.1 mg/mL of CNO solution (1 mg/kg) and the same volume of isotonic normal saline, respectively. 3 hours after the injection, the brain tissues were harvest and c-Fos immunofluorescence staining was performed to verify the efficiency of chemogenetic activation virus. Mice in the model control group and the CNO activated model group were subjected to chronic migraine modeling, with bilateral TNC injection of isotonic normal saline and CNO, respectively, on day 10. The mice in the negative control group were bilaterally intra-TNC injected with isotonic normal saline. After 30 minutes, the Von-Frey filament and acetone tests were used to assess the mechanical pain threshold and cold pain response time in the periorbital region of the mice in these three groups. Part Ⅳ: 24 male OXT-Cre transgenic C57BL/6J mice were bilaterally injected with the Cre-dependent chemogenetic activation virus into the PVN. These mice were randomly divided into four groups, with six mice in each group. Mice in the model control group, the CNO activated model group and the atosiban group were subjected to chronic migraine modeling. On day 10, mice in the negative control group and the model control group were intraperitoneally injected with isotonic normal saline, while mice in the CNO activated model group and the atosiban group were intraperitoneally injected with CNO. After 15 minutes, mice in the atosiban group were bilaterally intra-TNC injected with atosiban, while mice in other three groups were bilaterally intra-TNC injected with isotonic normal saline containing 1% dimethyl sulfoxide. After 15 minutes, the Von-Frey filament and acetone tests were used to assess the mechanical pain threshold and cold pain response time in the periorbital region of the mice. The GABA content in the bilateral TNC was detected by high-performance liquid chromatography (HPLC).
RESULTS:
Mice with chronic migraine models exhibited reduced periorbital mechanical pain thresholds and increased periorbital cold pain reaction time, accompanied by an increase in both the number of c-Fos+ neurons and the percentage of c-Fos+ OXT neurons in the PVN (all P<0.05). The anterograde tracing virus and RNAscope combined with immunofluorescence staining showed that PVN OXT neurons projected to TNC GABAergic neurons. Immuno-fluorescence staining demonstrated that compared with the control group, the percentage of c-Fos+ OXT neurons in the PVN of CNO group increased (P<0.05). In bilateral intra-TNC drug administration experiments, compared with the model control group, the periorbital mechanical pain threshold increased, and the periorbital cold pain reaction time decreased in the CNO activated model group (both P<0.05). In intraperitoneal drug administration experiments, compared with the CNO activate model group, the periorbital mechanical pain threshold decreased, and the periorbital cold pain reaction time increased in the atosiban group (both P<0.05). HPLC analysis showed that, compared with the negative control group, the model control group and the atosiban group, GABA level of TNC in the CNO activated model group increased (all P<0.05).
CONCLUSIONS
PVN OXT neurons exert a descending facilitatory effect on GABAergic neurons in the TNC via OXT release, thereby ameliorating pain sensitization in chronic migraine.

4.Correlation between blood lipid, body mass index and hyperuricemia in the elderly

Objective:To investigate the association of blood lipids and body mass index (BMI) with hyperuricemia (HUA) in the elderly.Methods:It was a cross-sectional study. A total of 114 391 elderly individuals received health examinations at primary healthcare institutions in Wujin District from January to December in 2022. The health examination included questionnaire survey, physical examination and laboratory examination. The multivariate logistic regression and restricted cubic spline (RCS) plots were used to analyze the association and dose-response relationship of blood lipid and BMI with HUA. The mediating effect model was used to explore the mediation effect of BMI on the association between blood lipid and HUA.Results:Among the 112 415 subjects, 18 506 (16.46%) were checked with HUA. After adjusting for relevant confounders, total cholesterol (TC) ( OR=1.20, 95% CI: 1.16-1.23), triglyceride (TG) ( OR=1.46, 95% CI: 1.44-1.49), high density lipoprotein cholesterol (HDL-C) ( OR=0.74, 95% CI: 0.73-0.76), low density lipoprotein cholesterol (LDL-C) ( OR=1.14, 95% CI: 1.12-1.15) and BMI ( OR=1.42, 95% CI: 1.39-1.44) were all associated with HUA (all P0.05). The RCS analysis revealed that TG, HDL-C, and LDL-C each exhibited a nonlinear dose-response relationship with HUA, the inflection points was 3.00 mmol/L, 1.57 mmol/L and 2.50 mmol/L, respectively (all P-nonlinear0.001). The results of interaction showed that there were additive interaction between high TC( S=1.27 , 95% CI: 1.17-1.37), high TG( S=1.32 , 95% CI: 1.25-1.40), high LDL-C( S=1.23 , 95% CI: 1.14-1.34) and overweight/obesity with HUA (all P0.05). The results of mediation effect analysis showed that the mediation effect of BMI on the association between blood lipids (HDL-C, LDL-C, TG and TC) and HUA, from high to low, were as follows: 22.5% (95% CI: 20.8%-24.2%), 13.9% (95% CI: 12.0%-16.2%), 13.5% (95% CI: 12.7%-14.4%) and-3.9% (95% CI:-6.6%--1.8%). Conclusion:The blood lipid levels and BMI are positively correlated with HUA in the elderly.
5.MALDI-TOF MS combined with machine learning for rapid identification of extended-spectrum β-lactamase-producing Escherichia coli

Objective:This study aims to develop a rapid identification technique for various genotypes of extended-spectrum β-lactamase (ESBL) producing Escherichia coli using matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS) in conjunction with machine learning algorithms. Methods:A total of 158 Escherichia coli strains were isolated from the clinical laboratory of the First Hospital of Jilin University from August 2018 to December 2022. Polymerase chain reaction (PCR) was employed to detect the CTX-M-1, CTX-M-8, CTX-M-9, and SHV genes. Mass spectral data of the bacterial strains were acquired by MALDI-TOF MS with a cooperative matrix of (E)-propyl α-cyano-4-hydroxycinnamate (CHCA-C3). Models based on random forest (RF), logistic regression (LR), and support vector machine (SVM) algorithms were constructed. The performance of the constructed models was evaluated using metrics including accuracy, sensitivity, specificity, and the area under the receiver operating characteristic curve (AUC). Mass spectral peaks exhibiting sensitivity and specificity exceeding 80% in the models were designated as characteristic peaks. To validate the efficacy of the cooperative matrix of CHCA-C3, clinical isolates of ESBL-producing Escherichia coli were analyzed by MALDI-TOF MS using the conventional CHCA matrix for comparative purposes. Results:Among the 158 strains of Escherichia coli, 91 strains produced ESBL, all of which were CTX-M genotype. The AUC values for the respective models were as follows: CTX-M-1 genotype exhibited AUC values of 0.98 for LR, 1.00 for RF, and 0.73 for SVM; CTX-M-9 genotype exhibited AUC values of 0.93 for LR, 0.99 for RF, and 0.76 for SVM; for CTX-M-8, all models achieved an AUC of 1.00, indicating excellent classification performance with respect to accuracy, specificity, and sensitivity. The characteristic mass spectral peaks associated with each genotype included: CTX-M-1 genotype at m/z 6 390; CTX-M-8 genotype at m/z 5 224, m/z 5 393, and m/z 9 021; CTX-M-9 genotype at m/z 5 161 and m/z 5 273. In the MALDI-TOF MS analysis conducted with the conventional CHCA matrix, the characteristic peak at m/z 9 021 for CTX-M-8 was the only one detected, with the characteristic peaks for CTX-M-1 and CTX-M-9 remaining undetected. Conclusion:The application of cooperative matrix of CHCA-C3 in conjunction with MALDI-TOF MS and machine learning algorithms facilitates the rapid and precise identification of extended-spectrum β-lactamase (ESBL)-producing Escherichia coli. This approach offers a feasible solution for evidence-based clinical therapy and the control of healthcare-associated infections.
6.Clinical efficacy comparison of ileal conduit,flap embedding method,and traditional cutaneous ureteros-tomy in patients with bladder cancer

Objective To compare the clinical efficacy of different urinary diversions,specifically exploring whether the flap embedding technique can improve bladder cancer patients'quality of life and reduce the incidence of related complications.Methods 63 bladder cancer patients undergoing radical cystectomy with urinary diver-sion,between December 2022 and December 2023,were divided into three groups:Ileal conduit group(n=21),flap embedding technique group(n=21),and traditional cutaneous ureterostomy group(n=21).General clini-cal data,surgical data,preoperative and postoperative renal function indicators,incidence of complications within 6 months postoperatively,and quality of life scores were compared among the groups.Results The operative time in the ileal conduit group was longer than that in the other two groups(P<0.05).The quality of life scores in the flap embedding technique group were superior to those in the traditional cutaneous ureterostomy group(P<0.05),but no significant difference was found compared to the ileal conduit group(P>0.05).In terms of postoperative complications,the incidence of intestinal obstruction in the flap embedding technique group was lower than that in the ileal conduit group(P<0.05),and the rate of reinsertion of a single-J stent for hydronephrosis in the flap embedding technique group was lower than that in the traditional cutaneous ureterostomy group(P<0.05).Postop-erative serum creatinine levels in the traditional cutaneous ureterostomy group were significantly higher than preop-erative levels(P<0.05),while no significant differences in renal function indicators were observed in the other two groups(P>0.05).Conclusion The flap embedding technique significantly improves patients'quality of life and reduces the incidence of postoperative complications.It is worthy of further promotion in clinical practice.

8.Exploring the Pathogenesis of Prostate Cancer and Its Prevention and Treatment with Moxibustion Based on"Excessive Fire Dispersing Qi and Mild Fire Generating Qi"in The Inner Canon of Huangdi

[Objective]To explore the pathogenesis of prostate cancer and the prevention and treatment with moxibustion based on the theory of"excessive fire dispersing Qi,and mild fire generating Qi".[Methods]Based on the original text of The Inner Canon of Huangdi,the theoretical connotation of"excessive fire dispersing Qi,and mild fire generating Qi",and its relationship with the pathogenesis of prostate cancer and the reinforcing and reducing of moxibustion were investigated.The application of moxibustion reinforcing and reducing,guided by classical theory,is established in the prevention and treatment of prostate cancer,and illustrated with typical medical cases to substantiate the argument.[Results]The concept of"excessive fire dispersing Qi,and mild fire generating Qi"describes the impact of physiological and pathological fire on the body's genuine Qi.Mild fire is gentle in nature and helps to generate the body's genuine Qi,while excessive fire is intense and can consume the body's genuine Qi.Dampness,heat,blood stasis,and toxins,which are factors that induce prostate cancer,cancer toxins and western medical treatments such as surgery,radiotherapy and chemotherapy can all manifest as"excessive fire"pattern,consume the body's genuine Qi,which cause the course of prostate cancer to present in different stages,such as the initial formation of cancer toxins,deficiency of genuine Qi with toxin accumulation and Yang deficiency with cold stagnation.The moxibustion of reinforcing technique is relatively gentle and suitable in temperature,embodying the momentum of"mild fire",while the moxibustion of reducing technique is relatively intense and high in temperature to expel pathogens,similar to the fierce momentum of"excessive fire".Guiding the treatment of prostate cancer with moxibustion based on"excessive fire dispersing Qi",and"mild fire generating Qi"should focus on nourishing the mild fire and suppressing the excessive fire,using moxibustion of reinforcing to nourish the deficiency with"mild fire generating Qi",and using moxibustion of reducing to suppress cancer toxins and reduce the consumption of Qi dynamics,highlighting the therapeutic effect in disease prevention and treatment,as well as in improving the adverse reactions of western medical treatment.In the medical case mentioned,the patient suffered from urinary incontinence due to the damage to genuine Qi caused by surgery and the failure to consolidate and control it properly.The patient was comprehensively diagnosed with a deficiency of both the spleen and kidney.In the treatment,mild moxibustion was used to strengthen the spleen,warm the kidney,and replenish and consolidate Qi.Emphasis was also placed on the combination of moxibustion and herbal medicine.After two months of treatment,the symptoms were brought under stable control.[Conclusion]Under the guidance of the theory that"excessive fire dispersing Qi,while mild fire generating Qi",moxibustion techniques are flexibly selected in order to drain excess to suppress"excessive fire"and tonify deficiency to enhance"mild fire",which is well-suited to the complex pathogenesis of prostate cancer,which is characterized by a combination of deficiency and excess.This method provides an important reference for the prevention and treatment of the disease.

The results of mediation effect analysis showed that the mediation effect of BMI on the association between blood lipids (HDL-C, LDL-C, TG and TC) and HUA, from high to low, were as follows: 22.5% (95% CI: 20.8%-24.2%), 13.9% (95% CI: 12.0%-16.2%), 13.5% (95% CI: 12.7%-14.4%) and-3.9% (95% CI:-6.6%--1.8%). Conclusion:The blood lipid levels and BMI are positively correlated with HUA in the elderly.
