1.Research progress on anti-inflammatory effects of traditional Chinese medicine under the guidance of syndrome differentiation and treatment

Inflammation is the body’s response to damage, infection or other stimuli, but its excessive or continuous development can lead to a variety of diseases. Although modern medical anti-inflammatory therapies were widely used, it is often accompanied by limitations such as more adverse reactions. Based on the “holistic view” and “differential treatment”, traditional Chinese medicine (TCM) regards inflammation as a manifestation of the imbalance of yin and yang in the body and the conflict between good and evil. The application of anti-inflammatory TCM is not only aimed at the pathological state of “inflammation”, but also based on the overall consideration of “syndrome”. According to different types of syndrome, anti-inflammatory TCM can be divided into heat-clearing and detoxifying agents (such as Lonicera japonica and Isatis indigotica ), heat-clearing and drying dampness agents (such as Coptidis Rhizoma), blood-activating and stasis-dissolving agents (such as Salvia miltiorrhiza ) and vital qi-strengthening and pathogenic factor-expelling agents (such as Panax ginseng ). Four types of anti-inflammatory TCM restore the body’s immune balance through the systematic regulation of multi-component, multi-target and multi-pathway, exhibiting a good anti-inflammatory effect. Future research should focus on integrating systematic biology, applying artificial intelligence, carrying out high-quality evidence-based research, and combining traditional Chinese medicine and Western medicine, so as to reveal the overall regulatory law of anti-inf lammatory effects of TCM and promote clinical rational use.
2.The effect of longitudinal umbilicus fixation umbilicoplasty in the aesthetic construction of female umbilicus

Objective:To evaluate the clinical outcome of umbilicoplasty with longitudinal umbilical fixation and provide a basis for constructing an umbilicus that conforms to the aesthetic characteristics of Chinese women.Methods:Patients undergoing umbilicoplasty with longitudinal umbilical fixation between December of 2021 and November of 2023 were retrospectively analyzed at Department of Plastic and Aesthetic Surgery, Xi’an International Medical Center Hospital. All patients underwent abdominoplasty to reshape the abdominal contour and umbilicoplasty with longitudinal umbilical fixation to construct a new umbilical. The preoperative and postoperative body mass index (BMI), waist-hip ratio (waist circumference/hip circumference), umbilical contour, and longitudinal and transverse diameter of the umbilical cord were collected. The patients themselves used a 5-point Likert scale to evaluate the satisfaction of umbilicus (from the four aspects of umbilicus size, shape, position, and overall abdominal coordination and aesthetics), abdominal contour satisfaction rate [satisfaction rate= (very satisfied+ satisfed) patients/(total patients)×100%], and the subjective evaluation of umbilicus scar (very not obvious, not obvious, average, obvious, and very obvious). SPSS 25.0 statistical software was used for statistical analysis. Paired t-test or χ 2 test was used for preoperative and postoperative comparison. P<0.05 was considered statistically significant. Results:A total of 438 female patients were included, aged (33.4±5.5) years, including the pregnancy childbirth patients (428 cases) and weight loss patients (10 cases). The median follow-up time was 216(194, 266) days. Preoperative umbilical appearance mainly for the circle, longitudinal oval, transverse oval and convex. After operation, it was mainly longitudinal oval and circle. Early complications related to the umbilical cord were mainly blood circulation disturbance (8 cases). Long-term umbilical complication was mainly periumbilical scar (6 cases). The BMI [(19.96±2.03)kg/m 2 vs. (20.44±2.18)kg/m 2], waist-hip ratio[(0.78±0.04) vs. (0.88±0.08)], transverse diameter of the umbilicus [(0.68±0.26) cm vs. (1.11±0.44) cm] and longitudinal diameter of the umbilicus[(1.66±0.35) cm vs. (1.29±0.44) cm] after the operation were compared with those before the operation, and the differences were statistically significant ( P<0.01). The number of people satisfied with umbilical position [31.51%(138/438)vs. 97.26%(426/438)], umbilical size[11.19%(49/438)vs. 87.44%(383/438)], umbilical shape[7.76%(34/438)vs. 82.19%(360/438)], overall evaluation[(11.42%(50/438)vs. 86.30%(378/438)] and abdominal contour evaluation [12.79%(56/438)vs. 89.04%(390/438)]increased significantly, and the differences were statistically significant compared with those before operation ( P<0.01). The subjective evaluation of periumbilical scar was very not obvious in 171 cases, not obvious in 163 cases, average in 98 cases, and obvious in 6 cases. Conclusion:Umbilicoplasty with longitudinal umbilical fixation is suitable for Chinese women. The vertical oval umbilical constructed by this surgical method demonstrates appropriate size and position, significantly enhancing the overall abdominal aesthetic effect. The patient satisfaction was high and the postoperative effect was stable.
3.Feasibility study of non-ECG-gated wide-body detector for left atrial CT angiography examination

Objective To investigate the feasibility of non-ECG-gated scan using wide-body detector in left atrial computed tomo-graphy angiography(CTA)and to assess its value in evaluating left atrial appendage closure(LAAC)outcomes.Methods Patients who underwent LAAC and completed left atrial CTA were prospectively selected.Participants were randomly divided into groups A and B.In group A,the heart rate was measured using a finger clip oximeter before the examination,and the heart rate was set as the simula-ted heart rate.The simulated signal was generated by the CT equipment to complete the examination.In group B,the conventional ECG-gated scan was adopted.The residual leakage of left atrial appendage occluder,image quality,patient comfort and radiation dose of the two groups were compared,and statistical analysis was performed for these indicators.Results Both groups completed the evaluation of the endothelialization degree of the left atrial appendage occluder,and there was no significant difference in the objective image quality evaluation or subjective image score between the two groups(P>0.05).The total scores of the physiological dimension,psy-chological dimension,social cultural dimension and environmental dimension of patients in group A were higher than those in group B(P<0.001),and the volume CT dose index(CTDIvol)and dose length product(DLP)decreased by 25%and 25.36%respectively,with statistically significant differences(P<0.001).Conclusion During the left atrial CTA examination,the wide-body detector with non-ECG-gated scan mode can provide good image quality for clinical use and observe the embolization of the left atrial appendage after LAAC surgery.At the same time,it can effectively reduce the radiation dose for patients and improve patient comfort dur-ing the examination,showing high clinical feasibility and appli-cation value.
4.MALDI-TOF MS combined with machine learning for rapid identification of extended-spectrum β-lactamase-producing Escherichia coli

Objective:This study aims to develop a rapid identification technique for various genotypes of extended-spectrum β-lactamase (ESBL) producing Escherichia coli using matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS) in conjunction with machine learning algorithms. Methods:A total of 158 Escherichia coli strains were isolated from the clinical laboratory of the First Hospital of Jilin University from August 2018 to December 2022. Polymerase chain reaction (PCR) was employed to detect the CTX-M-1, CTX-M-8, CTX-M-9, and SHV genes. Mass spectral data of the bacterial strains were acquired by MALDI-TOF MS with a cooperative matrix of (E)-propyl α-cyano-4-hydroxycinnamate (CHCA-C3). Models based on random forest (RF), logistic regression (LR), and support vector machine (SVM) algorithms were constructed. The performance of the constructed models was evaluated using metrics including accuracy, sensitivity, specificity, and the area under the receiver operating characteristic curve (AUC). Mass spectral peaks exhibiting sensitivity and specificity exceeding 80% in the models were designated as characteristic peaks. To validate the efficacy of the cooperative matrix of CHCA-C3, clinical isolates of ESBL-producing Escherichia coli were analyzed by MALDI-TOF MS using the conventional CHCA matrix for comparative purposes. Results:Among the 158 strains of Escherichia coli, 91 strains produced ESBL, all of which were CTX-M genotype. The AUC values for the respective models were as follows: CTX-M-1 genotype exhibited AUC values of 0.98 for LR, 1.00 for RF, and 0.73 for SVM; CTX-M-9 genotype exhibited AUC values of 0.93 for LR, 0.99 for RF, and 0.76 for SVM; for CTX-M-8, all models achieved an AUC of 1.00, indicating excellent classification performance with respect to accuracy, specificity, and sensitivity. The characteristic mass spectral peaks associated with each genotype included: CTX-M-1 genotype at m/z 6 390; CTX-M-8 genotype at m/z 5 224, m/z 5 393, and m/z 9 021; CTX-M-9 genotype at m/z 5 161 and m/z 5 273. In the MALDI-TOF MS analysis conducted with the conventional CHCA matrix, the characteristic peak at m/z 9 021 for CTX-M-8 was the only one detected, with the characteristic peaks for CTX-M-1 and CTX-M-9 remaining undetected. Conclusion:The application of cooperative matrix of CHCA-C3 in conjunction with MALDI-TOF MS and machine learning algorithms facilitates the rapid and precise identification of extended-spectrum β-lactamase (ESBL)-producing Escherichia coli. This approach offers a feasible solution for evidence-based clinical therapy and the control of healthcare-associated infections.
5.Clinical application of endoscopic-assisted dual-plane augmentation mammoplasty via abdominoplasty incision

Objective:To evaluate the clinical efficacy of endoscopic-assisted dual-plane augmentation mammoplasty via abdominoplasty incision.Methods:The clinical data of patients who underwent concurrent abdominoplasty and endoscopic breast augmentation utilizing a single incision at Xi’an International Medical Center Hospital from August 2023 to February 2024 were retrospectively analyzed. Through a lower abdominal incision, a 4 cm-wide subcutaneous tunnel was endoscopically dissected in a superolateral direction from the lateral aspect of the xiphoid region, traversing the abdominal wall flap dissection area. Initially, the medial-inferior origin of the pectoralis major muscle was exposed and excised. Subsequently, the posterior space of the pectoralis major muscle was dissected to create the prosthesis space, and the rib origin of the pectoralis major muscle was severed 1 cm above the inframammary fold to establish a type Ⅰ dual-plane configuration. Depending on the patients’ breast morphology, the glandular tissue could be dissected superiorly from the surface of the pectoralis major muscle to form a type Ⅱ or type Ⅲ dual-plane. The prosthesis was then implanted, and its position was adjusted accordingly. After the drainage was placed, the remaining operation of abdominoplasty was continued. Patients’ basic information, implant volume, postoperative extubation time, and postoperative complications were recorded. A Likert scale of five points was used to evaluate preoperative and postoperative breast satisfaction [ranges from 1 to 5 points, 1 = very dissatisfied, 2 = dissatisfied, 3 = average, 4 = satisfied, 5 = very satisfied, respectively, satisfaction rate calculated as: ( "very satisfied" + "satisfied" )cases/total cases×100%]. SPSS 25.0 statistical software was used for statistical analysis. Measurement data in accordance with normal distribution were expressed as Mean±SD, and satisfaction scores not in accordance with normal distribution were expressed as M( Q1, Q3), compared by Wilcoxon rank sum test before and after operation. The count data were expressed by cases (%). Results:A cohort of 25 patients were included in the study, with a mean age of (29.8±2.9) years and a mean body mass index (BMI) of (20.8±1.7) kg/m 2. The median volume of the implanted prosthesis was (290.4±40.8) ml (range: 250-410 ml), and the mean extubation time was (3.1±0.9) d (range: 2-6 d). The mean follow-up duration was (270.0±52.4) d (range: 188-356 d). All patients demonstrated full, symmetrical, and natural breast contours. No significant complications, such as infection, hematoma, abnormal breast morphology, implant displacement, or exposure, were observed. The patient satisfaction scores significantly improved from preoperative median 1 (1, 2) to postoperative median 5 (4, 5), and the difference was statistically significant ( Z=-4.44, P<0.001). Patient satisfaction rate increased significantly from 12% (3/25) to 100% (25/25). Conclusion:Endoscopic-assisted dual-plane augmentation mammoplasty via abdominoplasty incision is a straightforward, feasible, safe, and effective procedure that eliminates breast scarring. Postoperative outcomes indicate an absence of serious complications, and patient satisfaction rates are high.

10.Epidemiology and exposure management of rabies in Shanxi Province, 2011‒2022

ObjectiveTo analyze the epidemiological features and influencing factors of rabies in Shanxi Province,and to provide evidence to further promote the elimination of rabies in Shanxi Province. MethodsThe incidence data of rabies in Shanxi from 2011 to 2022 were collected and subjected to descriptive analysis. ResultsFrom 2011 to 2022, a total of 348 rabies cases were reported in Shanxi Province, with an average annual incidence of 0.080 3/105. The incidence of rabies showed a downward trend overall. The highest incidence was in August. The cases were mainly farmers, mostly males, and most cases were reported between 50 and 69 years old. The data of cases showed that dogs were the main animals attacking human (93.96%). The incubation period of most cases was 1‒3 months (37.37%).The main exposure site was hand(51.33%). Only 2.66% cases with grade Ⅲ exposure were injected with passive immune agents. ConclusionThe incidence of rabies in Shanxi Province continues to decrease, but there are still loopholes in prevention and control measures. It is necessary to strengthen the management and immunization of dogs,health education, and standardized procedures after exposure to maintain the achievements in the prevention and control of rabies.
