1.Frontalis sling surgery using polypropylene non-absorbable sutures: a novel approach for the treatment of senile ptosis

Objective:To investigate the effect of a new method of utilizing frontalis sling with polypropylene non-absorbable sutures for the treatment of senile ptosis.Methods:A retrospective analysis was conducted on the clinical data of senile patients with blepharoptosis who were treated with frontalis sling surgery with polypropylene non-absorbable sutures at the Department of Plastic Surgery, Affiliated Hospital of Nanjing University of Chinese Medicine between January 2022 and December 2023. The palpebral fissure height and margin reflex distance (MRD1) of the upper eyelid margin were measured and recorded before and after the operation, and the operation time and postoperative detumescence time were recorded. Postoperative complications and recurrence of ptosis were followed up. Patients’ satisfaction with the postoperative effect was investigated and divided into three levels: very satisfied, satisfied, and dissatisfied. Normal distribution measurement data were expressed as Mean±SD; counting data were expressed as frequency.Results:A total of 8 patients were enrolled, including 3 males and 5 females, with an age of (68.8±6.1) years. Six patients underwent bilateral ptosis correction surgery, and two underwent unilateral ptosis correction surgery. The ptosis was graded as follows: 1 case was mild, 2 cases were moderate, and 5 cases were severe. There were 2 cases of aponeurotic ptosis, 3 cases of congenital ptosis and 3 cases of traumatic ptosis (1 of which was prosthetic eye). The operative time of 8 patients (14 eyes) with unilateral ptosis was (43.9±4.9) min. The swelling resolved in (9.4±1.7) days. One patient still had symptoms of lagophthalmos and corneal irritation 7 days after the operation. The symptoms gradually relieved after wearing corneal protective goggles and applying eye ointment. The postoperative follow-up lasted from 2 to 15 months (mean 6.3 months). Palpebral cleft height [(9.8±0.6) mm vs. (3.3±1.2) mm] and MRD1[(4.1±0.5) mm vs. (-1.1±0.8) mm] in 8 patients (14 eyes) after surgery were significantly higher than those before the operation( P<0.01). There was no recurrence of ptosis after the operation. There were 2 cases of upper eyelid hysteresis, of which 1 case was mild upper eyelid hysteresis, and the other case had relatively obvious upper eyelid hysteresis on prosthetic side. Satisfaction survey showed that 7 patients were very satisfied with the result of the operation, and 1 patient was satisfied. Conclusion:Frontal muscle suspension with polypropylene non-absorbable suture is a feasible method for the treatment of ptosis in elderly patients with simple operation, little tissue damage, positive effect, rapid postoperative recovery and acceptable complications.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

5.Relationship between the expression of programmed death ligand 1,hypoxia inducible factor-1αand clinical pathological characteristics,prognosis of gastric cancer patients

Objective To investigate the relationship between the expression of programmed death ligand 1(PD-L1)and hypoxia inducible factor-1α(HIF-1 α)with the clinical pathological characteristics and prognosis of gastric cancer patients.Methods The cancer tissues of 100 gastric cancer patients who underwent radical gastrectomy at the First Hospital of Handan City from July 2019 to July 2020 were selected as the research subjects,and their adjacent tissues(normal tissues ≥ 5 cm from the cancer tissues)were as the control group.Immunohistochemical detection of PD-L1 and HIF-1 α was performed by SP method.Spearman correlation analysis was used to analyze PD-L1 and HIF-1 α in gastric cancer tissues.Kaplan-Meier method was used to analyze the 3-year survival relationship of gastric cancer patients.The influencing factors of prognosis and death in patients with gastric cancer were analyzed by univariate and multivariate Cox regression.Results Among 100 gastric cancer patients,52 were PD-L1 positive and 48 were negative;67 were HIF-1 α positive and 33 were HIF-1 α negative,the positive expression rates of PD-L1 and HIF-1α in gastric cancer tissues were 52.00%and 67.00%,respectively,which were obviously higher than those in adjacent tissues(11.00%、18.00%),the difference was statistically significant(P<0.05).Spearman correlation analysis showed that the expression of PD-L1 was positively correlated with that of HIF-1α in gastric cancer tissues(r=0.730,P<0.001).The expressions of PD-L1 and HIF-1α in patients with gastric cancer were correlated with TNM stage,lymph node metastasis and local invasion(P<0.05).The 3-year overall survival rate of gastric cancer patients was 48.00%after surgery,and the 3-years survival rate of patients with positive expression of PD-L1 and HIF-1α were 28.85%and 31.34%,which were lower than those of patients with negative expression of PD-L1 and HIF-1α(68.75%and 81.82%)(Log rank x2=25.155,P<0.001.Log rank x2=24.552,P<0.001).Moreover,positive expression of PD-L1 and HIF-1α,TNM staging of Ⅲ-Ⅳ,lymph node metastasis,and local infiltration were independent risk factors for prognosis and death in gastric cancer patients(P<0.05).Conclusion Both PD-L1 and HIF-1α are highly expressed in cancer tissues of gastric cancer patients,and they are positively correlated.They are also associated with clinical pathological features such as TNM staging,lymph node metastasis,and poor prognosis.
6.Infrared spectrum analysis of Taichong(LR3)and Taixi(KI3)points before and after menstruation in healthy women

Objective:To investigate the variations in infrared radiation at Taichong(LR3),Taixi(KI3),and control points before and after menstruation and to examine the infrared radiation patterns associated with Yuan-primordial points of Zang-Fu organs during the physiological menstrual cycle. Methods:Using a point infrared radiation spectrum detection system,we detected the infrared radiation spectra of Taichong(LR3),Taixi(KI3),and the control points located 1 cm away from the two points,in a range of 1.50-18.00 μm wavelengths during the premenstrual,menstrual,and postmenstrual phases in 32 healthy adult women.Subsequently,data mining and analysis were conducted. Results:Before,during,and after menstruation,the infrared spectral shapes of bilateral Taichong(LR3),Taixi(KI3),and their control points were generally consistent,with characteristic infrared spectral wavelengths located at 11.25 μm.Prior to menstruation,the total intensity of infrared radiation at the right Taixi(KI3)was significantly lower than that at the control point(P<0.05),and that at the left Taichong(LR3)was significantly lower than that at the control point(P<0.01).During and after menstruation,the total infrared radiation intensity at both Taixi(KI3)was significantly lower than that of the control point(P<0.05).The wavelength points exhibiting significant differences in the infrared radiation intensity between points and control points were concentrated at the primary peak of 7.50-14.25 μm and the secondary peak of 15.00-17.25 μm. Conclusion:During different menstrual phases,the infrared radiation spectra of Taichong(LR3)and Taixi(KI3)exhibited distinct point specificity,mainly evident in the infrared radiation intensity and wavelength.
7.Mechanism of Weiwei Tongtiao Decoction Against Chronic Atrophic Gastritis in Rats: Based on NF-κB Signaling Pathway

ObjectiveTo explore the effect of Weiwei Tongtiao decoction (WTD) on chronic atrophic gastritis (CAG) rats and the underlying mechanism. MethodA total of 90 SD rats were randomized into normal control group, model group, high-dose, medium-dose, and low-dose WTD groups (18, 9, 4.5 g·kg-1·d-1 WTD, respectively, ig), and weifuchun control group (0.45 g·kg-1·d-1 weifuchun aqueous solution, ig), with 15 rats in each group. The N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) was employed to induced CAG in rats. After the modeling (identified by histopathological examination), the administration began and lasted 12 weeks. Then, gastric mucosa tissues of the rats were collected and stained with hematoxylin and eosin (HE), and the pathological changes of gastric mucosa were observed under the optical microscope. Real-time PCR, immunohistochemistry (IHC), and Western blotting were applied to examine the mRNA and protein expression of indexes in nuclear factor kappa-B (NF-κB) signaling pathway in the gastric mucosa of rats. ResultCAG rats showed irregular arrangement and morphology of the inherent glands in gastric mucosa and the glands decreased or disappeared. In addition, inflammatory cell infiltration was observed in the lamina propria of CAG rats, and about 48.4% presented intestinal metaplasia. WTD significantly alleviated the reduction of the glands and intestinal metaplasia in a dose-dependent manner. Compared with the normal control group, the model group demonstrated increase in the mRNA expression of cyclooxygenase-2 (COX-2), NF-κB p65, interleukin (IL)-1α, IL-1β, IL-10, IL-8α, and IL-8β, and protein expression of COX-2, NF-κB p65, and IL-10 (P<0.01). WTD and weifuchun lowered the mRNA expression of COX-2, NF-κB p65, IL-1α, IL-1β, IL-10, IL-8α, and IL-8β, and the protein expression of COX-2, NF-κB p65, and IL-10 in gastric mucosa of CAG rats (P<0.01). The expression of the above indexes after the intervention with high-dose WTD was close to that of the normal control group. ConclusionWTD can improve or even reverse the diseased gastric mucosa of CAG rats, and the mechanism is the likelihood that WTD down-regulates the mRNA and protein expression of the indexes in NF-κB signaling pathway in gastric mucosa of CAG rats.
8.Effects of botulinum toxin A injection therapy with ultrasound-guided for benign masseter hypertrophy

Objective:To evaluate the efficacy of botulinum toxin A injection therapy with ultrasound-guided for benign masseter hypertrophy.Methods:Twenty cases (40 sides) of masseter hypertrophy were injected with botulinum toxin A, among which 10 cases were taken as experimental group and the other 10 cases as control group. The experimental group had preoperative ultrasound examination to measure the thickness of masseter, and use ultrasound-guided precise injection during the operation, and then ultrasound recheck to measure the postoperative masseter thickness; The other 10 cases of control group adopt traditional 3-point injection method.Results:The masseter thickness of the two groups at different period of times after treatment was decreased ( P<0.001), the most obviously decrease happened 4-12 weeks after injection. The mean reduction was 26.8% at 4 weeks and 28.4% at 12 weeks after injection. Masseter muscle thickness recovered by 22% at 24 weeks and by 20% at 36 weeks. The average follow-up was 6.0±2.4 months with no serious complications occurred. In the control group, there was a partial masseter bulge in one case. The satisfactory rate of the patients in the experimental group was higher than that of the control group. Conclusions:Botulinum toxin A injection therapy with ultrasound-guided for benign masseter hypertrophy is an effective treatment, which is more accurate and effective than the traditional injection method.
9.The surgical treatment strategy and medium and long-term follow-up of Ebstein’s anomaly

Objective:To explore the surgical treatment strategy and the results of medium and long-term follow-up of Ebstein's anomaly(EA) through analyzing 127 patients.Methods:From January 2010 to Decembe 2018, 127 patients with EA underwent operation. There were 59 male and 68 female patients, with age of(9.5±3.7) years(0.67-18.0 years), weight of(27.1±11.7) kg(6.4-76.0 kg).13 cases were diagnosed as WPW syndrome by Echocardiography. There were 11 cases of VSD, 42 cases of ASD, 34 cases of PFO, 6 cases of PS and 127 cases of TI. The surgical strategy included 92 cases of biventricular repair(including 20 cases retained atrial shunt) and 35 cases of one and a half ventricular repair(including 8 cases retained atrial shunt). Except 2 patients underwent simple tricuspid valve repair(including 1 case of tricuspid annuloplasty ring), 125 EA patients in this group were all treated with cone reconstruction method for atrioventricular folding and tricuspid valvuloplasty.Results:6 patients died within perioperative period because of low cardiac output, pulmonary infection and cerebral hemorrhage, with a mortality of 4.7%. The complete atrioventricular block occurred in 2 patients and pacemaker were implanted postoperatively. 7 patients(5.5%) underwent pericardial fenestration due to pericardial effusion. All the other patients recovered successfully and were discharged from hospital without serious complications. The echocardiographic result indicated that the degree of tricuspid insufficiency were significantly reduced and cardiac function were improved before all patients discharged. The ratio of functional right ventricle to atrialized right ventricle(FRV/ARV) in the whole group was 0.87±0.37. FRV/ARV ratios of biventricular repair group and one and a half ventricular repair group were 1.12±0.39 and 0.48±0.28, respectively. And the difference between the two groups was statistically significant( t=5.685, P< 0.001). 110 cases were followed up in 3 to 96 months. 3 patients(2.7%) died during medium and long-term follow-up. Conclusion:The principle of the techniques is to reconstruct the tricuspid valve and right ventricle anatomically. Surgical treatment strategies should be decided according to the condition of tricuspid valve, ratio of FRV/ARV. Surgical treatment has a good effect for Ebstein’s anomaly in the medium and long-term.

Objective To optimize the method of the directional differentiation of adipose-derived stem cells into keratinocytes.Methods Adipose-derived stem cells (ADSCs),separated by collagenase digestion method,were isolated and cultured.Then the expression of surface specific markers CD34,CD44 and CD90 were detected by flow cytometer.The effect of different induced mediums cultured for two weeks on the differentiation of ADSCs into KCs was demonstrated:Group 1,the DMEM supplied with 2% FBS and 49% supertant of KCs;group 2,KSFM medium;group 3,DMEM medium supplied with 10% FBS and 5 μM ATRA;10% FBS DMEM as the control group.Immunofluorescene staining was applied to detect the expression of keratin CK14 and F-actin.Results A flattened fibroblast-like morphology was observed in cells,the positive expression rate of CD34 was 0.08%,while those of CD44 and CD90 were 99%.The cells that could differentiate into osteoblasts and chondrocytes,indicated that the cells were ADSCs.There was no significant change in the cell morphology in the group 1 under the induction medium;about 10% of the cells in group 2 were altered;the morphological changes were obvious in group 3,and approximately 20% of the cells showed irregular polygon.The immunofluorescene staining of the cells in group 3 indicated that the cells showed cobblestone-like phenotype and an organized cytoskeletal network with dense actin fibers at the edges;some cells were positive for CK14.Conclusions ADSCs show higher induction rate under ATRA stimulation.
