1.Analysis of Acupoint and Reflection Area Rules of Foot Massage in the Treatment of Insomnia Based on Data Mining

[Objective]To analyze the application and compatibility rules of acupoints and reflex areas in foot massage for the treatment of insomnia through data mining technology,thereby providing an evidence-based basis for the clinical treatment of foot massage in patients with insomnia.[Methods]The literatures associated with the treatment of insomnia through foot massage were retrieved by computer from The Cochrane Library,Web of Science,Embase,PubMed,China National Knowledge Infrastructure,Wanfang,VIP,Chinese Biomedical Literature and other databases.The search period was set until January 1,2025.Excel 2016 software was employed to establish the database for the treatment of insomnia with foot massage,and SPSS 25.0 software was utilized to calculate the frequency of acupoints and reflection areas in the database.Association rules were analyzed by the Apriori module of SPSS Modeler 18.0 software.The association rules of acupoints,reflection areas and their compatibility were described.The association rules were visualized by using Cytoscape 3.9.1 and cluster analysis was visualized with Origin 2022,respectively.[Results]Finally,124 effective literatures were incorporated including 35 for massage treatment of the foot reflex area alone,42 for massage treatment of the foot point alone,and 47 for massage treatment of the combined area and point.The most frequently utilized foot reflex areas of foot massage were the brain,the abdominal nerve plexus,the kidney,the heart,etc.,and the foot points were Yongquan,insomnia points,Taixi,etc.,among which the foot points were mainly characterized by the foot Shaoyin kidney channel and the foot Jueyin liver channel.The five transport points were the most prevalent specific points.The statistical results indicated that the effect of multi-point and/or multi-zone compatibility treatment was superior.Compatibility analysis revealed that the efficacy of multi-point and/or multi-zone compatibility treatments was superior.Analysis result of 124 articles on insomnia treatment through foot massage demonstrated that the most supportive compatibitily for the treatment of insomnia by foot massage was brain-abdominal nerve plexus,which was consistent with the analysis results of 35 literatures on the treatment of insomnia by foot reflex area massage alone.Furthermore,an analysis of 42 articles on insomnia treatment through massage of foot acupoints indicated that Yongquan-Taixi was the most supportive foot acupoint combination.[Conclusion]The combination of Chinese and western medicine theories for guiding foot massage in the treatment of insomnia is highly recommended.Selecting points such as Yongquan,Taixi,insomnia points,brain,abdominal nerve plexus,kidney,heart,and others for the compatibility treatment effect of these areas is better.
2.Development of the Competency Assessment Questionnaire for Clinical Teachers of Traditional Chinese Medicine Specialist Nurses

Objective:To develop the Competency Assessment Questionnaire for Clinical Teachers of Traditional Chinese Medicine Specialist Nurses and to test its reliability and validity.Methods:The first draft of the questionnaire was formed through literature review and expert consultation based on the competency theory. Convenience sampling was used to survey 218 clinical teachers of traditional Chinese medicine specialist nurses in 5 ClassⅢ Grade A hospitals in Zhejiang and Jiangsu provinces from June to July 2024 to test the reliability and validity of the questionnaire.Results:The Competency Assessment Questionnaire for Clinical Teachers of Traditional Chinese Medicine Specialist Nurses contained 5 dimensions, including professionalism and personal attributes, professional knowledge and skills, clinical practice ability, clinical teaching ability, and research management ability, with a total of 35 items. The scale-level content validity index was 0.875. The Cronbach's α coefficient was 0.974, the folded half reliability was 0.845, and the retest reliability was 0.912.Conclusions:The Competency Assessment Questionnaire for Clinical Teachers of Traditional Chinese Medicine Specialist Nurses has good reliability and validity, and can be used as a valid tool for evaluating the core competencies of clinical teachers of traditional Chinese medicine specialist nurses.
3.Application of foot massage based on the brain-gut axis theory in hemodialysis and insomnia patients with spleen-kidney yang deficiency type

Objective To explore the application effect of foot massage based on the brain-gut axis theory in patients with spleen-kidney yang deficiency syndrome undergoing maintenance hemodialysis and suffering from insomnia,and to provide references for improving patients' sleep quality and gastrointestinal symptoms.Methods A total of 88 patients with spleen-kidney yang deficiency syndrome undergoing maintenance hemodialysis and suffering from insomnia,who were treated at a Blood Purification Center of a tertiary-A general hospital in Hangzhou,Zhejiang Province from April to September 2024,were selected as a study subjects.They were randomly divided into an experimental group and a control group using the random number table method,with 44 cases in each group.The experimental group received foot massage based on the guidance of the brain-gut axis theory in addition to conventional treatment and nursing care,while the control group received conventional treatment and nursing care only.The intervention lasted for 3 months.The differences in the Pittsburgh Sleep Quality Index(PSQI)scores,Gastrointestinal Symptom Rating Scale(GSRS)scores,traditional Chinese medicine(TCM)syndrome scores,and serum 5-hydroxytryptamine and melatonin levels between the 2 groups before and after the intervention were compared.Results During the study period,no cases were lost in the experimental group,while 5 cases were lost in the control group.Ultimately,44 cases were included in the experimental group and 39 cases in the control group.After the intervention,the PSQI scores,GSRS scores,and TCM syndrome scores in both groups were lower than those before the intervention,and those in the experimental group were lower than those in the control group.The serum 5-hydroxytryptamine and melatonin levels were higher than those before the intervention,and the levels in the experimental group were higher than those in the control group,with statistically significant differences(all P<0.001).Conclusion Foot massage based on the brain-gut axis theory can improve gastrointestinal symptoms and TCM syndromes in patients with spleen-kidney yang deficiency syndrome undergoing maintenance hemodialysis and suffering from insomnia,and enhance their sleep quality.

7.Hypothalamic-Pituitary-Adrenal Axis and Epilepsy

Epilepsy is a recurrent, transient seizure disorder of the nervous system that affects the intellectual development, life and work, and psychological health of patients. People with epilepsy worldwide experience great suffering. Stressful stimuli such as infection, mental stress, and sleep deprivation are important triggers of epilepsy, and chronic stressful stimuli can lead to frequent seizures and comorbidities. The hypothalamic-pituitary-adrenal (HPA) axis is the most important system involved in the body’s stress response, and dysfunction thereof is thought to be associated with core epilepsy symptoms and related psychopathology. This article explores the intrinsic relationships of corticotropin-releasing hormone, adrenocorticotropic hormone, and glucocorticoids with epilepsy in order to reveal the role of the HPA axis in the pathogenesis of epilepsy. We hope that this information will yield future possible directions and ideas for fully understanding the pathogenesis of epilepsy and developing antiepileptic drugs.
8.Application of CICARE mode on new entry guidance personnel communication skills training

Objective To investigate the effect of CICARE mode on communication skills training of new entry guidance personnel. Methods A total of 16 new attendants were set as the control group, and routine training was used (between May 2015 and June 2015). Another 18 new attendants named as the observation group were trained in CICARE mode on the basis of routine training (between May 2016 and June 2016). The communication skills and satisfaction degree were compared between two groups. Results The scores of establishing a harmonious relationship, keen to listen, to confirm the patient problem, participation, efficient information transfer, verifying the feeling after the new recruits training for guidance staff were 3.72±0.08, 3.77±0.17, 3.60±0.08, 2.96±0.32, 3.50±0.14, 3.06±0.50 in the observation group, 3.13 ± 0.11, 3.25 ± 0.24, 2.66 ± 0.12, 2.67 ± 0.37, 2.90 ± 0.08, 2.59 ± 0.19 in the control group, the differences were statistically significant (t =2.451-27.155, P<0.05). The scores of guiding service appearance, body language, active service, health consultation, medical order, answer questions and provide help, medical environment and overall satisfaction respectively was 99.67 ± 2.58, 98.00 ± 6.05, 98.00 ± 6.05, 96.00 ± 8.07, 99.33 ± 3.62, 98.00 ± 6.05, 96.00 ± 8.07, 94.00 ± 12.91, 97.50 ± 5.48 in the observation group, and 91.33 ± 10.56, 87.67 ± 14.19, 84.67 ± 14.31, 79.67 ± 15.27, 84.33 ± 14.17, 83.67 ± 14.37, 85.67 ± 12.70, 87.00 ± 10.85, 84.67 ± 9.91 in the control group, the differences were statistically significant (t=3.215-8.775, P<0.05). Conclusions CICARE mode can not only provide normative and orderly guidance for the guiding staff, but also improve the communication ability of the guiding staff in a short time, and let the patients participate in the guiding service. It is worthy of being popularize and applied in the guided diagnosis service to increase the patient′s participation, acceptance and satisfaction.
9.Preliminary study on WPBL teaching method in microbiological test teaching

Objective To explore the effect of WPBL teaching method in microbiologicla test teach-ing. Method A total of 120 students majoring in clinical laboratory medicine were divided into the experi-mental group (WPBL teaching) and the control group (the traditional teaching) with 60 in each group in 2015. Results Questionnaire survey showed that in the experimental group, 92.6%of the students believe that WPBL teaching method can stimulate learning interest and promote student self-directed learning, 97.3% believe that WPBL teaching method can improve the ability to analyze and solve problems, 94.7%believe that WPBL teaching method can strengthen teamwork and promote learning exchanges, and 99.6%hold that WPBL teaching method can help them recognize the importance of microbiological tests in the di-agnosis of clinical infectious diseases. The final scores and the usual performance of the experimental group were (75.23 ±11.21) and (24.15 ±1.71), respectively, while those of the control group were (63.42 ±8.21) and (20.26±1.48), respectively. The differences were statistically significant (P<0.005). Conclusions PBL teaching method can fully integrate teaching resources, stimulate students' enthusiasm and initiative, improve students' comprehensive qualities, including the ability to analyze and solve problems, teamwork ability and critical thinking ability, and improve the quality and effect of microbiological test teaching.
10.Effects of early oral feeding on postoperative recovery of patients undergoing gastrointestinal surgery:a Meta-analysis

Objective To evaluate the effects of early oral feeding on postoperative recovery of patients undergoing gastrointestinal surgery.Methods CNKI, China Biology Medicine database (CBM), PubMed, Embase, The Cochrane Library and Web of Science databases were screened. Randomized controlled trials (RCTs) and their references from 2006 to 2016 were recruited using Meta-analysis. RevMan 5.3 software was used for data processing.Results A total of 11 RCTs containing 2188 patients were included in the study. In patients with early oral feeding compared to patients with traditional fasting, there were statistically significant differences in first time of flatus [MD=-0.90 d, 95%CI (-1.12, -0.67),P<0.05], first time of stools [MD= -0.89 d, 95%CI (-1.11, -0.79),P<0.05], nasogastric tube reinsertion rate [RR=0.57, 95%CI (0.36, 0.90), P<0.05], fist time of regular diet [RR=0.57 d, 95%CI (0.36, 0.90),P<0.05], and infective complications rate [RR=0.45, 95%CI (0.28, 0.74),P<0.05]; No significant differences were observed for vomiting [RR=1.44, 95%CI (0.69, 3.19),P>0.05], abdominal distension [RR=1.29, 95%CI (0.70, 2.36),P>0.05], the total incidence rate of postoperative complications [RR=0.82, 95%CI (0.59, 1.15),P>0.05]and non-infective complications [RR=0.78, 95%CI (0.49, 1.25),P>0.05].Conclusions Early oral feeding is beneficial to the recovery of gastrointestinal function in patients with digestive system diseases, and significantly reduces the infective complications.
