Parkinson's disease (PD) is a complex neurodegenerative disease involving multiple systems and neurotransmitters. Due to the high clinical heterogeneity of PD,it is urgent to establish a comprehensive and long-term traditional Chinese medicine (TCM) management model. In this paper, the conceptual framework of full-cycle management of PD is preliminarily constructed:based on the evolution of the pathophysiological mechanisms of protein deposition and neurotransmitter disorder in PD,the three-stage syndrome characteristics of the prodromal stage (predominant healthy Qi with subtle pathogenic factors),the early clinical stage (declining healthy Qi with growing pathogenic factors) and the middle and late stages (overwhelming pathogenic factors with deficient healthy Qi) are longitudinally described. Through the syndrome differentiation of visceral manifestations, the etiology and pathogenesis of PD motor and non-motor symptoms were comprehensively analyzed, while the matching treatment methods and prescriptions were inferred, and the modular scheme of the combining main symptoms, accompanying symptoms and secondary symptoms was proposed. The conceptual gap of TCM regarding motor complications ('variable syndrome') and PD-related hyperpyrexia syndrome ('critical syndrome') was explained. This framework reflects the characteristics of combination of disease and syndrome and overall constant motion, and provides new theories and research ideas for individualized and whole-process management of PD in TCM.

Objective:To investigate anxiety symptoms and associated factors in relocated elderly residents of new townships, and to provide evidence for prevention interventions. Methods:A cross-sectional study was conduc-ted in relocated elderly residents in new townships of three urban areas in Shandong Province from 2021 to 2023.The study instruments included Ascertain Dementia-8,Generalized Anxiety Disorder-7,self-administered de-mographic characteristics information questionnaire.Multivariate analysis of factors associated with anxiety symp-toms in elderly residents was performed using multivariate logistic regression. Results:The prevalence rate of mild anxiety symptoms was 5.8%,and the rate of moderate-to-severe anxiety symptoms was 1.3%in 3 313 resi-dents.Multivariate analysis found that self-assessed general psychological condition(OR=0.52),good family envi-ronment(OR=0.34),no self-perceived cognitive impairment(OR=0.31),no chronic diseases(OR=0.42),and only one chronic disease(OR=0.61)were protective factors for mild anxiety symptoms, and very good dietary structure(OR=2.15)and fair dietary structure(OR=2.39)were risk factors for those.Very good family environ-ment(OR=0.11)and average family environment(OR=0.16),and no self-perceived cognitive impairment(OR=0.14)were protective factors for moderate-to-severe anxiety symptoms, and 0-3 years(OR=3.24)and 4-6 years(OR=3.28)of relocation were risk factors for those. Conclusion:Family environment, dietary structure, and duration since relocation are key factors associated with anxiety symptoms among relocated elderly residents in new townships.Targeted interventions should be implemented to address their mental health needs.

Objective:To analyze the development trajectory of sleep quality in patients with insomnia after cognitive behavioral therapy (CBT-I), and to explore the influencing factors of different development trajectories. Methods:A total of 418 patients with insomnia who received CBT-I at the Sleep Medicine Center of Xiamen Xianyue Hospital from February 2019 to June 2023 were recruited using convenience sampling. All patients completed the Pittsburgh sleep quality index (PSQI) at five time points: before CBT-I initiation (T1), at the end of treatment (T2), and at 3 months (T3), 6 months (T4), and 12 months (T5) post-treatment. At T1, all patients were evaluated by the insomnia severity index (ISI), Flinders fatigue scale (FFS), Beck depression inventory-Ⅱ (BDI-Ⅱ), Beck anxiety inventory (BAI), and dysfunctional beliefs and attitudes about sleep scale (DBAS). Latent class growth modeling (LCGM) was constructed and employed to analyze the development trajectories of sleep quality post-CBT-I using Mplus 8.0 software. Univariate analysis and Logistic regression analysis were conducted by SPSS 26.0 software to explore influencing factors of the sleep quality trajectories. Results:The PSQI scores of insomnia patients at T1, T2, T3, T4 and T5 were 15(12, 18), 8(6, 11), 5(3, 7), 5(3, 7) and 5(2, 7), respectively. Three distinct development trajectories were identified. Based on these trajectories, 418 patients were categorized into fluctuating development group ( n=44, 10.53%), slow improvement group ( n=193, 46.17%), and sustained improvement group ( n=181, 43.30%). Logistic regression analysis revealed that depressive symptom severity, anxiety symptom severity, family history of insomnia, and use of hypnotic medications were significant influencing factors for the sleep quality trajectories after CBT-I treatment (all P<0.05). Conclusion:The sleep quality of insomnia patients after CBT-I treatment shows different development trajectories, and its influencing factors are depression, anxiety, family history of insomnia and the use of hypnotic medications.

JOURNAL OF RARE DISEASES 2023;2(4):589-595
There are over 6000 rare diseases in the world, affecting more than 300 million people. Early and precise diagnosis of rare diseases has always been the goal in clinical medicine. Emerging computer vision technology now greatly enhance medicine and healthcare and shows the potential in assisting the diagnosis and treatment for rare diseases. The technology can be a useful tool for extracting disease-relevant patterns from medical imaging. However, the effectiveness of its application depends on the complexity of the medical cases. In this paper, we summarize the challenges and emerging solution for the application of computer vision in diagnosis, rehabilitation as well as management of rare musculoskeletal diseases.

Establishment of vaginal immune defenses at the mucosal interface layer through gene vaccines promise to prevent infectious diseases among females. Mucosal barriers composed of a flowing mucus hydrogel and tightly conjugated epithelial cells (ECs), which represent the main technical difficulties for vaccine development, reside in the harsh, acidic human vaginal environment. Different from frequently employed viral vectors, two types of nonviral nanocarriers were designed to concurrently overcome the barriers and induce immune responses. Differing design concepts include the charge-reversal property (DRLS) to mimic a virus that uses any cells as factories, as well as the addition of a hyaluronic acid coating (HA/RLS) to directly target dendritic cells (DCs). With a suitable size and electrostatic neutrality, these two nanoparticles penetrate a mucus hydrogel with similar diffusivity. The DRLS system expressed a higher level of the carried human papillomavirus type 16 L1 gene compared to HA/RLS in vivo. Therefore it induced more robust mucosal, cellular, and humoral immune responses. Moreover, the DLRS applied to intravaginal immunization induced high IgA levels compared with intramuscularly injected DNA (naked), indicating timely protection against pathogens at the mucus layer. These findings also offer important approaches for the design and fabrication of nonviral gene vaccines in other mucosal systems.
