Objective To explore the influencing factors of compliance with new oral anticoagulants(NOACs)in elderly patients with nonvalvular atrial fibrillation(NVAF)and its correlation with social support. Methods The clinical data of 126 patients with NVAF treated by NOACs in Lu 'an People's Hospital of Anhui Province from January 2022 to January 2024 were selected by convenient sampling method.General data of patients were collected.According to Morisky medication compliance scale, patients were divided into two groups:the good compliance group(n=74)and the poor compliance group(n=52).Social support was investigated by Social Support Rating Scale(SSRS).General data of patients were compared between the good compliance group and the poor compliance group. The relationship between medication compliance and social support was analyzed by Pearson correlation.Multivariate Logistic regression was used to analyze the influencing factors of patient medication compliance. Results There were statistically significant differences in educational level, the proportion of patients with anxiety and depression,CHA2DS2-VASc score,HAS-BLED score and SSRS score between the good compliance group and the poor compliance group(P<0.05).The results of the correlation analysis showed that the medication compliance of patients was positively correlated with objective support(r=0.461),subjective support(r=0.432),support utilization(r=0.323)and SSRS score(r=0.596,P<0.05).The results of multivariate Logistic regression analysis showed that low educational level, accompanied by anxiety, low HAS-BLED and SSRS scores were risk factors for poor medication compliance in elderly NVAF patients(P<0.05).Conclusion Improving the social support level of elderly patients with NVAF is conducive to the formation of good medication compliance.

Objective:This article summarized the characteristics of inpatients with the COVID-19 by psychiatric contact consultation in Wuhan.Method:This was a retrospective study. A total of 105 patients consulted by the Guangdong psychological medical team from March 1, 2020 to March 7, 2020 were selected. Data were collected by 20 psychiatrists responsible for the contact consultation, including age, gender, the main reasons for the contact consultation and the psychiatric treatment plan. The consultant doctor decided whether to enter the isolation ward according to the patient′s condition introduced by the attending physician, and whom were followed up within a week. Descriptive statistical analysis was used to analyze the data. Results:There were 105 patients from 5 designated hospitals, aged (58.3±18.9) years, 62.9% (66/105) were females, and 102 (97.1%) were consulted face-to-face in the isolated ward. Main reasons for consultation were anxiety (65, 61.9%), depression (27, 25.7%), insomnia (6, 5.7%), agitation (6, 5.7%), and illusion (1, 1%). Seventy-seven of those patients (73.3%) received individualized psychotherapy, 1(1.0%) was prescribed with psychiatric medication, and 27 (25.7%) were treated with both measures. Follow-up showed that psychological symptoms relieved in 102 cases (97.1%).Conclusion:Psychosocial symptoms such as anxiety and depression are common in infected patients, and thereby psychiatric contact and consultation is in great demand in the treatment of COVID-19. Early intervention of mental health professionals in the diagnosis and treatment of patients plays a positive role in the recovery of them.

Objective To evaluate the clinical effect of protecting the saphenous nerve with the suture anchor on repairing the medial collateral ligament injury of the knee joint. Methods From June 2014 to June 2016, 48 cases of medial collateral ligament injuries of the knee joint were repaired with suture anchors of the saphenous nerve in the 107th Hospital of PLA. Results All patients were followed up for 12-18 months, with an average of 14.6 months. All the patients were followed up with good stability of the knee joint. The 30 degree flexion stress test of the knee joint showed that 2 cases were positive forⅠdegree, and the stress test of the extension position was negative. The X-ray examination showed that the internal and external articular space of the knee joint was symmetrical and good, and the anchors did not loose or move in the bone. The average knee flexion before operation was (43.19 ± 2.60)°, and 1 year after operation was (135.62 ± 3.68) °. And the flexion of knee 1 year after operation was higher than that before operation (t=5.37, P<0.01). The Lysholm score was (43.19 ± 2.60) points before operation, and 1 year after operation was (93.69 ± 5.39) points, and the postoperative score was higher than that before operation (t=4.85, P<0.01). The grade of efficacy was excellent in 42 cases, good in 4 cases, fair in 2 cases, and the excellent and good rate was 95.8%(46/48). Conclusions The repair of medial collateral ligament injury with suture anchors under the protection of the hidden nerve can effectively prevent the symptoms of numbness in the medial area of the knee joint. It is simple, fixed and can be repaired under direct vision. It is reliable to restore the strength of the ligament and maintain the stability of the medial knee joint. It is worthy of clinical push and wide application.

ObjectiveTo explore the feasibility of psychiatric assessment scale of cognitive and volition capacity(PASCVC) in violent crimes.MethodsThe PASCVC,criminal responsibility rating scale for violence(CRRSV) and criminal responsibility rating scale(CRRS)were used respectively in 244 forensic psychiatric cases. All cases were classified by the judicial surveyor into three grades including complete lost, partial lost and normal, the results were compared with expert's opinions.Results①PASCVC was composed of two subscales:cognitive capacity and volition capacity. There were significant differences in rating average score of three groups (complete lost, partial lost and normal) of total scale, cognitive capacity and volition capacity subscales were 9.96±7.53,69.15±10.290,101.16±6.034;20.29±4.971,37.13±5.388,2.55±2.612;18.32±2.595,30.07±4.595,48.34±4.147 respectively and there were significant differences between them.②27 items had good correlation with the scale(r=0.157~0.915).③According to the reference demarcation points of scale,12 criteria were carried out by using stepwise discriminant analysis. The accuracy of retrospective cumulative square was 97.1%.④Total scale and subscales consistency of three demarcation point grading assessment results were consistent with expert's opinions, the Kappa values were 0.925,0.848 and 0.930 respectively (P<0.01);PASCVC consistency with CRRSV and CRRS,the correlation coefficients were 0.952 and 0.961 respectively (P<0.01),the Kappa values were 0.830 and 0.881 respectively (P<0.01).Conclusion The PASCVC is feasible to be used in violent crimes.

Objective To test the cut-off point and empirical validity of psychiatric assessment scale of cognitive and volition capacity for suspects in crime (PASCVC).Methods One thousand of forensic psychiatry cases from fifteen bodies of the seven administrative regions in China were collected for forensic assessment by the three scales of PASCVC,DCRRS and CRRSV in August 2009 to December 201 l.in all cases by the appraiser to make six grade judgment of the cognitive and volition capacity including completely lost, lost most, partial loss, partial loss of a small, slightly damaged and normal. Results ① Based on the results of frequency distribution table: complete loss of demarcation divided < 56 minutes, lost most 57-66,partial loss 67-76,partial loss of a small 77-87,slightly damaged 88-100 and normal ≥ 101.②The correlation coefficient of six demarcation point grading assessment results and expert opinion was 0.939(P<0.001).③PASCVC consistency with DCRRS and CRRSV,the Kappa values were 0.838 and 0.835 respectively(P<0.001).④Discriminant function were established by two methods of PASCVC and expert assessment, the correct rates of PASCVC and expert judgment on back substitution determine were 86.9% and 85.1%.Two kinds of classification results, the Kappa value of 0.857 (P<0.001).Conclusion PASCVC has good empirical validity, and can be used as an objective assessment tool for forensic assessment of criminal cognitive and volition capacity.
