1.Anti-inflammatory peptides for oral inflammatory diseases:regulation of inflammatory response to reduce tissue destruction and structural loss

BACKGROUND:The progression of chronic oral diseases is closely related to the continuous inflammatory response.Anti-inflammatory peptides are expected to become a substitute for traditional anti-inflammatory drugs due to their rich sources,easy absorption by the body and low side effects.OBJECTIVE:To review the types,anti-inflammatory mechanisms,and their application in oral related diseases.METHODS:CNKI,PubMed,and Web of Science were retrieved,with"polypeptide,anti-inflammatory,immunomodulation,oral inflammatory diseases"as Chinese and English search terms.111 articles related to the classification of anti-inflammatory peptides,anti-inflammatory mechanisms,and application of oral inflammatory diseases were selected for review.RESULTS AND CONCLUSION:(1)Anti-inflammatory peptides are abundant in nature,which can be extracted from plants,animals,and microorganisms.In addition to naturally occurring peptides and protein hydrolysates,peptides synthesized by chemical modification,computer simulation design,and genetic recombination technology can also exert anti-inflammatory effects.The composition,position,and properties of amino acids affect their anti-inflammatory activity.(2)Because the anti-inflammatory mechanism of anti-inflammatory peptides is still unclear,the activity verification is mostly cell experiments,and there is a lack of animal models,clinical trials and other further studies.(3)In the treatment of oral inflammatory diseases(including periodontitis,oral mucositis,caries,pulpitis,suppurative osteomyelitis of the jaw,and peri-implantitis),anti-inflammatory peptides can inhibit the release of inflammatory factors such as interleukin 6,interleukin 1β,and tumor necrosis factor α in oral tissues,regulate inflammatory responses,improve the chronic inflammatory environment,reduce tissue destruction and structural loss,and promote bone tissue regeneration,providing new ideas for the treatment of oral inflammatory diseases.
2.Application of outcome-oriented TECK teaching in standardized training of residents in the department of neurology

Objective:To explore the effectiveness of outcome-oriented TECK teaching in the standardized training of neurology resident physicians, and to evaluate its impact on clinical practice, clinical thinking, and self-learning abilities.Methods:A controlled study design was adopted, and 78 resident physicians who rotated through the Department of Neurology at Guangdong Provincial People's Hospital Affiliated to Southern Medical University between November 2022 and November 2024 were divided into a control group ( n=39) and an experimental group ( n=39) according to their admission order. The control group received the traditional "theoretical teaching, rotation training, exit assessment" mode. The experimental group received an outcome-oriented TECK teaching mode, which included four modules: theoretical class, experimental course/practice, case discussion, and knowledge reinforcement. The two groups of residents were evaluated for clinical practice ability, clinical thinking ability, and teaching satisfaction. SPSS 22.0 was used for t-test and chi-square test. Results:The experimental group outperformed the control group in all dimensions of clinical practice ability (except for doctor-patient communication ability) (all P<0.001), such as basic operational skills [(4.42±0.41) vs. (3.87±0.56)] and emergency management ability [(4.38±0.39) vs. (3.65±0.62)]. The clinical thinking ability scores of the experimental group were higher (all P<0.001), such as diagnostic logic [(4.43±0.38) vs. (3.92±0.51)] and decision-making basis [(4.45±0.42) vs. (3.78±0.63)]. In terms of self-learning ability, the experimental group showed significant improvements in literature search [(4.42±0.43) vs. (3.65±0.58)] and knowledge update [(4.38±0.39) vs. (3.42±0.61)] (both P<0.001). In terms of teaching satisfaction, the experimental group scored higher in dimensions such as course design [(4.42±0.41) vs. (3.89±0.58)] and clinical fit [(4.45±0.37) vs. (3.82±0.61)] (all P<0.001). Conclusions:The outcome-oriented TECK teaching mode can effectively enhance the clinical practice and thinking abilities of neurology residents in standardized training, and improve the effectiveness of self-learning, providing an innovative path for the training of specialized physicians. In the future, interdisciplinary integration and long-term tracking and evaluation need to be optimized.
3.The value of multiparametric MRI in the composition assessment of benign prostatic hyperplasia

Objective To investigate the application value of conventional MRI combined with diffusion tensor imaging(DTI)in evaluating the correlation between the texture composition of benign prostatic hyperplasia(BPH)and the International Prostate Symptom Score(IPSS).Methods Seventy patients with BPH confirmed by pathology were retrospectively analyzed and all patients underwent conventional MRI,DTI and IPSS before surgery.Evaluation metrics included:the mean signal intensity of T2WI(mean-SI-T2WI),apparent diffusion coefficient(ADC)and fractional anisotropy(FA)values.Independent samples t-test,partial correlation analysis,and receiver operating characteristic(ROC)curve were used to assess the correlation between the texture parameters of the prostate transition zone and IPSS.Results The mean-SI-T2WI was significantly negatively correlated with IPSS(r=-0.683,P<0.001);the average ADC value was slightly negatively correlated with IPSS(r=-0.467,P<0.001);and the average FA value was slightly positively correlated with IPSS(r=0.419,P<0.001).The predictive value of MRI texture parameters for IPSS in BPH patients,ranked from high to low,mean-SI-T2WI[area under the curve(AUC)=0.734],average ADC value(AUC=0.673),and average FA value(AUC=0.635);However,the combination of mean-SI-T2WI+ADC+FA(AUC=0.791)did not significantly improve the diagnostic efficacy by DeLong's test(P>0.05).Conclusion Mean-SI-T2WI,DWI and DTI can be used to evaluate the composition of the prostate,among which mean-SI-T2WI is the best,and the com-bination of them can not improve the diagnostic efficacy.
4.Construction of a predictive model for the efficacy of SNRI antidepressants in inpatients with moderate and severe depression based on machine learning

OBJECTIVE To construct a prediction model for the efficacy of serotonin-norepinephrine reuptake inhibitor(SNRI)in inpatients with moderate and severe depression by using a machine learning method.METHODS The case records of inpatients with moderate and severe depression treated with SNRI antidepressants were collected from a third-grade class-A hospital in Xinjiang from January 2022 to October 2024;those patients were divided into effective group and ineffective group based on the Hamilton depression scale-24 score reduction rate.After screening the characteristic variables related to the therapeutic efficacy of SNRI drugs through LASSO regression,five prediction models including support vector machine,k-nearest neighbor,random forest,lightweight gradient boosting machine and extreme gradient boosting were constructed using the training set.Bayesian optimization was used to adjust the hyperparameters of these models.The performance of the models was evaluated in the validation set to select the optimal model.The Shapley additive explanations method was used to perform explainable analysis on the best model.RESULTS The medical records from 355 hospitalized patients with moderate and severe depression were collected,comprising 285 cases in the effective group and 70 cases in the ineffective group,resulting in an overall therapeutic response rate of 80.28%.After feature variable screening,five characteristic variables for therapeutic efficacy were obtained,including Hamilton anxiety scale,blood urea nitrogen,combination of anti-anxiety drugs,drinking history,and first onset of the disease.Compared with other models,the random forest model performed the best.The area under the receiver operating characteristic curve was 0.85,the area under the precision-recall curve was 0.87,the accuracy was 0.74,and the recall rate value was 0.75.CONCLUSIONS The random forest model constructed based on five characteristic variables demonstrates potential for predicting the therapeutic efficacy of SNRI antidepressants in hospitalized patients with moderate and severe depression.
5.Systematic review of risk predictive models for chemotherapy-induced myelosuppression in breast cancer

OBJECTIVE To systematically evaluate risk prediction models for chemotherapy-induced myelosuppression in breast cancer, and provide a scientific reference for clinical healthcare workers in selecting or developing effective predictive models. METHODS A systematic search was conducted for studies on predictive models of the risk of chemotherapy-induced myelosuppression in breast cancer across the CNKI, VIP, Wanfang, PubMed, Web of Science, Cochrane Library, Embase, and Scopus databases, with a time frame of the establishment of the database to May 7, 2024. Literature was independently screened by 2 investigators, data were extracted according to critical appraisal and data extraction for systematic reviews of predictive model studies, and the risk of bias evaluation tool for predictive model studies was used to analyze the risk of bias and applicability of the included studies. RESULTS There were totally 7 studies, comprising 12 models. Among them, 11 models indicated an area under the subject operating characteristic curve of 0.600-0.908; 2 models indicated calibration. The common predictor variables of the included models were age, pre-chemotherapy neutrophil count, pre-chemotherapy lymphocyte count, and pre-chemotherapy albumin. The overall risk of bias of the 7 studies was high, which was mainly attributed to the flaws in the study design, insufficient sample sizes, inappropriate treatment of variables, non-reporting of missing data, and the lack of indicators for the assessment of the models, but the applicability was good. CONCLUSIONS The predictive performance of risk predictive models for chemotherapy-induced myelosuppression in breast cancer remains to be further enhanced, and the overall risk of model bias is high. Future studies should follow the specifications of model development and reporting, then combine machine learning algorithms to develop risk predictive models with good predictive performance, high stability, and low risk of bias, so as to provide a decision-making basis for the clinic.

9.Generation of a Cre recombinase knock-in mouse line labeling the mucous acinar cells of the sublingual gland

Objective To generate and identify the Itgbl1(integrin beta-like)promoter-driven Cre knock-in mouse line.Methods Itgbll-Cre knock-in mice were generated using clustered regularly interspaced short palindromic repeats(CRISPR)/CRISPR-associated protein 9(Cas9)gene editing.The Itgbl1-Cre mice were crossed with the Cre reporter ROSALSL-tdTomato)mice to detect the expression profile of Cre activity.The tdTomato expression pattern across tissues and cell-specific markers were used to identify the cell types of Itgbl1-expressing cells and their progeny.Results and Conclusion tdTomato was specifically expressed in mucous acinar cells of the sublingual gland,pancreatic islet cells,and gastric endocrine cells.In addition,tdTomato expression was also found in some of the neurons of the retina and brain,as well as in a few cells in the serosal layer of the intestine,articular cartilage,periosteum,and bone marrow.The first Itgbl1-Cre recombinase transgenic mouse line was established,which can specifically label the mucous acinar cells of the sublingual gland.
10.Assessment of outcome in hypothermia therapy for neonatal hypoxic-ischemic encephalopathy using amplitude-integrated electroencephalography

Objective To evaluate the efficacy of hypothermia therapy in neonates with hypoxic-ischemic encephalopathy(HIE)through the analysis of amplitude-integrated electroencephalography.Methods A retrospective analysis was conducted on 59 neonates with moderate to severe HIE.They were divided into observation group(n=31,hypothermia therapy)and control group(n=28,conventional therapy)according to different treatment protocols.Chi-square test,independent sample t-test,and one-factor Mann-Whitney U test were used for intergroup difference analysis.Results Significant differences between two groups were observed in lower boundary amplitude after 24 h of treatment,sleep-awake cycle after 72 h of treatment,and total scores after 72 h of treatment(P<0.05).After 48 and 72 h of treatment,the neonates in hypothermia therapy group had obviously lower neuro-specific enolase level than those in control group(P<0.05).Conclusion Early application of hypothermia therapy can significantly improve cerebral function in neonates with HIE and lower the neuro-specific enolase level.
