1.Theory and practice of medical humanistic care: a decade of implementation by the medical humanities team at Harbin Medical University

On October 9, 2024, the General Office of the National Health Commission, the General Office of the Ministry of Education, the General Department of the National Administration of Traditional Chinese Medicine, and the General Department of the National Bureau of Disease Control and Prevention jointly issued the Action Plan for Enhancing Medical Humanistic Care (2024-2027), stating that medical humanistic spirit is a concrete manifestation of humanistic spirit in the medical field. Aiming at caring for and respecting patients, this spirit reflects the attitude of medicine towards life. The cultivation of medical humanistic care competencies should serve as a core element of modern medical education, running through the entire process of the medical students’ training system and the full stage of medical workers’ career development. The humanities team of Harbin Medical University pioneered the “consistent system” concept of medical humanities education, inherited the “four-stage” with clinical practice as the primary approach, and closely focused on “three combinations, two considerations, and one assessment.” Through deeply integrating medicine with the humanities, as well as theory with practice, they have embarked on a distinctive implementation path of medical humanities, laying a solid foundation for the cultivation of high-quality medical talents with both skills and expertise in practice.
2.Palmar approach to on-top plasty for correction of radial polydactyly

Objective:To study the clinical outcome of palmar approach to on-top plasty in the treatment of unequal radial polydactyly.Methods:A retrospective analysis was conducted on patients with unequal radial polydactyly in which neither thumb duplicates possess both well-developed proximal and distal components. All the patients underwent palmar approach to on-top plasty between September 2013 and September 2023 at Shanghai Ninth People’s Hospital, Shanghai Jiao Tong University School of Medicine and Shanxi Children’s Hospital. At the final follow-up, the active flexion-extension range of motion of the first metacarpophalangeal and interphalangeal joints was measured, along with grip and pinch strength. The Vancouver scar scale was used to assess scar formation (total score 0-15, with higher scores indicating more severe scarring). Additionally, the pediatric quality of life inventory (total score 0-100, with higher scores indicating better quality of life, evaluated by the children themselves or by their parents/guardians on their behalf) and the satisfaction questionnaire (total score ranges from 1 to 5, with higher scores indicating greater satisfaction) were used to assess the patients’ daily quality of life and parents’satisfaction.Results:A total of 28 pediatric patients were included, comprising 15 males and 13 females, with an average age of 15 months (range: 10-36 months). All cases presented unilateral radial polydactyly, including 18 right hands and 10 left hands. The mean follow-up duration was 4.2 years (range: 1.5-6.0 years). All incisions healed in one stage, and the skin flaps survived well without infection. Joint stability was maintained without any deviation. All reconstructed thumbs demonstrated satisfactory aesthetic appearance. The reconstructed thumbs demonstrated mean active flexion-extension arcs of 76° (range 61°-85°) at the first metacarpophalangeal joint, and 42° (range 21°-65°) at the interphalangeal joint. The grip strength of the hand was 82% (63%-90%) of the normal opposite hand. The key, tip and tripod pinch strength were 76%(66%-83%), 77%(66%-85%) and 79%(68%-87%) of the normal opposite hand, respectively. No scar hyperplasia was found in all cases, and the Vancouver scar scale score was 1.2(0-3) points. The self-assessment score of the children on the pediatric quality of life inventory was 86 points, and the score of the parents/guardians was 91 points. In terms of family satisfaction, 23 patients scored 5, and 5 patients scored 4.Conclusion:The use of palmar approach to on-top plasty to treat unequal radial polydactyly can achieve functional and aesthetic results.
3.Different strategies to enhance mesenchymal stem cells in treatment of liver fibrosis:analysis of efficacy and potential risks

BACKGROUND:At present,a large number of studies have shown that mesenchymal stem cells can be combined with different strategies to more effectively improve liver fibrosis and inhibit its progression to end-stage liver disease.OBJECTIVE:To explore the mechanism of mesenchymal stem cells combined with different strategies to improve liver fibrosis compared with mesenchymal stem cells alone.METHODS:The first author used computers to search CNKI,WanFang,VIP,PubMed,Web of Science,and Nature databases involving mesenchymal stem cells combined with different strategies to improve liver fibrosis.The search terms were"mesenchymal stem cells,liver fibrosis,combination therapy,liver stellate cells"in Chinese,and"mesenchymal stem/stromal cells,liver/hepatic fibrosis/cirrhosis,combination therapy,hepatic stellate cells/HSCs"in English.By quickly browsing the title and abstract of the article,excluding the articles that are not closely related to the topic,104 articles were finally selected for review analysis.RESULTS AND CONCLUSION:Mesenchymal stem cells improve liver fibrosis by differentiating into hepato-like cells,inhibiting hepatic stellate cell activation,immune regulation,and other mechanisms.However,the low rate of liver colonization,low survival rate,and short action time of mesenchymal stem cells after transplantation limit their clinical application.Mesenchymal stem cells can improve liver fibrosis through a combination of drugs,gene modification,cytokines and other strategies,and the efficacy is better than that of mesenchymal stem cells alone.Moreover,the combination of mesenchymal stem cells and different strategies can effectively improve liver fibrosis by promoting mesenchymal stem cell homing,inhibiting hepatic stellate cell activation,regulating the microenvironment,and regulating signaling pathways.Mesenchymal stem cells can also show better liver-derived differentiation,homing and survival functions in reducing liver fibrosis through pretreatment,miRNA regulation and combination with other cells.The combination of mesenchymal stem cells and different strategies cannot avoid the potential risks of mesenchymal stem cells alone in the treatment of liver fibrosis,and the safety of these strategies(drugs,gene modifications,cytokines,etc.)is also worth considering.In addition,the number and route of mesenchymal stem cell transplantation need to be further studied.
4.Clinical and radiological follow-up analysis of posterior tibial slope reducing osteotomy combined with primary anterior cruciate ligament reconstruction in patients with steep posterior tibial slope and excessive anterior tibial subluxation

Objective To explore the clinical,radiological,and arthroscopic outcomes of anterior cru-ciate ligament(ACL)reconstruction combined with posterior tibial slope(PTS)reducing osteotomy in patients with increased PTS and excessive anterior tibial subluxation(ATS)due to primary ACL inju-ry.Methods This retrospective study included patients who underwent ACL reconstruction between 2016 and 2022 in our department.The inclusion criteria were PTS≥15° and ATS≥6 mm,and all pa-tients received ACL reconstruction combined with PTS reducing osteotomy with a follow-up of more than 2 years.Before and after the operation,they were assessed KT-1000 side-to-side difference(KT SSD),pivot shift grade,Lachman test,anterior tibial subluxation of the medial and lateral compart-ments(ASMC and ASLC),magnetic resonance imaging(MRI),and arthroscopic findings of associat-ed injuries.Moreover,graft laxity was defined as graft laxity observed on MRI or arthroscopy,KT SSD>3 mm,or pivot>1+,while graft failure was defined as graft rupture on MRI or arthroscopy,KT SSD>5 mm,or pivot>2+.Meanwhile,clinical outcomes were compared preoperatively and post-operatively,and graft status and associated injuries were described.Results A total of 30 patients were included,with an average age of 31 years,and 23%of female.The inter-observer reliability for mea-surements of PTS and ATS was greater than 0.80.Compared to preoperative values,postoperative PTS(18.2° vs.6.8°,P<0.001),KT SSD(7.8 mm vs.1.2 mm,P<0.001),ASLC(7.5 mm vs.4.5 mm,P=0.002),and ASMC(5.0 mm vs.2.7 mm,P=0.034),all showed significant improvements.In addi-tion,the proportion of patients with postoperative pivot grade of 2+to 3+(73.3%vs.6.7%,P<0.001)and Lachman grades Ⅱ-Ⅲ(70%vs.3.3%,P<0.001)reduced significantly.At the last follow-up,the incidence of graft laxity was 20%,and that of the graft failure was 13.3%.Among all pa-tients,1 patient(3.3%)had graft roof impingement or cyclops lesion,and 3(10%)suffered from concomitant cartilage injury,with 4(13.3%)undergoing intercondylar notchplasty due to intercondylar notch hyperplasia.However,no complications related to osteotomy,such as infection,non-union,or fixation failure,were observed.Conclusion Over a follow-up period of at least 2 years,for primary ACL injury patients with steep PTS and excessive ATS,combined ACL reconstruction with PTS reduc-ing osteotomy improves knee stability,but significantly reduces PTS,the incidence of severe pivot grade,anterior tibial subluxation in both medial and lateral compartments,and delays the occurrence of graft laxity and failure.
5.Analysis of medical cases and medication patterns for multiple system atrophy-orthostatic hypotension

Objective Using the Traditional Chinese Medicine Inheritance Assistance Platform V2.5 software,to analyze medical cases and prescription medication patterns of multiple system atrophy-orthostatic hypotension,and provide reference for clinical diagnosis and treatment of multiple system atrophy.Methods Relevant literature on the effectiveness of traditional Chinese medicine in treating multiple system atrophy-orthostatic hypotension was collected from China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,VIP,Chinese Medical Journal Full Text Database,and China Biomedical Literature Database.A database of medical cases and prescriptions was established on the Traditional Chinese Medicine Inheritance Assistance Platform.The software used mathematical algorithms based on association rules and entropy clustering to analyze the symptom ranking,symptom patterns,symptom clustering,and network display of medical cases,as well as the properties,taste,meridian tropism,frequency of application,and drug pair combinations of drugs in prescriptions.The core drugs were identified and the rules of diagnosis and treatment were explored.Results Through the Traditional Chinese Medicine Inheritance Assistance Platform V2.5 software,47 medical cases of multiple system atrophy were analyzed,including 73 symptoms.Among them,there were 8 symptoms with a frequency of ≥5,among which the most frequent symptom was pulse sinking,which appeared 8 times.And included 11 syndromes,the most frequent symptom was insufficient Qi syndrome,which appeared 12 times.The drug symptom network diagram,drug symptom network diagram,and drug symptom network diagram of multiple system atrophy-orthostatic hypotension medical cases were also drawn,which is the first time in the study.There are a total of 47 prescriptions for multiple system atrophy,including 106 types of drugs.Through analysis,the drugs used are mainly warm,cold,and flat,with less use of hot and cool drugs.The flavors are mainly sweet,spicy,and bitter,and the meridian tropism is mainly focused on the spleen,lungs,liver,heart,kidneys,and stomach.The top 10 drugs with the highest frequency of occurrence are Huangqi,Danggui,Shengma,Baizhu,Dangshen,Chenpi,Renshen,Chaihu,Fuzi,and Zhigancao.Among them,Huangqi has the highest frequency of occurrence,appearing 31 times,and has evolved into 66 core combinations of drugs and 3 new formulas.Conclusion The objective display of the medical case characteristics and medication features of multiple system atrophy-orthostatic hypotension through data mining has certain guiding significance for the research and clinical medication of this disease.
6.Analyzing the current status and influencing factors of occupational stress, job burnout and sleep quality of workers in the secondary industry in Jinshan District, Shanghai City

Objective To analyze the current status and influencing factors of occupational stress, job burnout and sleep quality among workers in the secondary industry in Jinshan District, Shanghai City. Methods A total of 1 418 workers from six key industries in Jinshan District, Shanghai City were selected as the study subjects by the stratified cluster sampling method. The Occupational Stress Core Scale, Maslash Burnout Inventory General Survey and Pittsburgh Sleep Quality Index were used to investigate occupational stress, job burnout and sleep quality of the workers. Results The detection rates of occupational stress, job burnout and sleep disturbance among the study subjects were 33.6%, 65.4% and 23.3%, respectively. Multivariate logistic regression analysis showed that the workers with a monthly income <5 000 yuan had a higher risk of occupational stress than those with a monthly income ≥5 000 yuan (P<0.01). The workers with ≥5.0 years of service had a higher risk than those with <1.0 year (P<0.05). Lack of physical exercise, employment in medium- and large-sized enterprises, and shift work were risk factors of occupational stress in the workers (all P<0.01). The workers aged 18-<30 years had a higher risk of job burnout than those aged 45-<60 years (P<0.05). The workers monthly income <5 000 yuan was associated with a higher risk of job burnout than those with ≥9 000 yuan (P<0.05). The workers with 1.0-<10.0 years or ≥15.0 years of service had higher job burnout risks than those with <1.0 year (all P<0.05). Being unmarried, lack of physical exercise, and employment in medium- and large-sized enterprises were risk factor of job burnout in the workers (all P<0.05). The workers with an educational level of high school or above had a higher risk of sleep disturbance than those with junior school or below (P<0.05). The workers who work >56 hours per week had a higher risk than those working ≤40 hours per week (P<0.01). Conclusion There is a high detection rate of occupational stress, job burnout, and sleep disturbance in the secondary industry workers in Jinshan District, Shanghai City. Special attention should be given to workers with low income, lack of physical exercise, employment in medium- and large-sized enterprises, shift work, long service duration, and long weekly working hours to protect their physical and mental health.
7.Natural product virtual-interact-phenotypic target characterization:A novel approach demonstrated with Salvia miltiorrhiza extract

Natural products(NPs)have historically been a fundamental source for drug discovery.Yet the complex nature of NPs presents substantial challenges in pinpointing bioactive constituents,and corresponding targets.In the present study,an innovative natural product virtual screening-interaction-phenotype(NP-VIP)strategy that integrates virtual screening,chemical proteomics,and metabolomics to identify and validate the bioactive targets of NPs.This approach reduces false positive results and enhances the ef-ficiency of target identification.Salvia miltiorrhiza(SM),a herb with recognized therapeutic potential against ischemic stroke(IS),was used to illustrate the workflow.Utilizing virtual screening,chemical proteomics,and metabolomics,potential therapeutic targets for SM in the IS treatment were identified,totaling 29,100,and 78,respectively.Further analysis via the NP-VIP strategy highlighted five high-confidence targets,including poly[ADP-ribose]polymerase 1(PARP1),signal transducer and activator of transcription 3(STAT3),amyloid precursor protein(APP),glutamate-ammonia ligase(GLUL),and glutamate decarboxylase 67(GAD67).These targets were subsequently validated and found to play critical roles in the neuroprotective effects of SM.The study not only underscores the importance of SM in treating IS but also sets a precedent for NP research,proposing a comprehensive approach that could be adapted for broader pharmacological explorations.

10.Influencing factors of health-related quality of life in patients after ileal ureteral replacement:a qualitative study

Objective To explore the influencing factors of health-related quality of life in patients after ileal ureteral replacement,and to provide a reference for formulating effective management plans.Methods Based on the conceptual model of health-related quality of life,16 patients who underwent ileal ureteral replacement in the Department of Urology of a tertiary A hospital in Beijing were selected for semi-structured interviews using descriptive research methods and purposive sampling method.The data were analyzed using directed content analysis.Results There are 6 themes and 10 sub-themes:symptom factors(distress of tube-related symptoms,distress of gastrointestinal symptoms);individual characteristics factors(obvious negative emotions,health behavior development);environmental factors(support from relatives and colleagues,lack of primary medical services);functional status factors(physical function rehabilitation,social function recovery);physiological function factors(test abnormal test index,risk of primary disease recurrence);general health perception factors.Conclusion The health-related quality of life of patients undergoing ileal ureteral replacement surgery was affected by symptoms,negative emotions,health behaviors,social support,functional status et al.In view of the above factors,medical staff should carry out effective evaluation and intervention in time,so as to improve the quality of life of patients.
