On October 9, 2024, the General Office of the National Health Commission, the General Office of the Ministry of Education, the General Department of the National Administration of Traditional Chinese Medicine, and the General Department of the National Bureau of Disease Control and Prevention jointly issued the Action Plan for Enhancing Medical Humanistic Care (2024-2027), stating that medical humanistic spirit is a concrete manifestation of humanistic spirit in the medical field. Aiming at caring for and respecting patients, this spirit reflects the attitude of medicine towards life. The cultivation of medical humanistic care competencies should serve as a core element of modern medical education, running through the entire process of the medical students’ training system and the full stage of medical workers’ career development. The humanities team of Harbin Medical University pioneered the “consistent system” concept of medical humanities education, inherited the “four-stage” with clinical practice as the primary approach, and closely focused on “three combinations, two considerations, and one assessment.” Through deeply integrating medicine with the humanities, as well as theory with practice, they have embarked on a distinctive implementation path of medical humanities, laying a solid foundation for the cultivation of high-quality medical talents with both skills and expertise in practice.

Background:
and Purpose In patients with large vessel occlusion (LVO), intravenous thrombolysis (IVT) frequently alters thrombus location; however, the clinical impact of this phenomenon remains unclear. We aimed to compare post-IVT thrombus dynamics between tenecteplase and alteplase and to evaluate the association between thrombus dynamics and 3-month outcomes.
Methods:
This retrospective study analyzed prospectively collected, multicenter data from consecutive patients with LVO who underwent bridging therapy between January 2022 and December 2024. Thrombus dynamics were classified as resolution, migration, or stability. Analyses incorporated propensity score matching with weighting to balance baseline characteristics.
Results:
Of the 806 initially included patients, 746 were included after matching (373 treated with tenecteplase and 373 treated with alteplase). The incidence of thrombus migration was significantly higher in the tenecteplase group than in the alteplase group (19.3% vs. 11.3%; odds ratio [OR]: 1.92; 95% confidence interval [CI] 1.27–2.91). The advantage of tenecteplase over alteplase was restricted to patients with an IVT-to-puncture time of <60 minutes (18.6% vs. 6.2%; p=0.001) and was no longer significant when the interval ≥60 minutes (19.7% vs. 15.0%; p=0.204; pinteraction=0.043). Additionally, thrombus migration was associated with a better functional outcome (OR: 1.62; 95% CI 1.04–2.53). Finally, tenecteplase was associated with improved functional independence compared with alteplase (OR: 1.43; 95% CI 1.04–1.95).
Conclusions
Tenecteplase demonstrated superior efficacy in inducing thrombus migration compared with alteplase, particularly within 60 minutes of IVT administration. Thrombus migration independently predicted improved functional independence. These findings support the preferential use of tenecteplase for bridging therapy in patients with LVO.

In-depth interviews were conducted with medical doctoral students engaged in animal experiments, animal laboratory management personnel, and ethics committee members, exploring the current status, challenges, and countermeasures regarding animal welfare protection after medical experiments based on NVivo 12 software. The study found that some laboratory animals still meet the conditions for continued breeding after the experiments, but face multiple challenges such as physiological health, psychological stress, adoption and placement, as well as ethical and legal issues. On this basis, this paper proposed countermeasures such as establishing adoption mechanisms, enhancing public awareness, and improving the ethical review system. This aimed to achieve a balance between the development of medical science and animal welfare, thereby ensuring that laboratory animals receive due respect and care after contributing to the cause of human health.

Chinese Medical Ethics 2026;39(10):1314-1321
ObjectiveTo systematically analyze the evolution characteristics and practical dilemmas of China’s scientific research integrity policies from 2004 to 2025, and to provide a reference for improving the governance system of science and technology ethics. MethodsBased on 53 policy documents retrieved from authoritative sources, such as the State Council policy document database, this study employed a three-stage coding process with NVivo 14, combined with word frequency analysis and policy metrology. ResultsThe policy evolution progressed through three phases regarding “partial exploration, system construction, and technological governance.” It demonstrated three characteristics, including systematic top-level design, rigid disciplinary mechanisms, and technologized supervision means. Nevertheless, the current governance system was still confronted with dilemmas such as biased institutional incentives, insufficient deterrence of disciplinary measures, and lagging technological supervision means. ConclusionThe study attempts to build a closed-loop governance paradigm of “institution-technology-culture,” deepen institutional reform to rectify incentive biases, adopt technology empowerment to promote full-process evidence preservation, and reshape academic culture to foster conscious integrity. This can systematically solve the problems of scientific research integrity anomie, thereby providing strong support for the high-quality development of scientific and technological innovation and the improvement of human well-being.

Objective To analyze the current status and influencing factors of occupational stress, job burnout and sleep quality among workers in the secondary industry in Jinshan District, Shanghai City. Methods A total of 1 418 workers from six key industries in Jinshan District, Shanghai City were selected as the study subjects by the stratified cluster sampling method. The Occupational Stress Core Scale, Maslash Burnout Inventory General Survey and Pittsburgh Sleep Quality Index were used to investigate occupational stress, job burnout and sleep quality of the workers. Results The detection rates of occupational stress, job burnout and sleep disturbance among the study subjects were 33.6%, 65.4% and 23.3%, respectively. Multivariate logistic regression analysis showed that the workers with a monthly income <5 000 yuan had a higher risk of occupational stress than those with a monthly income ≥5 000 yuan (P<0.01). The workers with ≥5.0 years of service had a higher risk than those with <1.0 year (P<0.05). Lack of physical exercise, employment in medium- and large-sized enterprises, and shift work were risk factors of occupational stress in the workers (all P<0.01). The workers aged 18-<30 years had a higher risk of job burnout than those aged 45-<60 years (P<0.05). The workers monthly income <5 000 yuan was associated with a higher risk of job burnout than those with ≥9 000 yuan (P<0.05). The workers with 1.0-<10.0 years or ≥15.0 years of service had higher job burnout risks than those with <1.0 year (all P<0.05). Being unmarried, lack of physical exercise, and employment in medium- and large-sized enterprises were risk factor of job burnout in the workers (all P<0.05). The workers with an educational level of high school or above had a higher risk of sleep disturbance than those with junior school or below (P<0.05). The workers who work >56 hours per week had a higher risk than those working ≤40 hours per week (P<0.01). Conclusion There is a high detection rate of occupational stress, job burnout, and sleep disturbance in the secondary industry workers in Jinshan District, Shanghai City. Special attention should be given to workers with low income, lack of physical exercise, employment in medium- and large-sized enterprises, shift work, long service duration, and long weekly working hours to protect their physical and mental health.

Objective:To investigate the correlation between the expression levels of breast cancer metastasis-like transcriptional repressor (BRMS1L), keratin 19 (KRT19), microRNA-101 (miR-101) in surgical resection specimens and postoperative recurrence and metastasis of differentiated thyroid cancer (DTC) .Methods:A total of 186 DTC patients were selected in Shanxi Province Cancer Hospital from Jan. 2021 to Jan. 2023. Their recurrence and metastasis status were assessed in 2 years after surgery, and they were categorized into the recurrence and metastasis group and the non-recurrence and metastasis group. The clinical data of the two groups, as well as the expression levels of BRMS1L, KRT19, and miR-101 in the surgical resection specimens, were compared. Logistic regression analysis was used to investigate the relationship between BRMS1L, KRT19, miR-101 in the surgical resection specimens and postoperative recurrence and metastasis of DTC. Receiver operating characteristic (ROC) curves and area under the curve (AUC) were used to evaluate the predictive value of BRMS1L, KRT19, and miR-101 in the surgical resection specimens for postoperative recurrence and metastasis of DTC, as well as their gain value in constructing predictive models. Results:Among 186 DTC patients were followed up for 2 years postoperatively, 4 were lost to follow-up. Of these, 137 did not experience recurrence or metastasis, while 45 did, resulting in a recurrence and metastasis rate of 24.73% (45/182). The tumor diameter, proportion of stage Ⅲ-Ⅳ, proportion of multiple lesions, and proportion of lymph node metastasis in the recurrent and metastatic group were all higher than those in the non-recurrent and non-metastatic group, while the proportion of postoperative 131I therapy was lower ( P<0.001, P=0.002, P=0.008, P<0.001). The KRT19 mRNA level in the recurrent and metastatic group was higher than that in the non-recurrent and non-metastatic group, while the BRMS1L mRNA and miR-101 levels were lower (both P<0.001). Logistic regression analysis showed that, before and after adjusting for other factors, BRMS1L, KRT19, and miR-101 were independent factors influencing postoperative recurrence and metastasis of DTC (both P<0.001). ROC curves showed that the AUCs of BRMS1L, KRT19, and miR-101 for predicting recurrent metastasis after DTC were 0.776, 0.802, 0.812, with sensitivities of 84.44%, 73.33%, and 84.44%, and specificities of 62.04%, 72.26%, and 79.56%; the R language package was used to construct a conventional prediction model consisting of clinical factors such as tumor diameter, clinical stage, lesion type, lymph node metastasis, and postoperative 131I treatment, and a new prediction model consisting of tumor diameter, clinical stage, lesion type, lymph node metastasis, postoperative 131I treatment, BRMS1L, KRT19, and miR-101. The test results indicated that both prediction models exhibited good fit ( P>0.05). The ROC curve analysis revealed that the AUCs for predicting postoperative recurrence and metastasis in DTC were 0.928 and 0.854 for the new and conventional prediction models, respectively. The sensitivity was 91.11% and 80.00%, and the specificity was 83.94% and 78.83%, respectively. The predictive value of the new prediction model was significantly better than that of the conventional prediction model ( Z=2.011, P=0.044) . Conclusions:The expression levels of BRMS1L, KRT19, and miR-101 in surgically resected specimens are closely correlated with postoperative recurrence and metastasis of DTC, serving as independent influencing factors with good predictive value. Furthermore, they exhibit significant gain value in constructing predictive models.
