1.Longitudinal Transitions of Fall States Based on a Multi-State Markov Model and Their Associated Risk Factors

Objective To investigate the transition intensity and transition probabilities of fall states among middle-aged and older adults in China,and to assess the impact of potential risk factors on falls.Methods We utilized in the study data from the China Health and Retirement Longitudinal Study(CHARLS)and employed a multi-state Markov model(MSM)to analyze the transition intensity and probabilities between states of no falls or falls without treatment,falls requiring treatment,and death.Results A total of 14722 participants were enrolled,with a mean age of(59.4 years±9.7 years),and 47.9%were male.The median follow-up period was 9 years(interquartile range[IQR].7-9 years).At baseline,12381 participants(84.1%)reported no falls or falls without treatment,while 2341(15.9%)reported falls requiring treatment.Participants who experienced falls requiring treatment within one follow-up cycle had a 55.2%probability of not falling again or only falling without treatment in the subsequent two years,a 37.6%probability of continuing to experience falls requiring treatment,and a 7.2%probability of death.The risk of transitioning from a state of no falls or falls without treatment to falls requiring treatment increased by 8.6%for every 5-year increase in age.The risk was 35.1%higher for females compared to males.Rural residents had a 10.1%higher risk.Those who were divorced,separated,widowed,or never married had a 20.7%higher risk.Higher degrees of physical function impairment were associated with an increased risk.Depressive symptoms increased the risk by 31.6%.Having one chronic disease raised the risk by 9.6%,while multimorbidity led to a 28.8%increase in risk.Conclusion According to the findings of the study,falls are a dynamic process and emphasis should be given to fall prevention for older adults,individuals with a history of fall-related medical visits,those living alone,those with impaired physical function,and those with depressive symptoms.
2.Analysis on positioning errors and adverse reactions of breast cancer

Objective To investigate the factors influencing the positioning error of conical beam CT(CBCT)radiotherapy and the occurrence of acute radiation dermatitis in breast cancer fixed by cervical and thoracic integrated membrane in order to provide a basis for personalized out-target external boundary,selec-tion of fixed mode and guidance of radiodermatitis clinical practice work.Methods A total of 147 patients with breast cancer in this hospital from June 2022 to October 2023 were selected as the study subjects,inclu-ding 74 cases of breast conserving operation and 73 cases of modified radical(excision)operation.During the treatment period,CBCT scanning once a week obtained the image and conducted the grey match with the planned CT images to acquire the translational error of X(left and right),Y(head and foot)and Z(abdominal and dorsal)and the rotational posing error of X(pitch),Y(roll)and Z(yaw)axis,and at the same time,the acute radiodermatitis in nearly 3 months during radiotherapeutic period was recorded.Results Compared with the Z axis,the translation error of X axis and Y axis was greater,the rotation error was smaller,and the differ-ences were statistically significant(P<0.05).The PTV external boundary of X,Y and Z axis calculated by the formula was 0.70,0.76,0.46 cm respectively.There was no statistically significant difference in the posi-tioning error of each direction of left and right breast between breast conservation operation and breast resec-tion operation(P>0.05).The correlation between the age with X,Y and Z axis translation errors was-0.14,0.18 and 0.10 respectively,which of the age with X,Y and Z axis rotation errors was 0.12,<0.01,0.02 respectively.The occurrence of grade 1 acute radiodermatitis in the patients with breast conserving oper-ation was 100.0%(74/74),which in the patients with excision operation was 13.7%(10/73),the occurrence of grade 2 was 82.2%,which of grade 3 was 4.1%(3/73),and the difference between them was statistically significant(P<0.05).Conclusion In the every day positioning and target area external expansion,particular attention should be paid to the X and Y axis translational directions and the Z axis rotation direction.Using the integrated cervicothoracic thermoplastic membrane for fixing breast cancer is not affected by the operation mode,affected side position and age.
3.Effective Ingredients of Chinese Medicine in Prevention and Treatment of Osteoarthritis by Regulating Oxidative Stress: A Review

Osteoarthritis (OA) is a common degenerative joint disease with a rising incidence rate year by year. Treatment often relies on analgesics and non-steroidal anti-inflammatory drugs (NSAIDs), which can lead to gastrointestinal damage with long-term use and the recurrence of symptoms. Chinese medicine has a long history of preventing and treating OA, with widespread application and fewer side effects. It offers unique advantages such as a broad treatment scope, multiple targets, and pathways. The effective components of Chinese medicine can reduce the content of reactive oxygen species (ROS), relieve oxidative stress (OS) damage, and increase the antioxidant capacity of the body by interfering with the expression of biomarkers of OS response such as malondialdehyde (MDA) and superoxide dismutase (SOD). Through the modulation of signaling pathways such as nuclear factor E2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1), phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt), nuclear factor kappa B (NF-κB), c-Jun N-terminal kinase (JNK), NOD-like receptor protein 3 (NLRP3), and osteoprotegerin (OPG), they downregulated the expression of inflammatory factors such as interleukin-6 (IL-6), interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α), thereby effectively relieving local joint inflammation, protecting chondrocytes and bone tissue, inhibiting chondrocyte apoptosis, and further alleviating the progression of OA. Currently, there are still certain limitations in the medical research status and development trends of OA, necessitating the continued advancement of traditional Chinese medicine. This paper reviewed the literature on the regulation of OS response by effective components of Chinese medicine for the prevention and treatment of OA, providing new directions and ideas for future research.
4.Application of whole blood thromboelastogram and its correlative kit for plasma thromboelastogram detection

【Objective】 To study the feasibility of whole blood thromboelastogram (WB-TEG) and its correlative kit for plasma thromboelastogram (P-TEG) detection and the characteristics of P-TEG in healthy subjects. 【Methods】 17 healthy volunteers were detected by WB-TEG instrument and its correlative kit, and the results were compared with those by P-TEG. The P-TEG characteristics of 17 healthy volunteers were analyzed. Three groups (7 cases/group)of plasma samples with different platelet (Plt) count and the other three groups of plasma(7, 6 and 4 cases, respectively) with different fibrinogen(Fib) concentration were tested for P-TEG. The effects of Plt and Fib on P-TEG detection were observed. 【Results】 There was no significant difference in R and MA value (P>0.05)as WB-TEG was compared with P-TEG in healthy subjects, while in K(min) (1.71±0.47 vs 1.07±0.45), A(°) (66.1±5.41 vs 75.59±5.77), and CI value (0.9±1.8 vs 2.52±2.58)(all P <0.05). Various parameters of healthy individuals were basically within the range of 95% CI of WB-TEG, but there were significant diffferences in K, A and CI value(P<0.05). When Plt count (×1011/L) was≥2.5 in plasma, the MA value of P-TEG was significantly extended than that of normal individuals(P<0.05); when Plt count (×1011/L) was 6.0 ~12.0, the MA and CI value of P-TEG significantly decreased(P<0.05). When Fib(g/L) was 6.4~6.91 in plasma, the R and K value of P-TEG were prolonged, but A, MA and CI value all decreased(P <0.05); when Fib(g/L) <1, the A and MA value significantly decreased(P<0.05), and K and CI value could not be detected. 【Conclusion】 The WB-TEG and its correlative kit can be used in P-TEG detection, and corresponding reference values of TEG parameters should be established in combination with the conditions of laboratories.
5.Effect of sildenafil on cardiovascular patients with erectile dysfunction:A review of recent researches

OBJECTIVE: To summarize the development of researches on sildenafil for treating cardiovascular patients with erectile dysfunction, so as to explore the safety of applying sildenafil and efficacy of treating related cardiovascular diseases.DATA SOURCES: The relevant articles to sildenafil between October 1999 and February 2006 were computer-searched in PubMed databasewith the key words of "sildenafil, erectile dysfunction, sexuality, cardiovascular disease" in English. Meanwhile, Medscape database was scanned with computer to identify relevant articles with key words of "sildenafil, cardiovascular disease". STUDY SELECTION: After the primary searching, literatures on relationship between sildenafil and cardiovascular disease were searched in full-text, while articles about erectile dysfunction but had nothing to do with cardiovascular disease were excluded. The repetitive experimental studies were also deleted. DATA EXTRACTION: A total of 55 articles were collected, and 22 of them were in accordance with the inclusion criteria and their full-texts were carefully looked through. DATA SYNTHESIS: The selected data were classified according to the relationship between erectile dysfunction and cardiovascular disease, effect of sildenafil on erectile dysfunction of patients with heart disease and rec ommendation of the Princeton conference. CONCLUSION: It is demonstrated in many literatures that the potential risk of sildenafil triggering cardiovascular disease is relatively small. Cardiac patients need not promote huge concerns when resorting to sildenafil for erectile dysfunction treatment. However, the protective cardiac effects of sildenafil should be further investigated.
6.Gender Differences on Echocardiography in Essential Hypertensives

Objective To study the gender differences in echocardiography in essential hypertensives. Methods Echocardiography measurement was performed in 108 subjects with Grade 1-2 essential hypertension (52 in female patient group, PGf, and 56 in male patient group, PGm). Forty two normotensive subjects (20 in female control group, CGf, and 22 in male control group, CGm) were as controls.Data were obtained by averaging measurements of the traced heart chambers and velocity curves in 5 cardiac cycles. Results Compared with PGm, the following echocardiographic features in PGf were showed:LAID (37.24±5.88 vs 32.14±3.80)mm,P<0.01] and MVa[(84.18±12.13 vs 81.71±12.30)cm/s, P<0. 05] were greater; LVMI [ ( 119.26 ± 22.33 vs 128.17 ± 27.00 ) g/m2 , P<0. 05], EF ( 75.13 % ±6.69% vs 83.00% ±3.68%,P<0. 01), FS (41.67% ±7.99%0 vs 49.03% ± 7.35%, P<0.01), MVe[(68. 28±8.66 vs 73. 73±11.46)cm/s, P<0. 05] and MVe/a(0.83±0. 08 vs 0. 93±0.11, P<0.01)were lower. The differences between CGf and CGm were not significant. Conclusion There are echocardiographic differences between sexes in hypertensives including cardiac structural and functional changes. Hypertensive woman is more susceptible to both cardiac structure damage and cardiac dysfunction.
