Rodent models play a crucial role in research on human periodontal diseases,providing key evidence for investigation into the pathological mechanisms of periodontal bone defects.Relevant research in the field involves gene expression,inflammatory regulation mechanisms,host-microbial interactions,as well as disease resolution and healing processes.Research methodology in the field falls under 2 categories-periodontal inflammation models and surgical defect models.The former simulates periodontal defects by inducing periodontal diseases,while the latter constructs clinically simulated periodontal defects through surgical removal of periodontal tissue.However,the currently available animal models of periodontitis face challenges in simultaneously capturing the disease complexity,tracking dynamic repair processes,and meeting translational needs.Herein,we reviewed and summarized the methods and characteristics of periodontal disease modeling in recent years.We proposed the establishment of a multimodal assessment framework integrating technologies such as spatial transcriptomics,single-cell sequencing,and in vivo fluorescence imaging,which may serve as a critical pathway for overcoming existing research challenges.

Objective To discuss the perioperative anesthesia management strategy for the surgical resection of rec-tal cancer in a patient with a rare neuromuscular disease-Facioscapulohumeral Muscular Dystrophy(FSHD)compli-cated by restrictive ventilatory dysfunction.Methods Clinical data of a patient with FSHD complicated by moder-ate-to-severe restrictive ventilatory dysfunction undergoing rectal cancer surgery were retrospectively collected;the clinical manifestations,complication,preoperative evaluation,intraoperative anesthesia management and postopera-tive pain treatment were analyzed and summarized.Results FSHD is a rare genetic disorder,and preopera-tive multidisciplinary evaluation is critical.In this case,total intravenous anesthesia was employed,with invasive arterial pressure monitoring,blood gas analysis,body temperature,sufficient analgesia,close respiratory monitoring and lung protective ventilation strategy after preoperative multidisciplinary evaluation.After thorough sputum suction,lung expansion,and complete recovery of muscle strength,the patient was successfully extubated;ensuring respiratory monitoring after surgery,sufficient analgesia was administered,and transferred to the ICU for monitoring,and ultimately discharged with satisfactory treatment results.Conclusions For patients with rare neuro-muscular diseases such as FSHD,thorough preoperative evaluation and optimization are essential.Clinicians should be aware of related complications,such as restrictive ventilatory dysfunction,and develop individualized anesthesia plans.Intraoperative monitoring,particularly of the respiratory and hemodynamic systems,should aim to prevent hypoxia and carbon dioxide retention,thereby reducing the risk of complications.

Objective To explore the relevant relationship and specificity between the implicit and explicit loyalty of military cadets in order to provide a theoretical basis and objective indicators for a more comprehensive and objective assessment for individual loyalty.Methods E-Prime 2.0,a classic implicit association paradigm was employed to construct an implicit association loyalty test for 64 military cadets.Simultaneously,an explicit loyalty measurement was conducted using the Chinese Military Personnel Loyalty Scale.Results ① Significant implicit effect was observed in the loyalty assessment of military cadets,indicating a general tendency to perceive higher levels of personal loyalty and lower levels of loyalty to external entities.② Explicit loyalty assessment revealed that the participants had the highest loyalty score towards the Party,the Nation,and the People(4.79±0.34),followed by the loyalty score to their profession(4.38±0.53),and the relatively lower loyalty score towards the unit and leaders(4.03±0.83).Among the 3 dimensions of loyalty,the normative loyalty score ranked highest,while continuance loyalty score took lower.③ There were no correlations among the scores of loyalty to the Party,the Nation,and the People(r=-0.030,P=0.823),to the profession(r=-0.047,P=0.728),to the unit(r=0.050,P=0.710),or to the leaders(r=0.043,P=0.749).Conclusion The implicit effect in the loyalty assessment is significant in military cadets,and there is no significant correlation between explicit and implicit loyalty measurements.Thus,we cannot rely solely on explicit measurements to assess their loyalty attitudes.

To analyze the correlation between preoperative hypoproteinemia and the risk of perioperative allogeneic erythrocyte transfusion in ovarian cancer patients undergoing cytoreductive surgery.

BACKGROUND: Vaccination has been shown effective in controlling the global coronavirus disease 2019 (COVID-19) pandemic and reducing severe cases. This study was to assess the flare and change in disease activity after COVID-19 vaccination in patients with stable rheumatoid arthritis (RA). METHODS: A prospective cohort of RA patients in remission or with low disease activity was divided into a vaccination group and a non-vaccination group based on their COVID-19 vaccination status. Each of them was examined every 3 to 6 months. In the vaccination group, disease activity was compared before and after vaccination. The rates of flare defined as disease activity scores based on 28-joint count (DAS28) >3.2 with ΔDAS28 ≥0.6 were compared between vaccination and non-vaccination groups. RESULTS: A total of 202 eligible RA patients were enrolled. Of these, 98 patients received no vaccine shot (non-vaccination group), and 104 patients received two doses of vaccine (vaccination group). The median time interval from pre-vaccination visit to the first immunization and from the second dose of vaccine to post-vaccination visit was 67 days and 83 days, respectively. The disease activity scores at pre-vaccination and post-vaccination visits in the vaccination group patients were similar. At enrollment, gender, RA disease course, seropositivity, and disease activity were comparable across the two groups. Flare was observed in five (4.8%) of the vaccination group patients and nine (9.2%) of the non-vaccination group patients at post-vaccination assessment ( P  = 0.221). In terms of safety, 29 (27.9%) patients experienced adverse events (AEs) after vaccination. No serious AEs occurred. CONCLUSIONS COVID-19 vaccinations had no significant effect on disease activity or risk of flare in RA patients in remission or with low disease activity. Patients with stable RA should be encouraged to receive the COVID-19 vaccination.

Objective:To compare the patient-centered doctor-patient communication skills in anesthesiologists with different working experience and genders.Methods:The second and third year residents who had been trained as residents in the Department of Anesthesiology in our hospital from September 2017 and September 2018 and senior physicians who had worked for 10-15 yr in our hospital, were recruited to perform a preoperative interview in a standardized patient counter.Consultation and Relational Empathy (CARE) measure was used to assess the communication skills among senior physicians, residents, and anesthesiologists of different genders.The residents also took the objective structured clinical examination (OSCE) and theory examination.Results:Thirty-six residents and 20 senior physicians were included.There was no significant difference in the CARE measure score between residents and senior physicians and in the total CARE measure score between residents who received doctor-patient communication training at the stage of medical students and those who did not ( P>0.05). Compared with the residents in the second year, the listening ability score and decision-making ability score were significantly decreased ( P<0.05), and no significant change was found in the scores for the other items in the residents in the third year ( P>0.05). Compared with residents, the listening ability scores were significantly increased, and the caring ability scores were decreased ( P<0.05), and no significant change was found in the scores for the other items in senior physicians ( P>0.05). There was no difference in CARE measure scores between the male and female anesthesiologists.The resident′s CARE measure score was positively correlated with the annual OSCE score ( r=0.486, P<0.05), and there was no correlation between the resident′s CARE measure score and annual theoretical examination grade ( r=0.308, P>0.05). Conclusion:Senior anesthesiologists with more clinical experience are not superior to the junior residents in patient-centered communication skills, and the doctor-patient communication skills of residents are not related to their medical theoretical grade.Moreover, there is no significant difference between male and female anesthesiologists in terms of doctor-patient communication skills before surgery.Therefore, training of patient-centered doctor-patient communication skills should be strengthened at all levels of anesthesiologists.

Objective:To investigate the viral molecular mutations of 2019 novel coronavirus (2019-nCoV) and host adaptability in Suzhou City.Methods:The throat swab specimens from nine local cases and six imported cases with 2019-nCoV viral nucleic acid test positive in Suzhou City were sequenced for the whole genome of the virus, and the Wuhan-Hu-1 strain was used as the reference sequence for alignment and analysis. The phylogenetic tree of the viral whole genome sequence was constructed by MEGA 7.0 software.Results:According to the Chinese typing method, Nextstrain typing method, Pangolin classification method and Global Initiative on Sharing All Influenza Data (GISAID) typing method, the 15 2019-nCoV genome sequences could be divided into seven types, six types, eight types and five subtypes, respectively. Compared with Wuhan-Hu-1 strain, the median number of amino acid sequence mutation sites based on nucleotide translation was three (ranging from 0 to 12). D614G mutation of spike protein was identified from all six imported viral strains, which could enhance the transmissibility. No Alpha, Beta, or Gamma mutants, which also could enhance the transmissibility, was found in the genomic sequences of the imported cases. The median number of nucleotide mutation sites in 15 sequences was eight (ranging from three to 23).Conclusions:2019-nCoV is constantly mutating, and a variety of evolutionary lineages/genotypes have been derived. All imported viruses in Suzhou City carry mutations that can increase infectivity.

Objective:To disclose the epidemiological characteristics of the first human case of H9N2 subtype avian influenza and genetic characteristics of hemagglutinin (HA) gene of H9N2 avian influenza virus in Jiangsu province.Methods:Reverse transcription polymerase chain reaction (RT-PCR) assay was used to detect the nucleic acid of influenza virus A/B and various subtypes from throat swab specimens of patients with acute and severe respiratory infections of unknown cause. Sanger method was used to sequence the vial HA gene of H9N2 positive specimens. N-J phylogenetic tree of HA gene was constructed by using MEGA X software.Results:The first laboratory confirmed case of human infection with H9N2 avian influenza virus in Jiangsu province in March 2019 was reported by Suzhou Center for Disease Control and Prevention. Sequence analysis of the HA gene showed that the A/Suzhou/S683/2019 (H9N2) strain belonged to Y280-like sub-lineage of the Eurasian lineage, which had 99.23% nucleic acid identity with A/chicken/Shanghai/S1171/2018 (H9N2) strain. The amino acid sequence of the HA protein cleavage site is PSRSSR↓GLF, and the key amino acid in receptor binding site shared Q226L mutation. The location of potential glycosylation sites of HA protein of A/Suzhou/S683/2019 (H9N2) have changed.Conclusions:The H9N2 avian influenza virus obtained from the first case in Jiangsu province has low pathogenicity, but it may have acquired part of the ability to adapt to human hosts.

Objective To explore the risk factors of urate deposition on ultrasound in patients with gout.Methods All the gout patients who visited our center between February 2015 and February 2017 and underwent ultrasound examination of bilateral knees,ankles and first metatarsophalangeal joints (MTP1) were enrolled.Subgroup analysis was done depending on whether double contour sign (DCS) or tophus was found on ultrasound.Main statistical analysis methods were t test,chi-square test and logistic regression model.Results One hundred and twenty-six patients were included.DCS was found in 50(39.7％) patients and tophus was found in 48 (38.1％) patients.The serum uric acid (SU) level of the DCS positive group was signi-ficantly higher than the DCS negative group [(602±79) μmol/L vs (538±101) μmol/L,t=3.998,P=0.044].The hyperuicemia duration of the two groups were (186±87) months and (130±77) months,which was significantly different (t=3.330,P=0.002).The hyperuicemia duration of the tophus positive group was significantly higher than tophus negative group [(175±102) months vs (138±96) months,t=2.003,P=0.045].The SU level and hyperuicemia duration were independent risk factors of positive DCS in gout patients [OR =1.006,95％ CI (1.002,1.01 1);OR=1.028,95％CI (1.013,1.042)].The hyperuicemia duration was independent risk factor of positive tophus in gout patients [OR=1.004,95％CI (1.000,1.007)].Receiver operating characteristic curve (ROC) curve showed gout patient whose hyperuricemia duration was longer than 94months and SU level was higher than 505.5 μmol/L were more likely to have positive DCS in joints;meanwhile,patient whose hyperuricemia duration was longer than 137 months were more likely to have positive tophus in joints.Conclusion Gout patients who have positive DCS and tophus on ultrasound have longer hyperuicemia duration.Positive DCS is also related with patients' higher serum levels.The hyperuicemia duration is an independent risk factor of urate deposition on ultrasound in patients with gout.

Objective To explore the impact of dyslipidemia on uric acid stones by analyzing the relationship between blood lipids and urolithiasis in primary gouty patients.Methods We retrospectively identified patients with nephrolithiasis in primary gout patients who underwent stone chemical analysis,compared with gout patients without nephrolithiasis.The clinical parameters,urine analysis and lipid levels were analyzed.Patients were divided into groups based on serum lipid levels.The groups were compared based on demographic data and stone composition.Correlations were analyzed between serum lipid,urinary pH and uric acid stones.Moreover,the risk factors of uric acid stones were determined by logistic regression analysis.Analysis of variance,t-test,chi-square test,Spearman's test and Logistic regression were used for statistical analysis.Results ① A total of 144 gout patients were included in study,48 patients with urolithiasis and 96 patients without urolithiasis.② Serum lipid levels were significantly lower in urolithiasis group than those patients without urolithiasis including triglyceride (TG) [1.6(0.9,2.1) mmol/L vs 2.2(1.4,3.2) mmol/L,Z=2.38,P=0.01],total cholesterol (TC) [(4.4±1.2) mmol/L vs (5.1±1.0) mmol/L,t=5.3,P=0.006];low density lipoprotein cholesterol (LDL-C) [(2.5±0.9) mmol/L vs (3.2±0.9) mmol/L,t=4.2,P=0.005].③ Compared to oxalate stone formers,uric acid stone formers had significantly higher TG [(1.8±0.6) mmol/L vs (0.9±0.5) mmol/L,t=4.9,P=0.001),TC [(4.4±1.1) mmol/L vs (3.8±1.0) mmol/L,t=1.8,P=0.001] and LDL-C [(2.8±0.9) mmol/L vs (2.0±0.7) mmol/L,t=3.5,P=0.045],while the high density lipoprotein (HDL) level was lower [(0.94±0.23) mmol/L vs (1.32±0.41) mmol/L,t=-4.0,P=0.002].④ Percentage of uric acid stones in high TG group was higher than normal TG group [85％(17/20 vs 46.4％(13/28),x2=7.4,P=0.007],in addition,the percentage of uric acid stones in low HDL group was higher than normal HDL group [(82.1％(23/28) vs 35.0％(7/20),x2=11.1,P=0.001].⑤ Uric acid stones were significantly correlated with high TG,low LDL and urinary pH(r=0.522,0.47,-0.212,respectively).Logistic analysis showed risk factors for uric acid stone in primary gouty patients were high TG [OR=2.38,95％CI(1.41,13.7);P=0.01] and lower HDL level [OR=0.01,95％CI(0.01,0.43);P=0.01].Conclusion There is a link between dyslipidemia and kidney uric acid stone risk in primary gout patients.Specific alterations in patient's lipid profile may portend unique aberrations in urine physico-chemistry and uric acid stone risk.
