Based on the principles of ergonomics, simulating the structure of the human skeleton,an exoskeleton device was designed that can effectively transmit the gravity of the lead coat to the ground from all parts of the human body, which can transfer the weight of the lead clothing without affecting the operation of the interventional doctor and ensuring the doctor's activity,so as to reduce the weight borne by the surgeon and alleviate occupational fatigue. The lead clothing support exoskeleton device included an upper support part and a lower support part. The upper support part and the lower support part were connected through a lumbar support frame, taking into account the advantages of practicality, wearing portability, lightweight, joint flexibility etc..The wearer can easily perform actions such as leg lifting, turning, bending over, etc.,as well as sit down and rest without removing lead or protective clothing. From the perspective of clinical interventional doctors, the device skillfully applies the mechanical support structure to realize the effective conduction of gravity, reduce the burden of doctors, increase the comfort of the wearer and improve work efficiency, which has practical and clinical application value.

Objective To analyze the risk factors for extrauterine growth retardation(EUGR)in late preterm infants appropriate for gestational age. Methods The clinical data in 1 402 preterm infants appropri-ate for gestational age delivered and hospitalized in this hospital from January 2016 to June 2022 were analyzed retrospectively. They were divided into the EUGR group(n=244)and the non-EUGR group(n=1 158)ac-cording to whether or not the body weight at discharge was below the 10th percentile of the growth curve for the same gestational age at the same period based on the Fenton's preterm growth curve. The clinical data of preterm infants and mothers of the two groups were collected. The risk factors for EUGR occurrence in pre-mature infants were analyzed. Results Among 1 402 preterm infants appropriate for gestational age,EUGR occurred in 244 cases with the EUGR incidence rate of 17.4%.The EUGR incidence rate had no statistical difference among the different fetal ages of premature infants(P>0.05).The EUGR incidence rate had sta-tistical difference among different birth weights of premature infants(P<0.05).The logistic regression anal-ysis showed that male(OR=1.694,95%CI:1.144-2.507),low birth weight(OR=0.989,95%CI:0.988-0.991),feeding intolerance(OR=2.719,95%CI:1.234-5.990),short gestational weeks(OR=0.146,95%CI:0.103-0.207)and hospitalization duration extension(OR=1.073,95%CI:1.031-1.117)were the risk factors for EUGR occurrence in late premature infants appropriate for gestational age in discharge. The sub-group analysis showed that male, low birth weight, feeding intolerance and hospitalization duration extension were the risk factors for EUGR occurrence in the preterm infants with gestational ages of 34-<36 weeks(P<0.05).Low birth weight and feeding intolerance only affected the preterm infants≥36 weeks of gesta-tional age(P<0.05).Conclusion Strengthening the pregnant duration health care and active nutritional sup-port after birth may reduce the risk of EUGR occurrence in late premature infants.

Journal of Clinical Hepatology 2020;36(9):2083-2086
Exosomes, as a type of multivesicular bodies secreted by various cells in human body, exist in many kinds of body fluids in the body. Exosomes have the structure of lipid bilayer membranes to protect the structure and function stability of their contents and they carry a large number of active substances including microRNA (miRNA) and mRNA. Studies have shown that exosomes and their miRNA are closely associated with the development and progression of liver fibrosis and regulate such process by acting on hepatic stellate cells to change their activation, proliferation, apoptosis, and migration. This article elaborates on the biological characteristics of exosome miRNA and its association with liver fibrosis, analyzes its important role in the development and progression, diagnosis and treatment, and prognostic evaluation of liver fibrosis, and points out that exosome miRNA may become a new potential target for the diagnosis, treatment, and prognostic evaluation of liver fibrosis.

Objective To investigate the prevalence of iron deficiency(ID)and iron deficiency anemia (IDA) in pregnant women in urban areas of China. Methods The study was a national cross-sectional survey conducted from September 19th, 2016 to November 20th, 2016. According to the classification of the National Bureau of Statistics, all survey sites were set up in 6 regions of the country. Pregnant women were continuously selected using multistage stratified sampling. A total of 12 403 pregnant women were collected and examined for serum ferritin and hemoglobin levels. Results The median serum ferritin level during pregnancy was 20.60 μg/L(11.78-36.98 μg/L), the hemoglobin level was(118±12)g/L. With the progress of pregnancy, the levels of serum ferritin and hemoglobin decreased gradually. The median serum ferritin levels in the first, second trimester and third trimester were 54.30 μg/L(34.48-94.01 μg/L), 28.60 μg/L(16.40-50.52 μg/L), and 16.70 μg/L(10.20-27.00 μg/L)respectively(P<0.01). The mean hemoglobin levels were(127 ± 10)g/L,(119 ± 11)g/L and(117 ± 11)g/L respectively(P<0.01). The prevalence of ID in urban pregnant women was 48.16%(5 973/12 403), and IDA prevalence was 13.87% (1 720/12 403). The prevalence of IDA in the first, second trimester and third trimester were 1.96% (20/1 019), 8.40%(293/3 487)and 17.82%(1 407/7 897), respectively(P<0.01). The prevalence of standardized ID and IDA were significantly different in various regions of China(P<0.01). The standardized prevalence of ID were relatively higher in East China and Northeast China, 57.37% and 53.41% respectively, while it was the lowest in Southwest China, 30.51%. The standardized prevalence of IDA in South Central, Northwest, and East China were relatively high, 21.30%, 16.97% and 17.53% respectively, and the standardized prevalence of IDA in Southwest China was the lowest, 5.44%,the differents in various regions were significant(all P<0.01). Conclusion The current phenomenon of ID and IDA in pregnant women is still very common, and nutrition and health care during pregnancy should be strengthened.

Objective To explore the efficacy and safety of double dosage of SHUGANJIEYU capsule on mod?erate depression. Methods The study was conducted by a multicenter, random, double-blind and parallel-controlled trial. Trial group received SHUGANJIEYU capsules 4# twice a day. Control group got two SHUGANJIEYU capsules and two simulated ones twice a day. The study lasted 56 days (8 weeks) for both groups. Primary efficacies were evalu? ated by the total score and score changed rate of Hamilton depression scale-17 (HAMD-17)and Hamilton anxiety scale (HAMA) as well safety by reported adverse events and laboratory tests for patients. Results One hundred twenty patients were recruited in trial group and 120 patients in control group. After a 8-week treatment, the remission ratio was 84.2% and 63.3% for the trial group and the control group, respectively according to the HAMD-17. The differ?ence in the remission ratio was significant (P<0.05). Furthermore, the reduction rate of the total scores of HAMD-17 was 35.4% and 27.1% for the trial group and the control group at the 14th day of treatment, respectively, whereas 79.0%and 65.9%for the trial group and the control group at the 56th day. The reduction ratio was significantly higher in trial group than control group (P<0.01). The median time to response was 14 days in both groups. Based on the HA?MA, the remission ratio was 93.3%and 77.5%for the trial group and the control group respectively after 8 weeks and the difference in the remission ratio was significant between the two groups (P<0.05). Incidences of adverse effect were not significantly different between the two groups (17.5%vs. 11.7%, P>0.05). Conclusion SHUGANJIEYU capsule ex?hibits a good therapeutic effect on the depressive symptoms and anxiety symptoms in patients with moderate depres?sion. Double doses of SHUGANJIEYU capsule can increase the effect of SHUGANJIEYU and accelerates SHUGAN?JIEYU-induced improvement of depression symptoms with an acceptable safety.
