The shortage of emergency and critical care resources has become increasingly prominent,seriously reducing the quality and safety of care.How to improve the efficiency of the emergency and critical care platform is an urgent problem to be solved.Since 2020,the emergency department of Peking University Third Hospital has achieved an increase of 10%-20%in the annual visits of emergency and critically ill patients,the reduction of the emergency department length of stay and the improvement of survival rate using Objectives and Key Results(OKR)as an advanced management tool.It provides a new paradigm for improving efficiency of emergency department in large general hospitals.

Objective:To provide references for improving the psychological support to fighter pilots by the investigation on their emotional states and typical negative emotions in different task stages and common causes during major tasks.Methods:The Chinese version of Brunel Mood Scale was used to evaluate the emotional states of 141 high performance fighter pilots in the stages of assignment, pre-task, in-task and post-task. The scores of the scale and the positive rate of the evaluation were analyzed by descriptive statistics. Combined with the on-the-spot interview of pilots′ actual experience, the causes of negative emotions were discussed.Results:The negative emotions with the score greater than 1 were "exhausted" "worried" "uncertain" "extreme fatigue" "anxious" "tired" "nervous". The positive rate of pilots′ self-evaluation of 7 emotions was more than 80%. There were 2-5 typical negative emotions in each stage of the task, which were mainly caused by 3 factors: task adaptation, interpersonal communication and family. The scores of 3 positive emotions were more than 2.8, and the positive rate of pilots′ self-assessment was 100%. Positive emotions were higher in the first 2 stages, decreased significantly in the task stage and increased slightly after the task.Conclusions:The apparent negative emotions exist among the high performance fighter pilots in major tasks. The corresponding regulations and protections should be applied in time in the form of psychological intervention, especially on the maintenance of task adaptation, interpersonal communication and family harmony.

Methods: The expressions of GTF2IP23 and GTF2I were detected in 40 cases of invasive breast cancer tumors and their counterparts by using quantitative real-time polymerase chain reaction (qRT-PCR). The effects of GTF2IP23 on the expression of GTF2I gene and cell proliferation and migration were analyzed by overexpression of GTF2IP23 in breast cancer cells. Results: The expression of GTF2IP23 mRNA in breast cancer tissues was significantly higher than that in adjacent tissues (P<0.001), while the expression of GTF2I mRNA was significantly lower than that in adjacent tissues (P=0.007). The expression of GTF2IP23 was negatively correlated with GTF2I (r=-0.335, P=0.025). The expression of GTF2IP23 in breast cancer cells was significantly higher than in normal breast cells (P<0.01), while GTF2I expression in breast cancer cells was significantly lower than that in normal breast cells (P<0.01). Overexpression of GTF2IP23 in ZR-75-30 cells significantly reduced the expression of GTF2I (P=0.034) and enhanced cell proliferation (P=0.017) and migration (P=0.026) capacity. Conclusions GTF2IP23 is distinctly upregulated in breast cancer, it inhibits the expression of real gene GTF2I and promotes the proliferation of breast cancer cells.

OBJECTIVE:To evaluate the effect of clinical antibiotics use special rectification in our hospital. METHODS:100 discharged medical records of typeⅠincision surgery were randomly sampled from our hospital during May in 2010 to Apr. in 2011,May in 2011 to Apr. in 2012,May in 2012 to Apr. in 2013,May in 2013 to Apr. in 2014,totaling 400 records. And then evaluation indicators were analyzed statistically,such as antibiotics use intensity,perioperative DDDs of antibiotics in typeⅠinci-sion surgery,DUI,types of antibiotics during perioperative period,medication time,etc. RESULTS:Since the implementation of clinical antibiotics use special rectification in May 2011,the utilization ratio of antibiotics in typeⅠincision surgery of our hospital decreased from 96% to 33%;DUI decreased from 1.44 to 0.79;while reasonable rate of drug selection increased from 19.8% to 100%,and that of medication time increased from 43.8% to 100%. CONCLUSIONS:Rational medication evaluation indicators in typeⅠincision surgery of our hospital have been improved after the implementation of clinical antibiotics use special rectification.
