ObjectiveTo observe the clinical efficacy and safety of Zhuyuwan in the treatment of hyperlipidemia. MethodsIn this study, hyperlipidemia patients treated in the Hospital of Chengdu University of Traditional Chinese Medicine (TCM) from September 2022 to December 2023 were randomly assigned into a control group and an observation group. Finally, 162 valid cases were included, encompassing 74 cases in the control group and 88 cases in the observation group. The control group was treated with atorvastatin calcium tablets, and the observation group with atorvastatin calcium tablets + Zhuyuwan extract granules. Both groups were treated for 8 weeks. The efficacy in terms of blood lipid level recovery, blood lipid levels, TCM syndrome distribution, efficacy in terms of TCM syndrome, and TCM symptom scores were compared between the two groups as well as between before and after treatment. Liver and kidney functions were monitored for safety assessment. ResultsIn terms of blood lipid level recovery, the total response rate in the observation group was 86.36% (76/88) and that in the control group was 86.49% (64/74), with no statistically significant difference between the two groups. After treatment, both groups showed declines in levels of triglyceride (TG), total cholesterol (TC), and low-density lipoprotein cholesterol (LDL-C) (P<0.05) and elevations in the level of high-density lipoprotein cholesterol (HDL-C) (P<0.05). Moreover, the observation group outperformed the control group in recovering the levels of TG, LDL-C, and HDL-C (P<0.05, P<0.01). In terms of TCM syndrome, hyperlipidemia was mostly caused by phlegm turbidity and obstruction. The total response rate in terms of TCM syndrome in the observation group was 87.30% (55/63), which was higher than that (63.46%, 33/52) in the control group (χ2=9.102, P<0.01). After treatment, the scores of total TCM symptoms, primary symptoms, and secondary symptoms decreased in both groups (P<0.05), and the observation group had lower scores than the control group (P<0.01). The observation group was superior to the control group in alleviating obesity, chest tightness, and low food intake (P<0.05). In terms of safety, the level of aminotransferase was slightly elevated in the control group, and no obvious adverse reaction was observed in the observation group, with no statistical significance in the incidence of adverse reactions. ConclusionZhuyuwan combined with atorvastatin can not only recover blood lipid levels and alleviate TCM symptoms but also reduce the occurrence of adverse reactions.

Based on the holistic view of "spleen-vessels-heart-spirit" system, this article explores the pathogenesis and progression of atrial fibrillation. It is proposed that the onset of atrial fibrillation is due to failure of the spleen to transport and disharmony of blood vessels; phlegm and blood stasis obstructing the collaterals and damage to the heart structure are the basis of its pathogenesis; the unclear mind and disorder of body and spirit are the causes of its progression. Based on the characteristics of pathological changes in different stages of the disease, it is proposed that early treatment should focus on restoring the middle jiao, clearing and promoting blood vessels, using modified Yigong Powder (异功散); during the progression of the disease, treatment should remove blood stasis and phlegm, nourish heart and protect the pulse, using self-prescribed modified Mengshi Tongmai Decoction (礞石通脉汤); meanwhile, calming mind and stabilizing palpitations, and regulating spirit should be sequentially incorporated, with self-prescribed Jiazao Ningmai Decoction (甲枣宁脉汤) or Shenying Dingji Decoction (参英定悸汤) and modified as appropriate. Clinical treatment should focus on the whole disease course of atrial fibrillation, implementing stage-based treatments to enable early intervention and holistic regulation.

This case report introduces Professor ZOU Wei 's experience of treating a patient with Wernicke encephalopathy using the "regulating spirit and promoting yang acupuncture method". The patient was diagnosed as spleen and stomach deficiency with internal liver wind. The treatment principle focused on regulating spirit and awakening the brain, strengthening the spleen, calming wind, and relaxing the tendons. Three groups of acupoints were selected: ①acupoints for awakening the brain by regulating spirit and unblocking meridians (Baihui [GV20], Qianshencong [EX-HN1] and bilateral Taiyang [EX-HN5], Fengchi [GB20]), etc.; ②acupoints for harmonizing the spleen, stomach, qi, and blood (bilateral Tianshu [ST25], Daheng [SP15], Taixi [KI3], etc.); ③acupoints for relaxing and softening the tendons (bilateral Waiguan [TE5], Hegu [LI4], Yanglingquan [GB34], Xuanzhong [GB39]).The needles were retained for 50 min per session, once daily, 7 days a week. After 16-week treatment, the patient was able to walk a few steps slowly with assistance, and other symptoms returned to normal. A two-month follow-up showed the patient's condition remained stable, walking distance further increased, and overall health significantly improved.

International Journal of Cerebrovascular Diseases 2025;33(4):252-259
Objective:To investigate the function changes in neurovascular coupling (NVC) in patients with white matter hyperintensities (WMHs) and its correlation with cognitive impairment. Methods:Thirty patients with moderate to severe WMHs admitted to the First People's Hospital of Changzhou from April 2023 to June 2024 (as case group), and 30 subjects with no or mild WMH matched by age, gender and educational level (as the control group) were included prospectively. The general clinical information was collected and cognitive assessment was conducted. Three-dimensional pseudo-continuous arterial spin labeling (3D-pCASL) and resting state functional magnetic resonance imaging (rs-fMRI) data were collected to quantify cerebral blood flow (CBF) and functional connectivity strength (FCS). NVC function was evaluated through voxel-wise CBF-FCS correlation analysis. The CBF-FCS coefficients between groups were compared, the correlation between NVC and cognitive function was explored using partial correlation analysis, controlling for the false discovery rate (FDR).Results:The prevalence of hypertension and diabetes in moderate to severe WMHs group was significantly higher than that in the no or mild WMH group (all P<0.05), while the overall cognitive function, executive function and episodic memory function were significantly lower than those in the no or mild WMH group (all P<0.05). There was no significant difference in the CBF-FCS coefficients of the whole brain gray matter between the moderate to severe WMHs group and the no or mild WMHs group. The CBF-FCS coefficients of the bilateral orbital frontal gyrus, right anterior cingulate gyrus, and bilateral hippocampus in the moderate to severe WMHs group were significantly lower than those in the no or mild WMHs group, while the CBF-FCS coefficients of the bilateral parietal gyrus and caudate nucleus were significantly higher than those in the no or mild WMHs group (all q<0.05 after FDR adjustment). Partial correlation analysis showed that after adjusting for age, gender, and education level, the CBF-FCS coefficients of the right anterior and lateral cingulate gyrus were negatively correlated with the executive function completion time in the moderate to severe WMHs group ( r=-0.55, q=0.04 after FDR adjustment), and the CBF-FCS coefficients of the right hippocampus were significantly positively correlated with the episodic memory scores ( r=0.59, q=0.03 after FDR adjustment). These correlations were not statistically significant in the no or mild WMHs group. Conclusion:Patients with moderate to severe WMHs exhibit NVC abnormalities in specific brain regions, which are closely associated with executive function and episodic memory impairment, suggesting that NVC dysfunction may be an important mechanism for WMHs-related cognitive impairment.

Chinese Journal of Emergency Medicine 2025;34(9):1222-1228
Objective:To investigate the effects and mechanisms of angiotensin-converting enzyme 2 (ACE2) and Captopril (CAP) on mechanical ventilation-induced lung injury (VILI) in mice.Methods:Seventy-two healthy male BALB/c mice were randomly assigned (using a random number table) into six groups ( n=12 per group): normal control (NC) group, VILI group, ACE2 group, VILI+ACE2 group, CAP group, and VILI+CAP group. One hour prior to mechanical ventilation, the ACE2 and VILI+ACE2 groups were intraperitoneally injected with ACE2 at a dose of 0.1 mg/kg, while the CAP and VILI+CAP groups were intraperitoneally injected with CAP at a dose of 2.5 mg/kg. Following mechanical ventilation, serum samples were collected and enzyme-linked immunosorbent assay (ELISA) was used to detect inflammatory factors [platelet activating factor (PAF), endothelin-1 (ET-1), soluble intercellular adhesion molecule-1 (sICAM-1), prostaglandin E2 (PGE2)] and cardiovascular system related indicators [von Willebrand factor (vWF), thrombomodulin (TM), angiotensin (Ang) (1-9), Ang (1-7), prostacyclin I 2 (PGI2)]. Bronchoalveolar lavage fluid (BALF) was gathered, and total protein concentration was determined using BCA method, and sICAM-1 levels were measured by ELISA. Lung tissues were collected and subjected to hematoxylin and eosin staining (HE staining) for the assessment of pathological lung injury and lung injury scoring. Western blot and reverse transcription quantitative polymerase chain reaction (RT-qPCR) were utilized to detect the relative expression levels of ACE2 protein and mRNA, respectively. Statistical analysis was performed using IBM SPSS 20.0 software. Intergroup comparisons were conducted using one-way analysis of variance followed by the least significant difference (LSD) test. Results:No statistically significant differences were observed in the levels of PAF, ET-1, sICAM-1, vWF, TM, Ang(1-9), Ang(1-7), and PGI2 in serum and lung tissues between the ACE2/CAP groups and the NC group (all P>0.05). Compared with the VILI group, the VILI+ACE2 and VILI+CAP groups exhibited significantly decreased serum and lung tissue levels of PAF, ET-1, sICAM-1, and vWF (all P<0.05), while the levels of TM, Ang(1-9), Ang(1-7), and PGI2 were significantly increased (all P<0.05). Pathological lung injury was alleviated, and the lung wet/dry weight ratio was significantly reduced (all P<0.05) in the VILI+ACE2 and VILI+CAP groups. Furthermore, both ACE2 protein and mRNA expression levels were significantly increased in these groups (all P<0.05). Conclusion:Both ACE2 and CAP can inhibit inflammation and protect the cardiovascular system, possibly by promoting the ACE2/Ang(1-9)/Ang(1-7) axis, thereby exerting a protective effect against VILI.

Journal of Traditional Chinese Medicine 2024;65(7):750-754
Paroxysmal atrial fibrillation can be attributed to the category of xinji (palpitations) and zhangchong (severe palpitations) in traditional Chinese medicine, and its onset has the characteristics of urgency, change, and movement, which is similar to the characteristics of diseases induced by wind pathogen. It is believed that the internal movement of wind pathogen runs through the whole course of this disease, and palpitations due to wind as the direct pathogenesis. Palpitations caused by wind pathogen showed different characteristics of deficiency and excess pattern. In the acute exacerbation period, excess wind is the main cause of disease. For excessive heat generating wind, the treatment is to clear the liver and extinguish wind by self-modified Lingxia Qinggan Decoction (羚夏清肝汤); for blood stasis generating wind, the treatment is to remove blood stasis and stop wind by self-modified Yandan Limai Decoction (延丹理脉汤); for phlegm-heat accumulation with wind, the treatment is to dissolve phlegm and eliminate wind by self-modified Lianlou Danxing Decoction (连蒌胆星汤). In the prolonged recovery period, deficiency wind is more common. For stirring of wind due to yin deficiency, the treatment is to nourish yin and extinguish wind by self-modified Zaoshao Zhenzhu Deoction (枣芍珍珠汤); for spleen deficiency generating wind, the treatment is to strengthen spleen and nourish wind by self-modified Shenying Dingji Deoction (参英定悸汤). Clinical prescriptions closely follow the characteristics of wind, weigh the changes of deficiency and excess, tailor with the patterns, and regulate qi and blood of the zang-fu organs, in order to extinguish wind and arrest convulsion.
