1.Prokaryotic expression of Argonaute protein from Thermus thermophilus and its application value in the detection of KRAS 12D mutation

Objective To obtain the recombinant Argonaute protein(TtAgo)from Thermus thermophilus using gene cloning and pro-tein purification techniques and detect its endonuclease activity against the KRAS 12D mutation in vitro,thereby to provide experimental evidence for its subsequent application in the detection of gene mutations.Methods The TtAgo gene sequence was amplified from the genome of Thermus thermophilus by the PCR technology.Then,the amplified sequence was inserted into the prokaryotic expression vec-tor pET-28a to construct the recombinant vector pET-28a-TtAgo.After the recombinant TtAgo protein was induced to be expressed,it was purified sequentially by the nickel ion affinity chromatography(Ni-NTA),heparin affinity chromatography,and size-exclusion chromatography.The high-specificity nuclease activity of the recombinant TtAgo protein mediated by a short DNA guide strand(gDNA)was verified in vitro.Results The prokaryotic expression vector pET-28a-TtAgo was successfully constructed by the gene cloning tech-nique,and the soluble expression of the recombinant TtAgo protein was achieved.With the help of protein purification technology,the high-purity recombinant TtAgo protein was obtained.Under the mediation of gDNA,the recombinant TtAgo protein could specifically cleave wild-type single-stranded DNA(ssDNA),double-stranded DNA(dsDNA),and RNA molecules,while had no cleavage activi-ty for tumor KRAS 12D mutant molecules.Conclusion The high-purity recombinant TtAgo protein with nuclease activity is successful-ly obtained,and its targeted cleavage activity towards the KRAS 12D mutation site is detected in vitro.
2.Establishment and validation of urine AD7c-NTP reference intervals for healthy adults in Mianyang area using the indirect method

Objective To establish the reference interval of urine Alzheimer-associated neuronal thread protein(AD7c-NTP)for healthy adults in Mianyang area using the indirect method.Methods The detection results of urine AD7c-NTP from 5 093 healthy in-dividuals were collected from the information management database of Medical Laboratory Department of Sichuan Science City Hospital from March 2017 to March 2022.Skewness-kurtosis and Kolmogorov-Smirnov tests were used to determine whether the data followed a normal distribution.After removing outliers using the Box Plots method,the enrolled subjects were grouped by gender and age.The Mann-Whitney U or Kruska-Wallis H tests were used to analyze the between-group differences of urine AD7c-NTP in healthy individu-als with different genders and ages.The adjacent age groups without statistically significant difference(P>0.05)were combined,and the indirect method(non-parametric test method)was used to calculate the reference intervals for different gender and age groups.Results Skewness-kurtosis and Kolmogorov-Smirnov tests showed that the data followed a non-normal distribution.After removing 293 outliers using the Box Plots method,a total of 4 800 subjects,including 3 199 males and 1 601 females,were enrolled.The enrolled subjects were grouped by gender and age,and the non-parametric test method were used to establish the reference intervals of urine AD7c-NTP in healthy populations with different genders.The Mann-Whitney U test confirmed that urine AD7c-NTP levels existed gen-der differences(Z=14.09,P<0.01),and the reference intervals for males and females were≤1.10 ng/mL and≤1.40 ng/mL,re-spectively.There were also statistical differences in urine AD7c-NTP levels among different age groups of the same gender.After combi-ning adjacent age groups without statistically significant difference(P>0.05),the reference intervals of urine AD7c-NTP in healthy populations with different genders and ages were established by the non-parametric test method,which were≤1.00 ng/mL for male 20-39 years old group,≤1.10 ng/mL for male 40-79 years old group,≤1.60 ng/mL for male≥80 years old group,≤1.30 ng/mL for female 20-69 years old group,and≤1.60 ng/mL for female≥70 years old group,respectively.The established reference intervals of urine AD7c-NTP were further verified by healthy individuals,and the results met the standards.Conclusion The reference intervals of urine AD7c-NTP in healthy populations with different genders and ages in Mianyang area are established successfully using the indi-rect method,which may help to predict the risk of Alzheimer's disease in clinical practice and provide support for the diagnosis and treatment of the disease.
3.Current status of orthopedic nurses' health science popularization information generation capacity and training needs in 54 hospitals in Beijing

Objective:To explore the current status of orthopedic nurses' health science popularization information generation capacity and training needs in Beijing.Methods:This study was a cross-sectional survey. Convenience sampling was used to select 818 orthopedic nurses from 54 hospitals (48 Class Ⅲ hospitals and 6 ClassⅡ hospitals) in Beijing in April 2024 for the study. General Information Questionnaire, Self-assessment Scale for Orthopedic Nurses' Health Science Popularization Information Generation Capacity, and Training Needs Scale for Orthopedic Nurses' Health Science Popularization Information Generation Capacity were used to survey the study participants. Multiple linear regression was used to analyze the factors influencing orthopedic nurses' health science popularization information generation capacity. A total of 818 questionnaires were distributed and 781 valid questionnaires were recovered, with a valid recovery rate of 95.48% (781/818) .Results:The 781 orthopedic nurses had a total score of [49.00 (42.00, 57.00) ] on the Self-assessment Scale for Orthopedic Nurses' Health Science Popularization Information Generation Capacity. Multiple linear regression analysis showed that years of working experience, health status, intention to participate in health science popularization, stage of health science popularization behavior, and frequency of participation in health science popularization training were the factors influencing orthopedic nurses' health science popularization information generation capacity ( P<0.05), which explained 17.30% of the total variation. Beijing orthopedic nurses had training needs in five areas of assessing audience needs, assessing message risk, revising and improving messages, generating messages, and pretesting messages. Conclusions:The Beijing orthopedic nurses' health science popularization information generation capacity is at a medium and high level, but there is still much room for improvement. It is recommended to pay attention to the physical and mental health of orthopedic nurses, improve the work commitment, and build a long-term incentive mechanism to stimulate the enthusiasm of orthopedic nurses in health science popularization. Medical institutions or specialty societies should actively formulate training programs, build training platforms, and gradually perfect the orthopedic nurses' health science popularization training system in order to comprehensively enhance the health science popularization information generation capacity and help China's great health.
4.Dosimetric study on different bladder filling status in cervical cancer radiotherapy based on artificial intelligence-assisted segmentation

Objective To evaluate the dosimetric impact of interfractional bladder fullness variation in volumetric modulated arc therapy for cervical cancer using artificial intelligence-assisted cone-beam CT(CBCT)image segmentation,and propose an imaging-based quantitative assessment criterion for bladder fullness,providing an objective basis for assessing bladder filling status during clinical treatment.Methods Fifty patients receiving volumetric modulated arc therapy for cervical cancer were selected.The criterion for determining bladder fullness was as follow:if the bladder longitudinal diameter measured on the CBCT mid-sagittal plane was greater than half of the bladder longitudinal diameter measured on the localizable CT,it was defined as≥50%bladder fullness;otherwise,it was defined as<50%bladder fullness.Based on this criterion,two CBCT images were selected for each patient(representing fractions with≥50%and<50%fullness,respectively).Borui auto-contouring system was applied to re-contour the target areas and organs at risk,followed by dose recalculation.Results Compared with≥50%bladder fullness group,<50%bladder fullness group had significantly increased bladder V40,and small bowel Dmax,Dmean,V55,V45,V40(P<0.05),indicating that during interfractional radiotherapy,a CBCT-measured bladder longitudinal diameter less than half of the bladder longitudinal diameter measured on the localizable CT could serve as a predictor for significantly increased radiation dose to the bladder and small bowel.Correlation analysis revealed that bladder volume change showed a positive correlation with bladder Dmax change(R=0.45),a significant negative correlation with bladder V30(R=-0.37),and negative correlations with small bowel Dmax,Dmean,V55,V45,V40(R=-0.31,-0.41,-0.39,-0.49,-0.61).The correlation results indicate that increasing bladder fullness could reduce the radiation dose to the bladder and small bowel.Conclusion Artificial intelligence-assisted segmentation confirms that in interfractional radiotherapy for cervical cancer,when the CBCT-measured bladder longitudinal diameter is less than half of the longitudinal diameter measured on the localizable CT,there is a significant increase in radiation dose to OAR.Maintaining an ideal state of bladder fullness contributes to reducing the radiation dose to the bladder and small bowel.It is crucial to provide patients with adequate bladder management education before treatment and implement strict bladder volume management strategies during treatment fractions.
5.Defocusing state and myopia control of single focus, defocus and orthokeratology in myopic children observed by multispectral refraction topography

AIM:To observe the defocus state and myopia control in myopic children wearing single-vision, defocus, and orthokeratology lenses using multispectral refraction topography(MRT).METHODS: A total of 279 myopic patients aged 8-14 years old, with a spherical equivalent(SE)from -7.00 to -0.50 D, treated at the Chengdu Aier Eye Hospital from June 2022 to December 2023. Patients who volunteered for the study were assigned to three groups. A total of 94 cases were provided with single-vision spectacle lenses(SVL group), 90 cases received individualized ocular refraction customization(IORC group), and 95 cases received orthokeratology lenses(OK group). Simultaneously, the three groups were further categorized into low(-3.00 to -0.50 D), moderate(-6.00 to -3.25 D), and high myopia(-7.00 to -6.25 D)groups according to different SE. MRT was used to measure and compare the defocus changes of the retina in supperior, inferior, nasal, and temporal quadrants(RDV-S, RDV-I, RDV-N, RDV-T), and three angles of field of view, including 0-15°, 15°-30°, and 30°-45°(RDV-15, RDV-30, RDV-45)in the three groups(the data divide for the connected regions is grouped to the latter group). A one-way analysis of variance was used for intergroup comparisons. Univariate and multivariate linear regression analyses were used to analyze the factors related to changes in the axial length(AL)at 1 a after intervention.RESULTS:There were significant differences in 1-year SE and AL growth among patients in the SVL, IORC, and OK groups before and after intervention(P<0.001). The 1-year SE and the difference of AL growth in patients with low myopia was significantly different among SVL, IORC, and OK groups(P<0.001); however, there was no significant difference between the IORC and OK groups(P>0.05); there were significant differences in the SE and AL growth changes between the OK group and the IORC and SVL groups in moderate myopia(P<0.001); and there were significant differences between the OK group and the IORC and SVL groups in SE and AL growth of high myopia group after wearing lenses for 1 a(P<0.001), while there were no significant differences between the IORC and SVL groups(P>0.05). In addition, there were significant differences in the relative peripheral refractive errors(RPRE)of 4 quadrants and 3 eccentric regions among the three groups of patients in different degrees of myopia groups(P<0.001). Pair-wise comparison of the growth difference of eccentric D-RDV-15 in low myopia group after wearing lenses for 1 a showed significant differences between the SVL, IORC, and OK groups(P<0.001), but no significant differences between the IORC and OK groups(P>0.05). The angle of field of view D-RDV-30 in moderate myopia subgroups was statistically different between the SVL group and the IORC and OK groups after wearing lenses for 1 a(P<0.001), while the IORC and OK groups showed no significant differences(P>0.05); the angle of field of view D-RDV-15 in high myopia subgroups was statistically different between the OK group and the IORC and SVL groups after wearing lenses for 1 a(P<0.001), but there was no significant difference between the IORC and SVL groups(P>0.05). Univariate and multivariate linear regression model analysis showed that the changes in D-TRVD, D-RDV-45, D-RDV-N, and D-RDV-I correlated with the increase in the difference in 1 a AL.CONCLUSION: MRT can be used to guide the clinical control of myopia. Myopia development is related to the peripheral retinal defocus state, and the difference of defocus quantity in the inferior nasal side at 30°-45° eccentricity may be a factor regulating the rapid progression of myopia.
6.Psychological experiences and care needs of elderly patients with chronic obstructive pulmonary disease following dysphagia: a qualitative research

ObjectiveTo explore psychological experiences and care needs of elderly patients with chronic obstructive pulmonary disease (COPD) following dysphagia. MethodsFrom April to June, 2024, 13 elderly patients with COPD and dysphagia received treatment in Yixing No. 2 People's Hospital (Yixing Occupational Disease Institute) and Northern Jiangsu People's Hospital were interviewed. Nvivo 11.0 and content analysis were employed to analyze and summarize themes. ResultsTwo main themes were identified. The psychological experiences included fear of eating due to swallowing and choking, swallowing anxiety in social situations, concerns about malnutrition, and emotional loss related to family. The care needs included improvement in swallowing function, adjustment of food texture, assistance with disease adaptation and effective access to health education information. ConclusionHealthcare professionals should thoroughly understand the psychological and needs of elderly patients with COPD-related dysphagia, and comprehensive nursing strategies should be developed and implemented to improve swallowing function and overall quality of life.
7.In vitro antibacterial effect of berberine combined with levofloxacin on MRSA and related mechanisms

OBJECTIVE T o understand the resistance capability of berberine combined with levofloxacin in vitro to methicillin-resistant Staphylococcus aureus(MRSA).METHODS Totally 100 strains of MRSA were isolated from Shanghai Eighth People's Hospital from Jul.2023 to Jun.2024,the minimum inhibitory concentrations(MICs)of berberine and levofloxacin against the strains were determined by microbroth dilution method.The anti-bacterial effect of the combined use of the two drugs on MRSA was observed.The effects of the single and com-bined use of berberine and levofloxacin on sterilization of MRSA standard strain USA 300-LAC and their influence on cellular morphology were observed.The influence of berberine on mRNA levels of drug resistance genes and ef-flux pump-related genes in the standard strain USA 300-LAC and clinical strains was determined by means of real-time fluorescent quantitative PCR.RESULTS Among the 100 clinical isolates of MRSA,most of them were isola-ted from respiratory tract specimens of respiratory medicine department and geriatrics department,and 75.00%of the strains were resistant to levofloxacin.The MICs of both berberine and levofloxacin against the standard strain USA300-LAC were 128 μg/ml;the MICs against the clinical MRSA isolates were 64 to 1024 μg/ml and 0.125 to 256 μg/ml,respectively.Berberine combined with levofloxacin showed additive effect on 17(44.74%)strains of MRSA.The combined use of the two drugs showed the most remarkable in vitro antibacterial effect.The cellular structures of the strains were unclear and the contents of bacterial bodies increased when berberine and levofloxacin were used in combination.Berberine showed various degree of inhibitory effect on gyrA,gyrB,grlA and nor A genes.CONCLUSIONS Berberine is capable of resisting the MRSA and shows synergistic or additive effect with levofloxacin.The combined use of the two drugs may reduce the MIC against levofloxacin,destroy the morpholog-ical structure,and inhibit the expression of gyrA,gyrB,grlA and nor A genes.
8.Current status of implementation of infection control core elements in grass-roots medical institutions under background of construction of"compact county-level medical communities"

OBJECTIVE To explore the implementation and standardized management of infection control core ele-ments in grass-roots medical institutions within county-level medical communities.METHODS From Mar.2024 to Apr.2024,the current status of implementation of infection control core elements in the grass-roots medical institu-tions within county-level medical communities was investigated by means of questionnaire survey and qualita-tive interview,and the implementation strategies were further explored.RESULTS The infection management or-ganizational system and functions of the two county-level medical institutions within the county medical communi-ties were completed,there is no independent hospital infection management department in the primary medical in-stitutions.The infection management personnel in the 16 grass-roots medical institutions were part-time person-nel,the personnel with the educational background below junior college accounted for 84.21%,the personnel with the professional background of nursing accounted for 100.00%,the personnel with less than 5 years of working experience accounted for 78.95%,none of them had an on-the-job training certificate.The monitoring programs of the county-level medical institutions within the county medical communities were completed,there was no infec-tion management monitoring information platform in the grass-roots medical institutions.The infection cases,hand hygiene,environmental health and occupational exposures were monitored by people.The grass-roots medi-cal institutions had the highest requirements for various professional trainings and increase of training contents of prevention and control of public health infectious diseases(100.00%).The county-level medical institutions had inadequate capabilities of professional examination of medical equipment replacement and construction of medi-cal architecture.CONCLUSION It is necessary for the country and local levels of governments to attach great importance to the implementation of the infection control core elements in the grass-roots medical institutions within the county-lev-el medical communities,establish the county-level regional information platform,formulate the corresponding surveil-lance indexes and homogenized management systems,complete the cultivation of talents,and offer financial support.

10.Effects of Agiophyllum Oligo Saccharides on inflammation and apoptosis of mouse synovial cells

BACKGROUND:For knee osteoarthritis,synovium inflammation and abnormal proliferation and activation of fibroblast-like synoviocytes caused by insuffcient apoptosis are important driving factors for disease progression.Toll-like receptor 4(TLR4)/nuclear factor-κB(NF-κB)signaling pathway plays a key role in regulating inflammatory response and apoptosis of synovial cells.As a traditional Mongolian medicine component,Agiophyllum Oligo Saccharides has anti-inflammatory and immunomodulatory functions,but its specific molecular mechanism has not been fully revealed.OBJECTIVE:To explore the effects and mechanism of Agiophyllum Oligo Saccharides on lipopolysaccharide-induced inflammation and apoptosis of mouse synovial cells.METHODS:The mouse synovial cells were randomly divided into a control group,a model group,an Agiophyllum Oligo Saccharides group and a Bay 11-7082 group.Osteoarthritic synoviocyte model was prepared using 1.0 μg/mL lipopolysaccharide intervention for 36 hour.The Agiophyllum Oligo Saccharides group was treated with 1.0 μg/mL lipopolysaccharide and 32 μg/mL Agiophyllum Oligo Saccharides for 36 hours;and the Bay 11-7082 group was firstly incubated for 4 hours with 1 μmol/L NF-κB pathway inhibitor Bay 11-7082,and then 1.0 μg/mL lipopolysaccharide and 32 μg/mL Agiophyllum Oligo Saccharides for 36 hours.Western blot was used to detect TLR4,MyD88,NF-κBp65,p-NF-κBp65,and IκBα protein expression.The level of tumor necrosis factor-α and interleukin-1β in the supernatant were analyzed by ELISA.To detect the expressions of Bax and Bcl-2,western blot and immunofluorescence analysis were performed.The intracellular reactive oxygen species level was detected by DCFH-DA.RESULTS AND CONCLUSION:(1)Compared with the control group,the levels of tumor necrosis factor-α,interleukin-1β and reactive oxygen species,the protein expressions of TLR4,MyD88,Bcl-2,and p-NF-κBp65/NF-κBp65 increased(P<0.05),whereas the protein expressions of IκBα and Bax decreased(P<0.05)in the model group.(2)Compared with the model group,the levels of tumor necrosis factor-α,interleukin-1β and reactive oxygen species,the protein expressions of TLR4,MyD88,Bcl-2,and p-NF-κBp65/NF-κBp65 decreased(P<0.05),while the expressions of IκBα and BAX increased(P<0.05)in the Agiophyllum Oligo Saccharides group.(3)The levels of tumor necrosis factor-α,interleukin-1β and reactive oxygen species and the protein expression of Bcl-2 in the Bay 11-7082 group were higher than those in the Agiophyllum Oligo Saccharides group(P<0.05),but still lower than those in the model group(P<0.05),whereas the expression of Bax protein was lower than that in the Agiophyllum Oligo Saccharides group(P<0.05),but still higher than that in the model group(P<0.05).(4)Compared with the model group and the Agiophyllum Oligo Saccharides group,the protein expressions of TLR4,MyD88 and p-NF-κBp65/NF-κBp65 decreased(P<0.05),whereas the protein expressions of IκBα increased(P<0.05)in the Bay 11-7082 group.To conclude,Agiophyllum Oligo Saccharides could inhibit inflammation of mouse synovial cells and promote apoptosis of synovial cells with inflammation,and the mechanisms may be correlated with regulating the TLR4/NF-κB signaling pathway.
