AIM: To investigate the specific molecular mechanism of Yinqiao Powder in affecting macrophage polarization in herpes simplex keratitis(HSK)through the cyclic GMP-AMP synthetase(cGAS)-stimulator of interferon genes(STING)-interferon regulatory factor 3(IRF3)molecular pathway.METHODS:Human corneal epithelial cells(HCE-T)were divided into control, HSK, and HSK + Yinqiao Powder groups. M0 macrophages were grouped as Ctrl, HSV-1, HSV-1+oe-cGAS, HSV-1+Yinqiao Powder, and HSV-1+oe-cGAS+Yinqiao Powder. Conditional medium(CM)from each group of M0 macrophages was collected to intervene in HCE-T cells and divided into Ctrl-CM, HSV-1-CM, HSV-1+oe-cGAS-CM, and HSV-1+Yinqiao Powder-CM groups. Cell viability was detected by MTT assay, and apoptosis was detected by TUNEL assay. ELISA was used to detect the concentrations of Arg-1 and iNOS in cell supernatants, and Western blotting was used to detect the relative protein expressions of cGAS, STING, and IRF3. Balb/c mice were divided into control, model, and drug groups. The model and drug groups were inoculated with HSV-1 on the cornea of Balb/c mice using the corneal scratch method to construct an HSK mouse model, and the drug group was treated with Yinqiao Powder. The incidence and mortality of the three groups were compared on days 1, 3, 5, 7, and 14 after modeling.RESULTS:Compared with the control group, the HCE-T cell viability in the HSK group was decreased but apoptosis was increased, which was reversed by Yinqiao Powder intervention. Compared with the Ctrl group, the Arg-1 concentration in the cell supernatant of the HSV-1 group was decreased, the iNOS concentration was increased, and the protein expressions of cGAS, STING, and IRF3 were decreased. Compared with the HSV-1 group, the Arg-1 concentration was increased, the iNOS concentration was decreased, and the protein expressions of cGAS, STING, and IRF3 were enhanced in the HSV-1+oe-cGAS group and the HSV-1+Yinqiao Powder group, and the same results were obtained in the HSV-1+oe-cGAS+Yinqiao Powder group. Compared with the Ctrl-CM group, the HCE-T cell viability was decreased and apoptosis was increased in the HSV-1-CM group, which was reversed by overexpressing cGAS in macrophages or intervening with Yinqiao Powder. In vivo experiments found that Yinqiao Powder intervention could improve the pathological progression of keratitis.CONCLUSION:Yinqiao Powder inhibits M1 polarization of macrophages through the cGAS-STING-IRF3 molecular pathway, thereby delaying the progression of HSK.

Objective To investigate the expression and clinical significance of serum Golgi protein 73 (GP73) and SMAD family member 2 (SMAD2)in endometriosis. Methods From March 2022 to September 2023,175 patients with endometriosis who visited Xinjiang Military Region General Hospital were collected as the observation group. According to the staging of endometriosis, patients were separated into mild group (stage Ⅰ,n=61),moderate group (stage Ⅱ,n=52 ) and severe group (stage Ⅲ-Ⅳ,n=62). The healthy group comprised 55 healthy individuals who underwent medical examinations from December 2020 to December 2022 at Xinjiang Military Region General Hospital. ELISA was applied to detect serum GP73 and SMAD2 levels, the general data of the observation group and the control group were compared. Logistic regression was used to analyze the relevant factors affecting the occurrence of endometriosis, the Pearson method was applied to investigate the correlation between serum GP73 and SMAD2 levels in patients with endometriosis.ROC curves were used to assess the diagnostic value of serum GP73,SMAD2 levels and their combination in patients with EMT. Results The presence or absence of dysmenorrhea and irregular menstruation was compared between the two groups, and the differences were statistically significant (x2=17.633,39.268,all P<0.001). Serum GP73 (73.68±19.23 ng/ml),SMAD2 (42.27±9.61 mg/L) expression levels in the observation group were higher than those in the control group (58.61±13.27 ng/ml,35.26±6.37 mg/L),and the differences were statistically significant (t=8.327,7.845,all P<0.05). GP73 (59.79±17.26,73.73±18.17,87.29±22.05ng/ml) and SMAD2 (35.18±7.39,39.97±9.45,51.17±11.96mg/L) increased sequentially in the mild, moderate and severe groups, and the differences were statistical significance (F=31.067,42.866,all P<0.05). Pearson analysis showed serum GP73 levels were positively correlated with SMAD2 levels in EMT patients (r=0.427,P<0.001).Multifactorial Logistic regression analysis showed that irregular menstruation, dysmenorrhea, serum GP73[OR(95%CI):2.035 (1.208~3.428)],SMAD2[OR(95%CI):1.972 (1.284~3.029)]levels were risk factors for the development of EMT in patients (all P<0.05). The AUC(95%CI) for the combined diagnosis of EMT by serum GP73 and SMAD2 was 0.821 (0.776~0.861),which was higher than that of serum GP73 and SMAD2 alone[0.763 (0.714~0.807),0.708 (0.656~0.756)],respectively,and the differences were statistically significant (Z=3.121,4.346,all P<0.05). Conclusion Elevated serum GP73 and SMAD2 levels in EMT patients are all related to the extent of the patient's condition and have some diagnostic value for EMT.

Chinese Journal of General Practitioners 2024;23(10):1075-1079
The non-randomized controlled study design was adopted. A total of 90 patients with knee osteoarthritis treated in Beijing Yuetan Community Health Service Center from September 2021 to August 2022 were enrolled in the study, including 45 patients received patellar loosening and then the quadriceps femoris pain point release (successive group) and remaining 45 patients received two manipulations simultaneously (simultaneous group). Patients were treated 3 times in a week, and each time received 5 groups of manipulation. The Visual Analogue Scale (VAS) and Lysholm scores were measured before treatment, immediately after treatment and 3 months after treatment. There was no exfoliation and no adverse events in both groups. There were no significant differences in VAS and Lysholm scores between the two groups before treatment ( P>0.05). Immediately after treatment, the VAS pain score in the simultaneous group was lower than that in the successive group ((3.36±1.12) vs.(2.24±0.93), P<0.01), while the Lysholm score in the simultaneous group was higher than that in the successive group ((89.76±6.61) vs.(74.07±16.80), P<0.01); the proportion of VAS score as“painless/mild pain”(88.9% (40/45) vs.64.4% (29/45), P<0.01) and Lysholm score as “excellent/good”(91.1% (41/45) vs. 44.4% (20/45), P<0.01) in the simultaneous group was significantly higher than that in the successive groups. At 3 months after treatment, there were still significant differences in VAS and Lysholm scores between the two groups. The simultaneous manipulations of patellar loosening and quadriceps femoris pain point release can better relieve the pain of patients with knee osteoarthritis.

Chinese Journal of Modern Nursing 2024;30(23):3139-3146
Objective:To explore the clinical effects of the best evidence-based practice plan for rehabilitation exercises in patients undergoing surgery for lumbar spinal stenosis (LSS) .Methods:Guided by the evidence-based practice model of the Joanna Briggs Institute (JBI) Center for Evidence-Based Healthcare in Australia, the best evidence was combined with the clinical context to develop quality review indicators for clinical practice. In December 2022, a baseline review was conducted with 30 patients undergoing surgery for LSS (before evidence application). From December 2022 to February 2023, the baseline review results were analyzed to identify facilitating and obstructive factors between the current status and the review indicators, and improvement strategies were implemented based on available resources. In February 2023, another 30 patients (after evidence application) were selected to evaluate the post-application effects. The Japanese Orthopedic Association (JOA) score, Oswestry Disability Index (ODI), Tampa Scale for Kinesiophobia (TSK) score, General Self-Efficacy Scale (GSES) score, and quality of life scores were compared between the two groups at 24 hours, 1 month, 3 months, and 6 months postoperatively. The execution rate of clinical quality review indicators and the knowledge scores on rehabilitation exercises of 25 nurses before and after training were also compared. Results:There were no statistically significant differences in JOA scores, ODI, TSK scores, GSES scores, and quality of life scores between the two groups at 24 hours postoperatively ( P > 0.05). However, there were statistically significant differences in these scores at 1, 3, and 6 months postoperatively ( P< 0.01). After evidence application, the execution rate of seven clinical quality review indicators improved, and the knowledge scores on rehabilitation exercises of the 25 nurses increased significantly ( P< 0.05) . Conclusions:The evidence-based practice for rehabilitation exercises in patients undergoing surgery for LSS is safe and feasible. It standardizes evidence-based nursing practice behavior, enhances nurses' knowledge levels, effectively improves postoperative lumbar function, promotes physical rehabilitation, and enhances the quality of life.

Chinese Journal of Clinical Infectious Diseases 2024;17(5):383-388
Objective:To analyze the clinical features of patients with Listeria monocytogenes(LM)infection in the central nervous system. Methods:The clinical data of 12 patients with central nervous system LM infection admitted to the Department of Neurology of the First Hospital of Jilin University from January 2020 to December 2023 were retrospectively analyzed,in terms of the clinical manifestations, laboratory results, imaging findings, treatment and prognosis. SPSS 27.0 software was used to analyse the data. Results:The clinical manifestations were fever(12/12),headache(10/12)and neck stiffness(10/12). There were 2 cases with focal neurological deficit symptoms and mental behavior abnormalities respectively. Seven patients(7/12)had autoimmune diseases and received immunosuppressive therapy. Laboratory tests showed that in cerebrospinal fluid(CSF),protein and leukocytes levels were elevated in all patients, chloride levels were decreased in 8 cases(8/12),and glucose levels were decreased in 3 cases(3/12). The metagenomic next-generation sequencing(mNGS)test was performed in CSF samples of 10 patients, and all showed positive results for LM. The bacterial culture was performed in 11 cases and showed positive results for 5 blood samples and 2 CSF samples, respectively. Eleven patients underwent enhanced MRI scan, which showed meningeal enhancement in 3 cases and thalamic nodular enhancement in 1 case. In addition,MRI also showed single brain abscess in 2 cases, hydrocephalus in 2 cases, and rhombencephalitis in 1 case. The time from onset to definitive diagnosis was 5.00(4.25,6.75)days. All patients were treated with a first-line regimen(ampicillin + gentamicin)after being definitively diagnosed,4 patients subsequently received second-line antimicrobial therapy(cotrimoxazole and/or meropenem). Of the 12 patients,2 cases were cured,1 case was dead against medical advice, and 9 cases were clinically improved,of whom 3 had neurological sequelae 3 months after discharge. Conclusions:The clinical manifestations and imaging of Listeria monocytogenes infection in the central nervous system are varied. The disease can be easily misdiagnosed, and detection rate of mNGS in CSF was higher than that of traditional methods. In the treatment the empiric antimicrobial spectrum should be covered LM.
