1.The construction of a hierarchical and classified science and technology ethics governance system,international experience and China's strategies

Science and technology ethics are important measures to strengthen science and technology governance and promote responsible innovation.China's science and technology ethics construction started relatively late.To adapt to China's science and technology management system and alleviate the contradiction between the constructions of science and technology ethics and the explosive growth of future scientific and technological activities,this paper constructed a"one-top-three-layers"hierarchical and classified science and technology ethics governance framework,as well as analyzed the current situation.It was found that China's science and technology ethics governance system has shortcomings in top-level systems,hierarchical management,ethics governance of emerging technologies,policy implementation in provinces and cities,and the responsibility of innovation subjects.Subsequently,in response to the shortcomings of China's hierarchical and classified science and technology ethics governance system,international advanced experiences were summarized and supplemented by specific cases.Finally,based on actual conditions and international experiences,recommendations for improving China's hierarchical and classified science and technology ethics governance system were proposed from seven aspects,including improving the science and technology ethics provisions in China's legal documents,clarifying the ethical risk levels of various scientific and technological activities in life sciences,medicine,and artificial intelligence,strengthening the ethical governance of emerging technologies,urging provincial government departments to improve their science and technology ethics governance policies,reinforcing the main responsibility of innovation subjects in the hierarchical and classified science and technology ethics governance system,encouraging third-party to take part in science and technology ethics governance work,as well as building a large language model of science and technology ethics governance.
2.Genotype drug resistance in human immunodeficiency virus/acquired immunodeficiency syndrome patients with antiviral therapy failure in Yunnan Province from 2021 to 2023

Objective:To investigate the overall drug resistance, drug resistance trend and distribution of drug resistance mutation sites in human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) patients with antiviral therapy failure in Yunnan Province.Methods:The demographic data and genotype drug resistance of HIV/AIDS population with antiviral therapy failure in Yunnan Province from January 2021 to December 2023 were collected and analyzed by cross-sectional investigation. Statistical analyses were performed using chi-square test.Results:Among 15 159 HIV/AIDS patients, 12 215 cases tested positive by amplification. The circulating recombinant form (CRF) 08_BC was the predominant genetic subtype, accounting for 54.97%(6 714/12 215), followed by CRF01_AE (16.14%(1 972/12 215)) and CRF07_BC (14.48% (1 769/12 215)). When the viral load was ≥200 to <1 000 copies/mL, the incidence of drug resistance was 21.48%(99/461). When it was ≥1 000 to <10 000 copies/mL, the incidence was 51.29%(2 867/5 590). When it was ≥10 000 to <100 000 copies/mL, the incidence was 69.39% (3 979/5 734). When it was ≥100 000 copies/mL, the incidence was 81.86%(352/430). A total of 7 297 drug resistant cases were detected, with a drug resistance rate of 59.74% (7 297/12 215), thus the estimated drug resistance incidence rate among the antiviral treated population in Yunnan Province was 2.00% (7 297/364 238). From 2021 to 2023, the annual drug resistance rates among patients were 60.71%(2 554/4 207), 60.28%(1 671/2 772), and 58.67% (3 072/5 236), respectively, with no statistically significant difference ( χ2=4.47, P=0.107). Among the population with antiviral therapy failure, the drug resistance rates of non-nucleoside reverse transcriptase inhibitor (NNRTI), nucleoside reverse transcriptase inhibitor (NRTI), and protease inhibitor (PI) were 93.70%(6 837/7 297), 44.10%(3 218/7 297) and 5.15%(376/7 297), respectively. The mutation sites with the highest frequencies among the three classes of drugs including NRTI, NNRTI and PI were M184V/I (46.13%(2 123/4 602)), K103N/S (37.14%(2 648/7 129)), L33F (15.50%(82/529)) and M46I/L (15.50%(82/529)), respectively. Analysis of the degree of drug resistance showed that among NNRTI drugs, nevirapine (49.01%(5 987/12 215)) and efavirenz (48.00%(5 863/12 215)) had the highest drug resistance rates, followed by emtricitabine (23.59%(2 882/12 215)) and lamivudine (23.58%(2 881/12 215)) among NRTI drugs. Conclusions:Among HIV/AIDS patients with antiviral therapy failure in Yunnan Province from 2021 to 2023, CRF08_BC is the main genetic subtype. The drug resistance rate of patients increases with the increase of HIV-1 viral load. There is no significant change in the drug resistance rate from 2021 to 2023. NNRTI has the highest drug resistance rate, followed by NRTI, and PI has the lowest. The main mutation sites are M184V/I for NRTI, K103N/S for NNRTI, and M46I/L and L33F for PI. The drug resistance rates of nevirapine, efavirenz, emtricitabine and lamivudine are relatively high.
3.Effect of dapagliflozin on myocardial function changes in early spontaneously hypertensive rats:a study based on longitudinal layer-specific strain

Objective To evaluate the effect of dapagliflozin on myocardial function in early spontaneously hypertensive rats(SHR)with layer-specific global longitudinal strain(GLS).Methods A total of 45 male SHR aged 6 weeks were randomly divided into control group(normal saline),dapagliflozin group[1 mg/(kg·day)],and losartan group[10 mg/(kg·day)].Fifteen male Wistar-Kyoto(WKY)rats at same age with normal blood pressure were subjected and served as blank control group.During 8 weeks of intervention,systolic blood pressure(SBP)was measured,and conventional echocardiography and two-dimensional speckle tracking echocardiography(2DSTE)were performed and the results were collected to acquire the longitudinal strain of each layer of left ventricular(LV)myocardium.The parameters were compared among the groups.The pathological changes of myocardium were observed in each group of rats.Results Compared with the WKY group,LV ejection fraction(LVEF)and LV fraction shortening(LVFS)at week 8 were decreased in the control group(P<0.05),but no such decreases were observed in the dapagliflozin group and the losartan group.The GLS of endo-myocardium(GLSendo)at the 6th week was decreased,and GLSendo,GLSmid and GLSepi at the 8th week were all decreased in the control group than the WKY group(all P<0.05).But there were no statistical differences in the above 3 indicators in the dapagliflozin and losartan groups when compared with the WKY group(all P>0.05).The pathological results showed that myocardial interstitial fibrosis was observed in the control group at the 6th week.Conclusion Dapagliflozin can effectively improve myocardial function in early SHR.
4.Construction and application of nomogram prediction model for trial of labor after cesarean

Objective To explore the influencing factors of the failure in trial of labor after cesarean(TOLAC),construct and verify a prediction model for the risk of TOLAC.Methods The clinical data of 273 pregnant women who underwent TOLAC in The First Affiliated Hospital of Naval Medical University from 2019 to 2022 were retrospectively analyzed.Logistic regression was used to analyze influencing factors of the failure in TOLAC,and a nomogram model was established for individualized risk assessment.The best threshold of failure risk of TOLAC was evaluated by the decision-making curve and clinical influence curve.Results There were statistically significant differences in the age,gestational week,body mass index(BMI)before delivery,time to the last cesarean,cervical Bishop score and delivery times between the successful trial delivery group and the failed trial delivery group.The best intervention threshold was 0.72,that is,vaginal trial delivery should to be stopped when the risk of TOLAC failure was more than 72%as evaluated by the prediction model.Conclusion Age,gestational week,BMI before delivery,time to the last cesarean,Bishop score of cervix and delivery times are influencing factors for TOLAC failure.The prediction model based on these factors can provide a quantifiable TOLAC risk for pregnant women.
5.Effectiveness of an emergency medical information management system in civil airport emergencies

Objective This study aims to develop and evaluate an emergency medical information management system for the medical rescue of mass casualties in public health emergencies at civil airports,focusing on on-site triage and emergency treatment.Methods A retrospective analysis was conducted to compare the effects of on-site emergency drills for mass-casualty rescue in public health emergencies at the Guangzhou Baiyun International Airport Emergency Center in 2018 and 2024.The 2018 drills involving 21 casualties for which conventional emergency medical rescue equipment and procedures were utilized were set as a control group,while the 2024 drills involving 19 casualties for which an emergency medical information processing system were set as an experimental group.Data were analyzed using descriptive epidemiology,chi-square test,and t-test in terms of in-jury conditions,triage time,and total transfer and handover time.By integrating the entire process and all elements to achieve in-formation-based handling in the practical emergency medical rescue drills,the role and advantages of the emergency medical in-formation processing system in scientific command,rapid response,rapid triage,and emergency treatment for mass casualties in public health emergencies were examined.Results The two drills had no difference in the number of casualties(x2=0.054,P=0.973).The time spent on casualty triage and the total time for transfer and handover in 2024 were significantly reduced com-pared with those in 2018(t′=18.992,14.787,both P<0.001).Conclusion The emergency medical information management system enhances the command,response capabilities and rescue efficiency and level of emergency medical rescue of mass casual-ties in public health emergencies at civil airports.It has promoted the informatization,digitalization and intelligence construction of emergency medical rescue for public health emergencies at civil airports in China,thereby offering a reference for optimizing the informatization model of emergency medical rescue at civil airports in China.
6.Efficacy of combined magnetic-electrical stimulation,intelligent exercise prescription,and novel matrix radiofrequency therapy in the treatment of pelvic organ prolapse

Objective To investigate the therapeutic effects of combined magnetic and electrical stimulation with an"intelligent exercise prescription"and novel matrix radiofrequency therapy in patients with pelvic organ prolapse(POP).Methods A total of 158 patients with POP who received treatment at the Gynecological Pelvic Floor Rehabilitation Center of the Eighth Affiliated Hospital of Southern Medical University between October 2022 and July 2025 were retrospectively enrolled and divided into an observation group(n=64)and a control group(n=94)based on their treatment plans.The control group underwent magnetic and electrical stimulation combined with an"intelligent exercise prescription"regimen.Specifically,patients received 10 sessions of electrical stimulation,5 sessions of magnetic stimulation,and performed 15~20 minutes of daily home exercise training guided by the"intelligent exercise prescription."The observation group received,in addition to the aforementioned treatments,four sessions of novel matrix radiofrequency therapy.Changes in the muscle strength grades of type Ⅰ and type Ⅱ pelvic floor muscles,Glazer surface electromyography(EMG)values,and POP-Q staging were compared between the two groups before and after treatment.Results After treatment,both groups demonstrated significant improvements in type Ⅰ and type Ⅱ muscle fiber strength compared to baseline(all P<0.05),with the observation group showing greater improvement in type Ⅰ muscle fiber strength than the control group(P<0.05).The muscle potential values of the observation group during rapid contraction,tense contraction,and endurance contraction stages were markedly increased compared to pre-treatment levels.Moreover,the muscle potential values during the pre-resting stage were significantly reduced after treatment(P<0.05).In the observation group,POP-Q grades of the anterior vaginal wall,uterus,and posterior vaginal wall were all significantly lower post-treatment than pre-treatment(all P<0.05).However,no statistically significant differences were observed between the observation group and the control group in these parameters(P>0.05).Both groups exhibited relatively high compliance rates(both≥75.0%),with no significant difference between them(P>0.05).The treatment cost for the observation group was significantly higher than that for the control group(P<0.05).Conclusions The combination of magneto-electrical stimulation,an"intelligent exercise prescription,"and novel matrix radiofrequency therapy can significantly improve pelvic floor muscle strength and muscle potential values in the short term,compared to pre-treatment levels.This integrated approach also effectively alleviates the prolapse of the anterior vaginal wall,uterus,and posterior vaginal wall.Furthermore,the combination of magnetic and electrical stimulation,"intelligent exercise prescription,"and matrix radiofrequency therapy demonstrates superior efficacy in enhancing type Ⅰ pelvic floor muscle fiber strength when compared to the combination of magnetic and electrical stimulation with"intelligent exercise prescription"alone.However,this treatment protocol entails a relatively high economic burden,and its clinical application should be carefully evaluated in consideration of patients'functional needs and financial conditions.

Objective To investigate the effects of gut microbiota regulation combined with exercise rehabilitation on non-motor symptoms and neurological function in Parkinson's disease patients.Methods A total of 154 Parkinson's disease patients admitted to our hospital from January 2022 to January 2024 were selected as the subjects of the study.Using a random number table,these 154 patients were evenly divided into a control group and a treatment group,with 77 patients in each group.Both groups received standard treatments,but the control group also underwent exercise rehabilitation therapy,while the treatment group received probiotic supplementation and exercise rehabilitation therapy.The effectiveness of the two groups was then compared.Results Following the 12 weeks,24 weeks therapeutic regimen,The treatment group showed significantly better outcomes(P<0.05).Clinically meaningful reductions were observed in Hoehn-Yahr staging,alongside decreased scores on standardized instruments assessing psychiatric symptoms HAMA,HAMD,SCOPA-AUT,UPDRS Ⅰ-IV and PDSS(P<0.05).Concurrently,the study group exhibited enhanced MMSE(P<0.05).Fecal microbiome analyses revealed a favorable ecological shift characterized by increased colonization of beneficial genera Bifidobacterium and Lactobacillus,with concomitant suppression of pathobionts Enterococcus and Enterobacteriaceae.Gait analysis revealed increased step length,speed,and frequency in the treatment group(P<0.05).Conclusion Patients with Parkinson's disease who received probiotics combined with exercise rehabilitation treatment could effectively improve non-motor symptoms and neurological function,while promoting the balance of intestinal flora,and reduce clinical symptoms.
