1.Volatile Component Differences in Xihuangwan Prepared with Natural and Artificial Musk Based on Non-targeted and Targeted Metabolomics

ObjectiveHeadspace solid-phase microextraction-gas chromatography-mass spectrometry(HS-SPME-GC-MS) and GC-triple quadrupole MS(GC-QqQ-MS) in combination with non-targeted and targeted metabolomics were employed to systematically analyze the chemical composition differences of Xihuangwan prepared with natural musk and artificial musk, and establish an identification system for them. MethodsThe volatile components of 9 batches of Xihuangwan samples from 8 manufacturers were analyzed by HS-SPME-GC-MS non-targeted metabolomics, and identified by comparing their MS data with the National Institute of Standards and Technology(NIST) spectral library. Orthogonal partial least squares-discriminant analysis(OPLS-DA) was used to identify differential volatile components of Xihuangwan prepared with natural musk and artificial musk. Additionally, GC-QqQ-MS targeted metabolomics was applied to quantify the levels of α-pinene, β-elemene, muscone, dehydroepiandrosterone, bornyl acetate, and octyl acetate in 27 batches of samples from 9 manufacturers. Cluster analysis, principal component analysis(PCA), and partial least squares-discriminant analysis(PLS-DA) were conducted to further explore the differences in volatile components between Xihuangwan samples prepared with natural musk and artificial musk. ResultsNon-targeted metabolomics identified 291 volatile compounds in Xihuangwan, including alkanes, esters, alkanes, alcohols, ketones, naphthalenes and others. OPLS-DA analysis revealed distinct separation between Xihuangwan samples containing artificial musk(A1, C1, D1, E1, F1, G1, I1) and those containing natural musk(H1, H3). A total of 30 differential metabolites were identified. The relative contents of these 30 differential metabolites were visualized using a radar chart, revealing significant differences in the levels of octanol, borneol acetate and muscone. Cluster analysis and PCA results from targeted metabolomics indicated that Xihuangwan could be classified into two distinct groups:one composed of natural musk(H1, H3) and the other of artificial musk, sample H2. PLS-DA identified muscone, octyl acetate, and dehydroepiandrosterone as key differential volatile components. Although no significant difference was observed in the content of octyl acetate between the two groups, statistically significant differences were found for muscone and dehydroepiandrosterone(P<0.05). ConclusionMuscone and dehydroepiandrosterone can be used for the differentiation of Xihuangwan samples containing natural musk from those containing artificial musk. This study systematically and comprehensively analyzed the differences in the types and contents of major volatile components in Xihuangwan prepared with natural musk and artificial musk, providing a scientific basis for quality evaluation and control of Xihuangwan.

3.Effect of Astragalus polysaccharides on intestinal immune inflammation of rats with dampness stagnancy due to spleen deficiency syndrome based on fecal microbiota transplantation

Objective:To explore effect of Astragalus polysaccharides(APS)through TLR4/NF-κB signal pathway on intesti-nal immune inflammation of rats with dampness stagnancy due to spleen deficiency syndrome based on fecal microbiota transplantation.Methods:Rats with dampness stagnancy due to spleen deficiency syndrome were fed with high-fat and low-protein diet and subjected to exhaustive swimming,transplant flora microbiota after intervention of APS.General condition,changes in weight gain,time of weight exhaustion swimming,spleen index and thymus index were tested.Pathological manifestations of duodenal tissue inflammation were observed by HE staining.Serum IL-1β,IL-6 and TNF-α concentrations of rats were determined by ELISA.Protein levels of TLR4,NF-κB p65 and IκBα in colon were determined by Western blot.Results:Compared with model group,general condition rating,changes in weight gain and time of weight exhaustion swimming in APS-FTG group were increased significantly(P<0.01);spleen index and thymus index were increased;pathological manifestations of duodenal inflammation were significantly relieved;serum IL-1β,IL-6 and TNF-α concentrations were decreased significantly(P<0.01),TLR4 and NF-κB p65 levels in colon were decreased significantly(P<0.05 and P<0.01),IκBα level was in colon increased significantly(P<0.01).Conclusion:APS improves intestinal microbiota of rats with dampness stagnancy due to spleen deficiency syndrome and inhibits TLR4/NF-κB pathway activation through intestinal flora,thus improving intestinal immune inflammation status in rats with dampness stagnancy due to spleen deficiency syndrome.
4.Research on a SECURE nursing pathway based on action research method in preventing MDRPI

Objective:To explore the applied effectiveness of the Skin and/or tissue,Education,Cooperation,Understanding,Report and Evaluate (SECURE) nursing pathway based on action research method in the prevention process of medical device-related pressure injuries (MDRPI) of patients in the Intensive Care Unit (ICU). Methods:The SECURE nursing pathway based on action research method included the establishment of a skin care team to identify problems,and the formulation of conducting intervention plan,and the development of targeting nursing interventions to prevent the occurrence of MDRPI. A total of 84 patients hospitalized in the ICU of Hebei General Hospital from March 2022 to March 2023 were selected,and they were randomly divided into a control group and an observation group,with 42 patients in each group. The control group implemented routine method to prevent MDRPI during ICU treatment,while the observation group implemented SECURE nursing pathway based on action research method to conduct prevention. The MDRPI incidence,the quality scores of ICU nursing,the scores of the Short Form Health Survey-36 (SF-36) of patients,the assessment of the improvement of MDRPI patients,and the satisfaction of patients for nursing care were compared between the two groups. Results:The MDRPI incidence during ICU treatment in the observation group was 4.76% (2/42),which was significantly lower than that in the control group,with a statistically significant difference (x2=11.052,P<0.05). The average scores for device usage,prevention assessment,positioning management and preventive nursing after nursing intervention in the observation group were respectively (88.45±2.05),(89.36±2.11),(89.25±2.15) and (91.45±2.25),all of which were significantly higher than those in the control group,with statistically significant differences (t=11.425,12.052,10.052,13.478,P<0.05). After nursing intervention,the average scores for physical function,social function,psychological function and material life as the SF-36 scale in the observation group were (85.47±2.05),(86.48±2.05),(88.46±2.15) and (90.25±2.44),respectively,which were respectively higher than those in the control group,with statistically significant differences (t=12.414,11.045,10.252,10.478,P<0.05). The occurrence time,improvement time of wound and ICU treatment time of presenting MDRPI patients in the observation group were respectively (4.05±0.54) days,(3.45±0.15) days,and (8.12±2.14) days. Compared to the control group,the occurrence time of the observation group was longer,while both the improvement time of wound and ICU treatment time of the observation group were shorter,with statistically significant differences (t=8.485,10.012,13.001,P<0.05). The satisfaction degree of nursing care of patients in the observation group was 95.24% (40/42),which was significantly higher than that in the control group (x2=12.054,P<0.05). Conclusion:The SECURE nursing pathway based on action research method,which prevents MDRPI of ICU patients,can reduce the MDRPI incidence rate of ICU patients,and enhance the work quality of nursing care for MDRPI. It is helpful to the recovery of ICU patients,which can also shorten the treatment time of ICU patients.

6.Palpitations, Shortness of Breath, Weakness in Limbs, Edema, and Dyspnea: A Rare Inflammatory Myopathy with Positive Aniti-mitochondrial Antibodies and Cardiac Involvement

This article presents a case study of a patient who visited the Geriatric Department of Peking Union Medical College Hospital due to "palpitations, shortness of breath for more than 2 years, limb weakness for 6 months, edema, and nocturnal dyspnea for 2 months". The patient exhibited decreased muscle strength in the limbs and involvement of swallowing and respiratory muscles, alongside complications of heart failure and various arrhythmias which were predominantly atrial. Laboratory tests revealed the presence of multiple autoantibodies and notably anti-mitochondrial antibodies. Following a comprehensive multidisciplinary evaluation, the patient was diagnosed with anti-mitochondrial antibody-associated inflammatory myopathy. Treatment involved a combination of glucocorticoids and immunosuppressants, along with resistance exercises for muscle strength and rehabilitation training for lung function, resulting in significant improvement of clinical symptoms. The case underscores the importance of collaborative multidisciplinary approaches in diagnosing and treating rare diseases in elderly patients, where careful consideration of clinical manifestations and subtle abnormal clinical data can lead to effective interventions.

8.Analysis of medical cases and medication patterns for multiple system atrophy-orthostatic hypotension

Objective Using the Traditional Chinese Medicine Inheritance Assistance Platform V2.5 software,to analyze medical cases and prescription medication patterns of multiple system atrophy-orthostatic hypotension,and provide reference for clinical diagnosis and treatment of multiple system atrophy.Methods Relevant literature on the effectiveness of traditional Chinese medicine in treating multiple system atrophy-orthostatic hypotension was collected from China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,VIP,Chinese Medical Journal Full Text Database,and China Biomedical Literature Database.A database of medical cases and prescriptions was established on the Traditional Chinese Medicine Inheritance Assistance Platform.The software used mathematical algorithms based on association rules and entropy clustering to analyze the symptom ranking,symptom patterns,symptom clustering,and network display of medical cases,as well as the properties,taste,meridian tropism,frequency of application,and drug pair combinations of drugs in prescriptions.The core drugs were identified and the rules of diagnosis and treatment were explored.Results Through the Traditional Chinese Medicine Inheritance Assistance Platform V2.5 software,47 medical cases of multiple system atrophy were analyzed,including 73 symptoms.Among them,there were 8 symptoms with a frequency of ≥5,among which the most frequent symptom was pulse sinking,which appeared 8 times.And included 11 syndromes,the most frequent symptom was insufficient Qi syndrome,which appeared 12 times.The drug symptom network diagram,drug symptom network diagram,and drug symptom network diagram of multiple system atrophy-orthostatic hypotension medical cases were also drawn,which is the first time in the study.There are a total of 47 prescriptions for multiple system atrophy,including 106 types of drugs.Through analysis,the drugs used are mainly warm,cold,and flat,with less use of hot and cool drugs.The flavors are mainly sweet,spicy,and bitter,and the meridian tropism is mainly focused on the spleen,lungs,liver,heart,kidneys,and stomach.The top 10 drugs with the highest frequency of occurrence are Huangqi,Danggui,Shengma,Baizhu,Dangshen,Chenpi,Renshen,Chaihu,Fuzi,and Zhigancao.Among them,Huangqi has the highest frequency of occurrence,appearing 31 times,and has evolved into 66 core combinations of drugs and 3 new formulas.Conclusion The objective display of the medical case characteristics and medication features of multiple system atrophy-orthostatic hypotension through data mining has certain guiding significance for the research and clinical medication of this disease.
9.Impact of medical guidance games combined with specialized nursing on treatment compliance and clinical symptoms in preschool children with pneumonia

Objective To analyze the impact of medical guidance games combined with special-ized nursing on treatment compliance and clinical symptoms in preschool children with pneumonia.Methods A total of 113 preschool children with pneumonia admitted to Beijing Daxing District Peo-ple's Hospital from February 2023 to June 2024 were selected as research subjects and randomly di-vided into control group(n=56)and observation group(n=57).The control group received routine nursing,while the observation group received medical guidance games combined with specialized nursing on the basis of the control group.Treatment compliance,improvement time of clinical symp-toms,and psychological states were compared between the two groups.Results The total treatment compliance rate in the observation group was 98.2%,which was higher than 89.3%in the control group,and the difference was statistically significant(P<0.05).The disappearance times of symp-toms such as cough,expectoration,dyspnea,and fever in the observation group were all shorter than those in the control group(P<0.05).After intervention,the scores of the Children's Medical Fear Scale(CMFS)and the Screen for Child Anxiety Related Emotional Disorders(SCARED)in the ob-servation group were both lower than those in the control group(P<0.05).Conclusion Medical guidance games combined with specialized nursing can improve treatment compliance in preschool children with pneumonia,shorten their recovery time,help alleviate negative emotions such as medi-cal fear and anxiety,and promote disease recovery.
10.Mendelian randomization study on the relationship between thyroid dysfunction and insomnia

Objective Two-sample Mendelian randomization(MR)analysis was used to explore the causal relationship between hyperthyroidism/hypothyroidism and insomnia.Methods A two-sample MR method was adopted.The exposure factors were hyperthyroidism/hypothyroidism,and the outcome was insomnia.The instrumental variables were screened.Inverse variance weighting(IVW)was mainly used to analyze the causal relationship between hyperthyroidism/hypothyroidism and insomnia.Meanwhile,MR-Egger,weighted mode,weighted median,and simple mode were used to supplement the IVW analysis results.Cochran's Q test was used to determine the statistical heterogeneity among single nucleotide polymorphism(SNP),and the intercept term of MR-Egger was used to evaluate whether SNP had horizontal pleomorphism.The influence of a single SNP on the IVW analysis results was evaluated by the retention of one method.To avoid reverse causality,insomnia was taken as the exposure factor and hyperthyroidism/hypothyroidism as the outcome event for reverse MR analysis.Results A total of 11 SNP strongly related to hyperthyroidism were screened out.After eliminating the palindromic sequences,9 SNP strongly related to hyperthyroidism were finally included.The IVW analysis results showed that hyperthyroidism increased the risk of insomnia(OR=1.01,95%CI:1.00-1.01,P=0.012).A total of 68 SNP strongly related to hypothyroidism were screened out.After removing the palindromic sequences,61 SNP highly related to hypothyroidism were finally included.The IVW analysis results showed that hypothyroidism increased the risk of insomnia(OR=1.01,95%CI:1.00-1.01,P=0.021).The results of Cochran's Q test showed that there was no heterogeneity in the instrumental variable hyperthyroidism(P=0.60).The MR-Egger intercept analysis indicated that studies with hyperthyroidism/hypothyroidism as exposure and insomnia as the outcome did not have horizontal pleiotropy(P>0.05).Reverse MR analysis revealed that exposure factors such as insomnia had no causal relationship with hyperthyroidism(OR=1.57,95%CI:0.55-4.50,P=0.40)or hypothyroidism(OR=1.09,95%CI:0.74-1.61,P=0.66).Conclusion Hyperthyroidism/hypothyroidism can increase the risk of insomnia.
