1.Efficacy of baricitinib combined with ruxolitinib cream in the treatment of six patients with progressive nonsegmental vitiligo: a clinical observation

Objective:To evaluate the efficacy and safety of baricitinib combined with ruxolitinib cream in the treatment of progressive nonsegmental vitiligo.Methods:Clinical data were retrospectively collected from patients with progressive nonsegmental vitiligo in Boao Super Hospital. All the patients were treated with oral baricitinib daily (2 mg/day for patients weighing ≤ 50 kg; 4 mg/day for those > 50 kg) in combination with topical application of ruxolitinib cream twice daily for 24 consecutive weeks. Disease severity was assessed using the facial vitiligo area scoring index (F-VASI) and total body VASI (T-VASI) at baseline, week 12, and week 24. Adverse reactions were monitored throughout the treatment course.Results:Six patients with progressive nonsegmental vitiligo were collected, including 3 males and 3 females, aged 26 - 42 years, with the disease duration ranging from 0.5 to 25 years. At week 12, 3 patients achieved a 50% ~ < 75% improvement in facial vitiligo lesions (F-VASI 50), 1 patient achieved F-VASI 75 (75% ~ < 90% improvement), and 1 patient achieved T-VASI 50; at week 24, 4 patients achieved F-VASI 50, 1 patient achieved F-VASI 75, 1 patient achieved F-VASI 90 (≥ 90% improvement), and 3 patients achieved T-VASI 50. During the treatment, upper respiratory infection occurred in 1 patient, acne in 1 patient, pruritus in 2 patients, elevation of total cholesterol levels in 2 patients, and increase of high-density lipoprotein levels in 2 patients. No severe adverse events were observed during the treatment.Conclusion:The combination therapy with baricitinib and ruxolitinib cream may have potential efficacy and safety in the treatment of progressive nonsegmental vitiligo.
2.Predictive Value of Tissue AMPK α and HNF4 Expression for Local Recurrence and Distant Metastasis after Laparoscopic Radical Gastrectomy for Gastric Cancer

Objective To study the predictive value of adenosine monophosphate-activated protein kinase(AMPKα)and hepatocyte nuclear factor 4(HNF4)expression for local recurrence and distant metastasis after laparoscopic radical gastrectomy for gastric cancer.Methods Two hundred and ninety-four patients who underwent laparoscopic radical gastric cancer surgery admitted to Suining Central Hospital from January 2020 to August 2022 were selected.These patients were categorized into a recurrence and metastasis group(n=59)and a non-recurrence and non-metastasis group(n=235)based on postoperative local recurrence and distant metastasis.Gastric cancer tissue specimens obtained from surgical resection were used to detect and compare the cancer tissues of AMPKα mRNA and HNF4 mRNA between the two groups,and the data were statistically analyzed.Results The AMPKα mRNA and HNF4 mRNA in the recurrence and metastasis group were lower than those in the non recurrence and metastasis group(P<0.05);the point-biserial correlation showed that AMPKα mRNA(r=-0.816)and HNF4 mRNA(r=-0.803)were negatively correlated with postoperative local recurrence and distant metastasis(P<0.001),and the net correlation analysis showed that this relationship still existed after excluding confounding factors(P<0.001).Logistic regression analysis showed that AMPKα mRNA and HNF4 mRNA were postoperative local recurrence and distant metastasis related influencing factors(P<0.001);the interaction analysis showed that AMPKα mRNA and HNF4 mRNA were in a super multiplication model,and the decrease of HNF4 mRNA had a positive interaction effect on the decrease of AMPKα mRNA(P<0.05);the AUC of AMPKα mRNA+HNF4 mRNA was higher than that of AMPKα mRNA or HNF4 mRNA(Z=4.261,4.275,P<0.05).Conclusion Co-mbined detection of AMPKα and HNF4 expression can predict the risk of local recurrence and distant metastasis after laparoscopic radical gastrectomy for gastric cancer.

4.The predictive value of stress hyperglycemia ratio on in-hospital mortality and mechan-ical complications in patients with acute ST-segment elevation myocardial infarction

Aim To explore the predictive value of stress hyperglycemia ratio(SHR)for in-hospital mortality and mechanical complications in patients with acute ST-segment elevation myocardial infarction(STEMI).Methods This study constituted a retrospective investigation that collected 995 patients diagnosed with acute STEMI at Suining Central Hospital from June 2019 to July 2023.Comparisons of baseline data were conducted using t-test,Mann-Whitney U test and chi-square test;Logistic regression was used to analyze the association between SHR and the risk of in-hospital mortality and mechanical complications in acute STEMI patients;Restricted cubic spline analysis based on the Logistic re-gression model was utilized to explore non-linear relationship between SHR and the risk of in-hospital mortality and mechan-ical complications;ROC curve was used to evaluate the diagnostic efficacy of SHR;Subgroup analysis was used to assess the predictive efficacy of SHR in each subgroup.Results Patients with high SHR had a significantly higher cardiovas-cular mortality(P=0.007).High SHR was an independent risk factor for in-hospital all-cause mortality(Model 1:OR=3.085,95% CI:1.719~5.538,P<0.001;Model 2:OR=2.738,95% CI:1.4439~5.132,P=0.002),cardiovascular mortality(Model 1:OR=3.406,95% CI:1.869~6.228,P<0.001;Model 2:OR=3.053,95% CI:1.595~5.817,P<0.001),ventricular aneurysm(Model 1:OR=3.203,95%CI:1.665~6.069,P<0.001;Model2:OR=3.93,95%CI:1.785~8.663,P<0.001),cardiac rupture(Model 1:OR=2.461,95% CI:1.389~4.312,P=0.002;Model 2:OR=2.302,95% CI:1.214~4.274,P=0.009)and composite endpoint(Model 1:OR=3.719,95% CI:2.226~6.332,P<0.001;Model 2:OR=2.919,95% CI:1.576~5.405,P<0.001)in patients with acute STEMI.SHR was positively correlated in a linear relationship with the risk of in-hospital all-cause mortality(P for non-linearity=0.250),cardiovascular mortality(P for non-linearity=0.129),ventricular aneurysm(P for non-linearity=0.588),cardiac rupture(P for non-linearity=0.787)and composite endpoint(P for non-linearity=0.399).The SHR had excellent diagnostic efficacy for in-hospital all-cause mortality(AUC=0.694),cardiovascular mortality(AUC=0.697),ventricular aneurysm(AUC=0.706),cardiac rupture(AUC=0.667)and composite endpoint(AUC=0.730),meanwhile SHR predicted efficacy consistently across subgroups.Conclusions High SHR is an independent risk factor for in-hospital all-cause mortality,cardiovascular mortality and cardiac mechanical complications in patients with a-cute STEMI.SHR holds significant predictive value for the prognosis of patients with STEMI.

Objective:To evaluate the long-term effectiveness of lingual mucosal graft ureteroplasty (LMGU) for managing long-segment (≥5 cm) ureteral strictures in a multi-institutional cohort of patients.Methods:A multi-center retrospective case series study was conducted on clinical data from 42 patients undergoing LMGU for long-segment ureteral strictures (≥5 cm) across five institutions between February 2017 and June 2024. The cohort comprised 31 males and 11 females, with an age of (43.4±12.0) years (range: 15 to 64 years) and a body mass index of (24.6±2.6) kg/m2 (range: 16.0 to 30.0 kg/m2). Strictures involved the left ureter in 24 cases and right ureter in 18 cases, demonstrating a stricture length of (6.4±1.5) cm (range: 5.0 to 11.5 cm). Surgical interventions included either onlay ureteroplasty or augmented anastomotic ureteroplasty, selected according to intraoperative findings. Intraoperative parameters, postoperative complications, and follow-up outcomes were analyzed.Results:Laparoscopic surgery was performed in 22 cases and robot-assisted surgery in 20 cases. Among the 42 patients, 22 underwent onlay ureteroplasty while 20 received augmented anastomotic ureteroplasty. The graft length was (5.9±1.8) cm (range: 3.0 to 12.0 cm), operative time (191.5±55.6) minutes (range: 105.0 to 350.0 minutes), and intraoperative estimated blood loss (86.7±73.6) ml (range: 10.0 to 400.0 ml). All procedures were successfully completed without conversion to open surgery. The postoperative hospital stay was (7.6±2.0) days (range: 4.0 to 15.0 days), with double-J stent removal at 6 to 8 weeks postoperatively. During a follow-up of (49.1±25.0) months (range: 12.0 to 99.0 months), no stricture recurrence was observed in any patient.Conclusion:LMGU is a safe, feasible, and effective long-term technique for managing long-segment (≥5 cm) ureteral strictures.

9.Effect and mechanism of LncRNA PICSAR knockdown on proliferation,migration and apoptosis of ovarian cancer cells

Objective To investigate the expression of long non-coding RNA(LncRNA)PICSAR in ovarian canc-er,and explore the effects of LncRNA PICSAR on the proliferation,migration,invasion and apoptosis of ovarian cancer cells as well as its possible mechanism of action.Methods The expression levels of LncRNA PICSAR in o-varian cancer tissue and cell line A2780,OVCAR-3,HO-8910 and normal ovarian tissue and cell line IOSE386 were detected by fluorescence quantitative PCR.Ovarian cancer cell lines with the highest expression of LncRNA PICSAR were divided into control group and experimental group,and transfected with negative control small inter-fering RNA(siRNA-NC)or PICSAR knockout small interfering RNA(siRNA-PICSAR)by liposome transfection technique,respectively.The effects of LncRNA PICSAR knockdown on the invasion,migration,proliferation and apoptosis of ovarian cancer cells were analyzed by cell counting assay(CCK-8),clonogenic assay,scratch assay,transwell assay and flow cytometry and so on.The expression levels of autophagy related proteins and apoptosis-re-lated proteins in each group were determined by Western blot.Ovarian cancer cells were treated with rapamycin and hydroxychloroquine as autophagy activator and inhibitor,and Western blot assay was used to detect apoptosis.Results The expression level of LncRNA PICSAR in ovarian cancer tissues was higher than that in normal ovarian tissues.Compared with IOSE386 cell line,LncRNA PICSAR expression levels in ovarian cancer cell lines HO-8910,OVCAR-3 and A2780 increased.Compared with the si-NC group,the proliferation,invasion and migration ability of ovarian cancer cells in si-PICSAR group decreased,and the apoptosis rate increased.The autophagy level of ovarian cancer cells in si-PICSAR group was lower than that in si-NC group.After transfection with siRNA-PIC-SAR,rapamycin activated autophagy to reduce apoptosis,while hydroxychloroquine inhibited autophagy to promote apoptosis.Conclusion LncRNA PICSAR is highly expressed in ovarian cancer tissues and cell lines,and the ma-lignant biological behavior of ovarian cancer cells can be inhibited by knockout of LncRNA PICSAR.The knock-down of LncRNA PICSAR may promote the apoptosis of ovarian cancer cells by regulating autophagy.
10.DING Yuanqing's Experience in Treating Young and Middle-Aged Post-Stroke Depression Patients with Regulating Qi and Promoting Blood Circulation Method

This paper summarizes the experience of professor DING Yuanqing in treating post-stroke depression (PSD) of young and middle-aged patients with the method of regulating qi and promoting blood circulation. PSD is a syndrome resulting by vascular injury and impairment of brain marrow and vital activity after the stroke. Factors such as poor lifestyle, improper control of chronic diseases and sleep disorders,etc.,which can be harmful individually, or they can interact. Over time,these factors can block yang of defensive qi,obstract blood circulationg and disturb qi movement. Reverse ascending of defensive qi can generate wind and fire,generate phlegm and stasis from the fluid the blood. Qi stagnation, phlegm and stasis can combined with stagnation heat, phlegm heat, blood stasis heat which caused by stroke , which can further aggravate pulse accumulation, damage the blood vessels and block the collaterals. Consequently, defensive qi is floating over and nutrient qi is not smooth, resulting in inadequate nourishment of the brain marrow,and disfunction of vital activity, causing depressive symptoms. Professor DING innovatively applied the method of regulating qi and promoting blood circulation. He selected the classic prescriptions such as Guizhi Decoction(桂枝汤), Baoyuan Decoction(保元汤), as well as self-fitting prescriptions like Erdan Decoction(二丹汤), Erzhu Decoction(二竹汤), to relieve qi and tonify qi,promote harmonious blood circulation, facilitate vasodilation, ease symptoms of depression, invigorate the mind, and provide an effective treatment for PSD.
