1.Analysis of inhalant and food allergen-specific IgE in a children′s hosipital in Suzhou City from 2017 to 2024

To investigate the distribution of common inhalant and food allergens test results in children in Suzhou by gender, age and disease group, and to analyse the changes in allergen distribution in the different years, to provide theoretical guidance for the diagnosis, treatment and monitoring of children′s allergic diseases in Suzhou City. A retrospective cross-sectional study was conducted to analyze the 2 736 children (1 654 males and 1 082 females) who were diagnosed and tested serum content of specific immunoglobulin E (sIgE) antibodies to inhalant and food allergens in the Children′s Hospital of Soochow University between October 2017 and June 2024. The allergen sIgE positive rates epidemiological characteristics were statistically analyzed by Chi square test by grouping based on different year, gender, age and disease. The results showed that a total of 2 736 children screened for allergens, with an overall positive allergen sIgE rate of 73.06%(1 999/2 736), and the top five allergen sIgE positive rates were Dermatophagoides farinae (40.75%, 1 115/2 736), Dermatophagoides pteronyssinus (38.78%, 1 061/2 736), milk (34.65%, 948/2 736), egg whites (32.68%, 894/2 736) and molds and yeasts mixes (mx2) (24.82%, 679/2 736). The positive rates of food allergen sIgE were higher in the 2017-2019 (60.46%, 370/612) than the 2020-2022 (53.79%, 149/227) and after the 2023-2024 (48.46%, 895/1 847) ( χ2=27.059, P<0.001); The positive rates of inhaled allergen sIgE were lower in the 2017-2019 (40.03%, 245/612) than the 2020-2022 (52.71%, 146/227) and the 2023-2024 (56.04%, 1 035/1 847)( χ2=47.223, P<0.001). The positive rates of inhaled allergen sIgE in males (54.96%, 909/1 654) were higher than those in females (47.78%, 517/1 082) ( χ2=13.497, P<0.001). The total positive rate for the food allergen sIgE was highest at 1-3 years of age (67.55%, 589/872) and then gradually decreased with age ( χ2=194.095, P<0.001); The total positive rate of inhaled allergen sIgE was lowest at the age of less than 1 year (8.33%, 22/206) and then gradually increased with age ( χ2=300.329, P<0.001). The positive rates of food allergen sIgE, in descending order from high to low were asthma (59.40%), atopic dermatitis and eczema (48.10%), other groups (48.02%), allergic rhinitis (45.73%) and urticaria (40.00%); The positive rates of inhaled allergen sIgE were asthma (71.05%), allergic rhinitis (63.57%), atopic dermatitis and eczema (62.17%), urticaria (40.00%) and other groups (49.76%), and the difference between disease subgroups were statistically significant( χ2 were 64.841 and 19.055, P<0.05). In conclusion, the top four allergen sIgE positive rates for children in the Suzhou area were Dermatophagoides farinae, Dermatophagoides pteronyssinus, milk and egg white. Total sIgE positive rates for food allergens decreased progressively in the 2017-2019, 2020-2022 and 2023-2024, and total positive rates for inhalant allergens increased progressively in the 2017-2019, 2020-2022 and 2023-2024. The distribution of allergen positive varies with gender, age and disease.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in healthcare facilities in major regions of China in 2023.Methods Clinical isolates collected from 73 hospitals across China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2023 Clinical & Laboratory Standards Institute (CLSI) breakpoints.Results A total of 445199 clinical isolates were collected in 2023,of which 29.0% were gram-positive and 71.0% were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species (excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi) (MRSA,MRSE and MRCNS) was 29.6%,81.9% and 78.5%,respectively.Methicillin-resistant strains showed significantly higher resistance rates to most antimicrobial agents than methicillin-susceptible strains (MSSA,MSSE and MSCNS).Overall,92.9% of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 91.4% of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis had significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 93.1% in the isolates from children and and 95.9% in the isolates from adults.The resistance rate to carbapenems was lower than 15.0% for most Enterobacterales species except for Klebsiella,22.5% and 23.6% of which were resistant to imipenem and meropenem,respectively .Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.6% to 10.0%.The resistance rate to imipenem and meropenem was 21.9% and 17.4% for Pseudomonas aeruginosa,respectively,and 67.5% and 68.1% for Acinetobacter baumannii,respectively.Conclusions Increasing resistance to the commonly used antimicrobial agents is still observed in clinical bacterial isolates.However,the prevalence of important crabapenem-resistant organisms such as crabapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a slightly decreasing trend.This finding suggests that strengthening bacterial resistance surveillance and multidisciplinary linkage are important for preventing the occurrence and development of bacterial resistance.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in tertiary hospitals in major regions of China in 2022.Methods Clinical isolates from 58 hospitals in China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2022 Clinical &Laboratory Standards Institute(CLSI)breakpoints.Results A total of 318 013 clinical isolates were collected from January 1,2022 to December 31,2022,of which 29.5%were gram-positive and 70.5%were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species(excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi)was 28.3%,76.7%and 77.9%,respectively.Overall,94.0%of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 90.8%of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis showed significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 94.2%in the isolates from children and 95.7%in the isolates from adults.The resistance rate to carbapenems was lower than 13.1%in most Enterobacterales species except for Klebsiella,21.7%-23.1%of which were resistant to carbapenems.Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.1%to 13.3%.The prevalence of meropenem-resistant strains decreased from 23.5%in 2019 to 18.0%in 2022 in Pseudomonas aeruginosa,and decreased from 79.0%in 2019 to 72.5%in 2022 in Acinetobacter baumannii.Conclusions The resistance of clinical isolates to the commonly used antimicrobial agents is still increasing in tertiary hospitals.However,the prevalence of important carbapenem-resistant organisms such as carbapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a downward trend in recent years.This finding suggests that the strategy of combining antimicrobial resistance surveillance with multidisciplinary concerted action works well in curbing the spread of resistant bacteria.

6.Clinical study on the treatment of QingxinWendan decoction in bipolar disorder manic episode patients

Objective:To study the clinical effects of QingxinWendan decoction in the treatment of bipolar disorder (BD) manic episode.Methods:60 patients with BD manic episode treated in Hunan Brain Hospital from February 2020 to December 2020 were prospectively selected. They were included in the control group and the observation group according to the random alphabet method, with 30 cases in each group. The control group was treated with magnesium valproate sustained-release tablets, and the observation group was treated with Qingxin Wendan decoction combined with magnesium valproate sustained-release tablets. The curative effect was evaluated after 4 weeks of continuous treatment. The degree of mania before and after treatment was evaluated by Beck-Rafaelsen mania scale (BRMS); the cognitive function before and after treatment was evaluated by Wechsler Adult Intelligence Scale (WAIS-RC) and Wechsler Memory Scale (WMS); The levels of interleukin (IL)-1β, tumor necrosis factor-α (TNF-α), neuron specific enolase (NSE) and amyloid β protein (Aβ) levels were measured by enzyme linked immunosorbent assay (ELISA) before and after treatment. Spearman correlation analysis was used to analyze the correlation between serum NSE and Aβ levels and WAIS-RC and WMS scores in the two groups.Results:The curative effect of the observation group was better than that of the control group, with statistically significant difference ( P<0.05). After treatment, the BRMS scores of the control group and the observation group decreased (all P<0.05), and the BRMS scores of the observation group were lower than those of the control group ( P<0.05); After treatment, the WAIS-RC and WMS scores of the control group and the observation group increased (all P<0.05), and the WAIS-RC and WMS scores of the observation group were higher than those of the control group (all P<0.05). After treatment, the serum levels of IL-1β, TNF-α, NSE and Aβ in two groups were decreased (all P<0.05), and the levels of IL-1β, TNF-α, NSE and Aβ in the observation group were lower than those in the control group (all P<0.05). NSE and Aβ levels were negatively correlated with WAIS-RC and WMS scores (all P<0.05). Conclusions:Magnesium valproate sustained-release tablets combined with Qingxin Wendan decoction in the treatment of patients with BD manic episode were superior to magnesium valproate sustained-release tablets alone in reducing manic score, IL-1β, TNF-α, NSE and Aβ levels, and improving the cognitive function of patients. The use of QingxinWendan decoction on top of valproate extended-release tablet treatment for BD manic episode was superior to treatment with valproate extended-release tablets alone in reducing mania scores, IL-1β, TNF-α, NSE and Aβ levels, as well as improving patients' cognitive function.
7.Retrospective analysis on endoscopic treatment for non-ampullary duodenal mucosal lesions

To study the clinical effect of non-ampullary duodenal mucosal lesions treated by endoscopic resection. A retrospective analysis was performed on the data of 58 cases of duodenal non-ampullary mucosal lesions treated by endoscopic resection from January 2016 to June 2019 from 3 hospitals. Among 58 cases, 27 lesions (46.6%) were located in the duodenal bulb and 31 (53.4%) in the duodenal descending part (including the ball-drop boundary). Forty-six patients (79.3%) received endoscopic mucosal resection, 7 (12.1%) received endoscopic submucosal dissection and 5 (8.6%) received pre-cut endoscopic mucosal resection. Few postoperative complications were found except for 1 case of intraoperative bleeding, 1 case of delayed bleeding 2 days after surgery, 1 case of lesion residual and 2 cases of postoperative abdominal pain. No perforation occurred. Endoscopic treatment of non-ampullary duodenal mucosal lesions is safe and effective.
8.The value of quantitative parameters of gemstone spectral CT in the diagnosis of pulmonary embolism

Objective To explore the value of quantitative parameters of gemstone spectral CT in the diagnosis of pulmonary embolism (PE). Methods 29 patients with PE were examined by gemstone spectral CT under GSI scanning mode.The pulmonary embolus on CTPA image were detected and the location and number of embolus were recorded.A variety of quantitative parameters of the embolic and normal areas were measured by the GSI Viewer.Paired t test was used to compare and analyze the quantitative parameters,and ROC curve was used to evaluate the diagnostic efficacy of quantitative parameters.Results 76 clots were diagnosed by CTPA in 29 patients.The iodine content,slope of energy spectrum curve and corresponding CT of 40 keV in the embolic area were significantly lower than those in the normal area (P<0.001),there were no significant differences in water content and effective atomic number between the embolic and normal area;The ROC curve analysis showed that iodine content had the largest area under the diagnosis curve of pulmonary embolism (0.84)with the specificity of 88% and the sensitivity of 65%.The AUC could be up to 0.89 when three quantitative parameters were combined to get the ROC curve,with the sensitivity of 82% and the specificity of 90%.Conclusion Some quantitative parameters of the pulmonary embolic area are lower than those in the normal area,which indirectly reflect the abnormal blood perfusion of the embolism and provide more basis for the early diagnosis,condition assessment and evaluation of therapeutic effect of PE.

10. Comparison of external fixation with or without limited internal fixation for open knee fractures

Objective:
To explore the characteristics and methods of different fixation methods and prevention of open knee joint fracture.
Methods:
The data of 86 cases of open knee joint fracture admitted from January 2002 to December 2015 in Department of Orthopaedics, Affiliated Hospital of Chengdu University were analyzed retrospectively.There were 65 males and 21 females aged of 38.6 years. There were 38 cases treated with trans articular external fixation alone, 48 cases were in the trans articular external fixation plus auxiliary limited internal fixation group. All the patients were treated according to the same three stages except for different fixation methods. Observation of external fixation and fracture fixation, fracture healing, wound healing and treatment, treatment and related factors of infection control and knee function recovery. χ2 test was used to analyze data.
Results:
Eleven patients had primary wound healing, accounting for 12.8%. Seventy-five patients had two wounds healed, accounting for 87.2%. Only 38 cases of trans articular external fixator group had 31 cases of articular surface reduction, accounting for 81.6%; Five cases of trans articular external fixator assisted limited internal fixation group had 5 cases of poor reduction, accounting for 10.4%; There was significant difference between the two groups (χ2=44.132,
