1.Correlation Study on the Odor Spectrum and Gut Microbiota in Chronic Atrophic Gastritis with Yin Deficiency Syndrome

Objective To collect oral exhaled odor spectrum of patients of chronic atrophic gastritis(CAG)with yin deficiency syndrome and detect their gut microbiota;To elucidate the mechanism of odor changes from the perspective of gut microbiota changes;To provide a basic research for the objectification of TCM olfactory diagnosis in CAG.Methods Totally 110 patients with CAG,including 55 patients with CAG yin deficiency syndrome,55 patients with CAG non-yin deficiency syndrome,and 30 healthy individuals were collected.The electronic nose technology was used to collect the oral exhaled odor spectrum of all subjects,and an improved Transformer model was used to identify the breath odor spectrum of CAG yin deficiency syndrome patients and healthy individuals,CAG non-yin deficiency syndrome patients and healthy individuals,CAG yin deficiency syndrome patients and CAG non-yin deficiency syndrome patients.At the same time,16S rRNA high-throughput sequencing method was used to detect the gut microbiota of the subjects'fecal samples,and the correlation analysis between the odor spectrum characteristics of CAG yin deficiency syndrome and gut microbiota was conducted.Results ① Analysis and recognition of odor spectrum characteristic.Amplitude characteristics:The response curves A,C,D,G,H,I and J of the odor spectrum in the CAG yin deficiency syndrome group and the CAG non-yin deficiency syndrome group were all lower in amplitude than those in the healthy group(P<0.01,P<0.05).Slope characteristics:The slopes of response curves A,B,C,D,E,G,H,I and J in the odor spectrum of the CAG yin deficiency syndrome group and the CAG non-yin deficiency syndrome group were lower than those of the healthy group(P<0.01,P<0.05).Pattern recognition:The accuracy of pattern recognition between the CAG yin deficiency syndrome group and the healthy group reached 0.904,with an area under ROC curve(AUC)of 0.91;the accuracy of pattern recognition between the CAG non-yin deficiency syndrome group and the healthy group reached 0.885,AUC=0.89;the accuracy of pattern recognition between the CAG yin deficiency syndrome group and the CAG non-yin deficiency syndrome group reached 0.747,AUC=0.75.② Species composition:At the genus level,compared with the healthy group,the abundance of Actinomyces,Escherichia-Shigella and Tyzzerella in the CAG yin deficiency syndrome group increased(P<0.05),while the abundance of Prevotella,Sutterella and Subdoligranulum decreased(P<0.05);the abundance of[Ruminococcus]_gnavus_group and Escherichia-Shigella in the CAG non-yin deficiency syndrome group increased significantly(P<0.01),while the abundance of Prevotella and Subdoligranulum decreased(P<0.05).Compared with the CAG yin deficiency syndrome group,the non-yin deficiency syndrome group showed significant enrichment of the Dialister(P<0.05).③ Correlation analysis between odor spectrum characteristics and gut microbiota in CAG yin deficiency syndrome:This study identified 17 bacterial genera that showed positive and negative correlations with the amplitude and slope characteristics of the odor spectrum in CAG yin deficiency syndrome,namely Lachnospiraceae_NK4A136_group,Lachnospiraceae_ND3007_group,Faecalibacterium,UCG-002,UCG-005,Coprococcus,CAG-352,Parabacteroides,Actinomyces,Streptococcus,Anaerostipes,Blautia,Dorea,[Eubacterium]_hallii_group,Phascolarctobacterium,Clostridium_sensu_stricto_1,Enterobacter.The above-mentioned bacterial genera could be classified into the following bacterial families:Trichomonas,Clostridia,Porphyromonas,Actinobacteria,Ruminococcus,Streptococcus,Bacteroidetes,Clostridium,Veillonellaceae and Enterobacteriaceae.Conclusion The use of electronic nose technology can accurately identify the oral exhaled odor of patients with CAG yin deficiency syndrome,CAG non-yin deficiency syndrome,and healthy individuals;the odor spectrum characteristics of patients with CAG yin deficiency syndrome are correlated with multiple bacterial genera,and the changes in related metabolites and gases produced by the disruption of their gut microbiota may be one of the biological bases for the changes in oral exhaled odor in CAG yin deficiency syndrome.
2.Study on the Clinical and Mechanism of Stomach Disease Involving Intestine in Chronic Atrophic Gastritis from the Correlation of"Qi-Bacteria-Symptom"

Objective Chronic atrophic gastritis(CAG)is often accompanied by intestinal flora disorder and intestinal symptoms,forming the phenomenon of"stomach disease involving intestine".This study explored the dynamic correlation between intestinal symptoms and qi-stagnation degree in patients with CAG qi-stagnation syndrome and analyzed the characteristics of gut microbiota from the perspective of"spleen-stomach system serving as the pivotal hub of qi movement"in TCM.Methods According to the syndrome element differentiation method,410 patients with CAG were divided into four groups:non-qi-stagnation group,mild qi-stagnation group,moderate qi-stagnation group and severe qi-stagnation group.Correlation analysis and 16S intestinal flora sequencing technology were used to analyze the correlation and differential flora between the degree of CAG qi-stagnation and intestinal symptoms.Results Patients with CAG qi-stagnation syndrome were often accompanied by intestinal symptoms such as frequent flatulence,poor defecation and alternating loose-constipated stools.The frequency of cases was significantly positively correlated with the degree of qi-stagnation"non-mild-moderate-severe"(P<0.05).There was a difference in the abundance of gut microbiota between the four groups of CAG qi-stagnation none,mild,moderate and severe.The relative abundance of Streptococcus,Subdoligranulum,Eubacterium_coprostanoligenes_group and Haemophilus was positively correlated with the degree of qi-stagnation.The relative abundance of Ruminococcus_torques_group and Butyricicoccus showed a negative correlation,and Haemophilus was statistically significant among the four groups(P<0.05).Conclusion This study can provide clinical evidence and micro-mechanism for the connotation of"gastrointestinal co-morbidities"and"different diseases with the same syndrome",which may open up new ideas for clinical diagnosis and treatment.

This paper focuses on extreme environments and systematically analyzes sleep disorders caused by multiple pathogenesis,including circadian rhythm disorder,yin-yang imbalance and qi-blood disorder under weightlessness,emotional depression due to environmental changes,abnormal diet and excretion,and six excesses pathogenic factors,in accordance with the theory of correspondence between nature and human.It proposes harmonizing yin and yang as the core therapeutic principle and guiding framework,with specific methods including calming rebellious qi-blood,regulating qi movement,harmonizing heart and kidney,and dredging and regulating blood vessels,thus forming the"one guiding principle and four specific methods"treatment strategy.Additionally,by integrating the modified application of classic formulas,a diagnostic and therapeutic approach targeting multi-dimensional pathogenesis is established.This study aims to promote the in-depth integration of Traditional Chinese Medicine(TCM)in extreme environmental medicine through TCM theories and innovative application of prescriptions,provide TCM-characterized solutions for health management of workers in extreme environments,and facilitate the transformation of extreme environmental medicine toward the modern medical model of"prevention-treatment integration".

Uveal melanoma (UM) poses a significant lethality, with approximately 50% of those developing metastases surviving less than one year. In the progression of UM, vasculogenic mimicry (VM) induced by hypoxia plays a pivotal role, which also partially explains the resistance of UM to anti-angiogenic therapies. Nevertheless, the crucial molecular mechanisms underlying VM in the progression of UM remain unclear. We identified ubiquitin conjugating enzyme E2 G2 (UBE2G2) as a critical suppressor through transcriptomic sequencing and metastasis correlation screening. In UM, hypoxia-induced VM and metastasis are markedly exacerbated by UBE2G2 knockdown and significantly alleviated by its overexpression. Mechanistically, UBE2G2 directly binds to galectin 3 binding protein (LGALS3BP) and forms a complex with the E3 ubiquitin ligase tripartite motif containing 38 (TRIM38), facilitating ubiquitination-mediated degradation of LGALS3BP at the K104 residue. Furthermore, UBE2G2 inhibits oncogenic phenotypes by inactivating intracellular PI3K/AKT signaling and reprogramming the tumor microenvironment. Therefore, targeting intercellular and intracellular molecular mechanisms of the hypoxia-UBE2G2-LGALS3BP axis may contribute to developing various therapeutic strategies for UM.
9.Construction and verification analysis of risk prediction model for re-pregnancy failure in patients with unexplained recurrent spontaneous abortion based on LASSO regression analysis

Objective To construct the related factors of re-pregnant failure in the patients with unex-plained recurrent spontaneous abortion(URSA)based on the least absolute shrinkage and selection operator(LASSO)regression analysis,and to construct the prediction model and conduct the verification.Methods The prospective study was adopted to select 420 URSA patients admitted and treated in Hangzhou Municipal Red Cross Hospital from February 2021 to February 2024 as the research subjects.They were divid-ed into modeling group and verification group by simple random sampling method with a ratio of 7∶3.All URSA patients were followed up until termination of pregnancy.They were divided into the pregnancy success group and pregnancy failure group based on the pregnancy outcome.The baseline data of the patients conduc-ted the statistics.LASSO regression was used to screen the risk factors that might affect the re-pregnancy fail-ure of URSA patients for conducting the binary logistic regression analysis,and the nomogram model was con-structed according to the relevant factors and a nomogram model was constructed according to the regression results.The Bootstrap internal verification method was used to verify the nomogram model,and the verifica-tion group data were used to conduct the external verification on the nomogram model.Results The optimal penalty term coefficient λ was determined by the three-fold cross-validation of the LASSO regression model.Nine potential related factors were screened at λ±s(x),which were the age,number of abortions,autoimmune factors,impaired fasting glucose,DFI,number of high-quality embryos,human chorionic gonadotropin(HCG),progesterone(P)and estradiol(E2).The results of binary logistic regression analysis showed that elder,more abortions,abnormal autoimmune factors,impaired fasting glucose and high DFI were the risk fac-tors for re-pregnancy failure in URSA patients(OR=1.286,4.574,3.114,3.204,1.234,P<0.05).The large number of high-quality embryos and high expression of serum HCG,P and E2 were the protective factors for the re-pregnancy failure in URSA patients(OR=0.547,0.826,0.948,0.995,P<0.05).The Bootstrap inter-nal validation method was used to verify the nomogram model of the modeling group and validation group.The consistency index(C-index)values were 0.959 and 0.968,respectively.The calibration curves of the two groups were well fitted with the ideal curve.The results of drawing the receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of the modeling group was 0.959(95%CI:0.937-0.981,P<0.001),the sensitivity was 86.6%,the specificity was 95.7%and the Youden index was 0.823.AUC of the validation group was 0.968(95%CI:0.949-0.988,P<0.001),the sensitivity was 88.5%,the specificity was 97.4%and the Youden index was 0.859.Conclusion The age,number of abor-tions,autoimmune factors,impaired fasting glucose,DFI,number of high-quality embryos,HCG,P and E2 are closely correlated to the failure of re-pregnancy in URSA patients.At the same time,the constructed nomo-gram model based on the above factors has a good predictive efficiency on the pregnant failure of the patients.
10.Application and Thinking on Multimodal Fusion Technology of Traditional Chinese Medicine Inspection

Multimodal fusion research in traditional Chinese medicine (TCM) inspection has greatly improved the accuracy of identification results with the help of emerging technologies in science and technology. At present, multimodal fusion technology is used in the integration of the parameters collected by the inspection instruments and the analysis of the parameters, which is represented by the development and application of the tongue inspection instrument, the face inspection instrument and the eye inspection instrument. However, the multimodal fusion research of TCM inspection has problems such as insufficient comprehensiveness and accuracy of parameter acquisition, difficulty in parameter integration for fusion analysis, and low clinical practicability of identification results. It is believed that, while keeping up with the cutting-edge science and technology, research on multimodal fusion of TCM inspection should be guided by the holism concept of TCM, and should focus on the enhancement of comprehensiveness and accuracy of multimodal parameter acquisition, standardisation of parameter fusion, and fusion of identification results, so as to gradually promote the objectivity, intelligence and modernisation of the four diagnostic methods of TCM.
