1.Characteristics of middle ear cholesteatoma with tympanicsclerosis.

Objective:To compare the preoperative presentation, intraoperative findings, and postoperative outcomes between middle ear cholesteatoma with tympanosclerosis (MECwTS) and middle ear cholesteatoma without tympanosclerosis (MECw/oTS), thereby investigating the clinical characteristics of MECwTS. Methods:A retrospective analysis was conducted on the clinical data of 120 patients with middle ear cholesteatoma. Patients were divided into two groups based on the presence or absence of concomitant tympanosclerosis: the MECwTS group (n=49) and the MECw/oTS group (n=71). All patients underwent preoperative evaluations including temporal bone CT, otoscopic examination, pure-tone audiometry, tympanometry, and assessment using the Zurich Chronic Middle Ear Inventory (ZCMEI-21) quality of life scale. All patients underwent canal wall down mastoidectomy with tympanoplasty. Concurrent ossicular chain reconstruction was performed: partial ossicular replacement prosthesis (PORP) in 83 cases and total ossicular replacement prosthesis (TORP) in 37 cases. Intraoperative disease severity was assessed using the Cholesteatoma Comprehensive Score Scale (CCSS). Postoperative follow-up lasted at least one year and included pure-tone audiometry, otoscopic examination, and the ZCMEI-21 scale administered at ≥1 year post-surgery. Preoperative and postoperative air-bone gap (ABG) and ZCMEI-21 scores were compared between the MECwTS and MECw/oTS groups. Additionally, surgical efficacy was defined as a postoperative ABG ≤20 dB; the hearing improvement efficacy of PORP versus TORP was compared based on this criterion. Results: ①Preoperative ABG showed no significant difference between the MECw/oTS and MECwTS groups(P>0.05). Postoperative ABG was (18.65±10.21) dB in the MECw/oTS group versus (22.55±9.53) dB in the MECwTS group, demonstrating a statistically significant intergroup difference (P<0.05). ②Intraoperative CCSS scores were significantly higher in the MECwTS group (8.04±2.18) compared to the MECw/oTS group (5.93±1.44) (P<0.05). ③Preoperative ZCMEI-21 scores showed no significant difference between groups (P>0.05). Postoperative ZCMEI-21 scores were (22.24±8.11) in the MECw/oTS group versus (27.02±7.21) in the MECwTS group, indicating a statistically significant difference (P<0.05). ④Postoperative ABG ≤20 dB was achieved in 54 patients (65.06%, 54/83) in the PORP group and 16 patients (43.24%, 16/37) in the TORP group. This difference in efficacy rates was statistically significant (P<0.05). The overall efficacy rate for ossiculoplasty was 58.33% (70/120). Conclusion: Patients with MECwTS exhibit more severe middle ear and mastoid pathology compared to those with MECw/oTS, resulting in poorer postoperative hearing levels and quality of life outcomes. Both PORP and TORP implantation can improve postoperative hearing to some extent; however, PORP appears to offer superior hearing improvement efficacy compared to TORP.

2.Rosa laevigata Michx. inhibits pulmonary arterial smooth muscle cell proliferation in hypertension by modulating the Src-AKT1 axis.

OBJECTIVES:
To investigate the synergistic mechanism of the traditional Chinese medicine Rosa laevigata Michx. (RLM) for treatment of pulmonary arterial hypertension (PAH).
METHODS:
Network pharmacological analysis was carried out to screen the active ingredients of RLM and PAH disease targets and construct the "component-target-disease" interaction network, followed by gene enrichment analysis and molecular docking studies. In the cell experiments, primary cultures of rat pulmonary arterial smooth muscle cells were exposed to hypoxia for 24 h and treated with solvent or 100, 200 and 300 mg/mL RLM, and the changes in cell proliferation were detected using Western blotting for PCNA and immunofluorescence staining. In the animal experiment, male SD rats were randomized into 5 control group, monocrotaline (MCT) solvent group, and MCT with RLM (100, 200 and 300 mg/mL) treatment groups. HE staining and immunofluorescence staining were used to observe histopathological changes in the pulmonary blood vessels of the rats.
RESULTS:
Seven core active ingredients (including β-sitosterol and kaempferol) in RLM and 39 key disease targets were identified, and molecular docking showed that SRC was a high-affinity target. KEGG enrichment analysis showed that the differential genes were significantly enriched in calcium signaling and PI3K-AKT pathways. In rat pulmonary arterial smooth muscle cells, hypoxic exposure significantly up-regulated cellular expression of PCNA and phosphorylation levels of Src and AKT1, which were obviously lowered by RLM treatment. In RLM-treated rat models, the mean pulmonary artery pressure and right ventricular hypertrophy index (Fulton index) were significantly reduced, the tricuspid annular plane systolic excursion (TAPSE) was improved, and pulmonary vascular wall thickening and fibrosis were obviously ameliorated.
CONCLUSIONS
RLM inhibits pulmonary arterial smooth muscle cell proliferation in rat models of hypertension possibly by regulating the Src-AKT1 axis, suggesting the potential of RLM as a new natural drug for treatment of pulmonary hypertension.

Background and aims:Metabolic dysfunction-associated fatty liver disease(MAFLD)is a common chronic condition that can lead to cancer due to its complex pathogenesis.Therapeutic agents targeting AMP-activated protein kinase(AMPK)activation have been suggested as potential treatments for metabolic disorders such as metabolic dysfunction-associated steatohepatitis(MASH).Rhizoma Atractylodis Mac-rocephalae(RAM)has been clinically used to treat obesity-related health problems,but its therapeutic effects on MAFLD and the underlying mechanism remain unclear.Therefore,this study was conducted to evaluate the function and underlying mechanism of RAM in the treatment of MAFLD.Methods:The effect of RAM decoction on MAFLD was evaluated using a high-fat diet(HFD)-induced MAFLD mouse model.In vitro studies were conducted using a palmitic acid/oleic acid-induced lipid accumulation model in the alpha mouse liver 12 cells and RAM-containing serum.The underlying mechanisms were elucidated through a combination of network pharmacology analysis,immunohis-tochemistry,western blotting,and polymerase chain reaction analysis.Results:Administration of RAM decoction significantly reduced body weight gain in MAFLD mice without changing food intake.The weights of the liver and inguinal adipose tissues were also reduced after RAM treatment.Additionally,RAM administration decreased serum levels of alanine aminotrans-ferase,aspartate transaminase,total cholesterol,triglyceride,low-density lipoprotein cholesterol,and glucose,while reducing lipid droplet accumulation in the liver tissues of MAFLD mice.The underlying mechanisms included the activation of the phosphorylation of AMPK and acetyl-CoA carboxylase(ACC),and inhibition of the expression of sterol regulatory element binding protein 1(SREBP1).However,RAM did not alter the protein expression levels of peroxisome proliferator-activated receptor α and carnitine palmitoyltransferase-1α.Furthermore,the RAM-induced upregulation of phosphorylated AMPK,phos-phorylated ACC,and SREBP1 expression,as well as the downregulation of fatty acid synthase expression,were reversed by using an AMPK inhibitor.Conclusions:Through a combination of network pharmacology and experimental validation,we demonstrated that RAM may exert therapeutic effects on MAFLD by inhibiting lipid synthesis and activating phosphorylated AMPK pathways.
5.Drug resistance and clinical characteristics of bloodstream infections caused by Enterobacter in children

OBJECTIVE To analyze the drug resistance,laboratory indicators and clinical characteristics of blood-stream infections caused by Enterobacter in children,providing references for early empirical treatment.METHODS A retrospective analysis was conducted on the clinical data of 302 children(aged 29 days to 12 years)with bloodstream infections caused by Enterobacter(monomicrobial infections)admitted to the Maternity and Child Health Care of Guangxi Zhuang Autonomous Region from Jan.2017 to Dec.2023.The drug resistance of Enterobacter and major pathogens were analyzed across different age groups of children.Based on prognosis,the children were divided into a favorable prognosis group(266 cases)and a poor prognosis group(36 cases),and their laboratory indicators and clinical characteristics were compared.Multivariate logistic regression was used to analyze the risk factors for poor prognosis.RESULTS The major pathogens causing infections were Salmonella,Escherichia coli and Klebsiella pneumoniae.The distribution difference of major pathogens across age groups(29 days-6 months,>6 months-1 year,>1-2 years,>2 years)was statistically significant(P<0.001).The drug resistance rates of Salmonella to ampicillin,chloramphenicol and sulfamethoxazole/trimethoprim were 68.61%,54.01%and 44.53%,respectively.E.coli exhibited drug resistance rates of 83.33%,52.22%and 47 78%to ampicillin,sulfamethoxazole/trimethoprim and cefazolin,respectively.K.pneumoniae showed drug resistance rates exceeding 50%to ampicillin/sulbactam,cefuroxime and cefazolin.Hypoalbuminemia(OR=3.319),sepsis(OR=3.122),ventricular purulent encephalitis(OR=5.104)and prior use of penicillin-class anti-bacterial drugs before culture positivity(OR=3.374)were identified as risk factors for poor prognosis of the chil-dren with Enterobacteriaceae bloodstream infections(P<0.05).CONCLUSIONS Bloodstream infections caused by Enterobacter in children predominantly occur in those under 2 years of age,with Salmonella,E.coli and K.pneumoniae as the major pathogens,exhibiting high drug resistance rates to penicillin and cephalosporin antibacte-rial drugs.Clinical therapeutic regimen should be adjusted early based on laboratory indicators and risk factors to improve prognosis.
6.Multimodal MRI-based neurophenotype correlated to structural bowel damage in Crohn's disease

Objective To characterize neurological alterations associated with structural bowel damage in patients with Crohn's disease(CD)through radiomics-assisted neurophenotyping,utilizing multiparametric brain MRI.Methods This prospective study enrolled patients with CD who underwent brain MRI,MR enterography,and ileocolonoscopy within one week.The Lémann Index was used to quantitatively assess cumulative structural bowel damage.CD patients were stratified into two groups based on a cutoff value of 4.8:those with bowel damage(LI>4.8)and those without bowel damage(LI≤4.8).A neurophenotype model was developed to characterize the neural changes associated with bowel damage in CD.Key features were selected from first-order features extracted from multiparametric brain MRI in the training cohort and validated in an independent test cohort.Results The final study population comprised 109 patients,including 51 individuals with bowel damage and 58 without bowel damage.The neurophenotype model scores were 0.785(95%CI:0.506~0.945)in the bowel damage group and 0.155(95%CI:0.093~0.394)in the non-bowel damage group,showing a statistically significant difference between the two groups(P<0.001).The developed model exhibited strong discriminative performance,with area under the receiver operating characteristic curve(AUC)values ranging from 0.824 to 0.918 across the training,vali-dation,and test cohorts(all P<0.05).Conclusion Our radiomics-assisted neurophenotype analysis reveals neural alterations in CD patients with bowel damage,which may indicate extraintestinal manifestations associated with cumulative intestinal injury.
7.Correlation between free triiodothyronine/free thyroxine ratio and early-phase insulin secretion in patients with type 2 diabetes mellitus

Objective To explore the correlation between the ratio of free triiodothyronine/free thyroxine(FT3/FT4)and the early-phase insulin secretion index(ΔI30/ΔG30)in type 2 diabetes mellitus(T2DM)patients with normal thyroid function.Methods 200 patients with T2DM with normal thyroid function in Hebei General Hospital from September 2019 to June 2021 were selected and divided into the Q1 group with FT3/FT4≤0.26(n=67),the Q2 group with 0.27≤FT3/FT4≤0.29(n=67),and the Q3 group with FT3/FT4≥0.30(n=66)according to the tertiles of the FT3/FT4.The general data,biochemical indicators and islet functions of the three groups were compared,and the relationship between the FT3/FT4 ratio and the ΔI30/ΔG30 as well as the islet β cell function was analyzed.Results The fasting insulin(FIns),2 h postprandial insulin(2 hIns),homeostatic model assessment of islet β cell(HOMA-β)and area under the curve of insulin(AUCI)in Q3 group were higher than those in Q1 and Q2 groups(P<0.05),2 h postprandial blood glucose(2 hPG)in Q3 group was lower than those in Q1 and Q2 groups(P<0.05).Systolic blood pressure in Q2 group was higher than those in Q1 group(P<0.05).Compared with Q1 group,diastolic blood pressure,body mass index(BMI),alanine aminotransferase(ALT),fasting C-peptide(FC-P),area under curve of C-P(AUCC),2 h postprandial C-peptide(2 hC-P)and ΔI30/ΔG30 in Q3 group were significantly higher(P<0.05),and HbA1c was significantly lower(P<0.05).Spearman correlation analysis showed that Δ I30/Δ G30,HOMA-β,AUCI,AUCC and HOMA-IR were positively correlated with BMI,ALT,FC-P,2 hC-P,FIns,2 hIns and FT3/FT4(P<0.05),it was negatively correlated with HbA1c and 2 hPG(P<0.05).Linear regression analysis showed that ΔI30/ΔG30,HOMA-β,AUCI and AUCC were the influencing factors of FT3/FT4 after adjusting for confounding factors.Conclusions ΔI30/ΔG30,HOMA-β,AUCI and AUCC are the influencing factors of FT3/FT4 in T2DM patients with normal thyroid function,suggesting that FT3/FT4 is higher in patients with better islet β cell secretion function.
8.Progress in research of health economics in HIV/AIDS prevention and treatment based on CiteSpace software analysis

Objective:An overview of the progress of health economics research in HIV/AIDS prevention and treatment based on CiteSpace analysis.Methods:With the help of visual literature analysis software CiteSpace 6.3.R1, English literature databases Web of Science, PubMed, Chinese literature databases CNKI, Wanfang Data Knowledge Service Platform, and Vip.com were searched to review the Chinese and English literature on health economics research in the field of AIDS prevention and treatment from 2010 to 2023. According to the literature clues, the main model methods in this field are supplemented.Results:The number of English publications averaged 468 per year and showed a gradual decline after reaching a peak in 2018. The number of Chinese publications averaged 50 per year and continued to decline after reaching a peak in 2014. The English literature mostly came from the CDC, HIV/AIDS medical institutions, and university medical departments in Boston, Seattle, and Washington. Most Chinese literature came from the China CDC, Shandong University, Anhui Medical University, etc. The keywords in the English literature study were antiviral treatment, South Africa, HIV detection, and pre-exposure prophylaxis (PrEP), respectively. The most frequent keywords in Chinese literature research are prostitution, mother-to-child transmission, entertainment places, and health education. Measurement methods on AIDS prevention and control service cost mainly include a top-down method, bottom-up method, activity-based costing method, etc; theoretical models are mainly HIV dynamic transmission model, Markov model optimization, and individual-based HIV comprehensive transmission model. Commonly used software tools are Spectrum, Optima HIV, MicroCOSM, EMOD-HIV, etc. Conclusions:In the field of HIV/AIDS prevention and control from 2010 to 2023, the number of English health economics research papers remained at a high level, while the number of Chinese health economics related papers declined significantly. Overseas studies have basically covered the health economics evaluation of a series of whole-chain intervention modes, such as HIV screening, pre-exposure prophylaxis (PrEP), antiviral therapy (ART) and bridging care for key populations. The domestic field needs to combine technological integration and innovation in the field of prevention and control, strengthen the health economics evaluation of innovative service models such as "Internet +" intervention, HIV self-test, PrEP, and rapid ART for key populations, deepen cross-unit and interdisciplinary cooperation, and expand methodological research.
9.Association between dietary behavior and type 2 diabetes in the older adults aged 65 years and over in 18 longevity areas of China

Objective:To explore the impact of plant and animal dietary behaviors on type 2 diabetes mellitus (T2DM) in older adults aged ≥65 in 18 longevity areas of China.Methods:The subjects were 5 223 older adults over 65 years old from the Healthy Ageing and Biomarkers Cohort Study (HABCS) in 18 longevity areas in China. Through a questionnaire survey and physical examination, information about their demographic characteristics, lifestyles, daily activities, self-health status, current diseases, and fasting venous blood were collected. Food Frequency and Questionnaire (FFQ) was used to collect data on food intake frequency. Based on the prior method, the plant-based diet index (PDI) and animal-based diet index (ADI) of 5 223 older adults were calculated. Subjects were divided into three groups (low-level group: PDI<39 or ADI<31, middle-level group: 39≤PDI≤42 or 31≤ADI≤34, high-level group: PDI>42 or ADI>34) by tertiles of PDI and ADI. Multivariate logistic regression was used to analyze the association between PDI and ADI and the risk of T2DM.Results:The average age of 5 223 subjects was (84.8±11.5) years, with the median ( Q1, Q3) of PDI about 41(38, 43) and the median ( Q1, Q3) of ADI about 33 (30, 35). The prevalence rate of T2DM was 16.41% (857/5 223). After adjusting for covariates, multivariate logistic regression showed that PDI was negatively associated with T2DM. Compared with the low-level group, the OR (95% CI) for T2DM in the high-level group was 0.83 (0.69-0.99). ADI was positively associated with T2DM, and compared with the low-level group, the OR (95% CI) for T2DM in the high-level group was 1.28 (1.06-1.55). For every one-point increase in PDI and ADI, the risk of T2DM decreased by 2% and increased by 3%, respectively, with the OR (95% CI) of 0.98 (0.96-1.00) and 1.03 (1.01-1.06), respectively. Conclusion:In Chinese older adults ≥65 years in 18 longevity areas, higher adherence to the plant-based behavior may be negatively associated with the risk of T2DM, while higher adherence to the animal-based behavior may be positively associated with the risk of T2DM.
