Objective To analyze the correlation between triglyceride glucose index (TyG) combined with different obesity indexes and new-onset ischemic stroke (IS) in 10 years in prediabetes population in Luzhou,Sichuan through cohort study. Methods A total of 2551 residents who participated in the epidemiological survey in Luzhou in 2011 and were diagnosed as having prediabetes were enrolled. According to the quartiles of baseline TyG-waist circumference (WC),TyG-waist to height ratio (WtHR),TyG-body mass index (BMI),and TyG-waist to hip ratio (WHR),they were divided into the 1st,2nd,3rd,and 4th quartiles groups (Q1,Q2,Q3,and Q4 groups). The incidence of new-onset IS in each group in 10 years was analyzed. The relationships between TyG-WC,TyG-WtHR,TyG-WHR and new-onset IS were analyzed by multivariate logistic regression. Results Among the 2551 prediabetes subjects enrolled in the study,123 died of various causes during the 10 years of non-intervention follow-up,with a mortality of 4.8%;142 had new-onset IS,with an incidence of 5.6%;and 224 had new-onset diabetes mellitus, including 22 also had new-onset IS,accounting for 9.8%. The quartile grouping analyses of TyG-WC and TyG-WtHR showed that the incidence of new-onset IS in prediabetes subjects over 10 years was Q4 group>Q3 group>Q2 group>Q1 group, with significant differences (P<0.05). The quartile grouping analysis of TyG-BMI showed that there was no significant difference in the incidence of new-onset IS (P>0.05). The quartile grouping analysis of TyG-WHR showed that the incidence of new-onset IS was Q3 group>Q4 group>Q2 group>Q1 group, with significant difference (P<0.05). Multivariate logistic regression analysis showed that TyG-WtHR was associated with new-onset IS in prediabetes subjects, either without adjusting any variable or after adjusting multiple factors. Conclusion TyG-WtHR is a risk factor of IS in prediabetes individuals. Enhancing management of prediabetes patients with high TyG-WtHR may reduce the risk of IS.

Clinical and Molecular Hepatology 2024;30(2):191-205
Background/Aims:
Four-week treatment of linvencorvir (RO7049389) was generally safe and well tolerated, and showed anti-viral activity in chronic hepatitis B (CHB) patients. This study evaluated the efficacy, safety, and pharmacokinetics of 48-week treatment with linvencorvir plus standard of care (SoC) in CHB patients.
Methods:
This was a multicentre, non-randomized, non-controlled, open-label phase 2 study enrolling three cohorts: nucleos(t)ide analogue (NUC)-suppressed patients received linvencorvir plus NUC (Cohort A, n=32); treatment-naïve patients received linvencorvir plus NUC without (Cohort B, n=10) or with (Cohort C, n=30) pegylated interferon-α (Peg-IFN-α). Treatment duration was 48 weeks, followed by NUC alone for 24 weeks.
Results:
68 patients completed the study. No patient achieved functional cure (sustained HBsAg loss and unquantifiable HBV DNA). By Week 48, 89% of treatment-naïve patients (10/10 Cohort B; 24/28 Cohort C) reached unquantifiable HBV DNA. Unquantifiable HBV RNA was achieved in 92% of patients with quantifiable baseline HBV RNA (14/15 Cohort A, 8/8 Cohort B, 22/25 Cohort C) at Week 48 along with partially sustained HBV RNA responses in treatment-naïve patients during follow-up period. Pronounced reductions in HBeAg and HBcrAg were observed in treatment-naïve patients, while HBsAg decline was only observed in Cohort C. Most adverse events were grade 1–2, and no linvencorvir-related serious adverse events were reported.
Conclusions
48-week linvencorvir plus SoC was generally safe and well tolerated, and resulted in potent HBV DNA and RNA suppression. However, 48-week linvencorvir plus NUC with or without Peg-IFN did not result in the achievement of functional cure in any patient.

Chinese journal of integrative medicine 2023;29(3):280-288
Psoriasis is a chronic skin disease and an important health concern. Western medicine and therapies are the main treatment strategies for psoriasis vulgaris (PV); however, the overall prognosis of patients with PV is still poor. Therefore, PV prevention is especially crucial. Chinese medicine (CM) has a long history of treating psoriasis, and it has unique wisdom in different cognitive angles and treatment modes from modern medicine. In this review, we first summarized the herbs and ancient CM formulas that have therapeutic effects on PV. Second, the research status and obstacles to the current development of CM in modern medicine were reviewed. Finally, the future of CM in the context of precision medicine and integrated medicine was discussed. After a detailed reading of the abundant literature, we believe that CM, through thousands of years of continuous development and clinical practice, has achieved high effectiveness and safety for PV treatment, despite its surrounding controversy. Moreover, precise analyses and systematic research methods have provided new approaches for the modernization of CM in the future. The treatment of PV with CM is worth popularizing, and we hope it can benefit more patients.

ObjectiveTo observe the effect of Hoveniae Semen, Flos Puerariae and their combinations on acute alcoholic liver disease and provide a scientific basis for the drug use in clinical practice and the research on other alcoholic diseases. MethodThe acute alcoholic liver injury model of mice was established by one-time gavage with 56% (V/V) Hongxing Erguotou liquor (12 mL·kg-1). One hundred and twenty male ICR mice were randomly assigned into blank group, model group, silybin group, Flos Puerariae group, Hoveniae Semen group, and Flos Puerariae-Hoveniae Semen combination groups (ratios of 1∶1, 1∶2 and 2∶1, respectively), with 15 mice in each group. Each group was administrated with 10 mL·kg-1 corresponding preventive drugs for 3 days by gavage. Except the blank group, the other groups were given Erguotou liquor by gavage at 12 mL·kg-1. The mice were sacrificed 12 h after drinking for the observation of liver function and oxidative stress. The pathological changes of liver were observed via hematoxylin-eosin (HE) staining. Western blot was employed to determine the expression levels of proteins in the Kelch-like Ech-associated protein 1 (Keap1)/nuclear factor E2-related factor 2 (Nrf2)/antioxidant response element (ARE) signaling pathway. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to detect mRNA levels of related genes. ResultCompared with control group, the modeling elevated the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and alkaline phosphatase (ALP) in the serum and the content of malondialdehyde (MDA) and reactive oxygen species (ROS) in liver tissue (P<0.01) and decreased the activities of glutathione (GSH) and superoxide dismutase (SOD) (P<0.01). The mRNA and protein expressions of Keap1 were significantly increased (P<0.05,P<0.01), mRNA and protein expressions of Nrf2 were significantly decreased (P<0.01). Compared with model group, Flos Puerariae-Hoveniae Semen 2∶1 lowered the levels of ALT, AST and ALP in the serum (P<0.01) and MDA and ROS in the liver (P<0.01), and increased the activities of GSH and SOD (P<0.01). Moreover, it alleviated the hepatic steatosis injury, up-regulated mRNA and protein levels of Nrf2 (P<0.01), and down-regulated the mRNA and protein levels of Keap1 (P<0.01). ConclusionFlos Puerariae, Hoveniae Semen and their combinations may exert the pre-protective effect on acute alcoholic liver injury in mice by regulating the Keap1/Nrf2/ARE pathway in the liver and restoring the liver oxidative balance destroyed by ethanol to inhibit the development of alcoholic liver disease .
