Esophageal cancer is a prevalent malignant tumor of the digestive tract in China, and radical surgery remains the cornerstone of its comprehensive treatment. However, multifactorial challenges such as postoperative gastrointestinal tract reconstruction, traumatic stress, and tumor-related metabolic disturbances render esophageal cancer patients highly susceptible to malnutrition. Perioperative nutritional support therapy plays a crucial role in enhancing surgical safety, improving clinical outcomes, and elevating patients' quality of life by regulating metabolic homeostasis, preserving organ function, and optimizing the immune microenvironment. This article reviews the mechanisms underlying malnutrition in esophageal cancer, methods for nutritional status assessment, and precision intervention pathways based on multi-omics evaluations. The aim is to strengthen clinicians' awareness of standardized perioperative nutritional management for esophageal cancer patients and promote its clinical implementation, thereby facilitating postoperative recovery and improving long-term quality of life.

High mobility group box protein 1(HMGB1)is one of the most widely expressed protein member in the HMGs family, which is well known for its involvement in the body inflammatory response.Previous researches have found that it plays a significant role in cell migration, immune identification and neuroprotection.Spinal cord injury is a disease that causes severe damage to the nervous system, and neural circuits are disrupted after a spinal cord injury, which leads to many conditions including ischemia and hypoxia, inflammatory responses, demyelinating lesions, and glial scar formation that are detrimental to nerve regeneration and repair, making it one of the most difficult diseases to treat in the modern spinal surgery field.HMGB1 is upregulated after spinal cord injury, thereby regulating neuroinflam-matory responses, and participating in the neuronal apoptosis, promoting neuronal regeneration, and inducing neural stem cell differentiation and migration, which plays an important role in the process of neural function recovery. This paper summarizes the structure and function of HMGB1,as well as its role in spinal cord injury,in order to provide direction for founding therapeutic target for neurological function recovery after spinal cord injury.

Aim To explore the mechanism by which the compound Chaijin Jieyu formula(CCJJY)regulates the TLR4/NLRP3 signaling pathway to inhibit central oxidative stress and improve hippocampal synaptic plasticity damage in depression. Methods SD rats were randomly divided into the control group, chronic unpredictable mild stress group, sleep deprivation group, chronic unpredictable mild stress combined with sleep deprivation group, positive drug group(venlafax-ine+melatonin),low-dose group of CCJJY,medium dose group of CCJJY,and high-dose group of CCJJY,with nine rats in each group.Except for the control group,a rat model of depression complicated with in-somnia was established using chronic unpredictable mild stress combined with sleep deprivation.Depres-sion-like and sleep behaviors in rats were evaluated through weight, food intake, water maze, and pento-barbital sodium tests.ELisa was used to detect ROS,AANAT,and HPLC-EC was used to detect 5-HT con-tent,while Western blot/RT-PCR was used to detect the expression of IL-1β,TLR4,NLRP3,PSD-95,and SYN related proteins and mRNA.HE and Golgic stai-ning were used to observe the pathological changes in the third ventricle, hippocampus, and neuronal synap-ses.Results Compared with the control group, the depression-like behaviors of the model group rats were significant. The expression of IL-1β,TLR4,and NL-RP3 in the hippocampus increased, while the expres-sion of PSD-95 and SYN decreased.Activation of NL-RP3 inflammasomes led to "sleeve like" pathological changes in the third ventricle, with hippocampal neu-rons undergoing apoptosis and significant damage to neuronal synaptic plasticity.Compared with the model group, after intervention with CCJJY,the expression of ROS,IL-1β,TLR4,and NLRP3 decreased, while the expression of AANAT,5-HT,PSD-95,and SYN in-creased.Pathological damage to the third ventricle and hippocampal neurons was repaired. Conclusion The CCJJY improves hippocampal synaptic plasticity dam-age in depression by regulating the TLR4/NLRP3 sig-naling pathway to inhibit central oxidative stress.

This study was aimed at investigating the effects of demethylase fat mass and obesity-associated protein(FTO)and methyltransferase methyltransferase like protein 3(METTL3),key molecules in N6-methyladenosine(m6A)modification,on the replication and proliferation of Japanese encephalitis virus(JEV).Recombinant lentiviruses were generated by packaging the FTO and green fluorescent protein into lentiviral vectors.Neuro2a cells,a mouse neuroblastoma cell line, were infected with the lentivirus, and stable FTO-expressing cell lines were obtained through puromycin selection.Successful overexpression of FTO was confirmed through fluorescence microscopy,real-time quantitative PCR,and western blot analysis. When Neuro2a cells overexpressing FTO were infected with JEV,the overexpression of FTO decreased JEV replication in the cells, and increased the expression of interferon(IFN)and related molecules.Additionally, treatment of JEV-infected Neuro2a cells with the METTL3-specific inhibitor STM2457 resulted in a dose-dependent decrease in JEV replication and viral protein expression. These findings suggested that lowering m6A methylation levels inhibits JEV replication, thus shedding light on the regulatory role of methylation modification in JEV replication.

Aim To explore the mechanism of baicalin combined with heat stimulation in treating acute lym-phoblastic leukemia(ALL)based on network pharma-cology and in vitro experiments. Methods The CCK-8 assay was used to screen the suitable conditions for heat stimulation to interfere ALL cell lines Jurkat,CCRF-CEM,Hut-78 and a normal lymphocyte HMy2.CIR,and the effects of baicalin combined with heat stimulation on the proliferation of three ALL cell lines and a normal lymphocyte were tested. The key targets of baicalin combined with fever stimulation for the treatment of ALL were obtained based on network phar-macological analysis, and the potential mechanisms were predicted by gene ontology(GO)annotation and kyoto encyclopedia of genes and genomes(KEGG)en-richment.The expression levels of TNF-α,AKT1,TYMS and CASP3 mRNA in ALL cell lines Jurkat and CCRF-CEM were examined by RT-qPCR with baicalin alone and baicalin combined with heat stimulation. Results The optimal conditions for heat stimulation to intervene ALL cells were 41 ℃ for 24 h,and heat stimulation combined with baicalin synergistically inhibited the growth of ALL cell lines and effectively reduced the cy-totoxicity of baicalin.Based on the network pharmaco-logical analysis,55 intersecting targets of baicalin with ALL diseases and 77 intersecting targets of baicalin with fever were obtained. The results of GO annotation and KEGG enrichment suggested that baicalin com-bined with fever stimulation to intervene ALL might be associated with influencing intracellular reactive oxygen species metabolism,DNA transcription and apoptotic processes involved in cysteine enzymes.Apoptosis,TNF and IL-17 signaling pathways were the key pathways for baicalin combined with heat stimulation in treating ALL.Under heat stimulation at 41 ℃ using SDHA gene as housekeeping gene,in vitro experiments showed that baicalin significantly up-regulated the expression of TNF-α and CASP3,and down-regulated the expression of TYMS in ALL cells. Conclusions Based on net-work pharmacologic analyses and in vitro experiments, baicalin combined with heat stimulation can regulate TNF-α and CASP3 gene levels in ALL cells and de-stroy cellular structure to promote cell apoptosis, thus synergistically treating ALL.

8.MKRN3 Regulates Central Precocious Puberty in Children
Chinese Journal of Biochemistry and Molecular Biology 2025;41(1):99-104
The pathogenesis of central precocious puberty(CPP)is due to the early initiation of the function of the hypothalamic-pituitary-gonadal axis, which is driven by the gonadotrophin-releasing hor-mone(GnRH)from the hypothalamus. The increase of GnRH secretion will promote the secretion of gon-adotropin by the pituitary gland, which leads to the development of reproductive organs and the secretion of sex hormones. The timing of puberty onset is influenced by the interaction between nutritional,envi-ronmental,and socioeconomic factors. In the study of pathogenic genes,loss-of-function mutations of MKRN3,LIN28 and DLK1 genes can lead to early onset of puberty, among which loss-of-function muta-tions of MKRN3 are one of the most common single-gene causes of familial CPP.Since 2013,when the MKRN3 mutation was first identified in five CPP families, the role of MKRN3 in adolescence has gradu-ally been discovered.MKRN3 is a member of the E3 ubiquitin ligase family and is expressed in the cen-tral nervous system of many eukaryotes.MKRN3 may act as an E3 ubiquitin ligase to inhibit GnRH activ-ity.Recently,more and more studies have explored the molecular mechanism of MKRN3 in CPP,revea-ling a variety of genes and proteins that interact with MKRN3.For example,MKRN3 can inhibit KISS1 and TAC3 activities, thereby affecting the expression of kisspeptin and neurokinin B to regulate GnRH se-cretion.MKRN3 also inhibited GNRH1 mRNA translation through PABPC1 ubiquitination. In addition, there are other targets such as MBD3,IGF2BP1 and NPTX1,all of which are involved in GnRH regula-tion downstream of MKRN3.Upstream of MKRN3,miR-30 can bind to three sites in the 3'-untranslated region of the MKRN3 gene to block MKRN3 transcription. Here we review the role of MKRN3 in the path-ogenesis of CPP and the research progress of its molecular mechanism, which will help to better under-stand the pathogenesis of CPP,provide new ideas and treatment methods for CPP-related fields in the fu-ture,and lay a theoretical foundation for further exploration of the molecular mechanism of MKRN3.

Objective:To investigate the clinical efficacy and factors influencing treatment of pediatric noninfectious uveitis with Adalimumab (ADA).Methods:A retrospective clinical study. A total of 86 pediatric patients with non-infectious uveitis, diagnosed and treated with ADA at Department of Uveitis Specialist of Xi'an People's Hospital (Xi' an Fourth Hospital) from January 1, 2021 to December 31, 2023, were included in this study. The age of all patients was ≤16 years. Among them, 55 (63.95%, 55/86) patients received ADA combined with one immunosuppressive agent, 28 (32.56%, 28/86) patients received ADA combined with ≥2 immunosuppressive agents, and 3 (3.49%, 3/86) patients received ADA alone without any immunosuppressive agents. All patients underwent best-corrected visual acuity (BCVA) and optical coherence tomography (OCT) examinations. The thickness of the retinal nerve fiber layer (RNFL) in the macular region was measured using an OCT device. The cumulative treatment effectiveness rate at 12 months post-treatment was evaluated using the Kaplan-Meier survival analysis. Multivariate analysis was performed using the Cox proportional hazards regression model, and the optimal predictive model was selected based on the Bayesian information criterion. The association between different treatment regimens and various clinical outcomes was assessed. Results:Among the 86 pediatric patients, 42 were male and 44 were female, with a mean age of (10.47±3.23) years. The distribution of uveitis types was as follows: anterior uveitis in 37 cases, intermediate uveitis in 15 cases, posterior uveitis in 10 cases, and panuveitis in 24 cases. Anterior chamber cells (ACC), keratic precipitates, and synechiae were present in 66, 55, and 38 cases, respectively. The cumulative treatment effectiveness at 12 months was 85.1% [95% confidence interval ( CI) 71.9-92.2], with a median time to treatment effectiveness of 3 months. Compared with baseline, after 6 months of treatment, the BCVA, RNFL thickness ( Z=?6.323, ?8.017), and the grading of ACC and vitreous haze ( χ2= ?6.917, ?5.027) showed significant improvement, with statistically significant differences ( P<0.05). Multivariate analysis revealed that ACC (hazard ratio=22.31, 95% CI 2.43-204.68) and anterior uveitis (hazard ratio=3.88, 95% CI 2.03-7.42) were significantly associated with treatment effectiveness ( P<0.05). Patients with ACC had a median time to treatment effectiveness of 2 months, with a 12-month cumulative treatment effectiveness of 95.5% (95% CI 86.3-98.5). Patients with anterior uveitis had a median time to treatment effectiveness of 2 months, with a 12-month cumulative treatment effectiveness of 97.3% (95% CI 81.3-99.6). Patients without anterior uveitis had a median time to treatment effectiveness of 5 months, with a 12-month cumulative treatment effectiveness of 76.7% (95% CI 54.1-88.2). The cumulative recurrence risk at 12 months was 15.6% (95% CI 6.2-24.1). Conclusion:ADA is safe and effective in treating pediatric non-infectious uveitis, and ACC and anterior uveitis are associated with response rate.

Guillain-Barré syndrome (GBS) is an autoimmune-mediated acute polyradiculoneuropathy. Its key feature is focal segmental demyelination along with inflammatory cell infiltration and axonal damage. In clinical practice, anti-ganglioside antibodies in the body fluid of GBS patients show heterogeneity, and conventional immunotherapy is ineffective in some GBS subtypes. Drawing on treatment options for other neuroimmune diseases, targeted regulation of antibody-complement system has now become an important direction in GBS treatment research. This paper examines the immune response mechanisms involving complement, antibodies and related inflammatory factors, explores new strategy in GBS immunotherapy, so as to provide theoretical basis and research entry points for GBS individualized treatment.
