Spine fracture and dislocation are common traumatic spinal conditions that often require surgical intervention due to compromised spinal stability. Surgical approaches include anterior, posterior, and combined anterior-posterior spinal procedures. According to the specific surgical requirements, patients may be placed in the prone position or repositioned between prone and supine positions during surgery. Intraoperative repositioning has become an essential step in patient positioning. However, during repositioning, patients with spinal fracture and dislocation are at increased risk for complications such as hemodynamic instability, nerve injury, and pressure injuries to the skin and soft tissue. Notably, due to the instability of the spinal cord, even minor manipulations can further exacerbate the damage, potentially leading to severe outcomes like paraplegia. Although the current clinical guidelines provide instructive recommendations for standard position, there remains no specific protocols for intraoperative repositioning in patients with spine fracture and dislocation. With a concern for the lack of clinical studies on positioning techniques, risk prevention, and operational norms for special patients, no applicable guidelines or standards are available. A consensus was required to provide clinical reference, meet the requirements of surgical treatment, and minimize the safety risks of patients caused by improper placement of positions. Professional Committee of Operating Room Nursing of Shaanxi Nursing Association organized experts in nursing management and operating room nursing from major hospitals across China to formulate Expert consensus on intraoperative repositioning for patients with spinal fracture and dislocation ( version 2025). The consensus provides 11 recommendations covering pre-repositioning preparation, intraoperative maneuvers, and post-repositioning observation, aiming to provide references for clinical standardization of the intraoperative repositioning process and protection of patients′ safety.

In this study, potential quality markers(Q-markers) of Schisandrae Chinensis Fructus for treating bronchial asthma were predicted based on analytic hierarchy process(AHP), entropy weight method(EWM), fingerprint, and network pharmacology. AHPEWM was employed to quantitatively identify the Q-markers of Schisandrae Chinensis Fructus. AHP was used to weight the primary indicators(effectiveness, measurability, and specificity), while EWM was employed to analyze the secondary indicators of each primer indicator. Further, through fingerprint combined with network pharmacology, a ″component-target-pathway″ network was constructed to screen the components of Schisandrae Chinensis Fructus for treating bronchial asthma. It was finally determined that schisandrol A,schisandrin A, and schisandrin B were potential Q-markers of Schisandrae Chinensis Fructus in the treatment of bronchial asthma. This study is the first to comprehensively use AHP-EWM, fingerprint, and network pharmacology to screen the key Q-markers of Schisandrae Chinensis Fructus in the treatment of bronchial asthma. This study provides a scientific basis for improving the quality standard of Schisandrae Chinensis Fructus and lays a foundation for studying its material basis in treating bronchial asthma.

7.The Predictive Value of Serum sIL-2R Combined with TNF-α,IgG and IgA in the Recurrence of Multiple Myeloma

Objective:To investigate the predictive value of serum soluble interleukin-2 receptor(sIL-2R),tumor necrosis factor alpha(TNF-α),IgG and IgA for the recurrence in patients with multiple myeloma(MM).Methods:A total of 108 MM patients who were initially diagnosed and treated in our hospital from January 2017 to March 2019,and 72 patients who met the diagnostic criteria and had complete follow-up data were selected as the study subjects.MM recurrence was the endpoint event,and follow-up was conducted until the occurrence of the endpoint event or the deadline of this study.MM patients were divided into recurrent group(RG)and non-recurrent group(NRG)based on whether they have relapsed or not.Venous blood was collected from patients at the first diagnosis and follow-up(at the occurrence of endpoint events or termination of the study),and enzyme-linked immunosorbent assay(ELISA)was used to detect sIL-2R and TNF-α levels in the patient's serum.An automatic immune analyzer was used to detect the levels of IgG and IgA in the patient's serum.The differences in expression levels of the factors between two groups were compared and the correlations between sIL-2R and TNF-α,IgG and IgA at the first diagnosis and follow-up were analyzed.At the same time,venous blood was collected from patients during complete remission,and their serum sIL-2R levels were measured to compare the differences in sIL-2R expression levels at the first diagnosis,complete remission and recurrence.Receiver operating characteristic(ROC)curves was used to determine the optimal cutoff values for serum sIL-2R,TNF-α,IgG and IgA,and the predictive value of sIL-2R,TNF-α,IgG and IgA in the recurrence of MM patients were analyzed based on the area under the curve(AUC).Results:The serum sIL-2R levels of MM patients at the first diagnosis and recurrence were significantly higher than at complete remission(P<0.05).At the first diagnosis,the hemoglobin content of RG was lower than that of NRG,while the β 2-microglobulin content was higher than that of NRG(P<0.001).There was no significant difference in other clinical parameters between the two groups(P>0.05).The levels of sIL-2R,TNF-α,IgG and IgA at the first diagnosis and follow-up of RG were higher than those of NRG(P<0.05).There was a significant correlation between sIL-2R and TNF-α,IgG and IgA at the first diagnosis and follow-up(P<0.001).The ROC curve showed that,at the first diagnosis,sIL-2R,TNF-α,IgG and IgA predicted the AUC of MM patients were 0.919,0.850,0.766 and 0.795,respectively,after follow-up,they predicted AUC of MM were 0.890,0.815,0.760 and 0.794,respectively(P<0.001).Conclusion:The serum sIL-2R has the highest predictive value for MM patient's recurrence,and it is possible to detect the TNF-α,IgG and IgA levels at specific times to infer changes in sIL-2R levels and evaluate the patient's prognosis.
8.Determination of concentration and pharmacokinetics of 3-demethylcolchicine in rat plasma by HPLC

Objective To establish a method based on high performance liquid chromatography for the determination of 3-demethylcolchicine(3 DMC)in rat plasma and to study its pharmacokinetic behavior in rats.Methods The concentration of 3 DMC in rat plasma was determined by high performance liquid chromatography(HPLC).Plasma samples were precipitated by acetonitrile and supernatant was centrifuged for analysis.The determination conditions were as follows:The chromatography was performed on Alphasil VC-C18 column(4.6 mm × 150.0 mm,5.0 μm)with mobile phase consisted of water-acetonitrile-methanol(79∶16∶5),iso-elution at the flow rate of 0.8 mL·min-1.The detection wavelength was set at 240 nm,with polydacryin as the internal standard.The specificity,standard curve and lower limit of quantitation,precision and recovery,matrix effect and stability of the method were investigated.The rats were randomly divided into low,medium and high dose groups(1,2 and 4 mg·kg-1 3DMC were injected into the tail vein,respectively),and blood was collected from orbital venous plexus.The concentration of 3DMC in plasma samples was measured;the pharmacokinetic parameters were calculated using Drug And Statistics(DAS)2.0.Results 3DMC has a good linear relationship between 0.39 and 25.00 μg·mL-1,and its standard curve is y=5.96 × 10-2x+7.60 × 10-3(r=0.999 5).The lower limit of quantitation was 0.39 μg·mL-1,the intra-day and inter-day relative standard deviations were-5.08%to 0.75%,the recovery was 93.23%to 108.90%,the matrix effect was 94.88%to 104.00%,and the stability relative standard deviation(RSD)was less than 8.72%.All the results met the requirements.Pharmacokinetic parameters of low,medium and high dose groups:Cmax were(1.66±0.24),(4.36±0.78)and(9.73±1.42)mg·L-1,respectively;t1/2 were(0.31±0.02),(0.32±0.11)and(0.48±0.06)h,respectively;AUC0-t were(0.50±0.09),(1.53±0.16)and(2.67±0.11)mg·L-1·h,respectively.Conclusion The established method for the determination of 3DMC concentration in rat plasma is simple,specific and sensitive,and is suitable for the determination of 3DMC concentration and pharmacokinetic study in rat plasma.

Coronary atherosclerotic heart disease(CHD)is an ischemic cardiovascular condition caused by the narrowing or blockage of the vascular lumen due to coronary atherosclerosis.Clinically,it presents as angina pectoris,heart failure,or sud-den cardiac death,and stands as one of the primary causes of mortality among both urban and rural populations in China.Cir-cRNA,classified as non-coding RNAs,can function as upstream regulatory molecules for miRNA or RNA-binding proteins.They actively participate in various pathological processes associated with CHD,including endothelial cell dysfunction,smooth mus-cle cell migration,macrophage-derived foam cell formation,an-giogenesis,myocardial injury,and repair,as well as post-in-farction heart failure.The expression pattern of these molecules is highly specific to the illness and tissue,indicating their poten-tial as therapeutic targets for disease management and as biomar-kers.Furthermore,they also open up new avenues for drug tar-get development in the field of traditional Chinese medicine.This article aims to provide an overview of the recent research progress on circRNA in the regulation of coronary heart disease,as well as the mechanisms involved in traditional Chinese medi-cine.It serves as a valuable reference for future research on cor-onary heart disease.
