Under the background of rapid development of artificial intelligence(AI),this paper systematically proposes AI-based education(AIBE).It empowers the teaching process,learning process,research process,and teaching management with AI,and constructs an AI-based educational paradigm,including AI-based teaching(AIBT),AI-based learning(AIBL),AI-based re-search(AIBR),and AI-based management(AIBM).Taking the AI course of immunology teaching as an example,this paper deeply analyzes the practices and explorations of implementing AIBT,AIBL,AIBR and AIBM based on the AI course,so as to accelerate the promotion of the transformation of the fourth generation of medical education.
2.Molecular epidemiological characteristics of 87 Clostridioides difficile isolates in Xi'an region

OBJECTIVE To understand the molecular epidemiological characteristics,antimicrobial sensitivity and resistance mechanisms of Clostridioides difficile in Xi'an region,and provide data support for the prevention and control of C.difficile infection and the rational clinical use of antibiotics.METHODS A total of 87 strains of C.difficile,which were successfully isolated from stool samples collected from 6 hospitals from Oct.2018 to Dec.2022,were tested for virulence genes,population structure and genetic diversity were detected by Multilocus sequence typing(MLST)and Ribotyping(RT)methods,the drug sensitivity was detected by Etest,additionally,amino acid variations in the quinolone resistance-determining regions GyrA and GyrB were detected.RESULTS There were 41 strains(62.12%)with the genotype A+B+CDT—,23 strains(34.85%)with the genotype A-B+CDT—,and 2 strains(3.03%)with the genotype A+B+CDT+.MLST was divided into 25 ST types,and the main types were ST3,ST42 and ST39.There were 37 RT types,mainly were RT012,RT106 and RT001.All strains were sensitive to vancomycin and metronidazole,and the resistance rates to ciprofloxacin,levofloxacin and moxifloxacin were 90.80%,28.73%and 21.84%,respectively.GyrA contains two amino acid variations at Thr82-Ile and Asp205-Glu,and GyrB contains 6 amino acid variations.CONCLUSIONS The predominant toxin-producing strain of C.difficile in Xi'an is of A+B+CDT-genotype and the primary molecular types are ST54/RT012,ST42/RT106,ST3/RT001 and ST37/RT017.No strains resistant to vancomycin or metronidazole are detected.Amino acid variations in GyrA or GyrB of C.difficile are associated with quinolone resistance.To ef-fectively prevent the outbreak of C.difficile infection,it is crucial to enhance molecular epidemiology studies and strengthen antimicrobial resistance surveillance efforts.
3.CT and MRI manifestations of gastritis cystica profunda

Objective To observe CT and MRI manifestations of gastritis cystica profunda(GCP).Methods Seventeen patients with GCP confirmed by operation or biopsy pathology were enrolled,and lesions'CT and MRI manifestations were observed.Results Among 17 cases,16 cases(16/17,94.12%)were found with single lesion and 1(1/17,5.88%)with diffuse multiple lesions.The lesion located in the fundus of stomach in 5 cases(5/17,29.41%),in the body of stomach in 4 cases(4/17,23.53%),in the cardia and antrum of stomach each in 3 cases(3/17,17.65%)and in the pylorus in 1 case(1/17,5.88%),while 1 case(1/17,5.88%)was found with diffused multiple lesions within stomach.Non-enhance CT showed local thickening of gastric wall in 10 cases(10/17,58.82%),all were isodensities,and the mucosa uniformly enhanced in contrast enhance CT(CECT).Predominately cystic lesion in 5 cases(5/17,29.41%)presented as submucosal cystic protrusions,and grew into the stomach cavity with circular or oblong low density in non-enhanced CT,while sandwich enhancement of mucosa was observed in CECT.Among these 5 cases(5/17,29.41%),MRI showed lesion confined to the submucosa with low signal on T1WI and high signal on T2WI,while diffusion weighted imaging showed unrestricted diffusion,and the enhancement pattern was consistent with that of CT in 2 cases.In other 2 cases(2/17,11.77%)with cystic-solid lesion,non-enhanced CT showed soft tissue density,while CECT showed lump-like stratified enhancement.Conclusion CT and MRI manifestations of GCP had certain characteristics.

BACKGROUND:Cervical spondylosis is a chronic degenerative disease that has become one of the most common and frequent diseases threatening human health.At present,the initial diagnosis of the cervical spine and its surrounding structures mainly relies on the interpretation of medical images by radiologists,which not only requires a high level of technical requirements for operators,but also has the disadvantages of strong subjectivity,high labor intensity,and low efficiency.With the rapid development of artificial intelligence technology,its powerful data processing and image recognition capabilities have shown broad application prospects in the medical field.Deep learning has also made certain progress in the research of spinal diseases.OBJECTIVE:To summarize the current status and research progress in the application of artificial intelligence technology in cervical spine imaging images in recent years,evaluating the performance of artificial intelligence models as well as future trends and challenges to be overcome.METHODS:The first author searched the relevant articles in WanFang,CNKI,and PubMed in June 2024.The Chinese search terms were"artificial intelligence,deep learning,cervical spine."English serach terms were"artificial intelligence,Al,cervical vertebrae,cervical."Finally,101 articles were included and analyzed.RESULTS AND CONCLUSION:(1)Artificial intelligence technology can realize automatic segmentation of cervical vertebrae and measurement of curvature change by segmentation,classification,landmarks recognition of medical image parts,detect cervical vertebral fracture,nerve root,and spinal cord type cervical spondylosis,identify cervical spine ossification of posterior longitudinal ligament,and predict post-surgery related risk factors and cervical vertebra maturation classification.(2)Although artificial intelligence technology has shown great potential in the field of cervical spine research,it is still in the early stages of exploration and rapid development,with unlimited room for development and innovation.
6.Application progress of micro-CT and finite element analysis techniques in scaphoid bone research

The scaphoid bone is one of the important bone of hand,which is frequently injured and difficult to treat in clinical practice.Therefore,it is very important to deeply study the microstructure and biomechanical characteristics of the scaphoid bone for understanding its injury mechanism and optimizing treatment scheme.Microcomputed tomography(micro-CT)provides high-resolution imaging of bone tissue,while finite element analysis can help to simulate the stress distribution and behavioral patterns of the scaphoid bone under various physiological and pathological states.The high-resolution three-dimensional image of the scaphoid bone obtained by micro-CT technology can be used to construct finite element models of real anatomical structure of the scaphoid bone,thus achieving accurate simulation of the mechanical properties of the scaphoid bone.The fusion of these two advanced technologies provides a new perspective for revealing the structural and functional relationships and injury mechanism of the scaphoid bone.Therefore,this paper reviews the anatomical characteristics of the scaphoid bone and its biomechanical behavior in different states,emphasizing the specific applications and advantages of micro-CT and finite element analysis techniques in the study of the scaphoid bone.By summarizing the research findings in recent years,this paper provides novel scientific basis and methods for the diagnosis,treatment,and prevention of scaphoid bone-related disorders.
7.Medical resource consumption of healthcare-associated infection based on disease diagnosis-related grouping payment model

Objective To analyze the medical resource consumption of healthcare-associated infection(HAI)in patients in different groups of disease diagnosis-related grouping(DRG)based on the DRG payment model,provide reference for optimizing prevention and control of HAI as well as resource management.Methods Medical records and DRG-related indicators of discharged patients from a municipal hospital in Sichuan Province from January 1 to December 31,2024 were analyzed retrospectively.Medical resource consumption of patients in HAI group and non-HAI group was compared.Differences in average length of hospital stay and average expense per hospitalization be-tween two groups of patients were analyzed using stratified analysis.Results In 2024,HAI incidence of discharged patients in DRG management in this hospital was 1.57%.There were statistically significant differences in age,gender,admission and discharge ways between the HAI group and the non-HAI group(all P<0.05).The main HAI sites were lower respiratory tract,surgical site,urinary tract,and blood.The time consumption index(1.63 vs 0.85),average length of hospital stay(21.00 vs 5.00 days),expense consumption index(1.53 vs 0.92),ave-rage expense per hospitalization(44 700 vs 7 300),and multiple expense in HAI group were all higher than those in non-HAI group(all P<0.05).The consumption of medical resources for bloodstream infection was relatively higher.Patients with HAI were mostly concentrated in the groups related to acute leukemia with major complications or co-morbidities(MCC),intracranial or craniotomy surgery with MCC,tracheotomy with mechanical ventilation for 96 hours,as well as gastric,esophageal,and duodenal surgery.The average length of hospital stay and average ex-pense per hospitalization of patients in HAI group were both higher than those in the non-HAI group,differences were statistically significant(both P<0.05).Conclusion HAI significantly increase the consumption of medical resources.Based on DRG analysis,key disease groups for infection prevention and control can be further identified,and the consumption of medical resources can be more accurately and precisely evaluated,thereby optimizing the allocation of medical resources and improving hospital operational efficiency.
8.Sj?gren disease complicated by primary breast lymphoma:A case report

This case report describes the diagnostic and therapeutic management of a 67-year-old fe-male with a 40-year history of Sj?gren disease(SjD)who was hospitalized for evaluation of recurrent fever lasting over one month.The patient's initial diagnosis of SjD was established four decades earlier based on clinical manifestations,serological findings,and evidence of glandular damage.Her clinical presenta-tion included recurrent parotid gland enlargement accompanied by sicca symptoms,notably persistent xerostomia and xerophthalmia,followed by progressive dental caries.Serological studies demonstrated positivity for antinuclear antibodies,anti-SSA/Ro,and anti-α-fodrin antibodies.Objective assessments confirmed significant ocular involvement(Schirmer's test≤5 mm/5 min)and pulmonary interstitial changes on chest CT,consistent with the 2016 American College of Rheumatology and European League Against Rheumatism(ACR/EULAR)classification criteria for SjD.The patient's condition remained sta-ble under low-dose corticosteroids and disease-modifying anti-rheumatic drugs(DMARDs)until the re-cent onset of prolonged fever,necessitating evaluation for fever of unknown origin.Differential diagnoses considered disease flare,infection,and malignancy.The European Sj?gren's Syndrome Disease Activity Index(ESSDAI)score was 5 points,indicating moderate systemic disease activity.Initial laboratory in-vestigations revealed no evidence of infection,and empirical anti-infective therapy proved ineffective.No-tably,despite the absence of lymphadenopathy,laboratory findings including borderline positive IgM λ M-protein,elevated lactate dehydrogenase,hyperferritinemia,and increased β2-microglobulin levels raised suspicion for lymphoproliferative disorders,given the established association between SjD and lymphoma.Bone marrow aspiration showed no significant abnormalities,but PET/CT imaging detected hypermetabolic lesions in the left breast and right distal femur,suggesting potential malignancy.Subse-quent histopathological examination of the breast lesion confirmed non-Hodgkin's lymphoma(NHL),specifically diffuse large B-cell lymphoma(DLBCL)of the germinal center B-cell(GCB)subtype.Treatment with R-CHOP chemotherapy(rituximab,cyclophosphamide,doxorubicin,vincristine,and prednisone)induced complete metabolic remission after three cycles.However,she subsequently developed treatment-related complications,including myelosuppression and pulmonary infection.This case underscores the importance of maintaining a high index of suspicion for atypical site involvement in SjD patients,particularly when lymphoma risk factors are present.Comprehensive differential diagnosis should include lymphoma and other malignancies,and the diagnostic value of PET/CT and histopatholog-ical examination in disease evaluation is emphasized.SjD complicated by breast lymphoma is exceptional-ly rare,and its pathogenesis may involve lymphocytic infiltration,abnormal activation of lymphocytes,formation of ectopic germinal centers in the breast,and eventual malignant transformation.These mecha-nisms require further investigation through clinical and basic research studies.

Stress refers to the non-specific responses of a stimulated body to different stressors and the subsequent maintenance or restoration of internal environmental homeostasis.Adverse stress reactions lead to general balance disruption and may cause digestive,neurological,and endocrine disorders,and decreased immune capacity,which seriously impact host health.As the core compo-nent of intestinal micro-ecology,the intestinal flora can greatly alter its own composition,distribu-tion,function,metabolic product output,and other aspects during stress,which cause disorders and aggravate homeostatic imbalance in internal environments.While the intestinal flora is of great sig-nificance to animal medicine and agricultural production,little is known about stress and its impact on intestinal flora.Therefore,we briefly reviewed the impact of stress on animal intestinal flora in combination with the latest research and provided theoretical insights on intestinal health research.
10.One case of elderly pregnant women with acute ST-segment elevation myocardial infarction was treated

Patients with advanced pregnancy complicated by acute ST-segment elevation myocardial infarction are relatively rare both domestically and internationally,and there are currently no relevant guidelines to guide clinical treatment.In this case,a 39-year-old pregnant woman was admitted to the hospital with sudden chest pain,and the electrocardiogram showed extensive anterior ST-segment elevation myocardial infarction.Coronary angiography showed subtotal subtotal occlusion of the proximal left anterior descending artery.Reperfusion and revascularization were given,and the blood flow grade of the trial of thrombolytic in myocardial infarction was restored.Complete intravascular ultrasonography to determine the cause of occlusion due to spontaneous hematoma of the left anterior descending artery.After the operation,a reasonable antiplatelet and anticoagulant treatment regimen was selected according to the coronary artery lesion,and the pregnancy was terminated at the appropriate time,and 1 live male baby was successfully delivered.Maternal and fetal health was followed up after 1、3 and 6 months.Through the experience and summary of the treatment process,combined with the literature,the pathogenesis of this disease is discussed,and the strategy of revascularization,the selection of antithrombotic drugs and the timing of pregnancy termination are discussed in depth for clinical reference.
