1.The Sequential Mediating Roles of Body Pain and Self-Reported Health Status in the Relationship between Sleep Duration and Life Satisfaction.

OBJECTIVE:
This study examines the sequential mediating roles of body pain and self-reported health in the association between sleep duration and self-reported life satisfaction among elderly Chinese adults.
METHODS:
Data from the fifth wave of the China Health and Retirement Longitudinal Survey (CHARLS) were used to analyse the relationships between sleep duration and body pain, self-reported health, and life satisfaction through logistic regression and Restricted Cubic Spline (RCS) analyses. The sequential mediation effects of body pain and self-reported health status were examined via chain mediation analysis.
RESULTS:
Logistic regression analysis showed that sleeping fewer than 6 hours or 6-7 hours was linked to higher risks of body pain, poor health, and dissatisfaction with life compared to sleeping 7-8 hours (all P < 0.05). Additionally, those sleeping more than 9 hours also had increased risks of poor health and dissatisfaction with life compared to those sleeping 7-8 hours (all P < 0.05). Chain mediation analysis showed that body pain and self-reported health status sequentially mediated 46.15% of the association between sleep duration and life satisfaction.
CONCLUSION
Body pain and self-reported health may shape the relationship between sleep duration and life satisfaction in elderly Chinese adults.

2.Value of Repeat Renal Biopsy in the Treatment and Prognosis of Patients With Severe Lupus Nephritis.

Objective To investigate the value of repeat renal biopsy in the treatment and prognosis of nephrotic syndrome(NS)and acute kidney injury(AKI)following immunosuppressive therapy in patients with lupus nephritis(LN). Methods A retrospective analysis was conducted for the clinicopathological data and follow-up records of LN patients undergoing repeat renal biopsy at Peking Union Medical College Hospital from January 1,2009 to December 31,2021. Results A total of 76 patients(55 females,72.4%)were included in this study,with the mean age at the first biopsy being(29.0±10.4)years,the median inter-biopsy interval of 4.0(2.0,7.0) years,and the median total follow-up duration of 7.5(5.0,13.8)years.Pathological transformation occurred in 46(60.5%)patients,and 2 patients had comorbid diabetic nephropathy.At repeat renal biopsy,50(65.8%) patients presented NS.These patients demonstrated lower estimated glomerular filtration rate(eGFR)(P<0.001),higher chronicity index(CI)(P=0.029),and higher complement C3(P<0.001)and C4(P<0.001)levels than those with NS at the first renal biopsy(n=50).Among the 28(36.8%) patients with AKI at repeat renal biopsy,8(28.6%)experienced acute exacerbation of chronic renal insufficiency.These patients exhibited higher serum creatinine level(P=0.002),C4 level(P=0.033),CI(P=0.042),and prevalence of thrombotic microangiopathy(P=0.046)than the patients showing AKI at the first renal biopsy(n=16),while the activity index(AI)showed no significant difference(P=0.051).Over 50% of NS and AKI patients underwent treatment modifications post-repeat renal biopsy,with clinical remission rates comparable to those after the first renal biopsy(both P>0.05).Elevated CI(≥5,P=0.001)and serum creatinine(≥140 μmol/L,P<0.001)at repeat renal biopsy were identified as independent risk factors for poor prognosis.The patients with AKI at repeat renal biopsy had higher incidence of endpoint events than the non-AKI patients(P=0.015).Neither AKI at the first renal biopsy nor NS at both biopsies had significant associations with prognosis. Conclusions Repeat renal biopsy reveals not only sustained high disease activity but also accelerates chronic progression in LN patients,which underscore its critical role in guiding the therapy for severe LN post-immunosuppression.AKI,CI≥5,and serum creatinine ≥140 μmol/L at repeat renal biopsy are strongly associated with poor prognosis.

3.Analysis and clinical characteristics of SLC26A4 gene mutations in 72 cases of large vestibular aqueduct syndrome.

Objective:To explore the genetic and clinical characteristics of Guizhou patients with enlarged vestibular aqueduct(EVA) syndrome through combined SLC26A4 variant analysis and clinical phenotype analysis. Methods:Seventy-two EVA patients underwent comprehensive genetic testing using a multiplex PCR-based deafness gene panel and next-generation sequencing(NGS). The audiological and temporal bone imaging characteristics were compared across mutation subtypes. Results:A total of 27 pathogenic loci of SLC26A4 were detected in 72 patients, including c.919-2A>G in 79.2%(57/72). A novel deletion(c.1703_1707+6del) was discovered. Among 65 cases, truncated mutations were 89.2%(58/65), 52.3%(34/65), 28(43.1%) and 7(10.8%). No significant differences were observed in the midpoint diameter of the vestibular aqueduct and the incidence of incomplete partitioning typeⅡ(IP-Ⅱ) of the cochlea among the three groups of patients. Moreover, there was no difference in the midpoint diameter of different vestibular pipes or the combination with IP-Ⅱ. Conclusion:The most common mutation site of SLC26A4 in EVA patients in Guizhou is c.919-2A>G, though genotype-phenotype correlations remain elusive. The detection of 27 mutation sites and the discovery of new mutation sites suggested the precise diagnostic significance of NGS technology in EVA patients in Guizhou.

4.Study on the current situation and influencing factors of nutritional risk in children in PICU

Objective:To investigate the nutritional risk status of children in PICU and analyze its influencing factors.Methods:From July 2021 to February 2023,all children aged 1 to 18 years admitted to PICU of Beijing Children's Hospital were investigated by using the pediatric Yorkhill Malnutrition Scoring tool(PYMS)and the clinical data questionnaire.Results:A total of 492 children in PICU were enrolled.The first nutritional risk screening results showed that there were 32 cases of no/low nutritional risk(6.5%),76 cases of medium risk(15.4%),and 384 cases of high risk(78.1%).The incidence of medium/high nutritional risk was as high as 93.5%.The PYMS score of nutritional risk in PICU was(2.61±1.42).The results of multiple linear regression analysis showed that weight,fever time before admission,white blood cells,body mass index,primary diagnosis,father's education,and diet before illness were the main influencing factors of nutritional risk of children in PICU(P<0.05).Conclusion:Children in PICU are in a state of high nutritional risk.It is suggested that children in PICU should carry out nutritional screening in a standardized manner,identify children with high nutritional risk and its influencing factors early.To actively conduct nutritional assessment and nutritional intervention could improve the clinical outcome of children in PICU.
5.Research Progress on Predicting Microvascular Infiltration in Hepatocellular Carcinoma Using Multimodal Magnetic Resonance Imaging

There was a close relationship between microvascular invasion(MVI)and the therapeutic efficacy and prognosis of hepatocellular carcinoma.With the continuous development of imaging examination methods,multimodal magnetic resonance imaging(MRI)was a non-invasive and quantitative examination method that com bines multiple imaging modes,including magnetic resonance T1/T2 weighted imaging(T1/T2WI),functional MRI[diffusion-weighted imaging(DWI),intravoxel incoherent motion(IVIM)and diffusion-kurtosis imaging(DKI)],dynamic contrast-enhanced MRI(DCE-MRI),hepatobiliary phase imaging(HBP),etc,it can obtain various imaging parameters and provide more comprehensive and accurate diagnostic information for clinicians,it has been widely used in the diagnosis and grading,efficacy evaluation,prognosis,and MVI prediction of hepatocellular carcinoma.Based on this,this article reviews the relevant literature on the application of multimodal MRI in MVI of hepatocellular carci noma in recent years,and summarizes the research status and progress of multimodal MRI in predicting hepatocellular carcinoma MVI,the aim was to further analyze the challenges of multimodal MRI in the application of hepatocellular carcinoma MVI,promote the clinical application of predicting hepatocellular carcinoma MVI,and improve the accuracy of predicting of hepatocellular carcinoma MVI.
6.Role and mechanism of TDO2 mediated apoptosis of renal tubular epithelial cells in Cis-AKI

Aim To investigate the role of tryptophan 2,3-dioxygenase(TDO2)in cisplatin-acute kidney in-jury(Cis-AKI)and to explore the mechanism of TDO2 in relation to apoptosis in tubular epithelial cells(TECs)to investigate the mechanism of TDO2 associ-ated with apoptosis.Methods An AKI model was es-tablished by intraperitoneal injection of cisplatin(Cis).Colorimetric assay was used to detect CRE and BUN levels,and PAS staining was employed to observe renal injury in mice.Immunohistochemistry was used to detect TDO2 protein expression and distribution and macrophage(F4/80+)infiltration;immunofluores-cence was used to detect the co-localization of TDO2 with the tubular marker LTL;TUNEL staining was used to detect apoptosis in mouse kidney;flow cytome-try was used to detect overexpression of human renal cortical proximal tubular epithelial cells(HK2)and apoptosis after administration of the TDO2 inhibitor 680C91;Western blot was used to detect TDO2 and NF-κB pathway protein levels in HK2 cells after over-expression and inhibition of TDO2.Results In the o-verall animal experiments,Cis-AKI mice showed signif-icantly higher levels of CRE and BUN and obvious tu-bular damage compared with the control group;at the same time,the renal tissues of Cis-AKI mice showed increased expression of F4/80,and the proportion of apoptotic cells in kidney cells was increased.Immuno-histochemistry and immunofluorescence showed that the expression of TDO2 increased,mainly localized in TECs.In cellular experiments,HK2 cells overexpress-ing TDO2 increased the proportion of apoptosis,and the expression of TDO2,p-IKBα,and p-p65 proteins was elevated,and p-IKBα/IκBα and p-p65/p65 were ele-vated;furthermore,the proportion of apoptosis was re-duced by the administration of 680C91,and the expres-sion of p-IκBα,and p-p65 proteins decreased,and the expression of p-IKBα/IKBα,and p-p65/p65 de-creased.Conclusions Elevated TDO2 in TECs is in-volved in the pathological mechanism of Cis-AKI,which may be related to its induction of apoptosis in TECs and activation of the NF-κB signaling pathway and consequently renal injury.
7.Teaching Case Construction and Implementation of The Biology Professional Degree Postgraduate—Taking'Advanced Immunology'Course as an Example

The case teaching mode,as an important practical way of teaching reform in graduate educa-tion,plays a positive role in stimulating learning interest and improving comprehensive ability.As a core course in biology,advanced immunology is characterised by the depth of its theoretical system and the boundaries of its research scope.In this paper,we first analyzed the significance and current situation of case library construction at home and abroad,and pointed out the key issues that need to be further opti-mised in the existing case teaching mode.Based on the construction strategy of"Advanced Immunolo-gy",three typical teaching cases,namely tumour treatment strategy,gene editing technology to overcome organ transplant rejection,and synthetic immunology,are selected for empirical research,focusing on graduate students' innovative ability to raise questions and solve problems.The effectiveness of teaching practice was summarized from the improvement of teachers' ability,the award-winning of students' disci-plinary competitions,and the co-construction between the university and enterprises,which has a signifi-cant effect and guarantees the benign optimization of the teaching reform of immunology,and also has the popularization value and exemplary effect on the high-quality development of immunology-related courses.
8.Practical research on nursing coordination training for rapid sequential intubation in children based on LSPPDM framework

Objective To investigate the practical effects of pediatric rapid sequence intubation(RSI)nursing coordination training based on the LSPPDM(learn,see,practice,prove,do,maintain)framework in order to provide evidence for optimizing pediatric RSI nursing training programs.Methods Nurses from the intensive care unit(ICU)of Children's Hospital,Fudan University during Feb 2023 and Jan 2024 were divided into the experimental group(n=35)and the control group(n=35)by block randomization.The experimental group received LSPPDM framework-based training,while the control group underwent conventional training with theoretical lectures and procedural demonstrations.Outcomes included training satisfaction,theoretical knowledge and procedural skill assessment scores,team collaboration compliance and RSI procedure time were compared between the two groups.Results The experimental group demonstrated significantly higher training satisfaction(123.80±2.04 vs.117.26±9.82,P<0.05),superior post-training theoretical knowledge and procedural skills(P<0.05),enhanced team collaboration compliance(P<0.05),and shorter RSI completion time(P<0.05)compared with the control group.Conclusion Pediatric RSI nursing coordination training based on the LSPPDM framework can effectively increase training satisfaction,promote theoretical and procedural skills and reduce completion time in nurses.
9.Establishment and validation of autoverification rules for pediatric coagulation tests

Objective To establish and validate an autoverification system for pediatric coagulation tests and apply it in clinical prac-tice.Methods A total of 31 633 specimens collected at the Children's Hospital of Soochow University,from August 1,2023 to De-cember 31,2023,were used to establish coagulation auto-verification rules,which were subsequently validated with 6 704 specimens collected during January 1-31,2024.The samples were analyzed using the SYSMEX fully automatic blood coagulation analyzer assem-bly line.Auto-verification rules were established based on quality control status,instrument alerts,sample trait,numerical anomalies,logical rules,linear deviations,specimen integrity,and delta checks.The pass rate and consistency rate following the auto-verification were subsequently evaluated.Results A total of 37 auto-verification rules were established,covering abnormal results for PT-INR,APTT,Fib,TT,DD,FDP and AT together with aberrant reaction curves,specimen status and instrument alerts.The pass rate of auto-verification in the establishment group was 57.44%,and the concordance rate between auto-verification and manual review was 99.82%,while the pass rate of auto-verification was 59.96%in the validation set,and the concordance rate between auto-verification and manual review was 100%.Conclusion A set of auto-verification rules for pediatric coagulation reports was established and imple-mented in routine practice,significantly expediting the efficiency of report review and effectively shortening the turn-around time(TAT).
10.Bioinformatics analysis of ferroptosis-related genes and immune infiltration in IgA nephropathy

Objective:This study based on comprehensive bioinformatics technology aimed to investigate the pathogenesis of ferroptosis in IgA nephropathy(IgAN)from an immunological perspective,and to identify the key genes and functional pathways.Methods:Differentially expressed ferroptosis-related genes(DE-FRGs)were identified from GSE93798 and analyzed by Kyoto Ency-clopedia of Genes and Genomes(KEGG)and Gene Ontology(GO).STRING and Cytoscape were used to construct a protein-protein interaction(PPI)network.Degree and MCODE algorithm were run.Combining LASSO regression,SVM-RFE and PLS-DA machine learning to construct the optimal feature selection hub gene.Cibersort and ssGESA algorithms were used to assess the immune infiltra-tion,as well as to explore the relationship between hub genes and immune infiltration.Single cell RNA sequence(scRNA-seq)data was used to analyze the location of hub genes in IgAN cell populations.Gene Set Enrichment Analysis(GSEA)was performed on hub genes.The HPA database was used to obtain the position of the specific protein of the hub genes,and the DsigDB database was used to predict the target drug.The GSE37460,GSE116626 and GSE73953 were treated as validation sets and diagnostic effectiveness evalua-tion.Collecting IgAN and healthy kidney tissue for immunohistochemical testing of hub genes expression.Results:This study identi-fied 94 DE-FRGs and 3 hub genes(JJUN,EGR1 and DDR2).KEGG and GO analysis indicated that the DE-FRGs were mainly con-centrated in pathways of ferroptosis,reactive oxygen species,responsing to oxidative stress and metal particles,mitochondrion,and transcriptional regulatory complex.GESA analysis involved in amino acid and fatty acid metabolism.The analysis of immune cell infil-tration in IgAN revealed an increased ratio of naive B cells,mast cells,and CD4+T cells.Differential analysis of immune function re-vealed that mechanisms such as chemokine receptor signaling pathways,human leukocyte antigen signaling pathways,and inflamma-tion promotion were more active in IgAN.Furthermore,the correlation was observed between immune infiltration and hub genes.ScRNA-seq analysis demonstrated that hub genes were localized in monocytes,macrophages,poximal convoluted tubule cells,princi-pal cells,and smooth muscle cells.Validations set analysis suggested that JUN and SIRT1 had a diagnostic value.The HPA database analysis showed that JUN was mainly located in the nucleus,while EGR1 was located in the membrane and cytoplasm.The 2 278 po-tential drugs for the treatment of IgAN were predicted.Finally,the results of immunohistochemistry and bioinformatics analysis were consistent.Conclusion:Ferroptosis may be associated with immune cell infiltration and immune related function during the occur-rence and development of IgAN.
