1.Regulatory mechanisms of exosome secretion and its application prospects in biomedicine

BACKGROUND:Exosomes,as a type of extracellular vesicle,have become a key medium for cell-to-cell communication due to their nanoscale size and enrichment of various bioactive substances.The study of exosome secretion regulation not only has important scientific value,but also has broad application prospects in clinical practice,and is of great significance for promoting medical progress and improving human health.OBJECTIVE:To review the biological characteristics,biological functions,biogenesis process and biochemical regulation mechanism of exosomes,and to explore the application prospects of exosomes in disease diagnosis,treatment and vaccine development,so as to provide theoretical basis and reference for basic research and clinical transformation of exosomes.METHODS:The first author searched PubMed and CNKI databases in October 2024 for relevant literature published from January 2010 to October 2024.Key words were"exosomes,biological functions,biogenesis,secretion or release,regulatory mechanisms,application prospects"in Chinese and English.Finally,92 articles were included for analysis.RESULTS AND CONCLUSION:The secretion level of exosomes can be regulated through physical or biochemical means.Exosomes show broad application prospects in the fields of disease diagnosis,treatment,and vaccine development,and may play a key role in the treatment of cardiovascular and cerebrovascular diseases as well as cancer.This review provides valuable information for the clinical translation and application research of exosomes,helping to promote future progress in exosome research and application.

3.Usefulness of intraoperative choledochoscopy in laparoscopic subtotal cholecystectomy for severe cholecystitis

Laparoscopic subtotal cholecystectomy (LSC) has been a safe and viable alternative to conversion to laparotomy in cases of severe cholecystitis. The objective of this study is to determine the utility of intraoperative choledochoscopy in LSC for the exploration of the gallbladder, cyst duct, and subsequent stone clearance of the cystic duct in cases of severe cholecystitis. A total of 72 patients diagnosed with severe cholecystitis received choledochoscopy-assisted laparoscopic subtotal cholecystectomy (CALSC). A choledochoscopy was performed to explore the gallbladder cavity and/or cystic duct, and to extract stones using a range of techniques. The clinical records, including the operative records and outcomes, were subjected to analysis. No LSC was converted to open surgery, and no bile duct or vascular injuries were sustained. All stones within the cystic duct were removed by a combination of techniques, including high-frequency needle knife electrotomy, basket, and electrohydraulic lithotripsy. A follow-up examination revealed the absence of residual bile duct stones, with the exception of one common bile duct stone, which was extracted via endoscopic retrograde cholangiopancreatography. In certain special cases, CALSC may prove to be an efficacious treatment for the management of severe cholecystitis. This technique allows for optimal comprehension of the situation within the gallbladder cavity and cystic duct, facilitating the removal of stones from the cystic duct and reducing the residue of the non-functional gallbladder remnant.
4.Compound Chaijin Jieyu formula modulates TLR4/NLRP3 signaling pathway to suppress central oxidative stress and ameliorate hippocampal synaptic plasticity impairment in depression

Aim To explore the mechanism by which the compound Chaijin Jieyu formula(CCJJY)regulates the TLR4/NLRP3 signaling pathway to inhibit central oxidative stress and improve hippocampal synaptic plasticity damage in depression.Methods SD rats were randomly divided into the control group,chronic unpredictable mild stress group,sleep deprivation group,chronic unpredictable mild stress combined with sleep deprivation group,positive drug group(venlafax-ine+melatonin),low-dose group of CCJJY,medium dose group of CCJJY,and high-dose group of CCJJY,with nine rats in each group.Except for the control group,a rat model of depression complicated with in-somnia was established using chronic unpredictable mild stress combined with sleep deprivation.Depres-sion-like and sleep behaviors in rats were evaluated through weight,food intake,water maze,and pento-barbital sodium tests.ELisa was used to detect ROS,AANAT,and HPLC-EC was used to detect 5-HT con-tent,while Western blot/RT-PCR was used to detect the expression of IL-1β,TLR4,NLRP3,PSD-95,and SYN related proteins and mRNA.HE and Golgic stai-ning were used to observe the pathological changes in the third ventricle,hippocampus,and neuronal synap-ses.Results Compared with the control group,the depression-like behaviors of the model group rats were significant.The expression of IL-1β,TLR4,and NL-RP3 in the hippocampus increased,while the expres-sion of PSD-95 and SYN decreased.Activation of NL-RP3 inflammasomes led to "sleeve like" pathological changes in the third ventricle,with hippocampal neu-rons undergoing apoptosis and significant damage to neuronal synaptic plasticity.Compared with the model group,after intervention with CCJJY,the expression of ROS,IL-1β,TLR4,and NLRP3 decreased,while the expression of AANAT,5-HT,PSD-95,and SYN in-creased.Pathological damage to the third ventricle and hippocampal neurons was repaired.Conclusion The CCJJY improves hippocampal synaptic plasticity dam-age in depression by regulating the TLR4/NLRP3 sig-naling pathway to inhibit central oxidative stress.
5.Effects of golden cicada anti-itch capsules combined with tacrolimus ointment on clinical symptoms and Th1/Th2 cytokine balance in older adult patients with itchy skin

Objective:To investigate the effects of golden cicada anti-itch capsules combined with tacrolimus ointment on clinical symptoms and T helper cell type 1/T helper cell type 2 (Th1/Th2) cytokine balance in older adult patients with itchy skin.Methods:This prospective study included 80 older adult patients with itchy skin who attended No. 215 Hospital of Shaanxi Nuclear Industry from January 2022 to January 2024. The patients were randomly divided into two groups ( n = 40 per group) using a random number table method. The ointment group received tacrolimus ointment, while the combination group was treated with golden cicada anti-itch capsules plus tacrolimus ointment. Both groups were treated for 4 weeks. Clinical symptom scores, Dermatology Life Quality Index (DLQI) scores, traditional Chinese medicine (TCM) syndrome scores, and the cytokine profiles of Th1 [interleukin-2 (IL-2) and interferon-gamma (IFN-γ)]/Th2 [interleukin-5 (IL-5), interleukin-13 (IL-13)] and skin barrier function were compared before and after 4 weeks of treatment. The incidence of adverse reactions and recurrence rates were recorded. Results:Before treatment, there were no statistically significant differences in clinical symptom scores, DLQI scores, or TCM syndrome scores between the ointment and combination groups ( t = -0.76, 0.76, -0.54, 0.33,-0.16, -0.36, all P > 0.0.5). After treatment, both groups showed reductions in severity of itching, duration, frequency of episodes, and area of skin lesions, as well as DLQI scores and TCM syndrome scores ( t = 16.14, 24.18, 14.65, 19.98, 18.21, 25.03, 15.42, 19.55, 12.36, 18.14, 15.65, 22.05, all P < 0.05). After treatment, the combination group had lower scores for severity of itching [(0.88 ± 0.24) points], duration [(0.91 ± 0.22) points], frequency of episodes [(1.06 ± 0.21) points], area of skin lesions [(1.18 ± 0.31) points], DLQI [(7.93 ± 2.17) points], and TCM syndrome scores [(8.89 ± 1.47) points] compared with the ointment group [(1.17 ± 0.33) points, (1.44 ± 0.26) points, (1.43 ± 0.25) points, (1.72 ± 0.44) points, (11.41 ± 3.05) points, (13.32 ± 2.06) points, t = 4.50, 9.84, 7.17, 6.35, 5.88, 11.07, all P < 0.05]. Before treatment, there was no statistically significant difference in Th1/Th2 cytokine level between the ointment and combination groups ( t = -0.15, -0.07, 0.21, -0.23, all P > 0.05). Both groups showed significant decreases in IL-2 and IFN-γ levels, and increases in IL-5 and IL-13 levels after treatment compared with their pre-treatment levels ( t = 4.56, 9.02, 11.11, 15.20, -5.63, -9.14, -6.01, -8.88, all P < 0.05). After treatment, the combination group had significantly lower IL-2 and IFN-γ levels and significantly higher IL-5 and IL-13 levels compared with the ointment group ( t = 7.25, 7.13, -6.69, -7.67, all P < 0.05). Before treatment, there was no statistically significant difference in skin barrier function between the two groups ( t = -0.16, 0.25, P > 0.05). After treatment, both groups exhibited significant reductions in transepidermal water loss and significant increases in stratum corneum hydration compared with their pre-treatment levels ( t = 6.25, 11.04, -4.82, -9.77, all P < 0.05). After treatment, the combination group demonstrated significantly lower transepidermal water loss and significantly greater stratum corneum hydration than the ointment group ( t = 4.13, 7.73, both P < 0.05). There was no significant difference in incidence of adverse reactions between the ointment group [7.50% (3/40)] and the combination group [12.50% (5/40), χ2 = 0.56, P = 0.456]. The recurrence rate was 27.50% (11/40) in the ointment group and 10.00% (4/40) in the combination group. Six months after treatment, the difference in recurrence rate between the ointment and combination groups was statistically significant ( χ2 = 4.02, P = 0.045). Conclusions:The use of Golden Cicada anti-itch capsules combined with tacrolimus ointment for treating itchy skin in older adult patients can help relieve clinical symptoms, improve skin barrier function, promote Th1/Th2 cytokine balance, and decrease the recurrence rate.
6.Epidemiological characteristics of allergens among children with allergic rhinitis in a children's hospital of Beijing from 2021 to 2023

OBJECTIVE To explore the epidemiological characteristics of common allergens among children with al-lergic rhinitis(AR)in a children's specialized hospital of Beijing.METHODS A total of 2120 children who met the diagnostic criteria of AR and were treated in otolaryngology department of Beijing Jingdu Children's Hospital from 2021 to 2023 were recruited as the research subjects.The specific immunoglobulin E(sIgE)of allergens was de-tected by western blot quantitative assay.The positive rates of the allergens were statistically analyzed according to the age,sex and years.The epidemiological characteristics and changing trends were observed.RESULTS The pos-itive rate of allergens was 47.78%among the 2120 children,in whom multiple allergies polysensitization were pre-dominant;the positive rate of allergens and polysensitization rate were the highest among the population aged be-tween 1 and 7 years old.The inhalational allergens were dominant,and the positive rate of the Candida group was highest.The positive rates of inhalant allergens and ingested allergens reached the peak at the age of 1-7 and then declined with age;milk and egg white were the two types of predominant ingested allergens in childhood.Among the 2120 children,there were significant differences in the positive rates of allergens between the male and the fe-male,and the positive rates of the male were higher than those of the female(P<0.001).The positive rates of al-lergens generally showed upward trends during the 3 years,the positive rates of the inhalant allergens such as mugwort,cat hair and dog dandruff ascended by a large margin,and the positive rates of the ingested allergens such as milk,egg white and egg yolk showed remarkable rising trends.CONCLUSIONS The causes of AR are complicated.There are a variety of factors affecting the allergen test.The epidemiological characteristics of the al-lergens among the AR children are observed from multiple perspectives,which may provide references for preven-tion and treatment of the children with AR.
7.High glucose inhibits expression of KIAA0753 and CCSAP proteins and disrupts osteoblast differentiation in mouse embryonic osteoblast progenitors MC3T3-E1 through impaired calcium signal transduction

Aim To explore the effects of high glucose on KIAA0753 and CCSAP and the relationship between KIAA0753 and CCSAP and osteogenic differentiation and calcium signaling pathway under high glucose con-ditions.Methods Mouse embryonic osteoblast pre-cursor cells(MC3T3-E1)were induced by osteoblast medium with glucose concentrations of 5.5 and 25 mmol·L-1,and the protein expressions of KIAA0753 and CCSAP were detected by Western blot.The over-expressed plasmid was transfected into human embry-onic kidney cells(HEK-293T),and the interaction between KIAA0753 and CCSAP was detected by co-im-munoprecipitation.MC3T3-E1 cells were treated in the osteogenic medium with different glucose concentrations and induction times.Alkaline phosphatase(ALP)ac-tivity was detected with a kit.The expression of KI-AA0753,CCSAP,osteopontin,osteocalcin,and other proteins were assessed using Western blot.and then 5.5,25 mmol·L-1,and 25 mmol·L-1+OE-CCSAP three groups of MC3T3-E1 cells were set.The expression of KIAA0753,OCN,OPN protein,and ALP activity were detected successively.The diabetic mouse model dataset in Gene Expression Omnibus was used to screen differential genes for bioinformatics a-nalysis.MC3T3-E1 cells were set up in three groups,5.5,25 mmol·L-1,and 25 mmol·L-1+OE-KI-AA0753,respectively.The calcium/calmodulin-de-pendent protein kinase Ⅱ beta(CAMK2B)and phos-pholamban(PLN)were detected by Western blot.Re-sults Compared with the 5.5 mmol·L-1 group,25 mmol·L-1 inhibited the expression of KIAA0753 and CCSAP proteins in osteoblasts,and there was an inter-action between KIAA0753 and CCSAP.At the same time,25 mmol·L-1 also inhibited the expression of ALP,OPN,and OCN proteins in osteoblasts.Overex-pression of CCSAP at 25 mmol·L-1 up-regulated the expression of KIAA0753,OCN,OPN,and ALP.The differential genes of the diabetic mouse model were mainly concentrated in the aspects of"signal receptor and signal regulation".25 mmol·L-1 glucose inhibi-ted the expression of CAMK2B and PLN proteins in os-teoblasts,and overexpression of KIAA0753 at 25 mmol·L-1 upregulated the expression of CAMK2B and PLN proteins.Conclusions High glucose inhibits the expression of KIAA0753 and CCSAP protein,inhibits the osteogenic differentiation and calcium signaling in mouse embryonic osteoblast precursor cells,overex-pression of CCSAP saves the inhibitory effect of high glucose on osteogenic differentiation,and overexpres-sion of KIAA0753 reverses the inhibitory effect of high glucose on calcium signaling pathway.
8.Role of GLUT1-dependent glycolysis in attenuation of oxygen-glucose deprivation-reoxygenation injury by dexmedetomidine in HK-2 cells

Aim To evaluate the role of the glucose transporter protein 1(GLUT1)-dependent glycolytic in the attenuation of oxygen-glucose deprivation-reoxygen-ation(OGD/R)injury in HK-2 cells by dexmedetomi-dine(Dex).Methods C57/BL6 mice were random-ly divided into three groups(n=6),namely,sham operation group(Sham group),renal ischemia reper-fusion group(I/R group)and Dex group(I/R+Dex group).Serum creatinine(Cr)and urea nitrogen(BUN)were measured,while the levels of key glyco-lytic enzymes HK2,PFKFB3 and GLUT1 were meas-ured.HK-2 cells were cultured and randomised into seven groups(n=6),which was treated with OGD/R,overexpression or interference with GLUT1,Dex and glycolysis inhibitor 2-DG.CCK-8 and LDH activi-ty were used to detect cellular damage.Glycolysis lev-els were detected by lactate and ECAR.The inflamma-tory level was reflected by qRT-PCR for IL-6 and TNF-α.qRT-PCR and Western blot were performed to de-tect the levels of GLUT1,HK2,and PFKFB3.Results Dex significantly ameliorated kidney injury and HK-2 cell injury(P<0.05).Dex inhibited the OGD/R-induced rise in lactate and extracellular acidification rate(ECAR),as evidenced by suppression of the ex-pression of GLUT1,HK2 and PFKFB3(P<0.05).In vitro experiments showed that GLUT1 knockdown sig-nificantly improved OGD/R-induced cellular damage.Lactate,ECAR,glycolysis-related mRNAs and pro-teins were inhibited by GLUT1 knockdown(P<0.05).Significantly,there were no significant differ-ences in above indexes after Dex treatment based on GLUT1 knockdown.Overexpression of GLUT1 abroga-ted the protective effects of Dex,while reversing the inhibitory effects of Dex on the expression of GLUT1,HK2,and PFKFB3(P<0.05).Conclusions Dexmedetomidine attenuates OGD/R induced injury in HK-2 cells by inhibiting GLUT1-dependent glycolysis.
9.Role and mechanism of TDO2 mediated apoptosis of renal tubular epithelial cells in Cis-AKI

Aim To investigate the role of tryptophan 2,3-dioxygenase(TDO2)in cisplatin-acute kidney in-jury(Cis-AKI)and to explore the mechanism of TDO2 in relation to apoptosis in tubular epithelial cells(TECs)to investigate the mechanism of TDO2 associ-ated with apoptosis.Methods An AKI model was es-tablished by intraperitoneal injection of cisplatin(Cis).Colorimetric assay was used to detect CRE and BUN levels,and PAS staining was employed to observe renal injury in mice.Immunohistochemistry was used to detect TDO2 protein expression and distribution and macrophage(F4/80+)infiltration;immunofluores-cence was used to detect the co-localization of TDO2 with the tubular marker LTL;TUNEL staining was used to detect apoptosis in mouse kidney;flow cytome-try was used to detect overexpression of human renal cortical proximal tubular epithelial cells(HK2)and apoptosis after administration of the TDO2 inhibitor 680C91;Western blot was used to detect TDO2 and NF-κB pathway protein levels in HK2 cells after over-expression and inhibition of TDO2.Results In the o-verall animal experiments,Cis-AKI mice showed signif-icantly higher levels of CRE and BUN and obvious tu-bular damage compared with the control group;at the same time,the renal tissues of Cis-AKI mice showed increased expression of F4/80,and the proportion of apoptotic cells in kidney cells was increased.Immuno-histochemistry and immunofluorescence showed that the expression of TDO2 increased,mainly localized in TECs.In cellular experiments,HK2 cells overexpress-ing TDO2 increased the proportion of apoptosis,and the expression of TDO2,p-IKBα,and p-p65 proteins was elevated,and p-IKBα/IκBα and p-p65/p65 were ele-vated;furthermore,the proportion of apoptosis was re-duced by the administration of 680C91,and the expres-sion of p-IκBα,and p-p65 proteins decreased,and the expression of p-IKBα/IKBα,and p-p65/p65 de-creased.Conclusions Elevated TDO2 in TECs is in-volved in the pathological mechanism of Cis-AKI,which may be related to its induction of apoptosis in TECs and activation of the NF-κB signaling pathway and consequently renal injury.
10.Oroxylin A induces apoptosis in Ishikawa cell line of endometrial cancer via PI3K/AKT signaling pathway

Aim To investigate the effect of oroxylin A(OA)on apoptosis in Ishikawa cell line of endometrial cancer and the underlying mechanism through the phosphatidylinositol-3 kinase/protein kinase B(PI3K/AKT)signaling pathway.Methods Ishikawa cells were treated with different concentrations of OA(0,4,8,10,12,and 20 μmol·L-1)for 24 h-72 h,the cell viability was detected by CCK-8 assay,apoptosis was detected by flow cytometry,and the protein ex-pression levels of B-cell lymphoma-2(Bcl-2),Bcl-2-associated X protein(Bax),PI3K/AKT,recombinant cytochrome P450 1B1(CYP1B1),and catechol-O-methyltransferase(COMT)were detected by Western blot technique.Results OA inhibited the prolifera-tion of Ishikawa cells in a concentration-and time-de-pendent manner.Compared with the blank control group,the expression of Bax protein increased signifi-cantly,while the expression of Bcl-2 protein decreased significantly with the increase of OA concentration.The expression of COMT protein increased significant-ly,while the expression of CYP1B1 protein decreased significantly.PI3K/AKT:IGF-1(PI3 K agonist)sup-plementation reversed the effect,the expression of COMT protein significantly decreased,and the expres-sion of CYP1B1 protein significantly increased.Con-clusions OA exerts anti-tumor effects in Ishikawa cells of endometrial cancer,which may be related to cell apoptosis mediated by the inhibition of the PI3K/AKT signaling pathway.
