With the accelerating trend of population aging, the number of elderly patients with lung cancer continues to rise, and the disease burden is becoming increasingly heavy. The clinical management of these patients faces severe challenges due to their decreased physiological reserve, complex comorbidities, and significant individual heterogeneity. Consequently, under traditional diagnosis and treatment models, doctors often struggle to identify the individualized risks of elderly patients in a timely and comprehensive manner, which can easily lead to decision biases such as undertreatment or overtreatment. In view of this, this study advocates for the establishment of an umbrella decision-making model specifically tailored for elderly lung cancer patients. Grounded in a multidisciplinary team (MDT) platform, this model deeply integrates oncological indicators with the comprehensive geriatric assessment (CGA) system. By holistically considering multidimensional variables including tumor burden, organ function, frailty index, cognitive status, and social support, the model establishes an operational mechanism characterized by "single entry, precise stratification, and targeted selection". Accordingly, patients can be scientifically triaged into distinct intervention tiers, such as active surveillance, minimally invasive surgery, drug therapy, radiotherapy, and best supportive care, thereby achieving real-time alignment between treatment intensity and patient fitness. This article elaborates on the construction logic and key operational procedures of this novel decision-making framework, aiming to guide clinical practice beyond the limitations of a tumor-centric perspective toward a holistic, dynamic, whole-course management strategy. This transition seeks to ensure optimal quality of life and clinical net benefit for elderly patients alongside survival prolongation.
2.Study on the treatment of chronic nonbacterial prostatitis caused by dampness-heat stasis with Oxalis Formula combined with transacupuncture

Objective:The aim of this study is to evaluate the clinical efficacy of Oxalicao Formula combined with transacu-puncture in the treatment of chronic nonbacterial prostatitis(CNP)characterized by dampness-heat stasis.Methods:A total of 70 patients diagnosed with CNP and characterized by dampness-heat stasis were randomly divided into control group and treatment group,with 35 cases in each group.The patients in control group received Qianlie Beixi capsules.While the patients in treatment group were administered with oxalis decoction in conjunction with acupuncture therapy which lasted for 8 weeks.Pre-and post-treatment evalua-tions for NIH-Chronic Prostatitis Symptom Index(NIH-CPSI),Traditional Chinese Medicine(TCM)symptom scores,urodynamic pa-rameters,immune cell subsets and inflammatory factors were performed.Results:Ultimately,65 patients completed the study with 33 in the treatment group and 32 in the control group.After 8 weeks of intervention,the patients in both of groups demonstrated signifi-cant improvements(P<0.05).Specifically,remarkable reductions in the NIH-CPSI total score including pain score,urination score,quality of life impact score,TCM symptom score and inflammatory cytokine levels were observed.Additionally,there were upward trends in maximum and average urinary flow rates as well as the CD4+/CD8+ratio of immune cells(P<0.05).Compared to the con-trol group,the treatment group exhibited superior outcomes in reducing the NIH-CPSI total score,pain score,urination score,quality of life impact score,TCM symptom score,and inflammatory cytokine levels,and increasing in CD4+/CD8+ratios,maximum and av-erage urine flow rates(P<0.05).Conclusion:The combination of Oxalicao Formula and transacupuncture for treating CNP charac-terized by dampness-heat stasis demonstrates significant therapeutic benefits,which has considerable clinical application value.
3.Relationship between adiponectin, high-sensitivity C-reactive protein to albumin ratio and the progression of acute cerebral infarction

Objective:To investigate the relationship between adiponectin (ADPN), the ratio of high-sensitivity C-reactive protein (hs-CRP) to albumin (ALB), and the progression of acute cerebral infarction (ACI).Methods:This case-control study involved 147 patients with ACI who were treated at Xingyuan Hospital of Yulin between January 2023 and March 2024. The National Institutes of Health Stroke Scale (NIHSS) was used to evaluate disease progression. Based on the progression of the disease, the patients were divided into two groups: the progressive group ( n = 38) and the non-progressive group ( n = 109). ADPN level and the hs-CRP/ALB ratio were compared between the two groups. Additionally, multivariate logistic regression analysis was conducted to identify risk factors for disease progression in ACI patients. The predictive value of ADPN and the hs-CRP/ALB ratio for ACI was assessed using receiver operating characteristic curve analysis. Results:In the progressive group, hs-CRP level and hs-CRP/ALB ratio were (14.38 ± 3.27) mg/L and (0.43 ± 0.13), respectively, both of which were significantly higher than those in the non-progressive group [(8.92 ± 2.73) mg/L, (0.20 ± 0.07), t = 9.22, 10.37, both P < 0.001]. In contrast, the levels of ADPN and albumin in the progressive group were (2.71 ± 0.59) mg/L and (33.54 ± 8.66) g/L, respectively, both of which were significantly lower than those in the non-progressive group [(5.36 ± 1.42) mg/L, (45.05 ± 10.42) g/L, t = -15.99, -6.68, both P < 0.001]. The progressive group had higher admission NIHSS scores, a higher proportion of patients with diabetes, higher white blood cell counts, higher hemoglobin A1c (HbA1c) and fibrinogen levels compared with the non-progressive group ( t = 8.43, 2.88, 5.79, 2.77, χ2 = 4.40, all P < 0.05). Multivariate logistic regression analysis results indicated that higher admission NIHSS scores, HbA1c levels, and hs-CRP/ALB ratios were independent risk factors for disease progression in ACI patients ( OR = 2.438, 1.600, 2.971, all P < 0.05), while higher levels of ADPN were identified as a protective factor against disease progression ( OR = 0.321, P < 0.05). The receiver operating characteristic curve analysis showed that the area under the curve for predicting disease progression after admission for ACI patients using serum ADPN and hs-CRP/ALB ratio alone were 0.730 (95% CI: 0.658-0.800) and 0.758 (95% CI: 0.685-0.826), respectively. The area under the curve for the combined prediction of serum ADPN and the hs-CRP/ALB ratio was 0.895 (95% CI: 0.845-0.940), demonstrating higher predictive efficacy ( Z = 1.80, 2.13, both P < 0.05). Conclusions:ADPN level and hs-CRP/ALB ratio are closely related to the progression of ACI and demonstrate good predictive efficacy for disease progression.

5.Phenotype and genomic characterization of a mucoid-type Salmonella Saintpaul ST50 isolate from a urinary tract infection patient

To investigate the phenotype and genomic characterization of a mucoid-type Salmonella Saintpaul ST50 isolate from a urinary tract infection patient,promoting clinical diagnosis and treatment for urinary tract infections caused by Salmo-nella spp.Culture-based quantitative counts of midstream urine sample from the patient were conducted,and further biochemi-cal identification,mass spectrometry detection,serum agglutination test and antimicrobial susceptibility test(AST)were con-ducted on Salmonella isolate(2024JD5).Whole-genome sequencing(WGS)was performed on isolate 2024JD5 to predict sero-type,multilocus sequence type(MLST),resistance genes,and virulence genes.Two smooth-type of Salmonella Saintpaul ST50 were selected as comparative genomic reference strains from the Chinese local Salmonella genome database.The literature reviews of global Salmonella serotype of urinary tract infection were summarized.Specific serum agglutination confir-mation of isolate 2024JD5 failed due to characterization of the mucus type.The strain 2024JD5 was predicted as Salmonella Saintpaul(4,5,12:e,h:1,2)ST50 using WGS,and was resistant to ciprofloxacin,nalidixic acid,chloramphenicol and tetracy-cline with carrying aminoglycoside resistance genes aac(6')-Ⅰaa and aph(3)-Ⅱa,chloramphenicol resistance gene floR,tetra-cycline resistance gene tet,quinolone resistance gene qnrS1,and S83Y substitution in the gyrA gene was found in the quinolo-ne resistance determination region(QRDR).In addition,the strain 2024JD4 carried six types of non-plasmid-based mobile ge-netic elements and 144 virulence genes,including 71 secretion transporter genes and 58 fimbriae adhesion genes,respectively.Four types of fimbriae regulatory genes(csgB,csgC,fimW,fimY)were absent in comparison with smooth-type Salmonella Saintpaul.The literature reviews showed Salmonella Saintpaul was currently a rare Salmonella serotype in cases of urinary tract infections worldwide.Salmonella Saintpaul ST50 with mucoid-type is the pathogen of urinary tract infection with multi-drug resistant phenotypic and genotypic characteristics,and the high mucoid expression may be related to the compensatory mechanism of fimbriae regulatory genes absence in urinary tract colonization and adaptation.WGS combined with the Chinese local Salmonella genome database can effectively solve the diagnosis and biosafety assessments of rare Salmonella phenotypes.
6.Immuneprotective efficacy of superoxide dismutase of Toxocarai canis

To investigate the immune protective effect induced by superoxide dismutase(SOD)of the Toxocara canis(T.canis)in mice,in this experiment,seventy-five Kunming mice of six-week-age were randomly divided into 5 groups for the immune protection experiment,the mice were subcutaneously immunized with 50,75 and 125 mg/L Tc-SOD(0.1 mL)on days 0,14 and 28,re-spectively.In the blank group,mice did not receive any treatment,while the PBS group was injected with equal amounts of sterility PBS.A total of three immunizations and each immunization interval of 14 days.14 days after the third immunization,the 3 000 infected worm eggs were given orally in all groups.Before each immunization,14 days after the third immunization,and after the T.canis attack on the 7th day,the blood was sampled from the tail vein and sera were separated,while the IgG levels in serum were detected by indirect ELISA.The proliferation ability of splenic lympho-cytes and the mRNA expression levels of Th1(IFN-γ,IL-2)and Th2 cytokines(IL-4,IL-10)were analyzed with CCK-8 and qRT-PCR.The larvae reduction rates were calculated on the 7th day after the T.canis attack,and the pathological changes in the livers and lungs were observed using H&E staining.The results showed that compared with the PBS group,the serum IgG antibody levels increased with the frequency and duration of Tc-SOD immunization.It remained at a high level af-ter the T.canis attack,and the differences were very significant(P<0.001).The mRNA expres-sion levels of IFN-γ,IL-2,IL-4,and IL-10 were dramatically increased(P<0.01),while IL-4 andIL-10 were significantly higher than IFN-γ and IL-2 on the 14th day after the third immunization(P<0.05),showing the Th2 type predominant immune response.And induce the proliferation of spleen lymphocytes in vitro(P<0.01).The reduction rates of larvae of 50,75 and 125 mg/L Tc-SOD immune groups were 18.7%,24.9%and 37.6%,respectively,with significant differences in the 125 mg/L Tc-SOD group(P<0.05),and had better immune protection effect.Moreover,the H&E staining results indicated that three Tc-SOD immune groups reduce the inflammatory cell infiltration(neutrophils,eosinophils,and macrophages)and hemorrhagic lesions in the livers and lungs of mice.The above results indicated that Tc-SOD could induce humoral and cellular immune responses in mice,mainly Th2 immune response,and provide immune protection against T.canis infection.

Objective This study investigated feasible cases and their significance in promoting the"generalized"development of inspection through digital-intelligent equipment.Methods A qualitative research approach was used,involving interviews conducted between February 2025 and March 2025 with experts in traditional Chinese medicine diagnostics,clinical research methodology,medical engineering integration,and related disciplines,using both online and offline methods.In accordance with the Consolidated Criteria for Reporting Qualitative Research,feasible cases involving the specific application of digital equipment in various parts of observation were collected through item enrichment.The significance of extending observation capabilities via these cases was analyzed,along with the overall implications of integrating digital technologies with traditional inspection method.Results Interviews were completed with 11 experts from domestic universities and research institutes in the fields of traditional Chinese medicine diagnosis,medical engineering integration,and related disciplines.A total of 78 feasible cases of digital-intelligent inspection were identified,along with 69 insights regarding the significance of enhancing the inspection capabilities.These insights were synthesized into two dimensions and 23 holistic meanings.The first dimension is to expand the scope of inspection,including obtaining internal environmental characteristics,observing external environmental characteristics,expanding thermodynamic characteristic data,and crossing time and space.The second dimension is to improve the quality of observation and diagnosis information collection and analysis,including 19 specific meanings,such as standardized collection environment,objective quantification,and refined observation.Conclusion Digital-intelligent equipment plays a significant role in expanding the scope of inspection content and achieving high-quality acquisition and analysis of extensive inspection information.These advancements extend and enrich the capabilities of traditional inspection method in traditional Chinese medicine.
8.Tissue and plasma proteomic signatures associated with the risk of gastric cancer

Objective:To identify proteins associated with the risk of gastric cancer (GC) and build a protein risk score for risk prediction of GC based on proteomic analysis.Methods:Gastric mucosal proteomics data were used to construct Dataset One, comprising 94 GC cases and 230 individuals with different stages of gastric mucosal lesions. The GC cases were recruited from the National Upper Gastrointestinal Cancer Early Detection (UGCED) Program in Linqu, Shandong Province, as well as clinical patients from the Fifth Medical Center, General Hospital of PLA, and Peking University Cancer Hospital. Non-cancer individuals were enrolled from the National UGCED Program in Linqu and community screening programs at the Dongfang Hospital. All participants were pathologically confirmed. Multivariate logistic regression analysis was employed to identify gastric mucosal proteins significantly associated with GC risk. Subsequently, plasma proteomics data from the UK Biobank Pharma Proteomics Project (UKB-PPP) were used to construct Dataset Two, including 40 baseline GC cases and 47 933 non-cancer individuals, and Dataset Three, comprising 138 incident GC cases and 47 933 non-cancer individuals during a prospective follow-up period. In Dataset Two, multivariate logistic regression analysis was conducted to assess associations between plasma protein levels and baseline GC risk. In Dataset Three, multivariate Cox regression analysis was used to examine associations with the risk of incident GC. A poly-protein risk score (PRS) was developed using a weighted summation method based on protein effect sizes from Dataset Two. Its associations with GC risk and the progression of gastric mucosal lesions were evaluated using linear regression trend tests.Results:A total of 324, 47 973 and 48 071 participants were included in Datasets One, Two, and Three, respectively. Across the three datasets, the proportions of males and individuals aged>60 years were higher in the GC group than in the non-GC group (all P values<0.05). The follow-up period in Dataset Three had a M ( P 25, P 75) of 14.47 (13.7, 15.2) years, with a median of 7.4 (4.6, 11.3) years for those who progressed to GC. Based on Dataset One, 2 524 tissue-differential proteins associated with GC risk were identified through multivariate logistic regression analysis adjusted for age and sex. Among these, seven proteins were consistently associated with GC risk across tissue and plasma levels in Datasets Two and Three, with consistent directions of association. Five proteins (MRC1, APOL1, BST2, PON2, and GGH) were positively associated with GC risk, while two (GSN and CLEC3B) were negatively associated. Analysis of the PRS based on these seven proteins showed that for each standard deviation increase in the tissue-derived PRS, the risk of GC increased by 6.26 times (95% CI: 4.02-9.75). In Dataset Two, each standard deviation increase in the plasma-derived PRS was associated with a 2.13-fold increase in GC risk (95% CI: 1.68-2.69). In the prospective cohort of Dataset Three, individuals in the high PRS group had a 2.27-fold higher risk of GC compared to the low PRS group (95% CI: 1.50-3.45). Moreover, each standard deviation increase in the plasma PRS was associated with a 57% higher risk of GC ( HR=1.57, 95% CI: 1.34-1.84). Additionally, the tissue-derived PRS showed an increasing trend with the progression of gastric mucosal lesions. Conclusion:The tissue and plasma proteomics identified seven individual proteins that may indicate the risk of developing gastric cancer, showing the potential as biomarkers for aiding in the screening of gastric cancer.
9.Research on practice guideline of health conversational artificial intelligence

The development of artificial intelligence(AI)technology has provided new avenues for the public to access health information.While both domestic and international research and practical applications of health conversational AI have emerged,there remains a lack of consensus on the key issues surrounding their development.Based on a comprehensive review of domestic and international literature and multiple rounds of expert consultations,the study focuses on six issues:the definition,characteristics,application scenarios,evaluation elements,evaluation methods,as well as prospects and challenges of AI-driven health consultation systems,and develops the practice guideline for health conversational artificial intelligence.The findings aim to provide scientific reference for promoting the innovative application of AI technology in the healthcare field.
10.Characteristics analysis of pediatric medicines with priority review and approval for marketing in China

OBJECTIVE To analyze the characteristics of pediatric medicines with priority review and approval for marketing in China,providing a reference for promoting enterprise R&D and production,as well as improving the supply guarantee mechanism for pediatric medicines.METHODS Based on publicly available data sources such as List of Approved Information for Pediatric Medications Subject to Priority Review and Approval,Pharnexcloud biomedical database,and National Medical Insurance Drug Directory,this study conducted a comprehensive analysis of the main characteristics of pediatric medicines with priority review and approval for marketing.RESULTS As of June 30,2024,a total of 68 pediatric medicines had been approved through the priority review and approval process,covering 12 therapeutic areas,with oral dosage forms accounting for 64.71%.The median time from application to inclusion in priority review was 35.50 days,with an average of 41.69 days.The median time from inclusion in priority review to market approval was 1.24 years,with an average of 1.42 years.This included 12 domestic new medicines,21 domestic generic medicines,35 imported medicines,as well as 29 pediatric-specific medicines and 21 orphan medicines.Additionally,31 of these medicines had been included in the medical insurance catalog,representing a proportion of 45.59%.CONCLUSIONS Currently,a trend of differentiated competition is emerging between domestic and imported pediatric medicines.The therapeutic areas for pediatric medicines are continuously expanding,and the dosage forms are becoming more tailored to children's needs.However,there are still issues such as slow progress in new medicine development,insufficient stability in the medicine review and approval process,and a need to increase the proportion of medicines included in medical insurance.
