1.Application and mechanism of virtual reality technology in treating attention deficit hyperactivity disorder

The core aberrant behaviors of attention deficit hyperactivity disorder (ADHD) are primarily manifested as inattention, hyperactivity, and impulsivity, which are closely associated with specific neuropathological features.Virtual reality (VR) technology, leveraging its immersive, interactive, and customizable properties, demonstrates potential in ameliorating the abnormal behavioral manifestations of individuals with ADHD. This article aims to synthesize existing evidence from neurobiological mechanism research and explore the therapeutic efficacy of VR interventions in improving abnormal behavioral presentations in ADHD patients. An evidence-integrated framework posits that: (1) VR enhances inhibitory control and working memory by promoting neural plasticity remodeling within key brain regions through highly contextualized simulation tasks, thereby optimizing functional connectivity.(2) VR reduces attentional lapses and impulsive behaviors by regulating the dynamic interplay efficiency among the default mode network, executive control network, and salience network.(3) VR elevates motivational levels and cognitive flexibility by dynamically modulating dopaminergic-noradrenergic system homeostasis, synergizing with brain-derived neurotrophic factor mediated synaptic plasticity. Delving deeper into these underlying mechanisms will facilitate the optimization of multi-target VR intervention design and provide novel insights for personalized behavioral therapy in ADHD.
2.Preliminary observation on the application of nasal endoscopic technique in curettage of large mandibular cyst

Objective:To evaluate the effects of nasal endoscopy in curettage of large mandibular cyst.Methods:20 cases of large mandibular cyst admitted to Xianyang Hosptial of Yan'an University from January 2022 to December 2023 were included.The curet-tage of mandibular cyst was performed under general anesthesia through intraoral incision by nasal endoscopy-assisted illumination and enlargement of the lesion area during the operation.The application effects were evaluated from the aspects of surgical incision length,nerve injury and cyst recurrence.Results:During operation,the surgical filed of view was clear and the operation was suc-ceeded in all cases.There was no complication in and after the nasal endoscopy-assisted curettage,no cyst recurrence was observed during 1-year follow-up.Conclusion:Nasal endoscopy-assisted curettage of large mandibular cyst is effective and safe.

5.Advances in the neural mechanisms of social disorders in autism

Social impairment is one of the core symptoms of autism spectrum disorder (ASD), which seriously affects patients' daily life. Using optogenetics, functional magnetic resonance imaging and other techniques, significant progress has been made in the study of the anatomical structure of the brain, functional connectivity and neural circuits in ASD patients. This study reviews the latest findings on the neural mechanisms of social impairment in ASD. In terms of the structure of brain regions, patients with ASD have structural abnormalities in key brain regions such as the putamen, amygdala and nucleus accumbens, and these abnormalities may affect emotion regulation and reward processing functions.In terms of the functional connectivity of brain regions, functional connectivity abnormalities of the brain in patients with ASD may lead to dysfunctional social behaviors. Further analysis revealed that abnormalities in neural circuits involving the prefrontal cortex, ventral tegmental area, hippocampus, and cerebellum were closely associated with social deficits in ASD patients. Meanwhile, abnormal synaptic function and imbalance of the neurotransmitter system are also considered to be important pathological mechanisms for social deficits in ASD patients. These findings allow for a deeper understanding of the neural mechanisms underlying social deficits in ASD patients and provide a scientific basis for developing targeted interventions to improve the social function and quality of life of ASD patients.
6.Research progress on the effect of blood flow restriction training on lower limb muscle strength in the elderly

The decline in muscle mass and strength in the lower limbs of the elderly can result in reduced exercise capacity and an increased risk of falls and mortality.Blood flow restriction training involves applying external pressure to the proximal end of a limb using a pressure device to restrict blood flow, leading to muscle adaptability similar to that achieved with high load training but at a lower intensity.This method, compared to traditional high load resistance training, offers the advantage of being low in load yet highly efficient.This article provides a summary of the clinical applications, mechanisms of action, and benefits of blood flow restriction training for enhancing lower limb muscle strength in the elderly, serving as a valuable reference for the development of scientifically sound blood flow restriction training programs for this population.

8.Exploration of Mechanism of Xiaonang Tiaojing Decoction in Treating PCOS-IR Model Rats Based on AMH/AMHR ⅡSignaling Pathway

ObjectiveTo investigate the mechanism of Xiaonang Tiaojing decoction(XNTJD)in improving polycystic ovary syndrome with insulin resistance(PCOS-IR)model rats based on anti-Müllerian hormone(AMH)/AMH type Ⅱ receptor(AMHRⅡ)signaling pathway. MethodForty-eight adult female SD rats were randomly divided into the blank group, model group, XNTJD group(11.4 g·kg-1·d-1) and Diane-35 group(0.21 g·kg-1·d-1), PCOS-IR model was established by high-fat and high-sugar diet combined with letrozole in rats of all groups except the blank group, rats in the administration groups were given the corresponding dose of drugs by gavage for 15 days with an interval of 1 d every 4 d, normal saline of the same volume was given to the blank group and the model group. Estrous cycle was recorded daily during treatment. At the end of treatment, body weight and Lee's index were recorded, AMH, luteinizing hormone(LH), LH/follicle stimulating hormone(FSH), testosterone(T)and estradiol(E2)levels were measured by enzyme-linked immunosorbent assay(ELISA), fasting plasma glucose(FPG)was measured by glucometer, fasting insulin(FINS) level was measured by radioimmunoassay(RIA), and the insulin resistance index(HOMA-IR) and insulin sensitivity index(QUICKI)were calculated, triglyceride(TG)and total cholesterol(TC)levels were measured by automatic biochemical analyzer, hematoxylin-eosin(HE)staining was used to observe the morphological changes of the ovary, the levels of AMHRⅡ, bone morphogenetic protein-15(BMP-15)and Smad5 in ovarian tissues were detected by immunohistochemistry(IHC),Western blot was used to analyze the protein expression levels of AMHRⅡ, BMP-15 and Smad5. ResultCompared with the blank group, a large number of leukocytes were observed in the vaginal exfoliated cells of rats in the model group, mainly in diestrus, the levels of body weight, Lee's index, glucose-lipid metabolism indexes(FPG, FINS, HOMA-IR, TG, TC), AMH and sex hormones(LH, LH/FSH, T)were significantly increased(P<0.01), and QUICKI and E2 levels were significantly decreased(P<0.01), there were more cystic bulges on the ovarian surface, more wet weight, more atretic and cystic dilated follicles in the ovarian tissues, and the thickness of granulosa cell layer was reduced without oocytes, the expression level of AMHRⅡ protein in ovarian tissues was significantly increased(P<0.01), and the expression levels of BMP-15 and Smad5 proteins were significantly decreased(P<0.01). Compared with the model group, the exfoliated cells in the vagina of rats treated with XNTJD group showed keratinocytes from the 5th to 6th day of treatment, and a stable estrous cycle gradually appeared, body weight, Lee's index, glucose-lipid metabolism indexes(FPG, FINS, HOMA-IR, TG, TC), AMH and sex hormones(LH, LH/FSH, T)levels were significantly decreased(P<0.05, P<0.01), QUICKI and E2 levels were significantly increased(P<0.01), ovarian surface was smoother and lighter in wet weight, oocytes and mature follicles were observed in ovarian tissues, the thickness of granulosa cell layer increased and the morphology was intact, the expression levels of BMP-15 and Smad5 proteins were significantly increased(P<0.01)and the expression level of AMHRⅡ protein was significantly decreased(P<0.01)in ovarian tissues. ConclusionXNTJD may mediate the up-regulation of BMP-15 and Smad5 in ovarian tissues of PCOS-IR rats by down-regulating AMH/AMHRⅡ, thereby improving ovarian function, sex hormones and glucose-lipid metabolism levels in PCOS-IR rats.
9.Research progress of abnormal glucose and lipid metabolism in preeclampsia

Preeclampsia is a kind of idiopathic disease during pregnancy. Its pathogenesis may involve many factors, such as mother, placenta and fetus. The study found that the abnormal metabolism of blood glucose and blood lipid during pregnancy may be closely related to the onset of preeclampsia. This paper reviews the research progress of abnormal glycolipid metabolism in preeclampsia at home and abroad in order to better guide the management of related aspects during pregnancy.
10.One-stage total knee arthroplasty combined with open reduction and internal fixation for knee osteoarthritis complicated with tibial stress fracture

Objective:To explore one-stage total knee arthroplasty (TKA) combined with open reduction and internal fixation (ORIF) for knee osteoarthritis complicated with tibial stress fracture.Methods:The 3 patients were retrospectively analyzed who had been treated for knee osteoarthritis complicated with tibial stress fracture at Department of Orthopedics, Ganzhou District People's Hospital from March 2018 to March 2020. They were all female, aged from 54 to 76 years (average, 66 years). There were 2 transverse fractures and one short oblique fracture; all of them had knee varus deformity. The Hospital for Special Surgery (HSS) scores averaged 37.6 (from 28 to 50) for the left knee and 28.3 (from 22 to 39) for the right knee. One-stage TKA was performed for the articular surface while ORIF for the right tibial stress fracture for all patients. Recorded were fracture union time, HSS knee score and range of articular motion.Results:The 3 patients were followed up for 25 to 44 months (average, 32 months).The fracture union time ranged from 4 to 7 months (average, 5 months). The last follow-ups revealed no such complications as prosthesis loosening, peri-prosthesis osteolysis or joint instability. Knee varus deformity was corrected in all patients. The HSS knee scores at the last follow-up averaged 89.6 (from 88 to 91) for the left knee and 88.3 (from 85 to 90) for the right knee.Conclusion:In the treatment of knee osteoarthritis complicated with tibial stress fracture, one-stage TKA combined with ORIF can restore the function of knee joint, leading to fine curative effects.
