1.Evaluation of the Effect of Chinese Medicine Formula Kai-Xin-San Combined with Fluoxetine on Hippocampal Neural Stem Cells in Chronic Stress Induced Depression Model Mice

Objective To evaluate the effect of Kaixin San(KXS)combined with fluoxetine on hippocampal neural stem cells in mice with chronic stress stress and depression.Methods A mouse model of depression was constructed using the method of chronic unpredictable stress stress,and the highest dose of KXS water extract and fluoxetine for clinical application was given for 28 days,and behavioral tests were carried out.Nissl staining was used to detect the pathological status of hippocampal tissues in mice.The expression of TUNEL and Nestin in mouse hippocampus was determined by immunofluorescence.Western blotting was used to detect the expressions of apoptotic proteins cleaved caspase-3 and caspase-3,pyroptosis proteins GSDMD and cleaved caspase-1,as well as the expression of neural stem cell marker Nestin in the hippocampus,and the expression of Wnt/β-catenin signaling pathway-related proteins in the hippocampus.Results The combination of KXS extract and fluoxetine can significantly improve the depression-like behavior of model mice,and the effect is better than fluoxetine alone.The combination inhibited the activation of apoptosis and pyroptosis signaling pathways in the hippocampus when used alone with high-dose fluoxetine,significantly upregulated the expression of Nestin,and regulated the expression of Wnt/β-catenin signaling pathway protein.Conclusion The combination of KXS and high-dose fluoxetine can improve apoptosis and pyroptosis in the hippocampus of stress stress and depression model mice,and upregulate the expression of neural stem cell marker Nestin by regulating the Wnt/β-catenin signaling pathway,which may be a key link to improve the antidepressant effect of the combination drug.
2.Potential profile analysis and influencing factors of social participation in colorectal cancer patients with stoma

Objective:To explore the potential category characteristics of social participation in colorectal cancer patients with stoma and analyze the influencing factors of different potential categories, so as to provide references for formulating and implementing targeted nursing measures.Methods:By using the convenience sampling method, colorectal cancer patients with stoma who were hospitalized or had outpatient follow-up visits at Guilin Hospital of Integrated Traditional Chinese and Western Medicine and the Second Affiliated Hospital of Guilin Medical University from January to December 2024 were selected as the survey subjects. A cross-sectional survey was conducted using the general information questionnaire, the voluntary participation Questionnaire, the Existential Anxiety Scale, the Family Support Scale and the Social Support Rating Scale. Potential profile analysis was used to identify the categories of social participation characteristics of colorectal cancer stoma patients, and multivariate Logistic regression was used to analyze the related influencing factors.Results:A total of 240 valid questionnaires were finally retrieved. Among the 240 colorectal cancer patients with stoma, 126 were male and 114 were female, aged 34-75 (55.63 ± 6.52) years. The social participation score of colorectal cancer patients with stoma was (55.84 ± 8.62) points. The results of potential profile analysis showed that the social participation of colorectal cancer patients with stoma could be divided into three potential categories: low participation-inefficient social life type accounted for 38.3% (92/240), medium participation-efficient indoor participation type accounted for 36.3% (87/240), and high participation-efficient outdoor participation type accounted for 25.4% (61/240). The results of multivariate Logistic regression analysis showed that, taking the high participation-efficient outdoor participation type as the reference, age, educational level, presence absence of stoma-related complications, existence of meaningless anxiety, family support, and social support were the influencing factors of low participation-inefficient social life type and medium participation-efficient indoor participation type ( OR values ranged from 0.163 to 1.879, all P<0.05). Conclusions:There are obvious classification characteristics in the social participation of colorectal cancer patients with stoma. Nursing staff can formulate and implement targeted intervention strategies based on different classification characteristics. Starting from the existence of meaningless anxiety in patients, an effective family and social support system should be established to improve their social participation.
3.Qualitative study on barriers and promoting factors of colonoscopy in patients with colorectal high-risk adenoma 3 months after resection

Objective:To understand the hindrance and promoting factors of colonoscopy compliance in high-risk adenoma patients 3 months after resection, and to provide reference for formulating targeted nursing intervention strategies.Methods:An interview outline was formulated based on the health action process approach theory. A descriptive qualitative research method was used to select 13 patients with high-risk adenoma after resection in Guilin Integrated Traditional Chinese and Western Medicine Hospital from April to May 2024 for semi-structured interviews according to the purposive sampling method. Content analysis was used to analyze the data and refine the themes.Results:Among the 13 patients, there were 8 males and 5 females, aged 42-70 years old.A total of 2 themes and 8 sub-themes were extracted, including hindering factors (fear of disease outcome, risk perception barriers, low self-efficacy and lack of behavior plan) and promoting factors (establishing correct health beliefs, perceiving the benefits of health behavior, enhancing personal efficacy and advocating health behavior plan).Conclusions:The compliance of colonoscopy in patients with high-risk adenoma after resection is affected by a series of obstacles and promoting factors. In the future, targeted intervention strategies can be formulated based on the health action process approach theory to improve their compliance with colonoscopy.
4.Simultaneous Determination of Four Off-flavors in Freshwater Fish by Gas Chromatography-Mass Spectrometry Combined with Silica Solid Phase Extraction

An effective method for simultaneously detecting four semivolatile earthy-musty odors in freshwater fish by gas chromatography-mass spectrometry(GC-MS)was developed.The concurrent extraction of geosmin(GSM),2-methylisoborneol(MIB),2-isopropyl-3-methoxypyrazine(IPMP),and 2-isobutyl-3-methoxypyrazine(IBMP)in fish tissue was conducted with n-hexane.The optimized QuEChERS material was implemented,and it was found that C18,primary secondary amine(PSA)and MgSO4 could adsorb the target analytes in n-hexane.So only the graphitized carbon black(GCB)could be used to purify the extraction.The adsorption rates of different materials for the four kinds of odors materials were explored in n-hexane and ethyl acetate.The experimental results revealed that the adsorption rates of silica for the four targets were 99.5%-100%in n-hexane and 0.7%-5.0%in ethyl acetate respectively.Then the silica solid phase extraction(SPE)method was utilized to eluent the compounds using 1.0 mL n-hexane/ethyl acetate in different proportions.The results of the comparative analysis demonstrated that n-hexane/ethyl acetate(4∶1,V/V)was the optimized eluent.Based on the obtained results,n-hexane extraction and GCB purification combined with silica SPE were used to isolate GSM,MIB,IPMP and IBMP from fish and the method was validated.It was found that the method showed good linearity in the range of 0.5-200 ng/mL,and with detection limits of 0.6 μg/kg for GSM and MIB,0.2 μg/kg for IPMP and IBMP.The limits of quantitation(LOQ)were 1.0 μg/kg for GSM and MIB,0.6 μg/kg for IPMP and IBMP.Good recoveries(77.5%-112.0%)and relative standard deviations(1.56%-9.42%)were also obtained.The use of silica SPE greatly mitigated the issue that the off-flavor compounds were easily lost in the gas blowing concentration process.There was no cross contamination in this method because the sample pretreatments were conducted separately,which was different with the most commonly used HS-SPME method for detecting semi-volatile substances.The sensitivity of this method was high enough to produce good quantitative results below the odor thresholds of the examined off-flavor compounds.

8.Bacterial pathogen spectrum and drug resistance in respiratory intensive care unit in 2020- 2022

Objective To understand the infectious pathogen characteristics and drug sensitivity of hospitalized patients in the respiratory intensive care unit (RICU) of Renmin Hospital of Wuhan University. Methods Bacterial culture samples sent to the RICU of our hospital from January 2020 to December 2022 were retrospectively analyzed. The bacterial types were identified by Bruker mass spectrometer, and the Phoenix 100 was used for drug sensitivity analysis. The antimicrobial susceptibility was analyzed by WHONET 5.6 software. Results A total of 1 157 strains of bacteria were isolated, including 878 strains of Gram-negative bacteria (75.89%) and 279 strains of Gram-positive bacteria (24.11%). The top five with the highest detection rate were Acinetobacter baumannii (25.50%), Pseudomonas aeruginosa (18.76%), Klebsiella pneumoniae (13.83%), Staphylococcus aureus (6.57%) and Escherichia coli (5.70%). Among them, Acinetobacter baumannii was extremely drug-resistant, only showing relatively high sensitivity to colistin, minocycline, and tigecycline. Staphylococcus aureus accounted for the highest proportion of Gram-positive bacteria (6.57%), with methicillin-resistant Staphylococcus (MRSA) showing a continuous increase. Conclusion In the past three years, Gram-negative bacteria have been the main pathogenic bacteria detected in the respiratory intensive care unit of our hospital. The main bacteria are Acinetobacter baumannii, Pseudomonas aeruginosa, and Klebsiella pneumoniae, which have a high resistance rate to various antibiotics. Therefore, clinical monitoring of resistant strains in RICU should be strengthened to facilitate rational use of antibiotics and improve antibacterial effect.
9.Effects of neohesperidin on the TLR4/NLRP3 signaling pathway in LPS-induced macrophage inflammatory model

Objective To investigate the effect of neohesperidin(NHP)on the Toll-like receptor 4(TLR4)/NOD-like receptor family pyrin domain containing 3(NLRP3)signaling pathway in lipopolysaccharide(LPS)-induced macrophage inflammation model.Methods RAW264.7 cells were divided into five groups:Control group,model group,experimental-L group,experimental-H group and MCC950 group.The control group was cultured normally,while the other groups were stimulated with 100 ng·mL-1 of LPS to induce the macrophage inflammation model.The experimental-L,-H groups were treated with 100 and 400 μmol·L-1 of NHP,respectively,while the MCC950 group was treated with 30 μmol·L-1 of the NLRP3 inhibitor MCC950.All interventions lasted for 24 hours.After the intervention,quantitative real-time polymerase chain reaction(qRT-PCR)was used to detect the mRNA expression levels of interleukin-1β(IL-1β),TLR4 and NLRP3 in RAW264.7 cells.Western blotting was used to detect the protein expression levels of TLR4 and NLRP3 in RAW264.7 cells.Results The mRNA expression levels of IL-1β in the control,model,experimental-L,experimental-H and MCC950 groups were 1.00±0.08,6.45±1.19,3.87±0.55,1.96±0.32 and 3.26±0.16,respectively;the mRNA expression levels of TLR4 were 1.00±0.13,2.69±0.35,1.92±0.22,1.32±0.23 and 3.38±0.33,respectively;the mRNA expression levels of NLRP3 were 1.00±0.14,1.28±0.19,0.83±0.02,0.87±0.15 and 0.95±0.25,respectively;the protein expression levels of TLR4 were 0.63±0.05,0.86±0.04,0.68±0.08,0.64±0.08 and 0.71±0.08,respectively;the protein expression levels of NLRP3 were 0.44±0.02,0.66±0.03,0.56±0.07,0.52±0.05 and 0.54±0.07,respectively.The differences between the model group and the control group were statistically significant(all P<0.05);the differences between the experimental-H group and the model group were statistically significant(all P<0.05).Conclusion NHP can improve macrophage inflammation,and its mechanism is related to inhibition of TLR4/NLRP3 signaling pathway.
10.Screen of key characteristic genes and analysis of immune cell infiltration in metastatic nasopharyngeal carcinoma base on machine learning

Objective To screen the key characteristic genes of metastatic nasopharyngeal carcinoma(mNPC)and analyze the immune cell infiltration in tumor microenvironment using machine learning algorithm.Methods Firstly,the training set GSE103611 was downloaded from the GEO database,and the data were subjected to differential expression gene(DEGs)screening,Gene Ontology(GO),Kyoto Encyclopedia of Genes and Genome(KEGG)and immune cell infiltration analysis.Second,the predicted genes in DEGs were screened by least absolute shrinkage and selector operation(LASSO)regression,and the characteristic genes were screened by using the expression level of the predicted genes and receiver operating characteristic(ROC).Third,the correlation between characteristic genes and immune cells was further analyzed to determine the key characteristic genes.Finally,the expression levels of key characteristic genes and ROC were verified using the reverse validation set GSE1245 data.Results A total of 136 DEGs were obtained,and their KEGG were mainly enriched in cytochrome P450,tumor necrosis factor(TNF)signaling pathway,prion disease,EB virus infection,and other pathways.GO was mainly enriched in the negative regulatory processes of peptide-based tyrosine phosphorylation modification,viral gene expression,and B cell and leukocyte activation.The difference in the degree of infiltration of the 22 immune cells in nasopharyngeal carcinoma(NPC)and mNPC was not significant.Two key characteristic genes(DAZ1 and SCCPDH)of mNPC were finally obtained by LASSO regression,and they were significantly correlated with immune cells in the mNPC microenvironment(P<0.05).In the reverse validation data set,the differential expressions of DAZ1 and SCCPDH between non-NPC(nNPC)and NPC groups were not significant(P>0.05),and the AUC values of ROC of both were less than 0.6.Conclusion DAZ1 and SCCPDH are the key characteristic genes of mNPC and can be used as important markers for mNPC and immunotherapy.
