1.Role of IRS-1/PI3K signaling axis in the effects of Bufei Jianpi formula on mitochondrial damage in skeletal muscle of chronic obstructive pulmonary disease rats

Objective To explore the action of Bufei Jianpi formula(BJF)on mitochondrial damage to skeletal muscle in chronic obstructive pulmonary disease(COPD)rats via its regulation of the IRS-1/PI3K signaling axis.Methods 60 SPF SD rats were randomly divided into Control group,Model group(COPD stable stage group),aminophylline(Am)group,BJF group,pioglitazone(PIO)group and BJF+PIO group,with 10 rats per group.A stable COPD rat model was established via forced smoking and Klebsiella pneumoniae nasal drip method.Samples were taken from the 9th week to the end of the 20th week,and the weight of the rats was measured every week.Routine sectioning and HE staining were performed on lung and skeletal muscle tissue,and corresponding pathological changes were observed under a light microscope.The lung function of the rats was observed by whole-body plethysmography in weeks 0,8,and 20,including tidal VT,PEF,and EF50.The mRNA expression of IRS-1,leptin,PGC1-α,and PI3K in rat skeletal muscle was detected by qPCR.The expression of PGC-1α,TFAM,IRS-1,PI3K,AKT,p-AKT,and leptin in rat skeletal muscle tissue was detected by Western blot.Results The Model group,but not the Control group,showed a large number of inflammatory cells infiltrating the alveolar interstitium and bronchus,indicative of lung disease;some alveolar walls had broken and fused to form air cavities,and fiber networks were destroyed.After drug treatment,the rats showed improved alveolar wall and fiber network integrity and reduced inflammatory cell infiltration in the bronchus,especially those in the BJF and Am groups.In the drug treatment groups,the skeletal muscle pathology of each group showed improved spatial arrangement,the atrophy and fracturing of muscle fibers were ameliorated to different degrees,and cytoplasmic staining of muscle cells was uneven,and the BJF group showed the most significant effects.Compared with the Control group,the Model group's PEF,VT,and EF50 significantly decreased from week 8(P<0.01),while the BJF,BJF+PIO and Am groups had significantly increased PEF and EF50(P<0.01).Compared with Control group,the Model group's mRNA and protein expression levels of IRS-1,PGC-1α,and PI3K were significantly decreased(P<0.05,P<0.01),the level of leptin was significantly increased(P<0.01).Compared with the Model group,the mRNA and protein expressions of IRS-1,PGC-1α and PI3K in the BJF group were significantly increased(P<0.05,P<0.01),and the mRNA expression of IRS-1 in the PIO group was significantly increased(P<0.01).The BJF+PIO group's mRNA levels of PGC-1α(P<0.01)and mRNA and protein levels of IRS-1 and PI3K were significantly increased(P<0.05,P<0.01).The mRNA and protein expression levels of PI3K in the Am group were significantly increased(P<0.01).The expression levels of leptin mRNA were significantly decreased in the four treatment groups(P<0.01),and the expression of leptin protein was significantly decreased in all treatment groups except the Am group(P<0.01).Compared with the Control group,the Model group's quadriceps femoris tissue showed a significant decrease in TFAM and p-AKT expression.TFAM and p-AKT expression in all the treatment groups showed an increasing trend,but the difference was not statistically significant(P>0.05).Conclusions By regulating the IRS-1/PI3K signaling axis,Bufei Jiempi reduces mitochondrial damage to skeletal muscle,increases the expression of PGC-1α and mitochondrial transcription factor TFAM,enhances mitochondrial biosynthesis,and reduces pathological damage to lung and skeletal muscle tissue.
2.Safety and efficacy analysis of a highly biomimetic modified esophagogastric anastomosis in gastrointestinal reconstruc-tion of proximal gastrectomy

Objective:To investigate the safety and anti-reflux effect of a highly bionic modi-fied esophagogastric anastomosis in the reconstruction of the digestive tract after proximal gas-trectomy,with the aim of providing a safe and simple new option with good anti-reflux effect for the reconstruction of the digestive tract after proximal gastrectomy.Methods:In this retrospec-tive study,30 patients(Study group)who underwent modified esophagogastric anastomosis after proximal gastrectomy in the Department of Gastrointestinal Surgery of Shandong Provincial Hospi-tal Affiliated to Shandong First Medical University from January 2022 to September 2023 were compared with 27(Control group)patients who underwent double-tract reconstruction which is currently recognized as having good anti-reflux effect during the same period,the main compari-sons were made between the surgery-related indexes,the quality of life after the surgery,the postoperative complications,and the anti-reflux effect of the surgery.Results:The study group was significantly better than the control group in terms of operation time and postoperative hospi-talization time,with statistically significant differences(P<0.05),and the difference between the two groups was comparable in terms of intraoperative bleeding and the anti-reflux effect of the opera-tion,with no statistically significant differences(P>0.05);in terms of postoperative complications,the incidence of anastomotic fistula and intestinal obstruction was slightly lower in the study group compared with the control group,but in terms of total incidence,the difference was not statistically significant;in terms of quality of life,the difference was not statistically significant when comparing the quality of life of the two groups after treatment(P<0.05).Conclusion:Highly bionic modified esophagogastric anastomosis is safe and feasible in the treatment of adenocarcinoma of the esophagogastric junction with excellent anti-reflux effect,but the procedure needs further high-quality randomized controlled studies and in-depth exploration.
3.Action mechanism of Huotu Jiji Pellets in the treatment of erectile dysfunction:An exploration based on network pharmacology and molecular docking

Objective:To explore the potential action mechanism of Huotu Jiji Pellets(HJP)in the treatment of erectile dys-function(ED)based on network pharmacology and molecular docking.Methods:We identified the main effective compounds and active molecular targets of HJP from the database of Traditional Chinese Medicine Systems Pharmacology(TCMSP)and Integrative Pharmacology-Based Research Platform of Traditional Chinese Medicine(TCMIP)and the therapeutic target genes of ED from the data-bases of Genecards.Then we obtained the common targets of HJP and ED using the Venny software,constructed a protein-protein in-teraction(PPI)network of HJP acting on ED,and screened out the core targets with the Cytoscape software.Lastly we performed GO functional enrichment and KEGG pathway enrichment analyses of the core targets followed by molecular docking of HJP and the core targets using Chem3D and AutoDock Tools and QuickVina-W software.Results:A total of 64 effective compounds,822 drug-related targets,1 783 disease-related targets and 320 common targets were obtained in this study.PPI network analysis showed that the core targets of HJP for ED included ESR1,HSP90AA1,SRC,and STAT3.GO functional enrichment analysis indicated the involvement of the core targets in such biological processes as response to xenobiotic stimulus,positive regulation of kinase activity,and positive regu-lation of MAPK cascade.KEGG pathway enrichment analysis suggested that PI3K-Akt,apoptosis,MAPK,HIF-1,VEGF,autophagy and other signaling pathways may be related to the mechanism of HJP acting on ED.Molecular docking prediction exhibited a good doc-king activity of the key active molecules of HJP with the core targets.Conclusion:This study showed that HJP acted on ED through multi-components,multi-targets and multi-pathways,which has provided some evidence and reference for the clinical treatment and subsequent studies of the disease.
4.Study on Decision Tree Model of Hyperactive Liver Yang Syndrome in Patients with Essential Hypertension

Objective To construct a diagnostic model based on the information of the TCM four diagnoses in hyperactive liver yang syndrome in patients with essential hypertension.Methods Syndromic investigation was carried out in patients with essential hypertension in some hospitals in Fujian and Beijing,and diagnostic information such as age,gender,symptoms,tongue and pulse were collected.On the basis of statistical analysis,this study adopted C5.0,CRT,CHAID and QUEST decision tree models respectively.After evaluating the stability and performance consistency of the models,the accuracy of the models was measured by diagnostic rate,and the optimal model of hyperactivity of liver yang in essential hypertension was selected.Results Totally 533 patients with essential hypertension were included,including 198 patients with hyperactive liver yang syndrome and 335 patients without hyperactive liver yang syndrome.The diagnostic rates of the four models were 75.2%,66.2%,67.7%and 65.0%,respectively.The diagnostic efficiency of C5.0 was better than that of CRT,CHAID and QUEST models."Aggravation after emotional excitement,poor complexion,string-like pulse,irritability,head swelling pain,bitter mouth,heavy pulse,fatigue,dark tongue,irritability,dizziness,thin pulse,yellow fur"could be regarded as the specific items of the syndrome model of hyperactive liver yang in essential hypertension.Conclusion The C5.0 decision tree model can clearly and intuitively identify the hyperactive liver yang syndrome in essential hypertension patients based on clinical TCM four diagnostic information data,and summarize diagnostic rules.
5.Anti-aging Effect of Ginseng Radix et Rhizoma:A Review

Ginseng Radix et Rhizoma(GRR) has the function of replenishing vital energy and can lighten the body and prolong the life when taken for a long time, which is suitable for the development of anti-aging products, so this paper intends to sort out the progress of anti-aging research on GRR. After combing, the results of modern studies have shown that a variety of components in GRR have anti-aging effect, which can prolong the lifespan of aging animal models, as well as delay the aging of various systems. The anti-aging mechanisms mainly include anti-cellular senescence, anti-oxidative stress, inhibiting telomere shortening, maintaining mitochondrial homeostasis and so on. The anti-aging ingredients of GRR involved in the researches mainly include ginsenoside Rg1 and ginsenoside Rb1, in addition, ginsenoside Rg3, ginsenoside Rd, ginsenoside Rg2, ginsenoside Re, ginsenoside Rb2, oligosaccharides of GRR, polysaccharides of GRR, water extract of GRR, total saponins of Panax ginseng stems and leaves are also included. Therefore, under current background of population aging, the in-depth development of GRR and its transformation into anti-aging products are of great significance for delaying senility and improving the health conditions of aging population.

G protein-coupled receptor (GPR) 40, as one of GPRs family, plays a potential role in regulating glucose and lipid metabolism. To study the effect of GPR40 novel agonist SZZ15-11 on hyperglycemia and hyperlipidemia and its potential mechanism, spontaneous type 2 diabetic KKAy mice, human hepatocellular carcinoma HepG2 cells and murine mature adipocyte 3T3-L1 cells were used. Cognitive dysfunction is the main manifestation of aging,which has become a hot topic of global concern as aging inten-sifies.This article proposes that essence is the origin of life,qi is the driving force of life,and spirit is the embodiment of life.Guided by the qiluo theory of essence,qi,and spirit,the key pathogenesis and corresponding treatment methods of cognitive impairment caused by aging are proposed:kidney essence deficiency and brain marrow insufficiency are the fundamental,primordial qi deficiency and brain meridian obstruction are the key,brain spirit dystrophy and spirit activity dysfunction are the manifestations;the treatment is to tonify the kidney and essence to fill brain marrow,warm and nourish primordial qi to unblock brain meridians,enhance brain sprit to improve spirit activity,expecting to be beneficial for delaying and preventing aging-caused cognitive impairment.
8.Coagulation Factors for Diagnosis of Periprosthetic Joint Infection

ObjectivePeriprosthetic joint infections (PJI) are currently the most calamitous complication after arthroplasty. Although achievements have been made in many markers for the diagnosis of PJI, the lack of a gold standard remains a great obstacle for early diagnosis. This study aimed to investigate the association between coagulation markers and the development of PJI in patients undergoing revision total joint arthroplasty (TJA). MethodsWe conducted a retrospective cohort study with a total of 2 517 patients who underwent hip or knee arthroplasties from January 2011 to January 2022 (2 394 with primary TJA, 87 with aseptic revision and 36 with PJI). We applied univariate analysis and multivariate logistic regression to analyze differences of coagulation factors between primary TJA and aseptic revision or PJI group. Receiver operating characteristic (ROC) curve and area under the curve (AUC) were used to measure the diagnostic value of coagulation factors in predicting PJI. ResultsCoagulation factors and their ratios including plasma fibrinogen (FBG), prothrombin time (PT), thrombin time (TT), activated partial thromboplastin time (APTT), platelet (PLT), mean platelet volume (MPV), platelet distribution width (PDW), plateletcrit (PCT), PLT / MPV, PLT / PDW and PLT / PCT were included in this study. High FGB level was strongly correlated with the risk of PJI compared to other coagulation factors. The optimal threshold value of FBG was 4.53 g/L with a sensitivity of 47.22%, a specificity of 93.07% (Primary TJA group vs. PJI group). Similarly, the optimal threshold value of FBG was 4.44 g/L with a sensitivity of 47.22%, a specificity of 95.40% between the other two groups (Aseptic revision group vs. PJI group). ROC curve analysis demonstrated moderate diagnostic performance of FBG (AUC value), indicating a potential to be a diagnostic marker for PJI. ConclusionsFBG is significantly correlated with PJI and it can be used as a potential non-invasive marker for early detection. It may serve as a safe and cost-effective tool for assessing PJI in clinical work.

To deepen the understanding of the acupoint indications, clarify the targeting of acupoints, and provide a basis for the composition of acupuncture prescriptions, it is suggested to add acupoint identification into the textbook Meridians and Acupoints, and a preliminary assumption that relevant acupoints can be identified by taking the indications, locations, and meridians as the key points is proposed. In this paper, acupoints for treating stomach disease, acupoints of eye region, and five-shu points of lung meridian are taken as examples, combined with ancient literature and modern scientific research achievements, the main indications of acupoint is identified, which is of great significance to the discipline's development, talent training, and achievement transformation.
