ObjectiveTo investigate the current situation of mutual recognition for ethical review outcomes in multicenter clinical research across 10 contracting medical institutions in Guangdong Province, analyze existing issues and propose improvement recommendations, thereby promoting the standardized management of mutual recognition for ethical review outcomes in medical institutions. MethodsData from 381 multicenter clinical studies conducted in these 10 medical institutions from January 24 to October 31, 2024, were collected. Text visualization was performed using Python and the WordCloud library, and statistical analyses were conducted with SPSS 25.0. ResultsOf the 381 studies investigated, industry-sponsored clinical trials (ISTs) accounted for 51.71%, while investigator-initiated clinical trials (IITs) constituted 48.29%. The proportions of ethical committees serving as primary reviewers and collaborative reviewers were 33.33% and 66.67%, respectively. The confirmation methods of mutual recognition outcomes were primarily expedited reviews (50.66%) and meeting reviews (49.34%), and no cases of “direct confirmation” were found. The Chi-square test revealed statistically significant differences in review confirmation methods based on project type (χ²=14.851, P<0.001) and ethics committee role (χ²= 69.435, P<0.001). The frequency distribution trend of the contingency table showed that IST projects and primary ethics committees preferred to employ meeting review (58.88% and 79.53% respectively, both higher than the average level of 49.34%), while IIT projects and the collaborative ethics committees more frequently utilized expedited review (60.87% and 65.75% respectively, both higher than the average level of 50.66%). ConclusionThe confirmation methods of mutual recognition for ethical review outcomes in multicenter clinical research are significantly associated with the role of the ethics committee and the type of project. It is recommended to improve management systems, enhance information construction and personnel training, as well as clarify mutual recognition responsibilities and strengthen supervision. This aims to ensure review quality while improving mutual recognition efficiency, thereby safeguarding the rights and interests of research participants.

Background/Aims:
Non-pharmacological treatments are crucial for managing irritable bowel syndrome (IBS), yet patient engagement remains a challenge. Understanding patients' knowledge, attitudes, and practices regarding these treatments is essential for improving care.
Methods:
A cross-sectional study was conducted across 5 hospitals, from October 2023 to February 2024. A self-designed knowledge, attitudes, and practices questionnaire along with the IBS quality of life and IBS severity scoring system was administered, and 496 valid responses were analyzed. Statistical analyses included correlation tests, multivariate linear regression, and mediation effect analysis.
Results:
The median scores for knowledge, attitude, and practice were 28, 25.5, and 21, respectively. Significant positive correlations were found between knowledge-attitude (r = 0.195), knowledge-practice (r = 0.364), and attitude-practice (r = 0.151). The multivariate linear regression analysis further indicated that knowledge (β = 0.399, P < 0.001) and attitude (β = 0.219, P = 0.022) positively correlated with the practical performance. SEM revealed that knowledge had a significant direct effect on both attitude (β = 0.186, P = 0.013) and practice (β = 0.356, P = 0.006). However, the direct effect of attitude on practice was not significant, and attitude did not mediate the relationship between knowledge and practice.
Conclusions
IBS patients exhibit a significant gap between their positive attitudes and their actual practices concerning non-pharmacological treatments. Knowledge is a direct driver of practice, but positive attitudes alone are insufficient to translate into behavior. Healthcare providers must move beyond simply fostering positive attitudes and focus on targeted educational interventions that provide actionable knowledge and skills to improve patient outcomes.

Objective To analyze the incidence and influencing factors of workplace violence (WPV) against nurses in tertiary hospitals in Western China, and to conduct a qualitative exploration based on the lived experiences of nurses who have suffered WPV. Methods A total of 1 294 nurses from 21 tertiary hospitals across 12 provinces, autonomous regions, and municipalities in western China were selected as the research subjects using a convenience sampling method. Investigation was performed among the nurses using the Hospital Workplace Violence Questionnaire and the Implementation Status of Hospital Violence Prevention Measures Questionnaire. Additionally, a total of 18 nurses who had experienced WPV were selected for semi-structured qualitative interviews. Results The overall incidence of WPV among the nurses was 47.5%, with incidences of psychological, physical, and sexual violence at 42.0%, 7.3%, and 3.5%, respectively. Binary logistic regression analysis showed that male nurses had a higher risk of experiencing WPV than female nurses (P<0.01); widowed/divorced nurses had a higher risk than unmarried nurses (P<0.05); both head nurses and department head nurses had a higher risk than general nurses (both P<0.01); and nurses with lower trust in hospitals had a higher risk than those with higher trust (P<0.01). Qualitative interviews revealed that WPV often stemmed from the accumulation of patients′ negative emotions during medical care, exacerbated by poor communication and unmet treatment expectations. Nurses tended to adopt avoidance and tolerance strategies when facing conflict, lacking effective coping skills. Following WPV incidents, nurses frequently experienced anxiety and burnout, relying mainly on self-regulation or confiding in colleagues to cope with the impact of WPV. Hospital responses to WPV are largely passive, with insufficient dedicated departments and psychological support mechanisms, indicating a need for improved institutional frameworks. Conclusion WPV among nurses in tertiary hospitals in Western China remains a serious concern. Hospital management should prioritize high-risk groups such as male and widowed/divorced nurses, and strengthen psychological support systems and standardized reporting procedures to mitigate WPV and to enhance nursing quality.

Space Medicine & Medical Engineering 2025;36(3):225-229
Objective Electrocardiogram(ECG)signal quality degrades when the level of activity is high and motion artifacts are severe.Poor quality signals may result in false alarms, poor patient monitoring, imprecise measurement, and misleading diagnosis. The quantitative assessment of ECG signal quality forms the basis of automatic ECG noise reduction and heart disease diagnosis. Methods The ECG signal quality index(SQI)was obtained by statistically analyzing the heartbeat similarity and the slope character, respectively, namely rSQI and kSQI.Results Using MIT-BIH Noise Stress Test Database to test, both rSQI and kSQI decreased when the Signal Noise Ratio(SNR)decreased, which revealed the ECG signal quality.Based on the quasiperiodic property, the waveform similarity,as a beat-to-beat index,is obtained by cross correlation between two ECG cycles with high precision but heavy computation.Slope-based method dispenses with QRS detection and is very simple and real-time,but its sensitivity is lower than similarity-based method and it only get statistical data. Conclusion Both morphology similarity and slope character algorithms could provide objective estimation of ECG quality.Slope-based method is an attractive measure due to its simplicity and mathematical convenience, while similarity-based method is more accurate and robust for ECG quality assessment.

OBJECTIVE:
To investigate chronic hepatitis C virus (HCV) infection's effect on gestational liver function, pregnancy and delivery complications, and neonatal development.
METHODS:
A total of 157 HCV antibody-positive (anti-HCV[+]) and HCV RNA(+) patients (Group C) and 121 anti-HCV(+) and HCV RNA(-) patients (Group B) were included as study participants, while 142 anti-HCV(-) and HCV RNA(-) patients (Group A) were the control group. Data on biochemical indices during pregnancy, pregnancy complications, delivery-related information, and neonatal complications were also collected.
RESULTS:
Elevated alanine aminotransferase (ALT) rates in Group C during early, middle, and late pregnancy were 59.87%, 43.95%, and 42.04%, respectively-significantly higher than Groups B (26.45%, 15.70%, 10.74%) and A (23.94%, 19.01%, 6.34%) ( P < 0.05). Median ALT levels in Group C were significantly higher than in Groups A and B at all pregnancy stages ( P < 0.05). No significant differences were found in neonatal malformation rates across groups ( P > 0.05). However, neonatal jaundice incidence was significantly greater in Group C (75.16%) compared to Groups A (42.25%) and B (57.02%) ( χ 2 = 33.552, P < 0.001). HCV RNA positivity during pregnancy was an independent risk factor for neonatal jaundice ( OR = 2.111, 95% CI 1.242-3.588, P = 0.006).
CONCLUSIONS
Chronic HCV infection can affect the liver function of pregnant women, but does not increase the pregnancy or delivery complication risks. HCV RNA(+) is an independent risk factor for neonatal jaundice.

Chinese Journal of Rheumatology 2025;29(5):372-379
Objective:To investigate the potential utility of the lymphocyte to monocyte ratio (LMR) and its modified index, albumin-LMR product, as predictive biomarkers for disease activity and presence of lupus nephritis in systemic lupus erythematosus (SLE).Methods:A total of 264 patients with newly diagnosed SLE who were treated at Henan Provincial People′s Hospital between December 2016 and September 2022 were included in this study. Their clinical data were subsequently collected for analysis. Patients were classified into non-active disease group (SLEDAI<5, n=55) and active disease group(SLEDAI≥5, n=209) based on the SLE SLEDAI. The Mann-Whitney U test was employed to compare the differences in clinical parameter levels, LMR and albumin-LMR product between the two groups. Additionally, the active disease group was further stratified into mild(SLEDAI 5~9, n=86), moderate(SLEDAI 10~14, n=96) and severe (SLEDAI≥15, n=27) subgroups to assess differences in LMR and albumin-LMR product. Further more patients were stratified into two groups based on renal involvement: those without lupus nephritis(non-LN) and those with lupus nephritis (LN). For non-parametric comparisons, the Mann-Whitney U test was used for intergroup comparisons between two groups, while the Kruskal-Wallis H test was applied for comparisons among three groups. If the Kruskal-Wallis H test revealed statistically significant differences ( P<0.05), pairwise comparisons were performed using the Mann-Whitney U test, and the significance level was adjusted using the Bonferroni method to account for multiple testing.Correlations between LMR, albumin-LMR product, and disease activity indicators were analyzed using Spearman′s correlation. The diagnostic value of LMR and albumin-LMR product for SLE activity was evaluated using the receiver operating characteristic (ROC) curves. Results:Both LMR and albumin-LMR product were significantly lower in active disease group compared to the non-active group {LMR:3.56 (2.15, 5.00) vs. 5.68 (3.89, 7.00); albumin-LMR product [93.21 (59.50, 143.98)g/L] vs. [187.89 (137.67, 260.90)]g/L, Z=-5.68, -7.05, P<0.001 for all}. Further subgroup analysis revealed that LMR and albumin-LMR product levels in severe, moderate, and mild active disease were also significantly decreased compared to the non-active disease group {LMR:3.83(1.78, 5.09)、3.09(2.06, 4.90)、3.65(2.45, 5.03) vs. 5.68(3.89, 7.00); albumin-LMR product: [95.69(66.57, 121.61)]g/L、[79.82(49.02, 126.91)]g/L、[104.73(69.21, 169.01)]g/L vs. [187.89(137.67, 260.90)]g/L, H=34.27, 58.29, P<0.001 for all}. A significant disparity in the levels of LMR and albumin-LMR product was detected between the non-LN and LN, with statistical significance ( Z=-3.44, P=0.001 and Z=-7.06, P<0.001). Correlation analysis indicated that LMR negatively correlated with SLEDAI( r=-0.31), urea( r=-0.29), creatinine ( r=-0.28) and 24-hour urinary protein level ( r=-0.27), all P<0.001, with no significant correlation to complement C3 or C4. Albumin-LMR product showed stronger negative correlations with SLEDAI ( r=-0.44), urea ( r=-0.40), creatinine ( r=-0.37), and 24-hour urinary protein ( r=-0.55), all P<0.001, and a positive correlation with complement C3 ( r=0.18, P=0.004). The areas under the ROC curves for LMR and LMR combined with complement C3 were 0.749 and 0.795, respectively, while for albumin-LMR product and its combination with complement C3, they were 0.809 and 0.833, indicating superior diagnostic efficacy for the modified albumin-LMR product. Conclusion:LMR and albumin-LMR product levels are significantly associated with SLE disease activity and may serve as potential biomarkers for assessing SLE activity and the degree of lupus nephritis.
