1.Effectiveness analysis on the standardization construction of the"full-chain"integrated medical and elderly care service model in a Tertiary Hospital

Objective:To explore the effectiveness of the"full-chain"integrated medical and elderly care service model in addressing key issues in medical-nursing services such as weak medical support capacity and insufficient provision of community-and home-based medical-nursing services.Methods:The development pathway for the"full-chain"integrated medical-elderly care service standardization system,encompassing core components such as operational mechanisms,smart platforms,policy documents,and quality control systems was systematically outlined.Effectiveness based on dimensions including service coverage,quality improvement,talent development,and social benefits was evaluated.With standardization as the core driver,the'1234567'management model was innovatively implemented.Results:The model leveraged the downward allocation of high-quality resources from tertiary general hospitals to strengthen subdistrict community health service centers.By collaborating with subdistrict elderly-care service centers,it established"subdistrict medical-elderly care and wellness service centers".These centers enhanced the capabilities of"community-embedded elderly-care complexes",including community daytime care centers,established two-way referral channels between medical and elderly care services,aligned with healthcare demands to provide elderly individuals with reliable medical support.It reduced the burden on families and society,stimulated market vitality,boosted domestic demand,promoted the development of integrated medical-elderly care and wellness initiatives,thereby advancing the silver economy.With provincial government endorsement,the model had been applied to 203 communities across 37 counties by the end of 2024.Conclusion:The established"full-chain"integrated medical-elderly care service model facilitates regional high-quality development in integrated care by consolidating healthcare group resources and seamlessly connecting the service chain across hospitals,nursing homes,community institutions,and home-based settings,thereby creating a practical paradigm for comprehensive elderly care service delivery.
2.Construction of a myocardial and skeletal muscle-specific Cyr61 gene knockout mouse model

Objective To construct a conditional knockout mice model of cysteine-rich angiogenesis inducer 61(Cyr61,also known as Ccn1)gene in myocardium and skeletal muscle regulated by CRE recombinant enzyme.Methods C57BL/6 mice were used to create CKmm-Cre+/-Cyr61flox/flox mice by using CRISPR/Cas9 technology.The successful construction of conditional knockout mice was confirmed by identifying the Cyr61flox/flox,3'LoxP and Cre enzyme sequences in mice.The knockout efficiency of Cyr61 was confirmed by Western blot.The skeletal mus-cle function of Cyr61 knockout mice was evaluated and following related indicators in the myocardium and skeletal muscle were detected by Western blot:Aging(p53,p21),inflammatory response(TNF-α,IL-18,IL-1β),fibrosis(vimentin,TGF-β,α-SMA,COL1).Results The mice were successfully bred and identified.In comparison with the control group,Cyr61 protein level showed a significant decrease in both myocardium and skeletal muscle in the experimental group(P<0.01).Compared with the control group,the experimental group mice showed increased skeletal muscle grip strength(P<0.05),enhanced maximum single contraction force(P<0.01),and increased average cross-sectional area of the anterior tibialis muscle(P<0.05).The expression level of p21,TNF-α,IL-18,IL-1β,TGF-β,and COL1 proteins in the myocardium and skeletal muscle of the experimental group was all lower than those of the control group(P<0.05).Conclusions Myocardial and skeletal muscle-specific Cyr61 gene condi-tional knockout mice were successfully constructed based on Cre-LoxP technology and heritable trait could be passed stably.

This study aimed to comprehensively examine the association of gallstones, cholecystectomy, and cancer risk. Multivariable logistic regressions were performed to estimate the observational associations of gallstones and cholecystectomy with cancer risk, using data from a nationwide cohort involving 239 799 participants. General and gender-specific two-sample Mendelian randomization (MR) analysis was further conducted to assess the causalities of the observed associations. Observationally, a history of gallstones without cholecystectomy was associated with a high risk of stomach cancer (adjusted odds ratio (aOR)=2.54, 95% confidence interval (CI) 1.50-4.28), liver and bile duct cancer (aOR=2.46, 95% CI 1.17-5.16), kidney cancer (aOR=2.04, 95% CI 1.05-3.94), and bladder cancer (aOR=2.23, 95% CI 1.01-5.13) in the general population, as well as cervical cancer (aOR=1.69, 95% CI 1.12-2.56) in women. Moreover, cholecystectomy was associated with high odds of stomach cancer (aOR=2.41, 95% CI 1.29-4.49), colorectal cancer (aOR=1.83, 95% CI 1.18-2.85), and cancer of liver and bile duct (aOR=2.58, 95% CI 1.11-6.02). MR analysis only supported the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer. This study added evidence to the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer, highlighting the importance of cancer screening in individuals with gallstones.

5.Effects of two driving pressure-based methods to set positive end-expiratory pressure on pulmonary mechan-ics and oxygenation in patients undergoing laparoscopic and thoracoscopic esophagectomy

Objective To investigate the effects of two driving pressure-based methods to set positive end-expiratory pressure on pulmonary mechanics and oxygenation in patients undergoing laparoscopic and thoracoscopic esophagectomy.Methods Sixty patients undergoing laparoscopic and thoracoscopic esophagectomy were divided into two groups(n=30 each):incremental group(group Ⅰ)and decremental group(group D).PEEP titration was performed in both groups during thoracoscopy and laparoscopy.Respiratory mechanics parameters,hemodynamic parameters,and blood gas analysis were collected for analysis before preoxygenation(T0),10 minutes after intuba-tion(T1),20 minutes after PEEP application for one-lung ventilation(T2),20 minutes after PEEP application for two-lung ventilation(T3),before extubation(T4),and 30 minutes after extubation(Ts).The postoperative pulmonary complications within 3 days and 7 days after operation,hospitalization duration,and costs were recorded.Results Compared with group Ⅰ,patients in group D showed higher oxygenation index and pulmonary compliance during surgery(P<0.05).In both groups,driving pressure decreased and compliance increased after PEEP titration(P<0.05).Conclusion Both driving pressure-guided incremental and decremental titration of individualized PEEP improved intraoperative respiratory mechanics in patients undergoing laparoscopic and thoracoscopic esopha-gectomy,and decremental titration was more effective in improving intraoperative respiratory mechanics and oxygenation in patients during operation.

Isoflavones which mainly distributed in leguminous plants have plenty of health benefits.Isoflavone synthase(IFS)is a membrane-associated cytochrome P450 enzyme(CYP450)which carries out the unique aryl-ring migration and hydroxylation.So far,few crystal structures of plant P450s have been obtained.We determined the crystal structure of IFS from Medicago truncatula at 1.9 ? by MAD method using a selenomethionine substituted crystal and conducted molecular docking and mutagenesis study.The structure of IFS complexed with imidazole exhibits the helix Ⅰa-loop-helix Ⅰβ motif which cor-responds to helix Ⅰ of other P450s.Compared with structures of common P450s,IFS/imidazole structure contains an extra domain,i.e.,the γ-domain.The structure reveals a homodimer in which the γ-domain of one molecule interacts with the β-domain of another.The plane of heme group makes an angle of ap-proximately 40° with the helix Ⅰa-loop-helix Ⅰβ motif.Molecular docking combined with mutagenesis study suggested that Trp-128 and Asp-300 might play important roles in substrate binding and recogni-tion.Phe-301,Ser-303 and Gly-305 from the helix Ⅰa-loop-helix Ⅰβ motif may play important roles in the aryl-ring migration.These novel structural features reveal insights into the unique reaction mechanism of IFS and provide a basis for engineering IFS in leguminous crops for health purpose.
8.Driver Genes in Hepatocellular Carcinoma

Hepatocellular carcinoma(HCC)is the third leading cause of cancer-related deaths,and al-though considerable progress has been made in the clinical diagnosis and treatment of HCC,the prognosis for patients remains poor,with a 5-year survival rate of only about 18%.The development of HCC is driven by the occurrence of mutations in driver genes that can directly promote cell proliferation,survival and metastasis.With the development of molecular biology and genomics technologies,researchers have elucidated that driver mutations that give HCC cells a selective growth advantage,the ability of the cells to resist apoptosis,maintain proliferative signals,initiate invasion and metastasis,induce angiogenesis,enable metabolic remodeling and immune escape.Exploring the key drivers of HCC development to fur-ther elucidate the pathogenesis of HCC may provide new directions for the diagnosis and treatment of HCC as well as improving the prognosis.In this paper,we summarize the driver gene mutations in HCC from various biological pathways such as telomere maintenance,cell cycle,the Wnt signaling pathway,oxida-tive stress,and epigenetic modifications.We also summarize the application prospects of driver gene mu-tations in the diagnosis and treatment of HCC.We aim to provide a reference for the diagnosis,treatment and research of HCC.
9.Impact of altitude on NIH-CPSI scores in patients with chronic prostatitis/chronic pelvic pain syndrome

Objective:To analyze the effect of altitude on NIH-CPSI score in patients with chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS)Methods:Clinical data and the results of NIH-CPSI Questionnaire of the 321 patients with CP/CPPS at different altitudes were collected from March 2021 to March 2022.And the influence of altitudes on NIH-CPSI score of CP/CPPS was analyzed.Result:The NIH-CPSI score of patients living at an altitude of 4 300 m was significantly higher than that of patients living at an altitude of 1 500 m and 2 200 m.The CP/CPPS patients who lived in the higher altitude had more severe symptoms of pain and urination as well as lower scores of life quality(P<0.05).Conclusion:NIH-CPSI score increased significantly with higher alti-tude,indicating more severe symptoms and decreased quality of life in CP/CPPS patients.These findings highlight the need for man-agement strategies for specific heights in patients with CP/CPPS.
