1.Efficacy and Safety of Blinatumomab in Adult Patients with B-Cell Acute Lymphoblastic Leukemia

Objective:To evaluate the efficacy and safety of blinatumomab in adult patients with relapsed/refractory(R/R)or measurable residual disease(MRD)positive B-cell acute lymphoblastic leukemia(B-ALL)in the real world.Methods:The clinical data of 30 B-ALL patients received at least 1 course of blinatumomab therapy in the Chinese PLA General Hospital from January 1st,2021 to December 31st,2023 were retrospectively analyzed,including pre-treatment baseline clinical feature,post-treatment complete response(CR),CR with partial hematologic recovery(CRh),CR with incomplete hematologic recovery(CRi),complete MRD response rate,MRD response rate(MRD<10-4),overall survival(OS),and disease-free survival(DFS),as well as drug-related adverse reactions.Results:Among 5 patients who were not assessed 4 were MRD negative and 1 did not receive bone marrow biopsy.In the R/R B-ALL group(13 cases),11 patients achieved CR/CRh/CRi and 10 patients achieved complete MRD response.In MRD+group(12 cases),9 patients achieved overall MRD response and 7 patients achieved complete MRD response.The median follow-up time was 8.4(95%CI:6.3-10.4)months.The median OS was 15.5(95%CI:0.7-30.3)months in the R/R group,while not reached in the MRD+group.The median DFS of the two groups were not reached.Drug-related adverse reactions occurred in 22 patients,and pyrexia was the most common(13 cases).Grade ≥3 adverse reactions occurred in 15 patients,and neutropenia was the most common(9 cases).Cytokine release syndrome occurred in 6 patients,including 5 cases with grade 1 and 1 case with grade 3.No patients interrupted therapy or died due to drug-related adverse reactions.Conclusion:Blinatumomab is effective in the treatment of R/R or continuous MRD+B-ALL with acceptable adverse reactions.
2.Pharmacokinetics/pharmacodynamics of antibiotics with different protein binding rates in hemodialysis

Objective·To evaluate the clearance and pharmacokinetics/pharmacodynamics(PK/PD)of antibiotics from the perspective of protein binding rates in critically ill patients undergoing intermittent hemodialysis(IHD),in order to explore the association between protein binding rate and dialysis clearance of antibiotics,and to provide theoretical basis for developing antibiotic dosing regimens during hemodialysis.Methods·Nineteen patients undergone low-flux hemodialysis and received antibiotic therapy at the Department of Nephrology,Shanghai Ninth People's Hospital,Shanghai Jiao Tong University School of Medicine,were enrolled and divided into the meropenem group(n=7),the vancomycin group(n=5)and the ceftriaxone group(n=7)according to the type of antibiotics.A liquid chromatography with tandem mass spectrometry(LC-MS/MS)method was established to detect meropenem,vancomycin,and ceftriaxone in human plasma/serum and dialysate.A two-compartment pharmacokinetic model was established using MATLAB.Instantaneous and total dialysis clearance rates were calculated,and PK/PD parameters were analyzed.Results·No significant differences were found in the clinical characteristics of subjects among the three groups.The dialysis clearance rates were as follows:meropenem group(5.14?5.97 L/h)>vancomycin group(2.87?3.77 L/h)>ceftriaxone group(1.21?1.90 L/h),with statistically significant differences(P<0.001).All three antibiotics showed good fit in the two-compartment pharmacokinetic model with a dialysate chamber(fval%<2),and the calculated PK/PD parameters were consistent with previous literature.For meropenem,the fraction of time that the free drug concentration remained above the minimal inhibitory concentration(%fT>MIC)values were 95.2%,60.8%and 32.4%at minimal inhibitory concentration(MIC)values of 2,8 and 16 μg/mL,respectively.For ceftriaxone(free concentration),the%fT>MIC values were all above 45.0%at MICs of 0.25,4 and 16 μg/mL.For vancomycin,only 14.0%of the trough concentrations reached the target range of 15?20 mg/L.Conclusion·The three antibiotics are well described by the two-compartment model.The plasma protein binding rate has a significant effect on the dialysis clearance of antibiotics in low-flux IHD,with higher protein binding associated with lower clearance.The regimens of meropenem(0.5 g/d)and ceftriaxone(2.0 g/d)are generally effective among patients undergoing low-flux IHD,while the vancomycin regimen with a loading dose of 1.0 g and a maintenance dose of 0.5 g/2 d carries a risk of treatment failure.
3.Application of cold snare endoscopic mucosal resection in treating small colorectal polyps

Objective To investigate the application effect of cold snare endoscopic mucosal resection(CS-EMR)in the treatment of 6 to 9 mm colorectal small polyps.Methods A total of 82 patients with small colonic polyps in our hospital from March 2022 to August 2023 were collected and divided into the observation group(45 cases received CS-EMR)and the control group[37 cases received hot snare endoscopic mucosal resection(HS-EMR)]according to different surgical methods.The clinical efficacy,polyp resection status,complete polyp resection rate,perioperative indicators and occurrence of complications were compared between the two groups.Results Follow-up for 1 month after operation,the effective rate of treatment in the observation group was higher than that in the control group(P<0.05).There was no statistically significant difference in the polyp resection status or perioperative indicators between the two groups(P>0.05).There was no statistically significant difference in the complete polyp resection rates of patients with different pathological types or total complete resection rate between the two groups(P>0.05).The incidence of delayed bleeding and endoscopic hemostasis rate in the observation group were lower than those in the control group(P<0.05).Conclusion The complete resection rate of 6~9 mm colorectal polyps through CS-EMR was comparable to that of HS-EMR,and CS-EMR has a better efficacy and lower risk of perioperative bleeding,along with higher safety.
4.Effects of Qinghua Zhixie Formula on macrophage polarization in a rat model of diarrhea-predominant irritable bowel syndrome via Nrf2/HO-1 and NF-κB signaling pathways

AIM To investigate the effects of Qinghua Zhixie Formula on improving symptoms of a rat model of diarrhea-predominant irritable bowel syndrome(IBS-D)and its impact on the polarization of M1/M2 macrophages.METHODS The SD rats were randomly divided into the blank group,the model group,the pirenzepine group(20 mg/kg),and the low-dose and high-dose Qinghua Zhixie Formula groups(13.64,27.29 g/kg),with 10 rats in each group.An IBS-D rat model was established using chronic restraint stress combined with gavage of senna leaf decoction.Each group underwent corresponding medication for 14 days.The rats had their changes of food intakes,stool characteristics,and body weight observed;their abdominal withdrawal reflex(AWR)measured to assess the intestinal pain sensitivity;their histopathological changes in the colon mucosa observed using HE staining;their colon mucosa apoptosis assessed using TUNEL staining;their colon expressions of ZO-1,Occludin and Nrf2 proteins detected using immunofluorescence staining;their ratio of M1/M2 macrophages in mesenteric lymph nodes measured by Flow cytometry;and their colon protein expressions of Nrf2,HO-1,p-IκBα and p-P65 analyzed by Western blot.RESULTS Compared with the model group,the groups intervened with Qinghua Zhixie Formula showed increased body weight and AWR pain threshold(P<0.05,P<0.01);generally intact colon tissue structure with orderly arrangement of epithelial cells and no significant cell degeneration or shedding;enhanced fluorescence intensity of colon ZO-1 and Occludin proteins(P<0.05,P<0.01);reduced apoptosis rate of colon tissue cells(P<0.05,P<0.01);more polarized M2 phenotype of macrophages in mesenteric lymph nodes(P<0.05,P<0.01);increased protein expressions of colon Nrf2 and HO-1(P<0.05,P<0.01);and decreased expressions of p-IKBα/IKBα and p-P65/P65(P<0.05,P<0.01).CONCLUSION Qinghua Zhixie Formula can improve the function of the intestinal epithelial mucosal barrier in the IBS-D rat models,and this may be related to the modulation of macrophage polarization toward the M2 phenotype via activation of the Nrf2/HO-1 signaling pathway and inhibited activation of NF-κB signaling pathway.
5.Pharmacokinetics/pharmacodynamics of antibiotics with different protein binding rates in hemodialysis

7.Retrospective Clinical Analysis on Lead Extraction and Reimplantation Strategies,Success and Complication Rates During Upgrade of Cardiovascular Implantable Electronic Devices in Non-infected Patients

Objectives:To analyze the clinical characteristics,strategies,success and complication rates of lead extraction and re-implantation during the upgrade of cardiovascular implantable electronic devices(CIED)in non-infectious patients.Methods:This retrospective study collected and analyzed the baseline clinical data and surgical data of 66 non-infected patients who had their existing CIEDs(including cardiac pacemaker,implantable cardioverter defibrillator[ICD],cardiac resynchronization therapy pacemaker[CRT-P])upgraded to ICD or CRT-P or cardiac resynchronization therapy defibrillator(CRT-D)or subcutaneous implantable cardioverter defibrillator(S-ICD)in Peking University People's Hospital from March 2018 to March 2024.We analyzed the strategies of lead extraction and reimplantation as well as the operation success rate and complication rate.Results:Among the 66 patients,preoperative imaging revealed that 12 patients(18.2%)had severe stenosis/occlusion of the venous access route,with lead wear/perforation in 26 patients(39.4%).32 patients(48.5%)underwent transvenous lead extraction(TLE),of which all leads were removed in 27 patients(84.4%),and only non-functional leads were removed in 5 patients(15.6%).The success rate of the TLE procedure was 100%and no complication occurred.Among the 66 patients,functional leads retained and new leads were implanted on the same side in 28 patients(42.4%),all leads were removed and new leads were reimplanted on the opposite side in 22 patients(33.3%),only non-functional leads were removed and new leads were reimplanted on the same side in 5 patients(7.6%),all leads were removed and new leads were reimplanted on the same side in 5 patients(7.6%),and 6 patients(9.1%)had the leads abandoned and then were re-implanled.The success rate of the upgrade surgery was 100%,no complications were reported.Conclusions:When the existing CIEDs(including cardiac pacemaker,ICD,CRT-P)of non-infected patients are upgraded to ICD,CRT-P,CRT-D or S-ICD,lead extraction and reimplantation are safe and feasible,and reimplantation can be performed on the ipsilateral or contralateral side.

9.The Construction and Empirical Research of the Operation and Management Capability Evaluation Indicator System in Tertiary Public Hospital

Objective:To construct an evaluation index system for the operational management capability in tertiary public hospitals and validate its feasibility,providing a reference for accurately assessing the operational management capability of public hospitals.Methods:A literature review and Delphi method were employed to construct the index system.The Analytic Hierarchy Process was used to calculate weights,and a weighted comprehensive index method was applied to evaluate the operational management capability of sample hospitals.Results:A hierarchical indicator system was established,comprising 6 first-level indicators,16 second-level indicators,and 40 third-level indicators,covering the dimensions of resource allocation,service efficiency,economic operation,risk prevention and control,development capacity,and social benefits.Empirical results indicated that the composite score of the sample hospital decreased from 71.5 in 2019 to 53.8 in 2020 and rebounded to 57.9 by 2023.Conclusions:The empirical outcomes align well with the operational capacity as understood by the operational management personnel of the sample hospital,demonstrating that the indicator system is scientifically sound and rational,possessing a high evaluative capability.
10.Radiation dose measurement for breast cone-beam CT-scanned examinees based on ART phantom of breast

Objective:To measure and estimate the radiation dose to breast cone-beam CT (CBCT) -scanned examinees, which can provide a dose reference for the selection of mammography equipment in the clinic.Methods:In this study, using a 400 cm 3 Alderson radiation therapy (ART) breast phantom and thermoluminescent detectors (TLDs), the distribution of absorbed dose, and the average glandular dose (AGD), to the examined breasts caused by a breast CBCT scanner was measured and calculated scanner at 50 and 100 mA tube currents. Results:At 50 and 100 mA tube currents, the range of breast absorbed dose inside the examined breast measured based on the phantom was 2.25-7.97 mGy and 3.88-15.68 mGy, respectively, with breast absorbed dose decreasing from the periphery to the centre of the phantom, and the related AGDs were 4.87 and 9.81 mGy, respectively.Conclusions:The AGDs to the breast of CBCT-scanned examinees measured in this study was higher than in the case of commonly used digital mammography. This will be meaningful to provide the guidance on the rational choice of imaging equipment in future clinical practice.
