1.The value of total volume response and total mass response in the therapeutic evaluation of lung metastasis of hepatocarcinoma

Objective To analyze the correlation between lesion volume,lesion mass,and maximum lesion diameter in the assessment of advanced hepatocarcinoma with lung metastasis,and to evaluate the application value of total volume response and total mass response of lung metastatic lesions in efficacy assessment.Methods A retrospective analysis was conducted on the CT imaging data of 20 patients clinically confirmed with hepatocarcinoma and lung metastases,followed by subsequent follow-up to monitor their survival outcomes.Volume measurement software was used to measure the volume of lesions before and after treatment.We recored lesion diameter,volume measurements and CT values,calculated the mass of the lesions.The correlation between lesion volume,mass and diameter was analyzed,as well as the correlation between the change rates of volume,mass and lesion diameter.Additionally,the total volume and total mass of all lesions were calculated.The correlation between the change rates of total volume/total mass and the change rate of pulmonary lesion diameter under the RECIST 1.1 criteria,as well as the correlation with changes in patients'tumor markers,were analyzed.Furthermore,the overall volume response and overall mass response of lesions were evaluated based on changes in total volume and total mass,and their consistencies with the RECIST 1.1 criteria for efficacy evaluation were analyzed.Finally,univariate Cox regression analysis was performed to explore the association between these variables and patient survival outcomes.Results There was strong correlation between lesion volume,mass and tumor diameter(r=0.771,0.775),between the rate of change in mass and the rate of change in lesion diameter(r=0.846),and between the rates of change in total volume/total mass and the rate of change in pulmonary lesion diameter under the RECIST 1.1 criteria(r=0.800,0.896).The correlation between the rates of change in total volume/total mass and patients'tumor markers was not statistically significant.There was moderate correlation between the rate of change in volume and the rate of change in lesion diameter(r=0.692).The evaluation results of total volume response and total mass response for pulmonary lesions in advanced hepatocarcinoma with lung metastasis were generally consistent with the RECIST 1.1 criteria(Kappa=0.486,0.426).Univariate Cox regression analysis revealed that total lesion volume(P=0.047)and total lesion mass(P=0.049)were independent prognostic factors for survival outcomes.Conclusion Lesion volume,mass,and diameter,as well as their respective change rates,were found to be interrelated.Furthermore,total lesion volume and total lesion mass were identified as independent prognostic factors for survival outcomes.The total volume response and total mass response are promising evaluation methods in evaluating the efficacy of lung metastasis of hepatocarcinoma,which are different from the RECIST 1.1 evaluation criteria.
2.Current status and suggestions on regulation of traditional Chinese medicine raw materials and preparations under regulatory system of drugs.

At present, the cause of traditional Chinese medicine(TCM) in China has entered a new period of high-quality development. How to strengthen the foundation for the TCM industry from the source is an important issue that deserves the attention of the authorities, industry, and academia. This study systematically analyzed the regulatory system of TCM raw materials and preparations. The study took the TCM industry chain and the product life cycle as a clue and focused on the dimensions of TCM resource protection and plant cultivation(farming), production and quality supervision of TCM raw materials and preparations, and their market access and distribution. It analyzed the current situation of the regulation of TCM raw materials and preparations under the regulatory system of drugs, discussed the main problems, and put forward corresponding suggestions. The results can provide an important reference value for the subsequent improvement of the regulatory system of drugs and the construction of a prominent regulatory system of drugs in accordance with TCM characteristics.

3.Efficacy of Chevron-Akin osteotomy with cannulated compression screw and absorbable screw fixation in the treatment of moderate and severe hallux valgus

Objective:To compare the clinical efficacy of Chevron-Akin osteotomy with cannulated compression screw and absorbable screw fixation in the treatment of moderate and severe hallux valgus.Methods:The data of 52 patients with moderate to severe hallux valgus treated in Sichuan Provincial Orthopaedic Hospital from January 2020 to December 2022 were retrospectively analyzed. According to the fixation method, they were divided into metal screw group and absorbable screw group. In the metal screw group, there were 25 cases (26 feet), 2 males (2 feet) and 23 females (24 feet), aged 36.7±4.9 years (range, 21-59 years), 12 feet on the left side, 14 feet on the right side, moderate 20 feet and severe 6 feet. There were 27 cases (29 feet) in the absorbable screw group, including 1 male (1 foot) and 26 female (28 feet), aged 34.1±5.5 years (range, 19-56 years), 16 feet on the left side, 13 feet on the right side, moderate 22 feet and severe 7 feet. The healing time and complications of osteotomy were recorded between the two groups, and the hallux valgus angle (HVA), first-second intermetatarsal angle (1-2 IMA), American Orthopaedic Foot and Ankle Society (AOFAS), hallux-metatarsophalangeal-interphalangeal joint score and visual analogue scale (VAS) for pain were compared before and after the operation.Results:All 52 patients were followed up. The follow-up time was 14.0±3.4 months (range, 11-27 months) in the metal screw group and 15.1±2.7 months (range, 13-24 months) in the absorbable screw group. In all patients, the incision healed in one stage, and no endovegetal rejection occurred. The osteotomy healing time was 3.8±0.8 months (range, 2-5 months) in the metal screw group and 3.4±1.1 months (range, 2.5-5 months) in the absorbable screw group, with no statistically significant difference ( t=1.014, P=0.417). In the metal screw group, there were symptoms of medial internal fixation irritation in 2 feet, medial osteophyte or thread knot irritation in 2 feet, and hallux valgus recurrence in 1 foot. The complication rate was 19%(5/26). In the absorbable screw group, medial osteophyte or thread knot stimulated one foot and hallux valgus recurred one foot, and the complication rate was 7%(2/29). The difference in complications between the two groups was statistically significant (χ 2=8.051, P=0.012). At 12 months after operation, for the metal screw group and the absorbable screw group, HVA (13.7°±2.1° and 12.9°±3.8°), 1-2 IMA (7.2°±1.5° and 7.8°±1.2°), AOFAS score (91.4±2.2 and 90.8±3.6 points) and VAS score (1.1±0.3 and 1.3±0.4 points), respectively, were significantly higher than those in the preoperative (HVA: 37.2°± 5.4° and 35.8°±4.7°, 1-2 IMA: 18.3°± 1.8° and 17.9°±1.7°, AOFAS score: 60.4±5.6 and 58.4±7.1 points, VAS: 6.4±0.6 and 6.8±0.4 points) improvement ( P<0.05), while there was no statistically significant difference between the two groups ( P>0.05). The incidence of complications in metal screw group was 19%(5/26), while in absorbable screw group it was 7%(2/29), with a statistically significant difference (χ 2=8.051, P=0.012). Conclusions:Both cannulated compression screw in Chevron-Akin osteotomy and absorbable screw fixation can have a good fixation effect in the treatment of moderate and severe hallux valgus. Absorbable screw fixation has the advantages of low complication rate, less interference with artifacts in later imaging examination and evaluation.

Objective To investigate the clinical characteristics and prognosis of pneumococcal meningitis(PM),and drug sensitivity of Streptococcus pneumoniae(SP)isolates in Chinese children.Methods A retrospective analysis was conducted on clinical information,laboratory data,and microbiological data of 160 hospitalized children under 15 years old with PM from January 2019 to December 2020 in 33 tertiary hospitals across the country.Results Among the 160 children with PM,there were 103 males and 57 females.The age ranged from 15 days to 15 years,with 109 cases(68.1% )aged 3 months to under 3 years.SP strains were isolated from 95 cases(59.4% )in cerebrospinal fluid cultures and from 57 cases(35.6% )in blood cultures.The positive rates of SP detection by cerebrospinal fluid metagenomic next-generation sequencing and cerebrospinal fluid SP antigen testing were 40% (35/87)and 27% (21/78),respectively.Fifty-five cases(34.4% )had one or more risk factors for purulent meningitis,113 cases(70.6% )had one or more extra-cranial infectious foci,and 18 cases(11.3% )had underlying diseases.The most common clinical symptoms were fever(147 cases,91.9% ),followed by lethargy(98 cases,61.3% )and vomiting(61 cases,38.1% ).Sixty-nine cases(43.1% )experienced intracranial complications during hospitalization,with subdural effusion and/or empyema being the most common complication[43 cases(26.9% )],followed by hydrocephalus in 24 cases(15.0% ),brain abscess in 23 cases(14.4% ),and cerebral hemorrhage in 8 cases(5.0% ).Subdural effusion and/or empyema and hydrocephalus mainly occurred in children under 1 year old,with rates of 91% (39/43)and 83% (20/24),respectively.SP strains exhibited complete sensitivity to vancomycin(100% ,75/75),linezolid(100% ,56/56),and meropenem(100% ,6/6).High sensitivity rates were also observed for levofloxacin(81% ,22/27),moxifloxacin(82% ,14/17),rifampicin(96% ,25/26),and chloramphenicol(91% ,21/23).However,low sensitivity rates were found for penicillin(16% ,11/68)and clindamycin(6% ,1/17),and SP strains were completely resistant to erythromycin(100% ,31/31).The rates of discharge with cure and improvement were 22.5% (36/160)and 66.2% (106/160),respectively,while 18 cases(11.3% )had adverse outcomes.Conclusions Pediatric PM is more common in children aged 3 months to under 3 years.Intracranial complications are more frequently observed in children under 1 year old.Fever is the most common clinical manifestation of PM,and subdural effusion/emphysema and hydrocephalus are the most frequent complications.Non-culture detection methods for cerebrospinal fluid can improve pathogen detection rates.Adverse outcomes can be noted in more than 10% of PM cases.SP strains are high sensitivity to vancomycin,linezolid,meropenem,levofloxacin,moxifloxacin,rifampicin,and chloramphenicol.[Chinese Journal of Contemporary Pediatrics,2024,26(2):131-138]
7.Visual analysis of research hotspots and development trends of Polygoni Multiflori Radix(processed)based on CiteSpace

Objective This study aims to explore the research hotspots and trends of Polygoni Multiflori Radix(processed)and provide a reference for follow-up research by bibliometric visualization analysis.Methods Relevant Polygoni Multiflori Radix literatures were collected through databases such as PubMed,Web of Science and China National Knowledge Infrastructure.Use CiteSpace 6.1.R6 software to do bibliometric analysis on the number and years of articles,research fields,countries/regions,institutions,authors,keywords,co-citation references,and build visual maps.Results A total of 1 517 literatures(1 131 Chinese literatures and 386 English literatures)were searched through databases in this review.We therefore conducted a quantitative analysis of accessible literature published over the last 30 years found that a very modest upward trend in publication volume from Chinese and a notable increase in publication over the world.Among the author teams,there was some cooperation,and the author team with the most published papers cooperated more closely,such as XIAO Xiao-he and WANG Jia-bo's team.In particular reducing toxicity and increasing efficiency by processing,and the toxicity and the rationalusage of Polygoni Multiflori Radix were a main research direction for nearly ten years.Meanwhile,through the co-occurrence analysis of keywords,we constructed and visualized a keyword network to explore the hotspots and future directions of Polygoni Multiflori Radix.We clearly found that effective and toxic homologous components,molecular mechanisms and early warning surveillance of hepatotoxicity might be the next topics for Polygoni Multiflori Radix.Conclusion The research on the homologous chemical components of Polygoni Multiflori Radix(processed)has been very active.The research on the conversion relationship and mechanism of homologous components of Polygoni Multiflori Radix(processed)and the mechanism of hepatotoxicity will be the research focus and development trend in this field.
8.Analysis of Material Basis of Famous Classical Formula Baoyuantang Based on UPLC-Q-TOF-MS/MS

ObjectiveA rapid method for identification of chemical constituents in Baoyuantang reference sample was established in order to clarify the material basis of this formula. MethodBased on ultra-performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS) and self-established database information, the chemical components in Baoyuantang were systematically characterized and identified. The chromatography was performed on a Waters ACQUITY UPLC HSS T3 column(2.1 mm×100 mm, 1.8 μm) with mobile phase of 0.1% formic acid aqueous solution(A)-0.1% formic acid acetonitrile solution(B) for gradient elution(0-3 min, 2%-19%B; 3-8 min, 19%B; 8-8.1 min, 19%-22%B; 8.1-14 min, 22%-29%B; 14-16 min, 29%B; 16-32 min, 29%-45%B; 32-32.1 min, 45%-90%B; 32.1-35 min, 90%-95%B; 35-36 min, 95%-98%B; 36-37 min, 98%-2%B; 37-40 min, 2%B). Based on electrospray ionization(ESI), continuum data format was collected in both positive and negative ion modes with a scanning range of m/z 50-1 500. Chemical constituents in the decoction of Baoyuantang were systematically analyzed by UNIFI 1.9.4 software matching, control comparison, The Encyclopedia of Traditional Chinese Medicine(ETCM) database search and literature reports. ResultA total of 229 components were identified under negative ion mode and 181 under positive ion mode, with a total of 322 components after removing duplicates, including 116 triterpene saponins, 66 flavonoids, 19 organic acids, 6 gingerphenols, 6 gingerols, 5 gingerones, 10 amino acids, 7 saccharides, 5 coumarins and 82 other components. Among them, 83, 141, 39, 35 and 38 components were attributed to Ginseng Radix et Rhizoma, Glycyrrhizae Radix et Rhizoma, Astragali Radix, Cinnamomi Cortex and Zingiberis Rhizoma Recens, respectively. ConclusionIn this study, the rapid characterization and identification of multi-class components in Baoyuantang was realized, and it was confirmed that the material basis of this formula was mainly triterpenoid saponins, flavonoids, gingerols and organic acids, and the chemical composition was attributed and analyzed, which provided a reference for the subsequent quality control research.
9.Application of Mitochondrial Targeting Strategy of Nano-delivery System in Tumor Diagnosis and Treatment

Tumor is one of the major diseases that endanger people’s health. At present, the treatments used for tumor include surgery, chemotherapy, radiotherapy and so on. Nonetheless, the traditional treatments have some disadvantages, such as insufficient treatment effect, liable to cause multidrug resistance, toxicity and side effect. Further research and exploration of tumor treatment schemes are still necessary. As the energy converter of cells, mitochondria are currently considered to be one of the most important targets for the design of new drugs for tumor, cardiovascular and neurological diseases. Nano-drug delivery carriers have the characteristics of being easily modified with active targeting groups, and it can achieve accurate targeted drug delivery to cells and organelles. This paper reviews the application of mitochondrial targeted nanoparticles in tumor diagnosis and treatment from the aspects of inhibiting tumor cell proliferation, promoting tumor cell apoptosis, inhibiting tumor recurrence and metastasis, and inducing cell autophagy.

AIM To investigate the effects of aloperine on the pulmonary expressions of TREM-1 and TREM-2 in septic rats.METHODS The rats were randomly divided into the control group,the model group and the low,medium and high dose aloperine groups ( 25,50 and 100 mg/kg ).The corresponding intraperitoneal aloperine injection was administered 0.5 h before modeling,and the rats'Penh,Cdyn and EF50 were detected 24 hrs after modeling.The rats had their pathological injury of the lung tissue observed by HE staining;their pulmonary mRNA expressions of TREM-1,TREM-2,TNF-α,IL-1β and IL-10 detected by RT-qPCR method;and their pulmonary protein expressions of TREM-1,TREM-2,TNF-α,IL-1β,IL-10,p-p65,p-PI3K and p-Akt detected by Western blot.RESULTS Compared with the control group,the model group displayed increased Penh (P<0.01),decreased Cdyn and EF50 ( P<0.01);destroyed lung tissue structure and thickened alveolar septum,increased mRNA and protein expressions of TREM-1,TNF-αand IL-1β( P<0.05,P<0.01);decreased mRNA and protein expressions of TREM-2 and IL-10 ( P<0.05,P<0.01);and increased protein expressions of p-p65,p-PI3K and p-Akt (P<0.01).Compared with the model group,the aloperine groups shared decreased Penh ( P<0.01);increased Cdyn and EF50 ( P<0.01);improved pulmonary structure and thinner alveolar wall,decreased mRNA and protein expressions of TREM-1,TNF-α and IL-1β( P<0.05,P<0.01 );increased mRNA and protein expressions of TREM-2 and IL-10 ( P<0.05,P<0.01);and decreased protein expressions of p-p65,p-PI3K and p-Akt (P<0.05,P<0.01).CONCLUSION Aloperine can alleviate the inflammatory response of septic rats by down-regulating TREM-1 expression and up-regulating TREM-2 expression,which may be related to the inhibited activation of PI3K signaling pathway.
