1.Traditional Chinese Medicine Regulates NF-κB Signaling Pathway for Treating CP/CPPS: A Review

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a prevalent and complex condition in urology and one of the leading causes of chronic pelvic pain. Due to its prolonged duration and recurrent episodes,CP/CPPS significantly impacts patients' quality of life. Abnormal activation of the nuclear transcription factor-κB (NF-κB) pathway influences pathological processes such as the inflammatory response,oxidative stress,apoptosis,and tissue fibrosis in the body,thereby promoting the progression of CP/CPPS. Currently,Western medical therapies include antibiotics,pain relievers,α-blockers,and physiotherapy,yet they still fail to halt the pathological process of CP/CPPS. Research indicates that traditional Chinese medicine (TCM) can modulate the NF-κB pathway through a multi-target and multi-pathway approach,effectively reducing prostate tissue damage and prostate index and alleviating pain and lower urinary tract symptoms. On the basis of existing studies,this paper systematically examines the intrinsic connection between CP/CPPS and the NF-κB pathway and summarizes the effects of TCM active ingredients (e.g.,emodin,quercetin,and gentiopicroside),extracts (e.g.,Oldenlandia diffusa extract),classical medidcinal pairs (e.g.,Phellodendri Chinensis Cortex-Smilacis Glabrae Rhizoma),and compound prescriptions and Chinese patent medicines (e.g.,Qianliexiao decoction,Jiedu Huoxue decoction,Qianlie Shutong capsules) in treating CP/CPPS by regulating the NF-κB signaling pathway. The review of the research findings on treating CP/CPPS through this pathway aims to offer insights for clinical practice of TCM in the prevention and treatment of CP/CPPS,as well as to provide references for basic research and new drug development in this domain.
2.Mechanisms of Action of Angelica sinensis in Prevention and Treatment of Alzheimer's Disease: A Review

Alzheimer's disease (AD) is a chronic neurodegenerative disorder with an unclear pathogenesis and currently no cure. It is characterized by a long disease course, diverse symptoms, and a persistent, difficult-to-treat nature, which greatly affects the quality of life of elderly patients. Traditional Chinese medicine (TCM) offers significant advantages in the treatment of AD, featuring multi-component, multi-target, and multi-pathway characteristics, with few adverse reactions and good safety. Angelica sinensis is a commonly used medicinal herb. As a frequently used clinical medicine for nourishing and activating blood, it shows significant efficacy in treating blood deficiency syndrome. In recent years, studies have revealed that A. sinensis exhibits significant activity in the prevention and treatment of AD. It contains various active components such as organic acids, volatile oils, and polysaccharides, and possesses pharmacological effects including anti-inflammatory, antioxidant, hepatoprotective, and neuroprotective properties. This article summarizes and outlines the mechanisms of A. sinensis and its compound formulas in the prevention and treatment of AD, providing a reference for its clinical application and the treatment of AD.
3.Current Research on Modulation of NF-κB Signaling Network by Traditional Chinese Medicine to Intervene in Rheumatoid Arthritis

Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease with complex pathogenesis, poor cure rate, long course of disease, and high disability rate, which seriously affects patients' quality of life. Western medicine treatment of RA mainly includes non-steroidal anti-inflammatory drugs, rheumatic drugs, glucocorticoids, biologics, and targeted small-molecule drugs, which have large side effects and many adverse reactions. Traditional Chinese medicine (TCM) has systematic, comprehensive, multi-target, and multi-mechanism advantages in the treatment of RA. Through the overall syndrome pattern identification, it is effective in relieving symptoms, delaying onset, and relieving pain in RA patients by dispelling wind and removing dampness, dissipating cold and dredging collaterals, clearing heat and removing toxin, resolving phlegm and removing blood stasis, and relaxing sinew and activating collaterals. In recent years, TCM has made remarkable progress in the intervention of RA by regulating the nuclear transcription factor-κB (NF-κB) signaling network. They include Toll-like receptor 4 (TLR4), phosphoinositol 3 kinase (PI3K)/protein kinase B (Akt), mitogen-activated protein kinase (MAPK), Nod-like receptor protein 3 (NLRP3), nuclear factor E2 associated factor 2 (Nrf2), Janus kinase 2 (JAK2)/signaling and transcriptional activator 3 (STAT3), and Toll-like receptor 2 (TLR2). The regulatory mechanism of NF-κB signaling network is complex: TLR4 is the upstream receptor of NF-κB, PI3K/Akt and MAPK pathways can not only regulate the activity of NF-κB, but also serve as its downstream effect pathway, Nrf2 and NF-κB often have antagonistic effects in the regulation of inflammatory response. Therefore, the research progress of regulating NF-κB signaling network by traditional Chinese medicine in the intervention of RA was summarized to provide a reference for the treatment of RA disease and the development of new drugs.
4.Improvement effects and mechanism of Bazheng powder on chronic urinary tract infection in rats

OBJECTIVE To investigate the improvement effects and its mechanism of Bazheng powder on chronic urinary tract infection (CUTI) induced by Escherichia coli in rats. METHODS The rats were divided into normal control group, model group, levofloxacin group (45 mg/kg) and Bazheng powder group (4.95 g/kg), with 10 rats in each group. Except for the normal control group, other groups were administered an intravesical injection of Escherichia coli suspension (1×10⁸ cfu/mL) via the urethra to establish CUTI model; at the same time, rats in each group were administered the corresponding medicinal solution or water by gavage once a day for 4 consecutive weeks. After the last medication, blood routine tests (white blood cell count and lymphocyte percentage), the levels of serum inflammatory factors [interleukin-1β (IL-1β), IL-6, IL-8, tumor necrosis factor-α (TNF-α)], and immune indicators [CD4, CD8, secretory immunoglobulin A (SIgA)], renal function indicators [cystatin C (Cys-C), α1- microglobulin (α1-MG), urea and creatinine] were all determined; the pathological changes in renal and bladder tissues in rats were observed. The protein expressions of Toll-like receptor 4 (TLR4), nuclear factor-κB (NF-κB), and nucleotide-binding domain leucine-rich repeat and pyrin domain-containing receptor 3 (NLRP3) in rat bladder tissues were detected. RESULTS Compared with the normal control group, the levels of IL-1β, IL-6, IL-8, TNF-α, CD8, Cys-C, α1-MG, urea and creatinine in serum, as well as the protein expressions of TLR4, NF-κB and NLRP3 in bladder tissues, were significantly elevated in the model group (P<0.05). Conversely, the levels of CD4 and SIgA were significantly decreased (P<0.05). Pathological changes, such as extensive infiltration of inflammatory cells, were observed in both renal and bladder tissues. Compared with the model group, the above quantitative indicators in the Bazheng powder group were significantly improved (P<0.05), with no obvious inflammatory lesions observed in either renal or bladder tissues. CONCLUSIONS Bazheng powder can alleviate inflammatory reaction and improve the immune function of CUTI rats, and its mechanism may be related to the inhibition of TLR4/NF-κB/ NLRP3 signaling pathway.

Benign prostatic hyperplasia (BPH) is a common chronic progressive disease in middle-aged and elderly men, characterized by prostate enlargement and bladder outlet obstruction, leading to symptoms such as frequent urination, urgency, and difficulty urinating. The pathogenesis of BPH involves factors such as aging, hormonal metabolic abnormalities, inflammatory responses, and imbalances in cell proliferation and apoptosis. Currently, the main treatment methods for BPH include medication, physical therapy, and surgical intervention. However, medication may cause side effects like sexual dysfunction and hypotension, physical therapy has limited efficacy, and surgery carries risks and postoperative complications. Therefore, there is an urgent need to find safer and more effective treatment options. Traditional Chinese medicine (TCM), with its focus on treatment based on syndrome differentiation and a holistic approach, offers therapeutic advantages through multiple pathways and mechanisms. Recent studies have shown that TCM regulates pathways such as phosphoinositide-3-kinase/protein kinase B (PI3K/Akt), nuclear factor-κB (NF-κB), mitogen-activated protein kinases (MAPK), nuclear factor E2-related factor 2/antioxidant response element (Nrf2/ARE), androgen receptor (AR), transforming growth factor-β (TGF-β)/Smad, and hypoxia-inducible factor-1α/vascular endothelial growth factor (HIF-1α/VEGF) to inhibit oxidative stress and inflammatory response, reduce prostate cell proliferation, and promote apoptosis, thus exerting therapeutic effects. This article summarizes and analyzes the roles of these signaling pathways in the occurrence and development of BPH and the mechanisms of TCM intervention, aiming to provide scientific evidence for clinical treatment and drug development for BPH.

7.Risk assessment of residual dizziness after repositioning in patients with benign paroxysmal positional vertigo according on multivariate analysis and nomogram.

Objective:To investigate the clinical characteristics of residual dizziness(RD) after repositioning in patients with benign paroxysmal positional vertigo(BPPV), identify its potential risk factors, and develop a predictive risk model. Methods:A total of 137 patients diagnosed with BPPV at the First Affiliated Hospital of Xi'an Jiaotong University between January 2023 and June 2023 were enrolled. Based on the presence or absence of subjective discomfort within 3 months after successful repositioning, patients were divided into the non-RD group(NRD, n=93) and the RD group(n=44). Differences in demographic characteristics, comorbidities, and disease-related features were compared between groups. Multivariate logistic regression analysis was used to identify independent risk factors for RD, and a nomogram was constructed based on these factors. The predictive performance of the model was assessed using the area under the curve(AUC). Results:The RD group showed significantly higher values in body mass index, prevalence of diabetes and motion sickness history, dizziness duration before repositioning, history of repositioning at external hospitals, number of treatments, and recurrence(all P<0.001). Multivariate logistic regression revealed that diabetes(adjusted OR=8.73, P=0.039), motion sickness history(adjusted OR=23.08, P<0.001), dizziness duration ≥30 days before repositioning(adjusted OR=15.16, P<0.001), and recurrence(adjusted OR=15.72, P=0.001) were independent risk factors for RD. The nomogram model based on these variables demonstrated good predictive ability, with an AUC of 0.804(95%CI 0.684-0.924). Conclusion:Diabetes, motion sickness history, dizziness duration ≥30 days, and recurrence are independent risk factors for RD after repositioning in patients with BPPV. The nomogram model based on these variables shows good predictive performance, with recurrence having the highest predictive value. This model can aid in early identification of high-risk patients and guide individualized intervention strategies.

8.Efficacy and safety of Rotarex mechanical thrombectomy combined with DCB versus PTA combined with DCB in the treatment of femoropopliteal artery in-stent restenosis

Objective:To compare the efficacy and safety of Rotarex mechanical thrombectomy (Rotarex) combined with drug-coated balloon (DCB) versus percutaneous transluminal angioplasty (PTA) combined with DCB in the treatment of femoropopliteal artery in-stent restenosis (ISR).Methods:A multicenter, prospective, randomized controlled trial was conducted. 46 patients with femoropopliteal artery ISR admitted to five hospitals (Beijing Friendship Hospital, Capital Medical University; Beijing Chaoyang Hospital, Capital Medical University; Beijing Jishuitan Hospital, Capital Medical University; Xuanwu Hospital, Capital Medical University; Beijing Tsinghua Changgung Hospital, School of Clinical Medicine, Tsinghua University) from July 2020 to June 2024 were enrolled. Patients were randomly divided into the Rotarex+ DCB group ( n=24) and the PTA+ DCB group ( n=22) using a random number table. The clinical data of the two groups were collected, including clinical characteristics, Fontaine classification, stent placement location, stent duration, and lesion length. The primary endpoint was the target blood vessel patency rate at 6 and 12 months postoperatively; the secondary endpoints included improvement in clinical symptoms (Fontaine classification), rate of reintervention, and safety indicators. Measurement data were expressed as mean±standard deviation ( ± s), and the t-test was used for comparison between groups; count data were expressed as the number of cases and percentages, and intergroup comparisons were performed using the Chi-test or Fisher exact probability method. Results:At 12 months postoperatively, the target blood vessel patency rate in the Rotarex+ DCB group was significantly higher than that in the PTA+ DCB group (81.8% vs 45.5%, P=0.012), and the proportion of patients in Fontaine classification stage I was also higher (86.4% vs 45.5%, P=0.004). The results at the 6-month follow-up were consistent (target blood vessel patency rate: 87.0% vs 59.1%, P=0.035). In terms of safety, no severe complications such as arterial rupture, amputation, or procedure-related death occurred during the perioperative period in either group. During the postoperative follow-up, no amputation or procedure-related deaths occurred in either group. Conclusion:For the treatment of femoropopliteal artery ISR, Rotarex mechanical thrombectomy combined with DCB is significantly superior to PTA+ DCB in terms of 12-month target blood vessel patency rate and improvement of clinical symptoms, with comparable safety.
9.A New Perspective on the Prevention and Treatment of Knee Osteoarthritis: the Mechanism and Intervention Strategy of TRPV1

Transient receptor potential vanilloid 1 is a non-selective cation channel, which is sensitive to heat, acidic environment and capsaicin. It is widely expressed in sensory neurons and various other tissues, and is closely associated with the development of several orthopedic disorders. Knee osteoarthritis (KOA) is a common degenerative joint disease characterized by articular cartilage degeneration, synovial inflammation and subchondral bone remodeling. In recent years, with the deepening of research, it has been found that Transient receptor potential vanilloid receptor 1 (TRPV1) plays an important role in the pathophysiological process of KOA, affecting the progression of the disease by regulating pain transmission, inflammatory response, ferroptosis of chondrocytes, bone homeostasis and other mechanisms. In recent years, by targeting the TRPV1 channel to maintain the homeostasis of the joint environment, certain breakthroughs have been made in the relief of symptoms and the prevention and treatment of KOA, showing a good application prospect. This review outlines the mechanisms and regulatory pathways of TRPV1 and discusses its roles in chondrocytes, synovial tissue, and subchondral bone, as well as its pathological relevance in the development of KOA. It further summarizes current strategies for targeting TRPV1 in KOA therapy and the progress of related therapeutic interventions. The aim of this study is to expand the treatment of knee osteoarthritis and provide new treatment ideas and strategies for KOA patients.
10.Optimization of alcohol precipitation process for Quhan Zhufeng Granules by overall desirability and central composite design

Objective:To optimize the alcohol precipitation process of Quhan Zhufeng Granules.Methods:Taking the volume fraction of ethanol, the relative density of the concentrated solution and the standing time as the investigation factors, the Z value of the total evaluation of gentiopicrin content, oleanolic acid content and dry paste yield of gentiana macrophylla as the evaluation indexes, the star-point design-response surface method was used to optimize the alcohol precipitation process.Results:The optimal the alcohol precipitation process of Quhan Zhufeng Granules: concentration relative density 1.08 g/ml (90-95 ℃), alcohol precipitation at the end of volume fraction of 62% ethanol, standing for 16 h.Conclusion:The alcohol precipitation process using overall desirability and central composite design is stable and feasible, and has good predictability, which can provide experimental basis for further scale production.
