1.Vitamin D and ferritin nutritional status among primary and secondary school students in the key nutrition monitoring counties of Xinjiang

Objective:
To analyze the relationship between vitamin D and ferritin nutritional status with physical indicators in primary and secondary school students in the key nutrition monitoring counties of Xinjiang, so as to provide reference for early prevention and intervention of students nutrition issues.
Methods:
From November to December 2023, 1 071 students aged 6-18 from key nutrition monitoring counties in the Xinjiang Nutrition Improvement Plan area were selected via stratified random sampling for physical and biochemical tests. The U test and Kruskal-Wallis H-test were used to compare the differences in physical indicators and the distribution of vitamin D and ferritin levels. Chi-square test was used to compare the prevalence of vitamin D and ferritin deficiencies among different groups of primary and secondary school students. Pearson correlation was used to analyze the relationship between the nutritional levels of vitamin D and ferritin and physical indicators.
Results:
The median vitamin D level was 14.7 (10.7, 19.0)ng/mL, with deficiency and insufficiency rates of 30.8% and 37.4% among primary and secondary school students in the key nutrition monitoring counties of Xinjiang Nutrition Improvement Plan area. Ferritin levels were 57.4 (37.7, 83.9)μg/L, with a deficiency rate of 5.7%. Males, primary school students, and rural residents had higher vitamin D and ferritin levels than females, secondary school students, and urban residents ( U =-11.35, -6.88, -4.52; -3.94, -9.17, -5.23, P <0.05). Vitamin D deficiency was more prevalent in females, secondary school students, and urban students ( χ 2=97.52, 49.01, 21.89, P <0.05), while ferritin deficiency was higher in primary school students and urban areas ( χ 2=34.11, 5.63, P <0.05). Significant differences in body mass index (BMI) and waist circumstance (WC) were observed across vitamin D/ferritin statuses ( U/H =35.47, 22.82; -4.19 , -5.36, P <0.05). Vitamin D and ferritin levels negatively correlated with age, BMI, and WC but positively with waist-to-height ratio (WHtR) ( r = -0.31, -0.19, -0.19, 0.20; -0.32, -0.13, -0.21, 0.08, P <0.05).
Conclusions
Vitamin D and ferritin levels in primary and secondary school students in key nutritional monitoring counties in Xinjiang are correlated with age, BMI, WC and WHtR, and there are nutrient deficiencies. Targeted measures are recommended to improve nutritional status and physical health.
2.Er, Cr: YSGG laser ablation combined with photodynamic therapy in the treatment of gingival leukoplakia: two case reports and literature review

Objective:
To evaluate the clinical feasibility and potential for malignant transformation prevention of hyperkeratotic oral leukoplakia (OLK) treatment using erbium, chromium:yttrium-scandium-gallium-garnet laser (Er,Cr:YSGG) ablation combined with 5-aminolevulinic acid (ALA)-mediated photodynamic therapy (PDT), providing a reference for clinical practice.
Methods:
Two cases of histopathologically confirmed gingival leukoplakia were treated. Following Er,Cr:YSGG (wavelength: 2,780 nm) ablation of the hyperkeratotic layer, 20% ALA solution was topically applied for 3 hours, followed by irradiation with a 635 nm diode laser (energy density: 100 J/cm²). Follow-up assessments included lesion regression and exfoliative cytology DNA ploidy stability, supplemented by a systematic literature review on OLK treatment advancements.
Results:
Case 1: at 1-year follow-up, > 90% lesion regression was observed (residual: 2 mm circular white patch) without recurrence, with stable diploid DNA ploidy. Case 2: complete lesion resolution and no recurrence at 1 year, with stable diploid DNA ploidy. Literature review showed that pharmacological therapies ameliorate OLK symptoms but fail to halt malignant progression, while surgical interventions carry recurrence risks. PDT demonstrates unique advantages by selectively targeting dysplastic cells. However, uneven ALA penetration due to the keratin barrier limits efficacy. Compared to needling, diode laser, or CO₂ laser pretreatment, Er,Cr:YSGG leverages water absorption for precise, low-thermal keratin ablation, substantially enhancing tissue permeability and optimizing ALA diffusion, thereby improving PDT outcomes.
Conclusion
Er,Cr:YSGG ablation combined with PDT serves as an optimized regimen for hyperkeratotic OLK, offering a novel approach to mitigate OLK carcinogenesis.
3.Development and validation of a dampness constitution prediction model based on clinical laboratory indicators

Objective:To develop a clinical predictive model for dampness constitution based on laboratory testing indicators.Methods:A retrospective cohort study was conducted on 1 355 healthy individuals who underwent physical examinations at the Health Examination Center of Guangdong Provincial Hospital of Traditional Chinese Medicine from October 1 st, 2022 to October 31 st, 2023. Basic information and blood routine, blood glucose, liver function, lipid metabolism, and kidney function test results of 1 355 apparently healthy individuals were collected. According to the diagnostic criteria for dampness constitution in traditional Chinese medicine, they were divided into a dampness constitution group (683 cases, including 394 with phlegm-dampness constitution and 289 with damp-heat constitution) and a non-dampness constitution group (672 cases). Among them, there were 547 males and 136 females in the dampness constitution group, with an age of 38.0 (32.0, 45.0) years; and there were 355 males and 317 females in the non-dampness constitution group, with an age of 33.0 (27.0, 41.0) years. A total of 1 355 apparently healthy individuals were randomly divided into a training set ( n=948) and a validation set ( n=407) using computer-generated random numbers in a 7∶3 ratio. Logistic regression analysis was employed to identify risk factors associated with dampness constitution. Utilizing these identified risk factors, a predictive model was constructed and subsequently visualized. The model′s predictive accuracy, consistency, and clinical utility were assessed using the receiver operating characteristic (ROC) curve, calibration curve, and decision curve analysis (DCA), respectively. Results:Among 1 355 subjects, there were statistically significant differences ( P<0.05) in gender, age, body mass index (BMI), blood glucose, some indicators of renal function, some indicators of blood routine, liver function, and four indicators of lipid metabolism between the dampness constitution group and the non-dampness constitution group. Gender ( OR=0.434,95 %CI 0.253-0.738), Cr ( OR=0.981,95 %CI 0.967-0.996), BMI ( OR=1.366,95 %CI 1.290-1.450), and LDL-C ( OR=1.388,95 %CI 1.014-1.897) were independent risk factors for dampness constitution ( P<0.05). A nomogram was subsequently developed based on these identified risk factors. The areas under the ROC curves (AUC) of the training set and validation set were 0.810 (95 %CI 0.783-0.837) and 0.804 (95 %CI 0.762-0.846), respectively. Conclusion:Gender,BMI,Cr and LDL-C were risk factors for the development of dampness constitution, and the clinical predictive model has clinical application value in predicting the risk of dampness constitution.
4.Knockout of IL-17A alleviates airway remodeling in a model of chronic asthma via a p38-dependent pathway

AIM:To investigate the ameliorative effect of interleukin 17A(IL-17A)gene knockout on airway remodeling in asthmatic mice and the specific mechanism involved.METHODS:In vivo,wild-type and IL-17A knockout(IL-17A-/-)mice were divided into 4 groups:control,IL-17A-/-,ovalbumin(OVA)and OVA+IL-17A-/-,with 6 mice in each group.Airway hyperresponsiveness was measured,pathological changes in the lungs were observed by staining tissue sections,the expression of the proteins related to airway remodeling,such as α-smooth muscle actin(α-SMA),matrix me-talloproteinase-9(MMP-9)and collagen type Ⅲ(Col Ⅲ),was detected by immunohistochemistry,and the level of p-p38 mitogen-activated protein kinase was detected by Western blot.In vitro,the regulatory effects of IL-17A on transforming growth factor β1(TGF-β1)-induced proliferation and migration of human airway smooth muscle(HASM)cells were as-sessed.Additionally,the level of p38 phosphorylation was detected,and the effect of a p38 inhibitor on airway remodel-ing-related protein expression was evaluated.RESULTS:Knockout of IL-17A significantly attenuated airway hyperrespon-siveness,airway inflammation and downregulated α-SMA(P<0.01),MMP-9(P<0.01)and Col Ⅲ(P<0.01)in asth-matic mice.In HASM cells,the proliferation and migration of these cells were inhibited following IL-17A intervention.Furthermore,a significant reduction in p38 phosphorylation was observed in both mouse lung tissue(P<0.01)and HASM cells(P<0.01).In addition,the gene expression of α-SMA(P<0.05),MMP-9(P<0.01)and Col Ⅲ(P<0.05)was re-duced upon further inhibition of p38 phosphorylation.CONCLUSION:Knockout of IL-17A attenuates airway remodeling in asthmatic mice by inhibiting the phosphorylation of p38 in airway smooth muscle.
5.Development and Validation of Dampness Syndrome of TCM Prediction Model Based on Blood Multiple Laboratory Indicators

Objective To explore the risk factors associated with the occurrence of dampness syndrome based on peripheral blood multiple laboratory indicators,construct predictive model and validate it.Methods A retrospective analysis was conducted on 180 patients who visited the Preventive Treatment Center of Guangdong Provincial Hospital of Chinese Medicine from May 2022 to December 2023.They were divided into two groups according to the diagnostic criteria:the damp syndrome of TCM group(n=118)and the balanced yin-yang constitution group(n=62),with the latter serving as the"non-syndrome"control group for dampness syndrome.Serum biochemical indicators were detected by electrochemiluminescence(ECL),immune cell subsets were analyzed through flow cytometer,and routine blood parameters were assessed using an automatic hematology analyzed.Logistic regression analysis was employed to screen risk factors and develop a predictive model.The Bootstrap method was used for data resampling to draw the receiver operating characteristic(ROC)curve,calibration curve,and clinical decision curve analysis(DCA)to evaluate the predictive value,consistency,and clinical efficacy of the model.Results Compared with the balanced yin-yang constitution group,the damp syndrome of TCM group showed increased levels of insulin(INS),non-high-density lipoprotein cholesterol(non HDL-C),red blood cells(RBC)and the proportion of CD4+T cells,the proportion of triglyceride(TG)>1.70 mmol/L,total cholesterol(TC)>5.20 mmol/L,low-density lipoprotein cholesterol(LDL-C)>3.37 mmol/L,and high-density lipoprotein cholesterol(HDL-C)≤1.15 mmol/L were also significantly higher,with statistical significance(U/t/χ2=-2.900~4 626,all P<0.05).Logistic regression analysis showed that INS,TC>5.20 mmol/L,HDL-C≤1.15 mmol/L,and the proportion of CD4+T cells were independent risk factors for the occurrence of damp syndrome of TCM(all P<0.05).Based on the screened risk factors,a forecasting model was established and a nomogram was plotted.The model had an area under the ROC curve area under curve(AUC)of 0.747(95%CI=0.672~0.822),a Brier score of 0.184 for the calibration curve,and demonstrated clinical net benefit at threshold probabilities ranging from 0.30 to 1.00.Conclusion The forecasting model constructed based on INS,TC>5.20 mmol/L,HDL-C≤1.15 mmol/L,and CD4+T cells ratio has a high predictive value for damp syndrome of TCM.

9.Current status and associated factors of high blood pressure among primary and secondary school students in Xinjiang in 2023

Objective:
To analyze and assess the current status of high blood pressure and associated factors among primary and secondary school students in Xinjiang in 2023, so as to provide a scientific basis for prevention and control decision making of high blood pressure among students.
Methods:
From September to November 2023, a total of 94 205 primary and secondary school students aged 8-17 from 14 prefectures and cities in Xinjiang, were selected for physical measurement and questionnaire survey using a stratified clustering random sampling method. The χ 2 test was employed to compare differences in high blood pressure rates among students with varying characteristics. Additionally, a Logistic regression model was developed to analyze associated factors with high blood pressure among primary and secondary school students.
Results:
The overall high blood pressure rate among primary and secondary school students in Xinjiang was 8.18%, with simple systolic hypertension being the main type at 52.16%. By educational stages, high blood pressure rates for elementary school (grades 4-6), middle school, high school and vocational high school were 8.04%, 8.59%, 7.65%, and 9.72% respectively ( χ 2=29.16, P <0.01). The high blood pressure rates among obese and overweight students were 9.93% and 17.88% respectively, significantly higher than 5.32% among normal weight students ( χ 2=1 643.14, P < 0.01 ). The high blood pressure rate among urban students (8.73%) was higher than that of rural students (7.15%) ( χ 2=71.58, P <0.01). Logistic regression model analysis showed increased high blood pressure risk for girls than boys ( OR = 1.06 ); middle school, high school, and vocational high school students had increased high blood pressure risk compared to elementary school students ( OR =1.22, 1.16, 1.70); rural students had lower high blood pressure risk than urban students ( OR =0.90); the risk of high blood pressure in overweight and obese groups was higher than that in the normal group ( OR =1.54, 3.00), and the risk of high blood pressure in boarding students was lower than that in non boarding students ( OR =0.71)( P <0.01).
Conclusions
In Xinjiang in 2023, primary and secondary school students have a certain rate of high blood pressure, mainly characterized by elevated systblood pressure lower. The main prevention and control targets are females, urban residents, those in higher academic stages, non boarding students, as well as overweight and obese primary and secondary school students.
10.De novo Huntington′s disease due to paternal intermediate alleles: a case report and literature review

Objective:To report the clinical manifestation and genetic characteristics of a case of de novo Huntington′s disease due to paternal intermediate alleles. Methods:Clinical data and imaging features of a middle-aged female, who complained of unstable walking without positive family history and was admitted to Xuanwu Hospital, Capital Medical University on September 20, 2022, were retrospectively analyzed. The serum samples of the patient and her parents were used to screen HTT gene dynamic mutation in accordance with the principle of informed consent and voluntary. And the relevant literatures were reviewed. Results:This is a 38-year-old female with progressive course, who presented as ataxia, involuntary movement at the end of extremities, dystonia, and cognitive impairment. Imaging results showed atrophy of bilateral caudate nuclei, as well as decreased glucose metabolism of bilateral caudate nuclei, putamen and partial cortex. Genetic testing showed the abnormal expansion of polymorphic trinucleotide (CAG) repeats in HTT gene and confirmed the diagnosis of Huntington′s disease. The CAG repeat length of the patient was 17/47 (pathopoiesis), of the father was 17/35 (intermediate alleles), and of the mother was 17/17 (normal). Conclusions:Paternal intermediate alleles may cause the first case of Huntington′s disease in a family. Importantly, HTT gene screening should be performed for the patient and parents when the diagnosis of Huntington′s disease is clinically possible despite negative family history, to prevent the misdiagnosis.
