ObjectiveTo investigate the feasibility and safety of endoscopic retrograde cholangiopancreatography (ERCP) combined with oral cholangiopancreatography in the treatment of major duodenal papilla gallbladder polyps. MethodsA retrospective analysis was performed for the clinical data of eight patients with choledocholithiasis and gallbladder polyps who underwent ERCP and combined with oral cholangiopancreatography for major duodenal papilla cholecystectomy in Center of Digestive Endoscopy, Jilin People’s Hospital, from May 2022 to June 2024, and related data were collected, including the success rate of surgery, the technical success rate of gallbladder polyp removal, the superselective method of cystic duct, the time of operation, the time of gallbladder polyp removal, and surgical complications. ResultsBoth the success rate of surgery and the technical success rate of gallbladder polyp removal reached 100%, and of all eight patients, three patients used guide wire to enter the gallbladder under direct view, while five patients received oral cholangiopancreatography to directly enter the gallbladder. The time of operation was 51.88±12.34 minutes, and the time of gallbladder polyp removal was 23.13±10.94 minutes. The diameter of gallbladder polyp was 2 — 8 mm, and pathological examination showed inflammatory polyps in three patients, adenomatous polyps in one patient, and cholesterol polyps in four patients. There were no complications during or after surgery. The patients were followed up for 2 — 27 months after surgery, and no recurrence of gallbladder polyp was observed. ConclusionOral cholangiopancreatography is technically safe and feasible in endoscopic major duodenal papilla cholecystectomy.

Objective:To establish 30-day of age transcutaneous bilirubin (TcB) reference curves for healthy neonates, and to investigate regional variations in bilirubin dynamics. Methods:A multicenter retrospective cohort study was conducted. A total of 220 950 healthy neonates born at a gestational age of 35-<42 weeks, with a birth weight ≥2 000 g, who did not receive phototherapy within 60 h after birth were recruited. All of them underwent remote TcB monitoring using the Bilibaby remote jaundice monitoring system between August 1 st, 2020 and December 31 st, 2024 in 426 hospitals. TcB data were collected within the period from birth to 30-day of age. The P40, P75, and P95 of TcB values were calculated, and dynamic TcB curves for 30-day of age were constructed. Patterns of bilirubin change, rates of change, and transition outcomes were described. Regional comparisons between South and North were conducted using linear mixed-effects models for TcB trajectories and Pearson′s chi-square test for outcome differences. Results:A total of 220 950 neonates were included, of whom 101 711 (46.03%) were female. Gestational age at birth was (38.75±1.12) weeks, and birth weight was (3 272±417) g. TcB levels increased rapidly within 3-day of age, peaked at 4-6-day of age, with peak values at P40, P75, and P95 of 200.6, 239.7 and 275.4 μmol/L (11.8, 14.1 and 16.2 mg/dl), respectively. TcB levels gradually declined thereafter and stabilized after 13-day of age, with values at P40, P75, and P95 fluctuating between 147.9-159.8, 190.4-200.6, and 231.2-239.7 μmol/L (8.7-9.4, 11.2-11.8, 13.6-14.1 mg/dl), respectively. Notably, among neonates categorized as low-or low-intermediate-risk within 3-day of age, 6 700 (12.76%) progressed to intermediate-high or high risk between 4 and 30 days of age. Before 13-day of age, TcB levels in the southern regions were consistently higher than those in the northern regions ( P=0.039); from 14 to 30 days of age, the overall TcB levels had no statistically difference, but the temporal changes in TcB still showed regional differences (degrees of freedom=3, all interaction P<0.05). Among neonates classified as low-or low-intermediate risk within 3-day of age, 25 326 were from southern regions, of whom 4 254 (16.80%) progressed to intermediate-high or high risk between 4 and 30 days of age. In northern regions, 27 193 neonates were classified as low-or low-intermediate risk within 3-day of age, among whom 2 446 (8.99%) progressed to intermediate-high or high risk. The risk progression between the 2 regions had statistically difference ( χ2=716.49, P<0.001). Conclusions:A TcB percentile curve for neonates within 30-day of age was established, revealing that both the overall TcB level and its temporal trend were higher in southern than in northern newborns. These findings provide baseline data to support continuous management of neonatal jaundice.

The anti-aging Klotho gene can not only delay the aging process but also regulate fibrosis and inflamma-tory responses by influencing inflammatory cells such as monocytes-macrophages,T cells, and B cells. It can regu-late the processes related to fibrosis and inflammatory responses through inflammatory pathways such as fibroblast growth factor(FGF),transforming growth factor-β(TGF-β),Toll-like receptor(TLR),nuclear factor-κB(NF-κB),and the renin-angiotensin system(RAS).Thus, elucidating the molecular mechanisms by which Klotho regulates inflammatory responses and fibrosis, and developing targeted intervention strategies to upregulate Klotho levels based on these mechanisms, may provide new theoretical basis and therapeutic targets for the prevention and treatment of related diseases.

Objective:To explore the human epidermal growth factor receptor 2 gene(HER2) expression amplification in endometrial cancer (EC) with different p53 expression patterns based on molecular typing, as well as their correlation and impact on prognosis. Methods:A retrospective analysis was conducted on 282 patients with EC who underwent surgical treatment at Dalian Women and Children′s Medical Center (Group) from January 2016 to December 2018 with complete clinical and pathological data. Immunohistochemistry and fluorescence in situ hybridization were used to detect the amplification of HER2 gene expression in paraffin- embedded tumor tissue of patients after surgery and confirmed the expression of p53. The clinical data were collected and tracked follow-up.Results:There were significant differences between HER2-positive patients and HER2-negative patients in the age of onset, pathological types of tumor tissue, depth of muscle infiltration, and tumor differentiation ( P< 0.05). The 5-year survival rate and 5-year progression-free survival rate of HER2-positive patients were lower than those of HER2-negative patients (45.45% vs. 95.16%, 45.45% vs. 95.11%), and there were statistical differences ( P<0.01). The positive expression of HER2 in EC tissues was positively correlated with p53 mutant expression ( r = 0.409, P<0.01). Among p53 wild-type EC patients, the 5-year survival rate was 98.4% for HER2-negative patients and 100.00% for HER2-positive patients. In the patients with p53 mutant EC, 58.05% and 37.50%, respectively, there was a statistical difference ( P<0.01). The 5-year progression-free survival rates were 97.50%, 100.00%, 60.20%, and 37.50%, respectively, and there was a statistical difference ( P<0.01). Conclusions:There is a significant positive correlation between HER2 positive overexpression and p53 mutant expression in EC tissue, and there is an inherent synergy between the two expressions. HER2 positive over expression is a key factor for poor prognosis in EC patients. Patients with HER2- positive over expression and p53 mutant expression have poorer 5-year overall survival and progression- free survival compared to other groups.
