Objective To investigate the epidemiological characteristics and influencing factors of Femoral Artery Compression-Related Skin Complications Around the Puncture Site(FACR-SCAPS)in patients with primary hepatocellular carcinoma(HCC)undergoing transarterial chemoembolization(TACE).Methods A multicenter cross-sectional study was conducted using convenience sampling.A total of 1 573 HCC patients who underwent TACE between April 2023 and October 2024 were recruited from interventional radiology departments,oncology units,and specialized centers across 10 hospitals in Beijing,Tianjin,Shandong,Hebei,Qinghai,and Inner Mongolia.Descriptive statistics,univariate analysis,and multivariate logistic regression were used to explore the epidemiological characteristics and influencing factors of FACR-SCAPS in this population.Results Among the 1 573 primary HCC patients undergoing TACE interventional therapy,FACR-SCAPS occurred in 28.99％(456/1 573),with a total of 476 complication instances recorded(30.26 per 100 patients).Patients with a single complication accounted for 96.93％,whereas those with multiple complications constituted 3.07％.The most prevalent types of complications were skin erythema,skin ecchymosis,and hard lumps formation,collectively accounting for 96.49％of all complications.Hematoma,blisters,and rupture complications collectively accounted for only 4.61％.Logistic regression analysis revealed that peak diastolic blood pressure during compression(OR=1.024,95％CI:1.013-1.035,P＜0.001),use of rotary compression hemostasis devices(OR=3.220,95％CI:2.120-4.891,P＜0.001),elevated PT-INR(OR=19.630,95％CI:6.039-63.810,P＜0.001),and anticoagulant use within the last three months(OR=1.909,95％CI:1.064-3.427,P=0.030)were significant influencing factors associated with FACR-SCAPS post-TACE.Conclusion FACR-SCAPS is commonly seen among primary HCC patients after TACE,its risk factors include peak diastolic blood pressure during compression,use of rotary compression devices,elevated PT-INR,and recent anticoagulant use.

Objective:To explore the effect of mind mapping-based health education on the discharge readiness of patients undergoing laryngectomy.Methods:From August 2022 to July 2023, convenience sampling was used to select patients who underwent first-time laryngectomy in the Otolaryngology and Head and Neck Surgery at Beijing Tongren Hospital as participants. The patients were divided into an observation group and a control group according to admission time, with 60 cases in each group. The control group received routine health education, while the observation group was treated with mind mapping-based health education. After the intervention, the discharge readiness and discharge guidance quality of two groups of patients were compared.Results:A total of 120 questionnaires were distributed and 120 valid questionnaires were collected, with a valid response rate of 100.00%. After the intervention, the discharge readiness and discharge guidance quality scores of the observation group were higher than those of the control group, and the difference was statistically significant ( P<0.01) . Conclusions:Mind mapping-based health education can improve the discharge guidance quality and discharge readiness for patients undergoing laryngectomy and provide a reference for medical and nursing staff.

Objective:To analyze the correlation between sleep midpoint and sleep quality in insomnia patients with type 2 diabetes mellitus (T2DM).Methods:By adopting current situation investigation research, total of 150 T2DM patients hospitalized in the Department of Endocrinology, the First Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from November 2021 to July 2022 were selected as the research objects. The general information questionnaire, Pittsburgh Sleep Quality Index (PSQI), anxiety scale (SAS) and Depression Scale (SDS) were used to investigate, and then analysis the datum.Results:Among 150 T2DM insomnia patients, 41 cases (27.33%) were in the early midpoint sleep group, 37 cases (24.67%) were in the middle midpoint sleep group, and 72 cases (48.00%) were in the late midpoint sleep group. There were significant differences in the distribution of sex, age and BMI level among different sleep midpoint groups ( χ2=7.24, 13.36, 15.93, all P<0.05). The scores of time to fall asleep at the midpoint of sleep in the 3 groups were (2.12 ± 1.25), (2.65 ± 0.79), (2.33 ± 1.02), the difference was significant ( F=2.14, P<0.05); the daytime disability scores in the 3 groups were (1.39 ± 1.36), (2.16 ± 1.12), (1.85 ± 1.32), the difference was significant ( F=3.17, P<0.05). Logistic regression analysis of disorder showed that the time to fall asleep ( OR=4.922, P<0.05) and daytime disability ( OR=4.043, P<0.05) had significant influence to the middle midpoint of sleep group when the early midpoint of sleep group as the control, while the male ( OR=2.182, P<0.05), 50 - 70 years old ( OR=5.005, P<0.05) and BMI over fat side ( OR=3.488, P<0.05) had significant influence to the late midpoint of sleep group. Conclusions:Medical staff should pay attention to the sleep quality of T2DM patients, pay attention to the sleep midpoint of patients, and improve patients′cognition of healthy sleep patterns.

Objective:To construct the nursing quality evaluation index system for Laryngeal Cancer Rehabilitation Nursing Clinic.Methods:The nursing quality evaluation index system for Laryngeal Cancer Rehabilitation Nursing Clinic was constructed through literature analysis and survey research. From October to December 2022, 17 experts were selected for two rounds of expert consultation, and indicators at all levels were screened, modified, and improved to establish the nursing quality evaluation index system for Laryngeal Cancer Rehabilitation Nursing Clinic.Results:In the two rounds of expert consultation, the effective response rate of the questionnaire was 100.0% (17/17), and the expert authority coefficients were 0.912 and 0.924, respectively. The Kendall harmony coefficients of the two rounds of consultation were 0.199 and 0.221, respectively ( P<0.05). The final constructed nursing quality evaluation index system for Laryngeal Cancer Rehabilitation Nursing Clinic included 3 primary indicators, 15 secondary indicators, and 90 tertiary indicators. Conclusions:The nursing quality evaluation index system for Laryngeal Cancer Rehabilitation Nursing Clinic is scientific and reliable, providing reference for nursing quality evaluation and standardized management of Laryngeal Cancer Rehabilitation Nursing Clinic.

Objective:To deeply explore the real experience, source and coping style of stigma in patients with diabetic foot, so as to provide a reference for nurses to formulate targeted intervention measures.Methods:From January to April 2021, 12 diabetic foot patients from the Department of Endocrinology, the First Affiliated Hospital of Guizhou University of Traditional Chinese Medicine were selected as the research object by objective sampling. The in-depth patient interview was conducted using the phenomenological method in qualitative research. The interview data were analyzed according to the Colaizzi 7-step analysis in phenomenology, and the themes were refined with the Nvivo 12.0.Results:The stigma experienced by diabetic foot patients was grouped into three themes. Theme one was the real experience of stigma, including shame, anxiety, depression, self-blame and regret, and low self-esteem. Theme two was the source of stigma, including family, friends and colleagues, media, and treatment costs. Theme three was stigma coping styles, including positive response, concealment, avoidance, and rejection.Conclusions:Diabetic foot patients have a strong sense of stigma and cannot take effective coping styles. Medical and nursing staff and family members of patients should pay full attention to the stigma of diabetic foot patients and provide them with nursing interventions and psychological support.

Objective:To explore the effect of enhanced recovery after surgery (ERAS) nursing in patients with chronic sinusitis during the perioperative period.Methods:From July 2019 to January 2021, 106 patients with chronic sinusitis admitted to the Otolaryngology and Head and Neck Surgery, Beijing Tongren Hospital Affiliated to Capital Medical University were selected by convenience sampling as the research object. According to the random number table method, the patients were divided into the control group (52 cases) and the observation group (54 cases) . The control group was given routine nursing, and the observation group was given ERAS nursing on this basis. The preoperative discomfort (thirst and hunger) scores, postoperative discomfort (shoulder, neck and low back discomfort) incidence, perioperative anxiety, comfort, and discharge satisfaction were compared between the two groups.Results:The preoperative thirst and hunger scores of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . The postoperative incidence of shoulder, neck and low back discomfort, and discharge anxiety scores of the observation group were lower than those of the control group, and the scores of comfort and discharge satisfaction were higher than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Perioperative ERAS nursing in patients with chronic sinusitis can reduce preoperative thirst, hunger and postoperative discomfort in the shoulder, neck, and lower back, improve patients' perioperative anxiety, comfort and discharge satisfaction, and promote patients' rapid recovery.

Objective:To understand the current status of postoperative self-care ability and quality of life in patients with laryngeal neoplasms, the correlation between them and the influencing factors of quality of life, so as to provide guidance for clinical formulation of targeted nursing measures.Methods:Using the convenient sampling method, a total of 126 patients who underwent laryngectomy for the first time in Department of Otorhinolaryngology and Head and Neck Surgery in Beijing Tongren Hospital, Capital Medical University from January to December 2018 were selected as the research objects. The Exercise of Self-Care Agency Scale (ESCA) and University of Washington-Quality of Life (UW-QOL) scale were used to investigate them.Results:The total score of ESCA of 126 patients with laryngeal neoplasms after operation was (110.35±16.05) , and the total score of UW-QOL scale was (557.32±70.00) . The total score of ESCA and scores of all dimensions were positively correlated with the total score of UW-QOL ( P<0.01) . Education level and self-care ability were the influencing factors of postoperative quality of life of patients with laryngeal neoplasms ( P<0.01) , which could explain 72.6% of the total variation. Conclusions:Postoperative self-care ability of patients with laryngeal neoplasms is at a moderate level. And the better the self-care ability of patients, the higher the quality of life.

Dysregulation of circadian rhythms associates with cardiovascular disorders. It is known that deletion of the core circadian gene Bmal1 in mice causes dilated cardiomyopathy. However, the biological rhythm regulation system in mouse is very different from that of humans. Whether BMAL1 plays a role in regulating human heart function remains unclear. Here we generated a BMAL1 knockout human embryonic stem cell (hESC) model and further derived human BMAL1 deficient cardiomyocytes. We show that BMAL1 deficient hESC-derived cardiomyocytes exhibited typical phenotypes of dilated cardiomyopathy including attenuated contractility, calcium dysregulation, and disorganized myofilaments. In addition, mitochondrial fission and mitophagy were suppressed in BMAL1 deficient hESC-cardiomyocytes, which resulted in significantly attenuated mitochondrial oxidative phosphorylation and compromised cardiomyocyte function. We also found that BMAL1 binds to the E-box element in the promoter region of BNIP3 gene and specifically controls BNIP3 protein expression. BMAL1 knockout directly reduced BNIP3 protein level, causing compromised mitophagy and mitochondria dysfunction and thereby leading to compromised cardiomyocyte function. Our data indicated that the core circadian gene BMAL1 is critical for normal mitochondria activities and cardiac function. Circadian rhythm disruption may directly link to compromised heart function and dilated cardiomyopathy in humans.
