AIM: To investigate the pharmacokinetic properties of the main active components of Dalitong extract in SD rats after oral administration using UPLC-MS / MS. METHODS: An UPLC-MS / MS method was established to simultaneously detect tetrahydropalmatine, nobiletin and costunolide in the plasma and tissues of SD rats. The method was applied to investigate the pharmacokinetic characteristics and tissue distribution. RESULTS: After a single oral administration, the three active components were rapidly absorbed into the body, with a peak concentration (Cmax) of (13.73 ± 7.50), (27.01 ± 17.69) and (6.73 ± 29.94) ng / mL for tetrahydropalmatine, nobiletin, and costunolide, respectively. The time to reach the peak concentration (Tmax) was (1.40 ± 0.93), (0.63 ± 0.28) and (2.38 ± 8.81) h, respectively. The area under the curve (AUC) was (80.43±40.03), (41.30±28.69) and (303.90 ± 136.69) ng · h · mL

Objective:Explore the feasibility and advantages of using proximal and distal ends of thoracodosal artery and vein as recipient vessels in deep inferior epigastric perforator flap immediate breast reconstruction.Methods:The clinical data of patients who underwent breast reconstruction surgery using the proximal and distal ends of the thoracodorsal vein as recipient vessels at the Department of Breast and Thyroid Surgery of the Second Affiliated Hospital of Hainan Medical University from March 2022 to June 2023 were analyzed retrospectively. Preoperative examinations included thoracoabdominal angiography and color Doppler ultrasonic localization of the main trunk and perforators of the inferior epigastric vessels. The procedure began with mastectomy and axillary lymph node dissection, followed by the isolation of bilateral perforators and the main trunk of the abdominal flap. The main trunks of the bilateral inferior epigastric arteries were then transected, and their vascular pedicles exposed and anastomosed respectively to the proximal and distal ends of the thoracodorsal artery and vein. Both arteries and veins were joined end-to-end. The flap after trimming and reconstruction was then implanted into the cavity left after mastectomy through the incision. Breast positioning was performed with the patient in a knee-bent and hip-flexed position. After adjusting the shape of the reconstructed breast. The donor site was closed, the umbilicus was reconstructed, drainage tubes were placed, and the breast incision was closed. Postoperative follow-up monitored complications associated with the flap and patient satisfaction with the breast reconstruction, utilizing a self-assessment method.Results:Four female patients were included, aged (46.0±6.5) years, ranging from 37 to 52 years. All four patients had bilateral vascular pedicles in the donor area, with three patients having thoracodorsal vessels at the distal and proximal ends as recipient vessels, and one patient having anterior serratus branch of the thoracodorsal vessels at the distal and proximal ends. All drainage tubes were removed within 7 to 10 days after surgery. Patients were discharged. Follow-up period ranged from 1 to 15 months, averaging 6 months. The patients recovered well postoperatively, with no flap-related complications occurring. All four patients were satisfied with the result of the reconstruction.Conclusion:The simultaneous application of the proximal and distal ends of the thoracodorsal artery and vein can ensure the safety of flap survival while reducing damage to the ribs and intercostal muscles, achieving better aesthetic result.

Objective:To investigate the relationship and intrinsic mechanism between disease uncertainty, coping style, social support and caregiver burden of primary caregivers of stroke, in order to provide a theoretical basis for reducing the burden of care and improving the quality of care for stroke caregivers.Methods:In this study, the main caregivers of 314 stroke patients hospitalized in Xinjiang Uygur Autonomous Region People's Hospital from December 2021 to May 2022 were selected by convenience sampling method, and cross-sectional surveys were conducted using the general data questionnaire, the Family Scale of Disease Uncertainty, the Simple Coping Style Questionnaire, the Social Support Rating Scale, and the Caregiver Burden Scale. Model 4 in Hayes′ SPSS-Process program was used to test the mediation effect, and Model 15 was used to test the mediation effect of social support.Results:The positive predictive effect of disease uncertainty in the primary caregiver of stroke patients on the burden of caregivers was significant ( β=0.665, P<0.01). Active coping played a partial mediating role between disease uncertainty and caregiver burden, with a mediating effect of 0.306 and a mediating effect of 31.8%. The interaction between disease uncertainty and social support had a significant predictive effect on caregiver burden ( β=-0.033, P<0.05), and the interaction item between active response and social support had a significant predictive effect on caregiver burden ( β=-0.019, P<0.05). Conclusions:Positive coping is an important psychological mechanism between disease uncertainty and caregiver burden, and social support can alleviate the influence of disease uncertainty on caregiver′s burden, and can also strengthen the weakening effect of positive coping on caregiver′s burden.

Objective:To explore the status quo of exercise rehabilitation attitudes and compliance in patients undergoing coronary artery bypass grafting (CABG), and analyze the influencing factors, so as to provide evidence for improving the compliance of rehabilitation exercise in CABG patients.Methods:From January to August 2022, convenience sampling was used to select 228 patients with CABG in three ClassⅢ Grade A hospitals in Urumqi City as the survey subject. One month after surgery, a questionnaire survey was conducted using the General Information and Disease Information Questionnaire, the Questionnaire of Rehabilitation Exercises (QRE), the Chinese version of the Tampa Scale of Kinesiophobia for the Heart (TSK Heart), and the Chinese version of the Multidimensional Fatigue Symptom Inventory-Short Form (MFSI-SF-C). This study conducted a latent category analysis of rehabilitation exercise attitudes and compliance among CABG patients, and identified the influencing factors of their latent categories through Logistic regression. A total of 220 valid questionnaires were collected, with an effective recovery rate of 96.49% (220/228) .Results:The fitting results of latent category analysis showed that the rehabilitation exercise attitudes and compliance of 220 CABG patients could be divided into 4 latent categories, which were respectively named as: excellent attitude-excellent compliance group, good attitude-single exercise group, moderate attitude-poor compliance group, poor attitude-poor compliance group. Logistic regression showed that education level, nationality, pain, exercise fear and fatigue were independent influencing factors for the latent categories of exercise rehabilitation attitudes and compliance in CABG patients ( P<0.05) . Conclusions:Postoperative exercise rehabilitation attitudes and compliance in CABG patients have 4 latent categories, and patient education level, nationality, pain, exercise fear and fatigue are all factors that affect compliance. Medical and nursing workers should formulate targeted interventions based on influencing factors to improve the compliance of exercise rehabilitation in patients with CABG.

Objective:To explore the correlation between technical anxiety and home care readiness in aged caregivers of stroke patients.Methods:From March to June 2022, 160 aged caregivers of stroke patients from the Department of Neurology and Rehabilitation of Xinjiang Uygur Autonomous Region People's Hospital were selected as research objects by convenience sampling and investigated with the general information questionnaire, the Chinese version of the Technical Anxiety Scale for the Aged and the Chinese version of the Caregiver's Readiness for Home Care Scale. Pearson correlation analysis was used to explore the correlation between technical anxiety and home care readiness in aged caregivers of stroke patients. Hierarchical linear regression analysis was used to explore the influencing factors of home care readiness of aged caregivers of stroke patients. A total of 160 questionnaires were distributed in this study, and 151 valid questionnaires were recovered, with an effective recovery rate of 94.4% (151/160) .Results:The home care readiness score of the aged caregivers of stroke patients was (49.42±7.54), and the technical anxiety score was (30.44±6.76). Pearson correlation analysis showed a negative correlation between home care readiness and technical anxiety among aged caregivers of stroke patients ( r=-0.577, P<0.01). Hierarchical regression analysis indicated that technical anxiety was an influencing factor of home care readiness among aged caregivers of stroke patients ( P<0.01), which could account for 9.50% of the variance in home care readiness. Conclusions:The home care readiness of the aged caregivers of stroke patients needs to be improved, and technical anxiety is an influencing factor of their home care readiness. Nursing staffs and community workers should help aged caregivers of stroke patients overcome anxiety and fear of new technologies, help them use digital health information and medical services, so as to improve home care readiness.

Abstract: Objective To investigate the clinical distribution characteristics, drug resistance trends and the carrying of antiseptic resistance gene of Pseudomonas aeruginosa infection in children in Suzhou, in order to provide theoretical basis for the prevention and treatment of Pseudomonas aeruginosa infection in children. Methods The clinical distribution characteristics and drug resistance trends of Pseudomonas aeruginosa isolated from Children's Hospital of Soochow University from 2016 to 2021 were retrospectively analyzed. Forthermore, 101 strains of Pseudomonas aeruginosa were randomly selected to detect the expression of 9 antiseptic resistance genes (qacEΔ1-sul1, qacE, qacEΔ1, qacG, sugE(p), sugE©, emrE, ydgE, ydgF) by polymerase chain reaction. Results Pseudomonas aeruginosa in Soochow University Children's Hospital was mainly isolated from respiratory specimen (47.83%), pus (28.60%) and urine (11.72%); the main departments were intensive care unit(21.45%), general surgery department (15.71%) and respiratory department (12.31%). Patients were mainly aged from 1 month to 1 year old and older than 6 years old (34.31% and 25.38%). The top three drug resistance rates of Pseudomonas aeruginosa were imipenem (11.25%), aztreonam (9.26%) and meropenem (8.02%). Among the 853 strains of Pseudomonas aeruginosa, the drug-resistant strains were mainly from the intensive care unit (58/183), hematology department (33/91), neonatology department (31/96), and there were 57 strains of multi-drug-resistant strains with the detection rate of 6.68%. There were 98 strains (11.49%) of Carbapenem resistant Pseudomonas aeruginosa, and the annual detection rates were 22.06%, 8.40%, 3.60%, 5.67%, 9.85% and 17.20%, respectively. Among the 9 antiseptic resistance genes, the carrying rate of ydgF, sugE© and qacE was 98.02%, 94.06% and 0 respectively. Conclusion Pseudomonas aeruginosa has high resistance to some drugs, so attention should be paid to rational drug use. The carriage rates of of two antiseptic resistance genes exceeded 90%, indicating the need to strengthen research on the mechanism of antiseptic resistance research and rational use of disinfectants

Objective: The urine concentrations of phthalate metabolites were used to estimate the cumulative risk assessment in preschool children in Ma’anshan of Anhui province. Methods: Based on the China-Anhui Birth Cohort, the demographic information and urine samples of 3 743 children were collected in Ma’anshan from April 2014 to April 2015. The concentrations of 7 metabolites’ [monomethyl phthalate (MMP), monoethyl phthalate (MEP), monobutyl phthalate (MBP), monobenzyl phthalate (MBzP), mono (2-ethylhexyl) phthalate (MEHP), mono (2-ethyl- 5-oxohexyl) phthalate (MEOHP) and mono (2-ethyl-5-hydroxyhexyl) phthalate (MEHHP)] of 5 phthalates [dimethyl phthalate (DMP), diethyl phthalate (DEP), dibutyl phthalate (DBP), butyl benzyl phthalate (BBzP), and di (2-ethylhexyl) phthalate (DEHP)] in the urine samples of the children were measured by solid-phase extration-triple quadrupole high performance liquid chromatography-tandem mass spectrometry-isotope method. In addition, the estimated daily intakes (EDIs) of 5 phthalates were calculated according to the metabolites’ concentrations. Cumulative risk assessment was performed using hazard quotient (HQ) and hazard index (HI) methods. Results: The M (Q_R) of seven metabolite concentrations were 29.58 (18.69-48.26), 26.65 (13.44-56.09), 256.86 (150.99-438.51), 0.12 (0.04-0.32), 6.27 (3.71-11.13), 17.94 (11.94-28.42) and 24.80 (16.05-40.32) μg/g creatinine, respectively. For the EDIs of 5 phthalates, DBP ranked first, followed by DEHP, DMP, DEP and BBzP with the M (Q_R) of 7.54 (4.41-12.85), 3.35 (2.20-5.42), 0.75 (0.47-1.24), 0.71 (0.36-1.52) and 0.003 (0.001-0.009) μg/(kg·d), respectively. The HQ and HI varied with age, gender and sampling season, the differences were significant (P<0.05). Conclusions These results indicated that risk of cumulative exposure to phthalates was high in preschool children aged 3-6 years in Ma’anshan. Age, gender and sampling season were influencing factors.

Objective: To investigate the association between the internal exposure levels of phthalic acid esters (PAEs) and executive function (EF) of preschool children. Methods: Between October 2008 and October 2010, pregnant women who accepted pregnancy health care services in four municipal medical and health institutions in Ma'anshan city, Anhui Province, were recruited as study objects. A total of 5 084 pregnant women and 4 669 singleton live births were enrolled in this cohort. The follow-up study was conducted from April 2014 to April 2015. A total of 3 725 data-completed preschool children aged 3 to 6 years older entered in this study. The method of analysis seven metabolites of phthalates in urine was high performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) and divided objects into low (P₀-P₂₄), medium (P₂₅-P₇₄) and high (P₇₅-P₁₀₀) groups according to their exposure concentrations. To investigate the executive function of preschool children, we used the Behavior Rating Inventory of Executive Function-Preschool Version (BRIEF-P). Univariate and multivariate statistical method was used to analyze the etiology association between the phthalate metabolites levels and preschool children's executive function. Results: In this study, 53.6% (1 997/3 725) of preschoolers were boys, children's age was (51.5±5.6) months. The detection rates of seven phthalate esters were: mono-n-methyl phthalate (MMP) was 99.89% (3 721/3 725); mono-ethyl phthalate (MEP) was 99.97% (3 724/3 725); mono-benzyl phthalate (MBzP) was 69.10% (2 574/3 725); mono-butyl phthalate (MBP), mono-(2-ethyl-5-oxohexyl) phthalate (MEOHP) and mono- (2-ethyl-5-hydroxyhexyl) phthalate (MEHHP) were 100.00% and mono-ethylhexyl phthalate (MEHP) was 99.95% (3 723/3 725). The median concentrations of the seven phthalate metabolites were: 17.71, 15.36, 0.07, 155.24, 10.73, 14.67, 3.59 µg/L, respectively. The median concentrations corrected by urinary creatinine were 29.65, 26.65, 0.12, 257.73, 17.94, 24.80, 6.27 µg/g Cr, respectively. The P₂₅ and P₇5 concentration of the total PAEs metabolites corrected by urinary creatinine were 1.20 µmol/g Cr and 3.04 µmol/g Cr. After adjusted the relevant demographic information: children sex, children age in month, maternal age, parental education levels, household exposure to secondhand smoke and whether the child is the only child as confounds, multivariate logistic regression model showed that the risk of inhibitory self-control index (ISCI) dysplasia in MEHHP high concentration group and MEOHP high concentration group were 1.71 and 1.54 times (OR=1.71, 95% CI: 1.11-2.62; OR=1.54, 95% CI: 1.01-2.34) than in low concentration group. The risk of ISCI dysplasia in total PAEs metabolites high concentration group was 1.55 times (OR=1.55, 95% CI: 1.00-2.38) than in low concentration group. Conclusion Phthalates exposure may damage the executive function of preschool children.

OBJECTIVETo investigate the relationship between traditional bullying, cyberbullying and depression in adolescents, with the involvement frequency and different roles considered.

METHODSA stratified cluster sampling method was used to randomly select 5 726 middle school students in three cities of Anhui province in December 2013. Those students were surveyed by self-designed questionnaire of adolescent health-related behaviors in Anhui province. Self-reports on the general demographic information, physical, verbal, relational bullying and cyberbullying involvement and depression were collected. Additionally, the items assessing depression were based on Self-rating Depression Scale (SDS). The chi-square test used to compare prevalent rates of bullying involvement and depression in different groups of students with different demographic features. Multinomial logistic regressions were performed to explore the relationships between all four types of bullying and depression, after controlling confounders.

RESULTSWe got a final effective sample of 5 726 student. The response rate was 99.4% in this study. Among 5 726 adolescents, the prevalence rates of bullying in the last two months were 23.4% (1 339/5 726) physically, 55.0% (3 147/5 726) verbally, 34.2% (1 958/5 726) socially, and 8.9% (512/5 726) electronically. The prevalence rate of depression was 64.8% (3 711/5 726). The detection rate was higher in participants who involved in physical, verbal, relational, and cyber bullying than those who not involved in bullying. After controlling the confounding factors, the different frequency and role of verbal and relational bullying were risk factors of depression (OR = 1.42-3.71, all P values < 0.001). As for physical bullying, except for occasional bully-victims (OR = 1.08, 95% CI: 0.79-1.49), others were more likely to be depressed than noninvolved students. (OR = 1.40-7.84, all P values < 0.001). Frequent involvement in cyberbullying as bullies and (or) victims were more likely to be depressed than noninvolved students (OR = 2.30-4.55, all P values < 0.001), but occasional involvement in cyberbullying as bullies and (or) victims were not the risk factors of adolescents' depression (OR = 1.39, 95% CI:0.97-1.99; OR = 1.58, 95% CI: 0.88-2.82; OR = 1.50, 95% CI:0.82-2.74).

CONCLUSIONInvolvement in physical, verbal, relational, and cyberbullying were risk factors for depression among adolescent students.
