1.Risk factors for delayed hemorrhage in patients with early gastric carcinoma after endoscopic submucosal dissection and its predictive value

Objective To analyze the risk factors for delayed hemorrhage in patients with early gastric carcinoma(EGC)after endoscopic submucosal dissection(ESD),as well as to explore the predictive value of predictive models.Methods The clinical data of 386 EGC treated with ESD patients from July 2019 to March 2024 were retrospectively analyzed.They were followed up for a short period of time(24 h~30 d postoperatively)after ESD,and were divided into the group of occurrence(50 cases)and the group of non-occurrence(336 cases)according to the occurrence or non-occurrence of delayed hemorrhage.To understand the clinical data of the patients,multivariate Logistic regression was used to analyze the risk factors for delayed hemorrhage in patients with EGC after ESD,and plot receiver operating characteristic curve(ROC curve)to analyze the predicted value.Results The proportions of cardia gastric fundus,lesion diameter ≥ 3 cm,submucosa with fibrosis,coarse blood vessels,and having ulcer or scar in the occurrence group were significantly higher than those in the non-occurrence group,the differences were statistically significant(P<0.05).The results of multivariate Logistic regression analysis(with an introduction level of 0.05)showed that the lesion location was cardia-gastric fundus(OR=1.088,95%CI:1.040~1.138),the diameter of the lesion was ≥3 cm(OR=1.095,95%CI:1.057~1.135),and fibrosis under the mucosa(OR=1.130,95%CI:1.061~1.203),thick blood vessels under the mucosa(OR=1.177,95%CI:1.116~1.241),and the presence of ulcer or scar(OR=1.082,95%CI:1.057~1.108)were the risk factors for the occurrence of delayed hemorrhage in patients with EGC after ESD(P<0.05).A prediction model was established:Logit(P)=-9.238+Location of lesion as cardia-gastric fundus × 0.084+Diameter of lesion ≥ 3 cm× 0.091+Submucosal with fibrosis × 0.122+Submucosal with thick blood vessels × 0.163+Ulcer or scar×0.079(P<0.05),the likelihood ratio test yielded x2=131.09,DF=8,P<0.05,which indicated that the constructed prediction model was valid.Hosmer-Lemeshow goodness-of-fit test showed a good model fit(P>0.05).The ROC curve showed an area under the curve(AUC)value of 0.929 at a cutoff value of 12.00,with a sensitivity of 88.00%(95%CI:0.757~0.955)and a specificity of 83.04%(95%CI:0.780~0.869).Conclusion Postoperative delayed hemorrhage in patients with EGC treated with ESD is associated with cardia-gastric fundus,lesion diameter ≥3 cm,submucosal fibrosis,thick blood vessels,and the presence of ulcer or scar,and the prediction value of the prediction model constructed accordingly is good.

3.Research progress of the therapy in treatment-resistant depression

Treatment resistant depression is a common and severe mental disorder associated with significant burden of disease .Most individuals receiving conventional pharmacotheraphy fail to achieve and sustain remission.So this is still one of the difficult challenges for the psychiatrist .Much of the research pro-vided indications that the efficacy of treatment for the disease was not optimistic, but the treatment was still made great progress .The common treatments for this disease included pharmacotherapy,psychotherapy,elec-troconvulsive therapy(ECT),transcranial magnetic stimulation (TMS),magnetic resonance guided focused ultrasound surgery( MRgFUS) ,deep brain stimulation ( DBS) ,aerobic exercise,light therapy and so on.Each therapeutic strategy has its own features, and could be suitable or unsuitable in some situations.The high rates of non-remission with first-line treatment strategies make the combination of antidepressant and non-drug treatments to be the new trend of the treatments for treatment resistant depression in the future.
4.Application of team-based learning in practice teaching of internal medicine

Objective To explore the feasibility of team-based learning (TBL) for practice teaching of internal medicine.Methods TBL was carried out in 40 interns from clinical medical students of eight-year program.Every team included 4 ~ 5 members.The course of rheumatoid arthritis was chosen according to the course syllabus.Results The students discussed enthusiastically in the team test and in-class application exercises.More than 80% of the students expressed that TBL was helpful to understand the course content better,to learn the disease better,to elevate personal study ability and to know others' thought processes.76% of the students supported to carrry out TBL again.Conclusions TBL is helpful for senior medical students to elevate their ability of applying course concepts to solve practical problems through a series of team activities.Improvement of teaching idea and skill is the key to perform TBL successfully.
5.Rosiglitazone inhibits receptor activator of NF-κB ligand expression in rheumatoid synovial fibroblasts through p-ERK pathway

ObjectiveTo investigate the effect of rosiglitazone (RSG), a high-affinity synthetic agonist for PPAR-γ, on the expression of receptor activator of NF-κB ligand (RANKL) and osteoprotegerin (OPG)in rheumatoid arthritis fibroblast-like synoviocyte (RA-FLS) and the underlying mechanisms. MethodsRAFLS were incubated with different concentrations of RSG(0, 5, 10, 15, and 20 μmol/L) for 3 days, RAFLS proliferation was detected by Cell Counting Kit-8 assay, and then the expression of RANKL and OPG was examined by real-time PCR and Western blot. The expression of phosphorylated ERK, p38 and JNK was also detected after RA-FLS was incubated with RSG(0, 15, and 20 μmol/L) for 3 days. One-way ANOVA and Bonferroni were used for statistical analysis. ResultsRSG (5, 10, 15, and 20 μmol/L) had significantly inhibited the expression of RANKL mRNA(0.503±0.005, 0.438±0.031, 0.161±0.042, 0.050±0.018) and protein (1.72±0.09, 1.58x±0.05, 1.46±0.11, 1.22x±0.14) in RA-FLS (F=200.820, F=13.602, P<0.01 ), while the expression of OPG mRNA (2.8 ±0.5, 7.0 ±3.2, 8.4 ±2.3, 25.7 ±5.1 ) and protein ( 1.21 ±0.11, 1.52 ±0.16, 1.63±0.11, 1.91 ±0.03) increased significantly (F=26.531, F=24.872, P<0.01), both in a dose dependent manner. The ratio of RANKL/OPG mRNA and protein significantly reduced (F=453.425, P<0.01 ;F=4.173, P<0.05 ). RSG (15, 20 μ mol/L) significantly inhibited the expression of p-ERK-1/2 protein (0.55±0.06, 0.45±0.06, F=6.991, P<0.05). ConclusionRSG can inhibit RANKL expression in RA-FLS through p-ERK pathway.
6.Research on the relationship between sensation seeking, personality charateristic and health-risk behaviors of medical freshmen

Objective To study the relationship between sensation seeking,personality characteristics and health-risk behaviors of medical freshmen.Methods Sensation Seeking Scale ( SSS),Eysenck Personality Questionnaire( EPQ),Adolescent Health-Risk Behaviors Questionnaire were applied in this survey to 384 freshmen from two medical colleges.Results ① The health risk behaviors were comnmon in the medical college freshmen (5.2%,48.2% ),and the score of health risk behaviors had a significant difference between boys and girls,boys risk behaviors were much more than girls(P< 0.05 ).②The correlations between the risk behaviors (such as smoking,drinking,fighting,suicidal ideas,using internet too long,poor eating behavior,lack of physical exercise) and sensation seeking personality traits were strong.The health-risk behaviors were all correlated with extraversion(E)and neuroticism (N) significantly( 66.78 ± 10.29 vs 60.57 ± 9.09,t =2.54,P < 0.01 ;7.11 ± 3.23 vs 5.33 ±2.89,t =2.81,P < 0.05 ).③The results of logistic regression analysis showed that male mother' s high education level,sensation seeking,Eysenck' s P,N dimension were effective prediction factors of freshmen ' s health-risk behaviors.Conclusion Medical college freshmen who have higher levels of sensation seeking,Eysenck' s P,N dimension are more prone to health risk behaviors.
