Clinical practice guidelines serve as a crucial bridge linking research evidence, clinical practice, and health policy decision-making. However, the guideline domain continues to face persistent challenges, including duplicate guideline development, variable methodological quality, lack of transparent reporting, insufficient implementation and translation, and delayed updates. Focusing solely on "how to develop a high-quality guideline" is no longer sufficient to meet the modern healthcare system's demands for timely, trustworthy, actionable, and dynamically updated recommendations. The emergence of the guideline ecosystem marks a shift in guideline research and practice from a linear development process toward a more systematic governance model. Rooted in the evidence ecosystem, the guideline ecosystem has its own distinct functional positioning, encompassing recommendation formulation, clinical implementation, and life-cycle updating. Looking ahead, China should leverage guideline registries, integrated quality assessment systems, implementation science, dynamic updating mechanisms, and artificial intelligence technologies to promote the transformation of its guideline system from scale expansion to an ecosystem-based model characterized by high quality and sustainability.

ObjectiveTo observe ovarian microvascular damage in rats with cold coagulation and blood stasis syndrome and to explore the mechanism by which Liangfang Wenjing decoction improves this condition in rats. MethodsFifty SPF female SD rats were randomly divided into a blank group， a model group， low-dose （8.1 g·kg^-1） and high-dose groups （16.2 g·kg^-1） of Liangfang Wenjing decoction， and a 4-phenylbutyric acid （0.1 g·kg^-1） group， with 10 rats in each group. The ice-water bath method was employed to establish the rat model of cold coagulation and blood stasis syndrome. Concurrent with modeling， Liangfang Wenjing decoction was administered continuously for 21 days， once daily. The rats' syndrome manifestations and estrous cycles were recorded. The enzyme-linked immunosorbent assay （ELISA） was used to detect serum reproductive hormone levels and levels of endothelin-1 （ET-1）， nitric oxide （NO）， thrombomodulin （TM）， and von Willebrand factor （vWF） in ovarian tissue. Prothrombin time （PT）， activated partial thromboplastin time （APTT）， thrombin time （TT）， and fibrinogen （FIB） were measured. The ovarian microcirculatory blood perfusion was detected by laser speckle contrast imaging. Hematoxylin-eosin （HE） staining was performed to observe the ovarian histopathology， flow cytometry to detect ovarian apoptosis rate， and transmission electron microscopy to observe the ultrastructure of ovarian microvascular endothelial cells. Western blot was employed to detect the protein expression of endothelial nitric oxide synthase （eNOS）， phosphorylated eNOS （p-eNOS）， Caspase-3， B-cell lymphoma-2 （Bcl-2）， Bcl-2-associated X protein （Bax）， glucose-regulated protein 78 （GRP78）， C/EBP homologous protein （CHOP）， inositol-requiring enzyme1α （IRE1α）， p-IRE1α， apoptosis signal-regulating kinase 1 （ASK1）， p-ASK1， c-Jun N-terminal kinase （JNK）， and p-JNK. Immunofluorescence was used to detect ovarian Bax and Bcl-2 expression in microvascular endothelial cells. ResultsCompared with the blank group， the model group showed signs of cold coagulation and blood stasis syndrome， prolonged estrus cycles， and reproductive hormone disorders. Histopathological results revealed a decrease in follicle counts at all stages and disorganized granulosa cell arrangement. Ovarian microcirculatory perfusion was significantly decreased （P<0.01）. PT， APTT， and TT were reduced （P<0.05， P<0.01）， while FIB levels were increased （P<0.05）. In ovarian tissue， NO content was decreased， while ET-1， vWF， and TM levels were increased significantly （P<0.01）. The apoptosis rate of ovarian cells was markedly increased （P<0.01）. Furthermore， p-eNOS/eNOS and Bcl-2 were decreased （P<0.05）， whereas Bax， cleaved-Caspase-3/Caspase-3， GRP78， CHOP， p-IRE1α/IRE1α， p-ASK1/ASK1， and p-JNK/JNK expression showed significant increases （P<0.05， P<0.01）. Compared with the model group， Liangfang Wenjing decoction intervention alleviated the symptoms of cold coagulation and blood stasis， gradually restored the estrus cycle， and improved ovarian histopathology and endothelial cell ultrastructure. Microcirculatory blood perfusion was significantly elevated （P<0.05）. NO content in ovarian tissue was elevated， while ET-1， vWF， and TM levels were significantly decreased （P<0.05， P<0.01）. The p-eNOS/eNOS ratio and Bcl-2 expression were significantly elevated （P<0.05）， while the expression of Bax， cleaved-Caspase-3/Caspase-3， GRP78， CHOP， p-IRE1α/IRE1α， p-ASK1/ASK1， and p-JNK/JNK was significantly decreased （P<0.05， P<0.01）. ConclusionLiangfang Wenjing decoction may regulate the IRE1α/ASK1/JNK signaling pathway to inhibit endoplasmic reticulum stress， attenuate apoptosis， and improve microvascular endothelial injury in ovaries of rats with cold coagulation and blood stasis syndrome.

Objective:To investigate the clinical features and prognosis of neonatal patients with congenital idiopathic vocal cord paralysis (VCP).Methods:A retrospective analysis was conducted on the general condition,clinical manifestations,auxiliary examinations,complications,treatment,and follow-up of 41 cases of congenital idiopathic VCP at the Neonatal Center,Xi'an Children's Hospital,from January 2018 to June 2023.Patients were divided into unilateral vocal cord paralysis (UVCP) and bilateral vocal cord paralysis (BVCP) groups based on the side of the affected vocal cord.The Fisher exact test or Mann-Whitney U test was employed to compare the follow-up indicators between the two groups. Results:A total of 41 cases (26 males,15 females) were included.All were full-term infants with a mean birth weight of (3 240.0±553.9) g.The median age at first visit was 3.0 (1.0,12.5) days.All 41 cases exhibited varying degrees of hoarseness,with 33 (80.5%) experiencing inspiratory stridor,10 (24.4%) having feeding difficulties,and 9 (22.0%) presenting with dyspnea and cyanosis.Serum vitamin D levels were found to be deficient or insufficient in 19 of 24 (79.2%) cases.Eighteen (43.9%) cases were misdiagnosed,and 4 (9.8%) cases were missed.Seven (17.1%) cases were complicated by laryngomalacia.Among the 33 (80.5%) cases in the UVCP group,30 (73.2%) exhibited left VCP and 3 (7.3%) had right VCP.Eight (19.5%) cases were classified in the BVCP group.Nine patients required respiratory support,with 2 undergoing tracheotomy.Ten patients with feeding difficulties received tube feeding,and some underwent swallowing and sucking training.A total of 37 (90.2%) cases were followed up for a median of 2 (1,5) months; 29 (78.4%) had no symptoms,4 (10.8%) showed symptom improvement,2 (5.4%) had persistent symptoms,and 2 (5.4%) died due to treatment interruption.The rates of symptom disappearance and improvement in the UVCP group were significantly higher than those in the BVCP group,while the rate of symptom persistence and mortality were significantly lower in the UVCP group (83.3% vs.57.1%,13.3% vs.0,0 vs.28.6%,3.3% vs.14.3%, P<0.05).No statistically significant difference was found between the two groups regarding the age of symptom disappearance ( P>0.05). Conclusion:Neonatal congenital idiopathic VCP is more prevalent among full-term infants,with most cases presenting hoarseness,vitamin D deficiency,and some associated with laryngomalacia.The condition is often misdiagnosed or missed,but most cases demonstrate a favorable prognosis.

Objective To analyze the trends in disease burden and risk factors of depression among the elderly population in China from 1990 to 2021,and to provide a theoretical basis for the prevention,treatment,and policy-making of geriatric depression in China.Methods Data on the disease burden of geriatric depres-sion in China from 1990 to 2021,including the number of incident cases,disability-adjusted life years(DALYs),incidence rate,and DALY rate,were extracted from the 2021 Global Burden of Disease(GBD)database.The Joinpoint regression model was used to analyze the trends by calculating the annual percentage change(APC)and average annual percentage change(AAPC).The autoregressive integrated moving average(ARIMA)model was employed to predict the disease burden of geriatric depression over the next five years.Population attributable fractions(PAFs)were used to describe the risk factors for geriatric depression in China in 1990 and 2021.Results From 1990 to 2021,the number of incident cases and the incidence rate of geri-atric depression in China showed an overall upward trend.The most significant increase in incidence was ob-served in the 60-64 age group,while the prevalence rate increased notably in the ≥ 95 age group.The DALY rate showed the most pronounced upward trend in the 65-69 age group.The incidence,prevalence,and DALY rates of geriatric depression were higher in women than in men.Major risk factors included child-hood sexual abuse and intimate partner violence,with the impact of intimate partner violence being particular-ly significant among women.The ARIMA model predicted that the incidence,prevalence,and DALY rates of geriatric depression in China would decline over the next five years,with a greater decline observed in women than in men.Conclusions From 1990 to 2021,the incidence,prevalence,and DALY rates of geriatric de-pression in China showed an overall upward trend,with higher rates observed in women than in men.Greater attention should be paid to the elderly female population,with a focus on early prevention to reduce the dis-ease burden of geriatric depression.

Objective:To investigate the awareness levels of doctors from various departments regarding fertility preservation to provide reference and suggestions for further improving awareness and promoting multidisciplinary development in fertility preservation.Methods:From November to December 2023, the China Alliance of Fertility Preservation initiated this survey covering 88 hospitals in 23 cities across 20 provinces in China. The survey focused on the degree of understanding of basic concepts, technological perceptions, treatment concepts, and related challenges regarding fertility preservation. Descriptive statistics and Fisher's exact test were used to analyze the difference in awareness levels across various departments.Results:There was a significant difference in the awareness of fertility preservation guidelines among physicians from different departments ( P<0.001). Reproductive center physicians had the highest level of understanding [68.3% (41/60)]. Doctors in the reproductive centers had a higher awareness rate of the five fertility preservation techniques (ovarian tissue cryopreservation, oocyte cryopreservation, sperm cryopreservation, testicular tissue cryopreservation, and embryo cryopreservation) than that in other departments (all P<0.05). Regarding treatment concepts, more reproductive center physicians believed that psychological counselling about fertility preservation before oncological treatment was necessary ( P=0.016). Hematologists had more worries and concerns about fertility preservation treatment. Additionally, 96.7% (58/60) of reproductive center physicians expressed the need to establish effective referral mechanisms within the same city, province, or across provinces. However, 55.6% (35/63) of oncologists, 64.6% (31/48) of hematologists, and 77.8% (7/9) of pediatricians considered treating primary tumors far more important than fertility preservation. Conclusion:Doctors from different departments have varying levels of understanding regarding fertility preservation. It is necessary to strengthen the dissemination and training of fertility preservation knowledge among physicians from various departments, as well as to promote a multidisciplinary treatment model to improve efficiency, to establish a comprehensive referral mechanism to improve patients' opportunities for fertility preservation.

Objective To explore the effects of exosomes of mesenchymal stem cells(MSC-Exos)on the proliferation,apoptosis,migration and invasion of esophageal cancer ECA109 cells.Methods Human umbilical cord mesenchymal stem cells were cultured,exosomes were extracted and isolated,and identified by transmission electron microscopy.The nanoparticle size determination and protein characterization(TSG101,CD63)were measured by transmission electron microscope.There were the MSC-Exo group(MSC-Exos co-cultured with esophageal cancer ECA109 cells)and the blank control group(only esophageal cancer ECA109 cells),and cells were cultured for 0,24 and 48 h,respectively.CCK-8 proliferation test and scratch test were used to detect the proliferation and migration ability of esophageal cancer EAC109 cells in each group,respectively.After 48 h of culture,cell apoptosis and cell cycle changes were detected by flow cytometry.The protein expression levels of phosphoinositol 3-kinase phosphorylation(p-PI3K),rabbit phosphorylated protein kinase B phosphorylation(p-Akt)and β-catenin were detected by Western blot assay.Results After identification,the obtained MSC-Exos meeted the required standard.Transmission electron microscopy,particle size measurement and marker protein results confirmed that the extracted exosomes of mesenchymal stem cells meeted the identification criteria.At 0 h of cell culture,there were no significant differences in cell proliferation and migration healing rate between the two groups(P＞0.05).After 24 h culture,the cell proliferation ability was lower in the MSC-Exo group than that of the blank control group(P＜0.05).After 48 h culture,the cell proliferation and migration healing rate were lower in the MSC-Exo group than those of the blank control group(P＜0.05).The apoptosis rate of the MSC-Exo group was higher than that of the blank control group,and the proportion of G2+S phase cells was lower than that of blank control group(P＜0.05).The expression levels of p-PI3K,p-Akt and β-catenin protein were significantly lower in the MSC-Exo group than those in the blank control group(P＜0.05).Conclusion MSC-Exos can inhibit the proliferation and migration of esophageal cancer cells and promote cell apoptosis.The inhibitory effect of MSC-Exos on esophageal cancer cells may be related to inhibiting the activation of PI3K and Akt protein and the down-regulating expression of β-catenin protein.

Objective To observe the clinical efficacy of"three modulation acupuncture"combined with repeated functional magnetic stimulation(rFMS)in the treatment of neurogenic bladder with detrusor muscle weakness after spinal cord injury.Methods A total of 120 patients with neurogenic bladder with detrusor muscle weakness after spinal cord injury were divided into conventional treatment group,"three modulation acupuncture"treatment group,rFMS treatment group and comprehensive treatment group according to the random number table method,with 30 patients per group.The conventional treatment group was given conventional rehabilitation treatment,the"three modulation acupuncture"treatment group was treated with"three modulation acupuncture"(modulating spirit,modulating reflex arc,and modulating lower jiao and waterway)based on conventional rehabilitation treatment,the rFMS treatment group was treated with rFMS based on conventional rehabilitation treatment,and the comprehensive treatment group was treated with"three modulation acupuncture"and rFMS based on conventional rehabilitation treatment.The first desire to void(FDV),maximum cystometric capacity(MCC),maximum detrusor pressure of urine storage period(Pdet.max),maximum intravesical pressure of urine storage period(Pves.max),average daily urination frequency,average daily urine leakage,residual urine volume,and neurogenic bladder symptom scores of the patients were compared before and after treatment,and the clinical effectiveness of each group was evaluated.Results After treatment,the FDV,MCC,and Pdet.max of the four groups were all increased compared with those before treatment,and Pves.max,the average daily urination frequency,the average daily frequency of urine leakage,the residual urine volume,and the neurogenic bladder symptom scores were all decreased(P<0.05).After treatment,the FDV,MCC,and Pdet.max of the"three modulation acupuncture"treatment group,the rFMS treatment group and the comprehensive treatment group were all higher than those of the conventional treatment group,and Pves.max,the average daily urination frequency,the average daily frequency of urine leakage,the residual urine volume,and the neurogenic bladder symptom scores were all lower(P<0.05).After treatment,the comprehensive treatment group had a higher FDV,MCC,and Pdet.max than the"three modulation acupuncture"treatment group and rFMS treatment group,and Pves.max,the average daily urination frequency,the average daily frequency of urine leakage,the residual urine volume,and the neurogenic bladder symptom scores were lower(P<0.05).The curative efficiency rates were 86.2%(25/29)in the"three modulation acupuncture"treatment group,85.7%(24/28)in the rFMS treatment group,and 92.6%(25/27)in the comprehensive treatment group,which was higher than that of the conventional treatment group,which was 75.9%(22/29).Conclusion"Three modulation acupuncture"and rFMS can effectively improve the functional status of the bladder in patients of neurogenic bladder with detrusor muscle weakness after spinal cord injury,and their combined application has a synergistic effect.
