The key link to colorectal cancer progression is that epigenetic derangements promote reprogramming of glucose metabolism.Based on many studies on the relationship between spleen being the origin of acquired constitution and epigenetics, spleen qi dispersing essence and glucose metabolism, combined with the extensive experience of traditional Chinese medicine in the treatment of colorectal cancer and the accumulation of researches on glucose metabolism,it is believed that the"epigenetic-reprogramming of glucose metabolism"pathology of colorectal cancer is the microscopic essence of the key pathogenesis of"dysfunction of spleen transforming essences".The"promoting spleen and dispersing essences"method means that"promoting spleen"to reverse epigenetic derangements and"dispersing essences"to regulate reprogramming of glucose metabolism was put forward to treat colorectal cancer.Look forward to enrich the scientific connotation of the pathogenesis of"dysfunction of spleen transforming essences",new ideas and potential empirical basis for traditional Chinese medicine intervention in colorectal cancer were provided.

Objective To explore the diagnostic value of 18F-fluorodeoxyglucose(FDG)PET/CT in primary pulmonary enteric adenocarcinoma. Methods The clinical and imaging data of 9 patients with primary pulmonary enteric adenocarcinoma who under-went 18F-FDG PET/CT examination were retrospectively analyzed, including lesion distribution, morphology, maximum standardized uptake value(SUVmax),clinical symptoms and signs, gastroscopy finding, puncture pathological results, and serum tumor markers[carbohydrate antigen 72-4(CA72-4),cytokeratin 19 fragment antigen 21-1(CYFRA21-1),carcinoembryonic antigen(CEA),carbo-hydrate antigen 199(CA199)].Results Pathological examination confirmed a diagnosis of primary pulmonary enteric adenocarcinoma after excluding gastrointestinal primary tumors through clinical evaluation. In all nine patients,18F-FDG PET/CT examination did not reveal any evidence of digestive system malignancies, and gastrointestinal microscopy was negative.Primary lesions were observed as masses or nodular types in 6 cases(5 in the left lung and 1 in the right lung),while 3 cases exhibited diffuse bilateral pulmonary involvement(manifested as multiple patchy opacities,nodules,ground-glass opacities, and consolidations). All pulmonary primary lesions showed increased 18F-FDG uptake, with SUVmax ranging from 2.7 to 12.8,mean 8.6±3.7.The six masses-or nodular-type primary lesions showed maximum diameters ranging from 2.1 to 10.5 cm,mean(5.23±3.06)cm.Four cases demonstrated hilar and mediastinal lymph node metastases, intrapulmonary metastases, and distant metastases, while 1 case showed only distant metastasis.Elevated levels of serum tumor markers were observed as follows:CA72-4 in 7 cases(10-273.3 U/mL),CEA in 7 cases(5-147.4 ng/mL),CA199 in 6 cases(31.22-4 364 U/mL),and CYFRA21-1 in 5 cases(8.31-99.7 ng/mL).Conclusion When pathological biopsy of a pulmonary lesion suggests primary pulmonary enteric adenocarcinoma after excluding gastrointestinal primary tumors, and 18F-FDG PET/CT shows no gastrointestinal masses, this may support the diagnosis of primary pulmonary enteric adenocarcinoma.

Journal of Practical Stomatology 2024;40(4):584-586
Oral and maxillofacial space infection(OMSI)as a mostly mixed infection, often requires the combination of anti-aerobe and an-ti-anaerobe antibiotics. The use of third-generation quinolones, such as levofloxacin,is limited in OMSI due to the tendon rupture and other serious adverse drug reactions(ADR).In the treatment of a case with OMSI and anaphilactic shock induced by levofloxacin, the association evaluation method of ADR and literature review method participated by clinical pharmacists were used.Suggestions on rational drug use and ADR treatment were gained, the patient was timely rescued from anaphylactic and cured of OMSI.

OBJECTIVE
To investigate the potential of low-frequency, low-power ultrasound to enhance the transdermal absorption and efficacy of ketoprofen gel.
METHODS
Ketoprofen gel was used as a model drug to compare the in vitro transdermal permeation of ultrasound treated group and untreated group. Additionally, a rat model of collagen-induced inflammation provided a basis for evaluating pharmacodynamic differences. Pharmacokinetic studies further elucidated the effects of ultrasound on ketoprofen gel's penetration process.
RESULTS
Ultrasound treatment enhanced the cumulative transdermal permeation of ketoprofen gel by 3.5-fold over 24 hours compared to untreated. Significant pharmacokinetic improvements in AUC0-t from (4289.02±763.58)ng·h·mL−1 to (11301.10±3386.30)ng·h·mL−1 and a reduction in Tmax from (6.0±1.4)h to (3.0±2.0)h. Ultrasound notably improved the gel's anti-inflammatory effects in the rat model, effectively and rapidly reducing inflammation-induced swelling.
CONCLUSION
Low-frequency, low-power ultrasound can significantly improve the amount and rate of transdermal absorption of ketoprofen gel and enhance its pharmacological potency, from the aspects of skin permeation tests, pharmacodynamic evaluation, and pharmacokinetic studies, which is an effective penetration enhancer for transdermal administration of ketoprofen gel.

Chinese Journal of Clinical Pharmacology and Therapeutics 2023;28(5):556-560
AIM: To explore the role of clinical pharmacist in the treatment of patients with transjugular intrahepatic portosystemic shunt. METHODS: Clinical pharmacist participated in the treatment of a patient with repeated oral bleeding after transjugular intrahepatic portal shunt followed treatment with multiple antithrombotic drugs, which assisted the physician to diagnose and adjust the antithrombotic treatment plan as well as provided the patient with whole-process pharmaceutical care and online management. RESULTS: Based on the inquiry about the patient's past and current medical history and medication consumption, the pharmacist considered that there was weak correlation between oral hemorrhage and antithrombotic drugs and advised for dentist inspection. Thereafter, the patient was diagnosed with chronic gingivitis. The dosage of warfarin was adjusted, and the pharmacists managed it online after discharge to achieve stable INR of the patient. In the later online follow-up, an abnormal increase of INR was encountered, By asking about the history of medication, it was considered that the increase in INR was related to taking amoxicillin capsules. Therefore, the pharmacist suggested to stop amoxicillin capsule and to gradually adjust the dose of warfarin to the original level to improve the treatment. CONCLUSION: The involvement of clinical pharmacists in clinical treatment facilitates comprehensive pharmaceutical care of patients, which plays a positive role in the efficacy and safety of medication therapy.
