This paper summarizes professor ZHU Shengliang's academic idea and clinical experience in treating gastroesophageal reflux disease (GERD) with qi constraint and phlegm obstruction syndrome. It is believed that the core pathogenesis of GERD is the dysfunction of the liver, gallbladder, spleen and stomach, with binding of phlegm and stasis. The basic pathogenesis is the liver and gallbladder failing to dredge and disperse, the stomach failing to harmonize and descend, and the stomach qi counterflowing upward. In clinical practice, the treatment principle of regulating qi as the priority, resolving phlegm as the key, and activating blood as the assistant is applied, with harmonizing the stomach and directing counterflow downward throughout the whole process. A self-made Shugan Hewei Huatan Formula (疏肝和胃化痰方) is employed with the function of soothing the liver and rectifying qi, opening constraint and dissipating knot, dissolving phlegm and dispelling stasis, harmonizing stomach and directing counterflow downward, with an emphasis on flexible modification according to the syndrome as well as careful protection of stomach qi.

Objective:To explore the clinicopathological characteristics and prognosis of fumarate hydratase-deficient uterine leiomyoma (FH-dUL).Methods:Clinical data and follow-up information for 117 patients with FH-dUL diagnosed through surgical pathology and immunohistochemistry in the First Affiliated Hospital of Nanjing Medical University from January 2020 to December 2024, were collected. A control group of 130 patients with common uterine leiomyomas was also included. The differences between the two groups in clinical, imaging, and pathological characteristics were compared. Additionally, recurrence rates, fertility outcomes for FH-dUL patients, and the incidence of renal cancer in FH germline mutation carriers were monitored.Results:(1) Comparison of clinicopathological characteristics: the median age of 117 FH-dUL patients was 35 years, and the median age at first diagnosis of uterine leiomyomas was 29 years, both significantly younger than the control group (41 and 36 years; both P<0.01). The FH-dUL group showed significantly higher incidences of uterine myomectomy, multiple leiomyomas, diffusion restriction on pelvic magnetic resonance imaging diffusion weighted imaging, and typical pathological features (candelabra-like vessels, bizarre nuclei, cytoplasmic eosinophilic globules, perinuclear halo, cellular atypia) and higher ultrasound blood flow score (all P<0.05). Of the 30 FH-dUL patients who underwent genetic testing, 9 had germline mutations, 3 had somatic mutations, and 6 had mutations of unclear origin. Among the 9 FH gene germline mutation patients, 2 had already developed renal cell carcinoma. (2) Recurrence analysis: among the 56 patients who underwent uterine myomectomy, 22 (39.3%, 22/56) experienced recurrence during follow-up, compared to 12 (21.8%, 12/55) of the 55 patients in the control group, the difference between the two groups was statistically significant ( P=0.046). Multivariate binary logistic regression analysis showed that cellular leiomyomas ( OR=9.489, 95% CI: 1.740-51.755; P=0.009) and multiple uterine leiomyomas ( OR=10.709, 95% CI: 1.354-84.683; P=0.025) were significant risk factors for recurrence in FH-dUL. (3) Fertility analysis: among the 66 FH-dUL patients who underwent fertility-preserving surgery, 16 had the intention to have fertility desire, only 2 (2/16) completed their fertility plans during follow-up. Conclusions:Clinicopathological features and imaging features help to differentiate FH-dUL from common type uterine fibroids, but lack specificity, and the diagnosis of FH-dUL is based on immunohistochemistry. The recurrence rate after resection of FH-dUL is high, and cellular and multiple leiomyomas are important predictors of recurrence. It is crucial to perform genetic testing, genetic counseling, drug treatment to prevent recurrence, fertility guidance, and long-term comprehensive management after surgery for FH-dUL management.
4.Early screening and risk factors for stroke-related sarcopenia
Journal of Apoplexy and Nervous Diseases 2025;42(5):454-458
Objective To investigate the incidence rate of sarcopenia and related risk factors in patients with stroke. Methods A retrospective analysis was performed for the patients with stroke who were admitted to the stroke center of a grade A tertiary hospital in Changchun, China, from March 2023 to June 2024. The method of bioelectrical impedance was used to perform body composition analysis for all patients on day(7.0±1.0)after admission, and the incidence rate of stroke-related sarcopenia was analyzed. A binary logistic regression analysis was used to investigate the risk factors for stroke-related sarcopenia. Results A total of 666 patients were included in the study, among whom 150(22.5%) developed sarcopenia (95%CI 0.193‒ 0.257). Low body mass index, low phase angle, low triglyceride, advanced age, and low Barthel index were risk factors for the early onset of sarcopenia in patients with stroke. Conclusion There is a relatively high incidence rate of sarcopenia in stroke patients, with complex influencing factors. Medical staff should pay more attention to the elderly stroke patients, as well as those patients with emaciation, low phase angle, low triglyceride, and limited activities of daily living. Early nutritional supplementation and functional exercise can help to prevent the onset of stroke-related sarcopenia.

Objective:To identify the incidence and prevalence of inflammatory bowel disease (IBD) in the northern Chinese population of Jinan, Shangdong Province, along with its temporal trends from 2005 to 2022.Methods:By utilizing the data from the Jinan basic medical insurance system, a population-based IBD cohort was constructed. This facilitated the computation of both the incidence and prevalence rates of IBD, alongside their temporal trends throughout the 2005 to 2022 timeframe. The 95% confidence intervals were estimated using poisson regression. Results:The overall incidence rate of IBD showed a yearly increasing trend, with age-standardized incidence rates rising from 0.03/100 000 in 2005 to 5.39/100 000 in 2022. The age-standardized incidence rate of ulcerative colitis (UC) increased from 0.03/100 000 in 2005 to 4.97/100 000 in 2022. The age-standardized incidence rate of Crohn's disease (CD) rose from 0.05/100 000 in 2011 to 0.44/100 000 in 2022. The crude prevalence of IBD increased from 0.60/100 000 in 2005 to 32.39/100 000 in 2022. Specifically, the crude prevalence of UC increased from 0.60/100 000 in 2005 to 31.44/100 000 in 2022, while the crude prevalence of CD increased from 0.05/100 000 in 2011 to 1.19/100 000 in 2022.Conclusions:Analysis of recent medical insurance data reveals a continuous uptrend in both the incidence and prevalence of IBD in Jinan, a northern city in China. This underscores the urgent need for enhanced medical resources and healthcare guaruntee to ensure the well-being of individuals afflicted with IBD.

BACKGROUND:Steroid-induced osteonecrosis of the femoral head is a key risk factor for non-traumatic femoral head necrosis, and the incidence of it has gradually increased in recent years, but its specific pathogenesis is still unclear, and finding a reasonable animal model is essential for disease research and treatment.OBJECTIVE:To review the commonly used animal models of steroid-induced osteonecrosis of the femoral head in recent years, and to analyze the advantages and disadvantages of different modeling methods and evaluation criteria,so as to provide a reference for follow-up research. METHODS:The articles published from 2013 to 2023 were searched in CNKI,WanFang Data, and PubMed with the keywords "femoral head necrosis, osteonecrosis of the femoral head,steroid-induced osteonecrosis of the femoral head, animal model." Finally,61 articles were involved for comprehensive analysis according to the inclusion criteria, including 38 English articles and 23 Chinese articles.RESULTS AND CONCLUSION:(1) Rabbits, rats, and chickens are the animals that are widely used in the study of steroid-induced osteonecrosis of the femoral head models. (2) Hormone combined with lipopolysaccharide or horse serum modeling has low mortality, high success rate, and strong stability. (3) Histopathology is the gold standard for the evaluation model, but the experiment needs to be terminated, which is not conducive to subsequent experimental research,so finding a non-invasive alternative method is still the direction of future efforts. (4) An ideal model of steroid-induced osteonecrosis of the femoral head has not yet been explored, and future researchers need to continue to strive to achieve breakthroughs in this field as soon as possible.
