1.Values of non-invasive myocardial work and tissue motion annular displacement in evaluating anthracycline-induced cardiotoxicity

Objective To explore the clinical values of non-invasive myocardial work (MW) and tissue motion annular displacement (TMAD) in evaluation of anthracycline therapy-related cardiac dysfunction in patients with non-Hodgkin lymphoma. Methods A total of 62 patients with non-Hodgkin lymphoma who received standardized chemotherapy based on doxorubicin. Two and three dimensional transthoracic echocardiography, along with two dimensional speckle tracking echocardiography, were performed one day before chemotherapy and at 3, 6, and 9 months after chemotherapy to assess left ventricular ejection fraction, global longitudinal strain (GLS), MW parameters, and TMAD. Logistic regression analysis was used to evaluate the risk factors for cancer therapy-related cardiac dysfunction (CTRCD). The receiver operating characteristic curve was used to assess the diagnostic values of MW- and TMAD-related parameters for CTRCD. Results Compared to baseline, GLS, global work index (GWI), global constructive work (GCW), global work efficiency (GWE), TMAD at midpoint (TMADmid), and TMADmid percentage of left ventricular long-axis diameter (TMADmid%) decreased at 3 months after chemotherapy, while global wasted work (GWW) increased at 6 months after chemotherapy (P<0.05). Logistic regression analysis showed that the relative reduction in GLS and TMADmid% at 3 months after chemotherapy were independent predictors for CTRCD (P<0.05), while MW parameters were not independent predictors for CTRCD. GLS reduction≥10.3% and TMADmid% reduction≥15.8% at 3 months after chemotherapy predicted CTRCD with 0.866 and 0.824 of area under the curve (AUC), 92% and 75% of sensitivity, and 74% and 80% of specificity, respectively. AUC of combination of two indexes improved to 0.905, with 75% of sensitivity and 90% of specificity. Conclusions In non-Hodgkin lymphoma patients, the combination of GLS and TMADmid% is helpful of predicting CTRCD early, TMAD may be a novel diagnostic index for CTRCD, and GLS has superior predictive performance than MW for CTRCD.
2.Clinical experience of low-dose PTCY combined with ATG in preventing graft versus host disease after hematopoietic stem cell transplantation in children with β-thalassemia

Objective:To evaluate the efficacy and safety of low-dose post-transplant cyclophosphamide (PTCY) combined with anti-thymocyte globulin (ATG) in preventing graft-versus-host disease (GVHD) in children with β-thalassemia after hematopoietic stem-cell transplantation (HSCT).Method:A retrospective analysis was conducted on 42 children with transfusion-dependent β-thalassemia who underwent HSCT at Zhongshan Hospital, Xiamen University between March 2019 and June 2023. Based on donor source, recipients were grouped into the haploidentical donor group (Haplo-RD, 10 cases) and the unrelated donor group (UD, 32 cases). The UD group was further subdivided into HLA 8/10 matched (2 cases), HLA 9/10 matched (15 cases), and fully HLA-matched (15 cases). The conditioning regimen included fludarabine, busulfan, cyclophosphamide, and thiotepa. GVHD prophylaxis consisted of ATG (4.5 mg/kg), cyclophosphamide (25 mg/kg for 2 days), cyclosporine A (CsA), and mycophenolate mofetil (MMF). Engraftment, GVHD incidence, survival, mortality, and virus reactivation rates were evaluated.Result:The median age was 6 years (range, 2~12). All patients achieved hematopoietic reconstitution. The median times to neutrophil and platelet engraftment were 11 days (range, 10~15) days and 12 days (range, 6~31), respectively. All recipients had >95% peripheral blood STR chimerism by day 30. Grade Ⅲ~Ⅳ acute GVHD occurred in 3 recipients (7.14%), and chronic GVHD occurred in 5 recipients (11.90%) -1 case extensive, 4 cases limited. Both overall survival (OS) and disease-free survival (DFS) were 92.86%. All children in the Haplo-RD group achieved DFS. Eight patients developed cytomegalovirus (CMV) viremia (no CMV disease), with a reactivation rate of 19.05%, and 9 recipients had BK virus-related urinary tract infections (6 cases in the UD group and 3 cases in the Haplo-RD group), for a total incidence of 21.43%.Conclusion:The combination of low-dose PTCY and ATG is a safe and effective strategy to prevent GVHD following haploidentical or unrelated donor HSCT in pediatric β-thalassemia. It is associated with reduced infection and viral reactivation post-transplant and contributes to high survival rates.
3.Visualization analysis based on research into adverse reactions of CT enhanced examination using CiteSpace

Objective:To summarize the domestic and international research progress on adverse reactions to contrast-enhanced CT examinations, and forecast research themes, future hotspots, and trends.Methods:Using CiteSpace bibliometric methodology, retrieve Chinese and English literature on adverse reactions in patients undergoing contrast-enhanced CT examinations from China National Knowledge Infrastructure(CNKI), Wanfang Data Knowledge Service Platform, VIP Chinese Science and Technology Periodical Database, and Web of Science. The search period covers database inception to December 31, 2023. After deduplication, CiteSpace and other software are employed to conduct statistics and analyses on publication volume, publishing countries, institutions, and keywords for both Chinese and English literature separately.Results:A total of 3 749 articles were included, comprising 3 044 Chinese-language articles and 705 English-language publications. Visual analysis revealed that the annual number of publications on CT contrast-enhanced examination-related adverse reactions exhibited fluctuating growth from 1994 to 2023. The United States emerged as the leading contributor in this research domain, while China demonstrated rapid growth in recent years. Network analysis of research institutions revealed insufficient collaboration and communication among domestic institutions. Thematic clustering identified 22 clusters and 25 emerging keywords in Chinese literature, compared to 20 clusters and 7 burst keywords in English publications. Domestic research priorities focused predominantly on adverse reactions to contrast agents, nursing interventions, and nursing prevention strategies. International research trends emphasized safety, risk factor identification for adverse reactions, contrast-induced acute kidney injury (CI-AKI) prevention, and the prevention of adverse reactions using contrast-agent nanomaterials.Conclusions:Both domestic and international research in this field exhibits distinct emphases but universally underscores the importance of nursing. It is imperative to strengthen multidisciplinary collaboration among domestic institutions in the future. By focusing on research hotspots and cutting-edge frontiers, expanding the depth and breadth of China's research in this domain, and continuously optimizing nursing strategies, we can effectively enhance nursing quality for CT contrast-enhanced examination patients, optimize patient experience, and contribute to the high-quality development of nursing practice in this field.

Objective To investigate the distribution and antimicrobial resistance profiles of clinically isolated Haemophilus influenzae and Moraxella catarrhalis in hospitals across China from 2015 to 2021,and provide evidence for rational use of antimicrobial agents.Methods Data of H.influenzae and M.catarrhalis strains isolated from 2015 to 2021 in CHINET program were collected for analysis,and antimicrobial susceptibility testing was performed by disc diffusion method or automated systems according to the uniform protocol of CHINET.The results were interpreted according to the CLSI breakpoints in 2022.Beta-lactamases was detected by using nitrocefin disk.Results From 2015 to 2021,a total of 43 642 strains of Haemophilus species were isolated,accounting for 2.91%of the total clinical isolates and 4.07%of Gram-negative bacteria in CHINET program.Among the 40 437 strains of H.influenzae,66.89%were isolated from children and 33.11%were isolated from adults.More than 90%of the H.influenzae strains were isolated from respiratory tract specimens.The prevalence of β-lactamase was 53.79%in H.influenzae strains.The H.influenzae strains isolated from children showed higher resistance rate than the strains isolated from adults.Overall,779 strains of H.influenzae did not produce β-lactamase but were resistant to ampicillin(BLNAR).Beta-lactamase-producing strains showed significantly higher resistance rates to these antimicrobial agents than the β-lactamase-nonproducing strains.Of the 16 191 M.catarrhalis strains,80.06%were isolated from children and 19.94%isolated from adults.M.catarrhalis strains were mostly susceptible to both amoxicillin-clavulanic acid and cefuroxime,evidenced by resistance rate lower than 2.0%.Conclusions The emergence of antibiotic-resistant H.influenzae due to β-lactamase production poses a challenge for clinical anti-infective treatment.Therefore,it is very important to implement antibiotic resistance surveillance for H.influenzae and guide rational antibiotic use.All local clinical microbiology laboratories should actively improve antibiotic susceptibility testing and strengthen antibiotic resistance surveillance for H.influenzae.

Burkitt lymphoma is a highly aggressive B-cell lymphoma and the fastest proliferating human malignant tumor.If the disease is found in the early stage,the patient could have a high possibility to be cured successfully,whereas the prognosis is poor in the late stage.Burkitt lymphoma can occur in children and adults,and it is categorized as local(Africa),sporadic,and immunodeficiency associated type.Sporadic Burkitt lymphoma mainly affects children and ado-lescents,and the most common initial sites are abdominal organs and lymph nodes.Sporadic Burkitt lymphoma manifested by initial oral and maxillofacial lesions is rela-tively rare.Here,a case of pediatric sporadic Burkitt lym-phoma,with oral and maxillofacial lesions as the first symptoms,was reported.The patient was treated in the Department of Periodontology,Shandong University School and Hospital of Stomatology.After timely checkup was pro-vided,the patient was transferred to another hospital and had good results.In this article,an incidence of Burkitt lympho-ma,with oral and maxillofacial lesions as the first symptom,was reviewed to provide reference for oral clinicians to achieve early diagnosis and treatment of patients with Burkitt lymphoma with oral diseases and improve the success rate of treatment.
9.Development and validation of an Assessment Scale of Proactive Health Behavior Ability for the Disabled Elderly in Nursing Homes

Objective To develop the assessment scale of proactive health behavior ability for the disabled elderly in nursing homes and to test its reliability and validity.Methods The first draft of the scale was formed by literature review,qualitative interviews and Delphi method.From December 2023 to March 2024,525 disabled elderly people from 9 nursing homes in Sichuan Province and Chongqing City were selected as the survey subjects,and item analysis and reliability and validity test were carried out on the scale.30 disabled elderly people were re-investigated after 2 weeks to calculate the retest reliability of the scale.Results The scale consisted of 4 dimensions and 27 items.Exploratory factor analysis extracted 4 common factors,with the cumulative vanance contribution rate of 65.992%,and confirmatory factor analysis showed that the modified model fitting index was within acceptable range.The content validity index at item level was 0.917-1.000,and that at scale level was 0.997.The Cronbach's α coefficient,test-retest reliability and split-half reliability of the total scale were 0.944,0.997 and 0.882,respectively.Conclusion The scale has good reliability and validity,and it can be used to evaluate the proactive health behavior ability of the disabled elderly in nursing homes.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in healthcare facilities in major regions of China in 2023.Methods Clinical isolates collected from 73 hospitals across China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2023 Clinical & Laboratory Standards Institute (CLSI) breakpoints.Results A total of 445199 clinical isolates were collected in 2023,of which 29.0% were gram-positive and 71.0% were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species (excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi) (MRSA,MRSE and MRCNS) was 29.6%,81.9% and 78.5%,respectively.Methicillin-resistant strains showed significantly higher resistance rates to most antimicrobial agents than methicillin-susceptible strains (MSSA,MSSE and MSCNS).Overall,92.9% of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 91.4% of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis had significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 93.1% in the isolates from children and and 95.9% in the isolates from adults.The resistance rate to carbapenems was lower than 15.0% for most Enterobacterales species except for Klebsiella,22.5% and 23.6% of which were resistant to imipenem and meropenem,respectively .Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.6% to 10.0%.The resistance rate to imipenem and meropenem was 21.9% and 17.4% for Pseudomonas aeruginosa,respectively,and 67.5% and 68.1% for Acinetobacter baumannii,respectively.Conclusions Increasing resistance to the commonly used antimicrobial agents is still observed in clinical bacterial isolates.However,the prevalence of important crabapenem-resistant organisms such as crabapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a slightly decreasing trend.This finding suggests that strengthening bacterial resistance surveillance and multidisciplinary linkage are important for preventing the occurrence and development of bacterial resistance.
