5.Attaching importance to the study of referral and transport of critically ill patients: my humble opinion on non-emergency medical referral and transporting service

The transfer of critically ill patients is related to the treatment and prognosis of patients. It is not only the work of saving lives and injuries in the medical and health system, but also a measure of the ability and level of public health services in a region. "Non-emergency medical transshipment service" is a new form of business which is emerging at present. The phenomenon of "illegal ambulance" derived from it has attracted the attention of relevant government departments and has become the focus of media. The solution of this problem involves many aspects. The perspectives of defining the orientation of transfer work, constructing transfer service system, formulating transfer process norms and improving the quality of transfer efficiency are discussed in this paper, and it clearly puts forward that the development of "non-emergency medical transfer service" should adhere to the development path of marketization, diversification, standardization and specialization.
6.Clinical biological characteristics of metastatic carcinoma of the thyroid: a report of 30 cases

Objectives To study the clinical characteristics,diagnosis,treatment and prognosis of cancer metastases to the thyroid gland.Methods At Tianjin Medical University Cancer Hospital,from 1980 to 2016 30 patients were identified with a metastatic malignant tumor of the thyroid gland.Results Primary tumor sites were in the esophagus (26.7%),lung (16.7%),and kidney (13.3%).The median age at discovery of thyroid metastasis was 56 years old.The time lapse ranged from 0 to 108 months.There were 18 patients with metachronous metastasis with median survival of 36 months,12 patients were with synchronous metastasis,and the median survival was 8 months (P < 0.01).20 patients were treated with thyroid surgery,and of which 15 patients were given post-op radiotherapy and chemotherapy.Patients who underwent thyroid resection had a median survival of 15 months,while that was 8 months in those without thyoid surgery (P < 0.01).Conclusions Metastases to the thyroid gland is a rare clinical phenomenon,the prognosis of patients is mainly related to the biological behavior of primary tumors,but surgical resection and combined treatment can increase the survival rate.
7.Forward and maneuver: how to save life and injuries in an air rescue room or intensive care unit?

Aviation medicine is being recognized as a rising star and an indispensable part of the "trinity"medical emergency network composed of land, water and air. The unparalleled mobility and traffic advantages of aircraft, especially rotorcraft (helicopter), are playing an increasingly important role in rescuing the dying and wounded, especially the critical patients, and thus derive a new medical operation mode. How to adapt to the development of the situation and the progress of the times, give full play to the advantages of aviation medical care? The key factors are improvement of the level of service efficiency, strength of learning, practice courage, innovation, and good at summing up to keep pace with the times.
8.Genomic characterization and pathogenic study of two porcine reproductive and respiratory syndrome viruses with different virulence in Fujian, China

Two strains of porcine reproductive and respiratory syndrome virus (PRRSV) were isolated in 2006 and 2016 and designated as FZ06A and FZ16A, respectively. Inoculation experiments showed that FZ06A caused 100% morbidity and 60% mortality, while FZ16A caused 100% morbidity without death. By using genomic sequence and phylogenetic analyses, close relationships between a Chinese highly pathogenic PRRSV strain and the FZ06A and FZ16A strains were observed. Based on the achieved results, multiple genomic variations in Nsp2, a unique N-glycosylation site (N³³→K³³), and a K151 amino acid (AA) substitution for virulence in the GP5 of FZ16A were detected; except the 30 AA deletion in the Nsp2-coding region. Inoculation experiments were conducted and weaker virulence of FZ16A than FZ06A was observed. Based on our results, a 30 AA deletion in the Nsp2-coding region is an unreliable genomic indicator of a high virulence PRRSV strain. The Nsp2 and GP5 differences, in addition to the virulence difference between these two highly pathogenic PRRSV strains, have the potential to be used to establish a basis for further study of PRRSV virulence determinants and to provide data useful in the development of vaccines against this economically devastating disease.
